    Promised incentives on a random digit dial survey

    Die Verfasser legen die Ergebnisse von zwei Experimenten vor, mit denen die Wirkung von versprochenen Belohnungen bei einer Random-Digit-Dial-Untersuchung im Stadium der Gesprächseröffnung sowie im Stadium der Aufgabe einer anfänglichen Verweigerungshaltung untersucht werden sollte. In beiden Fällen zeigte sich kein Effekt. Dieses Ergebnis steht im Widerspruch zu mindestens einem weiteren Experiment aus jüngster Zeit sowie auch zum Einsatz versprochener Belohnungen zur Reduktion von Antwortverweigerung bei mehreren Meinungsforschungsinstituten. The results of the experiments find no effect of at either stage of the survey process. These results are contrary to those found by at least one other recent experiment, as well as the use of promised incentives used routinely to convert reluctant respondents by several survey organizations.     Practicing doctors' perceptions on new learning objectives for Vietnamese medical schools

    <p>Abstract</p> <p>Background</p> <p>As part of the process to develop more community-oriented medical teaching in Vietnam, eight medical schools prepared a set of standard learning objectives with attention to the needs of a doctor working with the community. Because they were prepared based on government documents and the opinions of the teachers, it was necessary to check them with doctors who had already graduated and were working at different sites in the community.</p> <p>Methods</p> <p>Each of the eight medical faculties asked 100 practising recent graduates to complete a questionnaire to check the relevance of the skills that the teachers considered most important. We used mean and standard deviation to summarize the scores rated by the respondents for each skill and percentile at four points: p50, p25, p10 and p5 to describe the variation of scores among the respondents. Correlation coefficient was used to measure the relationship between skill levels set by the teachers and the perception of practicing doctors regarding frequency of using skills and priority for each skill. Additional information was taken from the records of focus group discussions to clarify, explain or expand on the results from the quantitative data.</p> <p>Results</p> <p>In many cases the skills considered important by teachers were also rated as highly necessary and/or frequently used by the respondents. There were, however, discrepancies: some skills important to teachers were seldom used and not considered important by the doctors. In focus group discussions the doctors also identified skills that are not taught at all in the medical schools but would be needed by practising doctors. </p> <p>Conclusion</p> <p>Although most of the skills and skill levels included in the learning objectives by the teachers were consistent with the opinions of their graduates, the match was not perfect. The experience of the graduates and their additional comments should be included as inputs to the definition of learning objectives for medical students.</p

    Mortalidad en menores de cinco años, departamento del Putumayo

    La mortalidad por desnutrición en menores de cinco años para el país ha presentado una disminución entre los años 2005 a 2014, presentando tasas de 14,87 a 6,82 muertes por cada 100,000 menores de cinco años respectivamente. Los departamentos que presentaron tasas de mortalidad por desnutrición por encima de la tasa nacional uno de los departamentos fue el Putumayo. Más del 70% de la mortalidad por este evento en menores de cinco años se concentró en el 50% de la población que presentaba menor acceso a fuentes de agua potable, personas con necesidades básicas insatisfechas y con mayores barreras a los servicios de salud para la primera infancia. Por lo anterior, este evento ingresa como nuevo al Sistema de vigilancia en salud pública, de Colombia durante el año 2013 en su fase de implementación. Su vigilancia responde al propósito expuesto en alianza público – privada por la seguridad alimentaria y nutricional de acuerdo para la prosperidad N|° 29 de la alta consejería para las Regiones y la Participación Ciudadana de abril del 2011. Cerca de la mitad de las muertes de los menores de cinco años podrían atribuirse a la subnutrición, esto es cerca de 2,95 millones de niños para el 2015. la desnutrición o subnutrición en la población infantil tiene efectos negativos en la salud, en la educación y a lo largo del tiempo en la productividad y el crecimiento económico de un país, pues se pueden ver implicados las habilidades, el desarrollo cognitivo, la prevalencia de enfermedades transmisibles y no transmisibles y la mortalidad.Mortality due to malnutrition in children under five years of age for the country has shown a decrease between the years 2005 to 2014, presenting rates of 14.87 to 6.82 deaths per 100,000 children under five years of age respectively. The departments that presented mortality rates due to malnutrition above the national rate, one of the departments was Putumayo. More than 70% of the mortality due to this event in children under five years of age was concentrated in the 50% of the population that had less access to drinking water sources, people with unsatisfied basic needs and greater barriers to health services for early childhood. Due to the above, this event enters Colombia's public health surveillance system as new during 2013 in its implementation phase. Its surveillance responds to the purpose set out in the public-private alliance for food and nutritional security according to agreement for prosperity No. 29 of the High Council for the Regions and Citizen Participation of April 2011. About half of the deaths of children under five years of age could be attributed to undernutrition, this is about 2.95 million children in 2015. Malnutrition or undernutrition in the child population has negative effects on health, education and over time in the productivity and economic growth of a country, since skills, cognitive development, the prevalence of communicable and non-communicable diseases and mortality can be implicated

    Report from “Observatório COVID19 - Grupo: Redes de Contágio – Laboratório de Estudos de Defesa” for the Northeast Region of Brazil. We combined data on confirmed cases of the new coronavirus (SARS-CoV-2) as reported by public authorities by 02/04 and structural analyses of road transport networks within and between the nine States of Northeast Brazil to predict the potential influence of the 187 micro-regions on the geographic propagation of the pandemic.  Este é o primeiro relatório do Observatório COVID1920 - Grupo: Redes de Contágio – Laboratório de Estudos de Defesa para a região Nordeste do Brasil.     This is the first report by the COVID19 Observatory - Group: Contagion Networks analyzing mortality data from the city of São Paulo. In this report, we integrated mortality data for the city of São Paulo between 04/02/2020 and 04/28/2020, with information on the flow of victims between hospitals and cemeteries/crematoriums. We included in our analyzes both confirmed and suspected deaths from COVID-19. The main objectives of this report were: (1) to describe the structure of the flow of victims between locations and (2) to suggest changes in the current flow based on geographical distances in order to avoid a potential overload of the mortuary system. We suggest that the city of São Paulo should plan for a potential overload of the mortuary system (that is, the number of burials), based on the presented results. Thus, our results reinforce the need to adopt specific planning for the management of the extraordinary number of victims of this pandemic. Our predictions are based on the structural analysis of the COVID-19 victim flow network, which shows several hotspots with high vulnerability to system overload. These hotspots concentrate with either the greatest number of deaths (hospital) or of burials (cemetery or crematorium), and therefore have high potential to become overwhelmed by receiving many bodies due to the increase in victims of the pandemic. We recommend special attention to be given to localities on the east side of São Paulo, which has both the most vulnerable hospitals in the city, and also houses cemeteries and crematoriums that have a central role in the network and / or are vulnerable. Based on our optimization analysis, we suggest logistical changes in the current flow of bodies from hospitals to cemeteries/crematoriums so as not to overload the funeral system and minimize transportation costs. In this sense, our results are potentially useful for improving the operational planning of the Municipality of São Paulo, ratifying or rectifying actions underway at the municipal level.Este é o primeiro relatório do Observatório COVID19 - Grupo: Redes de Contágio analisando os dados de óbitos da cidade de São Paulo. Neste relatório, integramos os dados de óbitos da cidade de São Paulo entre os dias 02/04/2020 e 28/04/2020 com informações sobre o fluxo de vítimas entre os hospitais e os cemitérios e crematórios da cidade de São Paulo. Incluímos em nossas análises óbitos confirmados e óbitos suspeitos de COVID-19. Os principais objetivos deste relatório são: (1) descrever a estrutura do fluxo de vítimas entre localidades e (2) sugerir mudanças no fluxo com base em distâncias geográficas de maneira a evitar uma potencial sobrecarga do sistema funerário.&nbsp;Sugere-se à prefeitura da cidade de São Paulo que seja realizado um planejamento para uma potencial sobrecarga do sistema funerário (isto é, número de sepultamentos) da cidade de São Paulo com base nos resultados apresentados. Desta forma, nossos resultados reforçam a necessidade de ser adotado planejamento específico para a gestão dos casos extraordinários visualizados no contexto da pandemia. Esta previsão está baseada na análise estrutural da rede de fluxos de vítimas da COVID-19, que indica a concentração de vários locais com alta vulnerabilidade à sobrecarga do sistema. Tais locais concentram a maior quantidade de óbitos (hospitais) ou a maior concentração de sepultamentos (cemitérios ou crematórios) e tem portanto alto potencial de tornarem-se sobrecarregados por receberem muitos corpos devido ao aumento de vítimas da pandemia. Recomenda-se especial atenção à localidades da zona leste de São Paulo, que apresenta os hospitais mais vulneráveis da cidade e abriga cemitérios e crematórios que possuem papel central na rede e/ou encontram-se vulneráveis. Com base em nossa análise de otimização, sugerimos mudanças logísticas no atual fluxo de corpos de hospitais para cemitérios/crematórios de modo a não sobrecarregar o sistema funerário e minimizar os custos de transporte. Neste sentido, nossos resultados são potencialmente úteis ao aperfeiçoamento do planejamento operacional da Prefeitura Municipal de São Paulo, ratificando ou retificando ações em curso no âmbito municipal

    Contribution of copy number variants to schizophrenia from a genome-wide study of 41,321 subjects

    Copy number variants (CNVs) have been strongly implicated in the genetic etiology of schizophrenia (SCZ). However, genome-wide investigation of the contribution of CNV to risk has been hampered by limited sample sizes. We sought to address this obstacle by applying a centralized analysis pipeline to a SCZ cohort of 21,094 cases and 20,227 controls. A global enrichment of CNV burden was observed in cases (OR=1.11, P=5.7×10−15), which persisted after excluding loci implicated in previous studies (OR=1.07, P=1.7 ×10−6). CNV burden was enriched for genes associated with synaptic function (OR = 1.68, P = 2.8 ×10−11) and neurobehavioral phenotypes in mouse (OR = 1.18, P= 7.3 ×10−5). Genome-wide significant evidence was obtained for eight loci, including 1q21.1, 2p16.3 (NRXN1), 3q29, 7q11.2, 15q13.3, distal 16p11.2, proximal 16p11.2 and 22q11.2. Suggestive support was found for eight additional candidate susceptibility and protective loci, which consisted predominantly of CNVs mediated by non-allelic homologous recombination

    Genetic correlation between amyotrophic lateral sclerosis and schizophrenia

Palotie on työryhmän Schizophrenia Working Grp Psychiat jäsen.We have previously shown higher-than-expected rates of schizophrenia in relatives of patients with amyotrophic lateral sclerosis (ALS), suggesting an aetiological relationship between the diseases. Here, we investigate the genetic relationship between ALS and schizophrenia using genome-wide association study data from over 100,000 unique individuals. Using linkage disequilibrium score regression, we estimate the genetic correlation between ALS and schizophrenia to be 14.3% (7.05-21.6; P = 1 x 10(-4)) with schizophrenia polygenic risk scores explaining up to 0.12% of the variance in ALS (P = 8.4 x 10(-7)). A modest increase in comorbidity of ALS and schizophrenia is expected given these findings (odds ratio 1.08-1.26) but this would require very large studies to observe epidemiologically. We identify five potential novel ALS-associated loci using conditional false discovery rate analysis. It is likely that shared neurobiological mechanisms between these two disorders will engender novel hypotheses in future preclinical and clinical studies.Peer reviewe

    No Reliable Association between Runs of Homozygosity and Schizophrenia in a Well-Powered Replication Study

    It is well known that inbreeding increases the risk of recessive monogenic diseases, but it is less certain whether it contributes to the etiology of complex diseases such as schizophrenia. One way to estimate the effects of inbreeding is to examine the association between disease diagnosis and genome-wide autozygosity estimated using runs of homozygosity (ROH) in genome-wide single nucleotide polymorphism arrays. Using data for schizophrenia from the Psychiatric Genomics Consortium (n = 21,868), Keller et al. (2012) estimated that the odds of developing schizophrenia increased by approximately 17% for every additional percent of the genome that is autozygous (β = 16.1, CI(β) = [6.93, 25.7], Z = 3.44, p = 0.0006). Here we describe replication results from 22 independent schizophrenia case-control datasets from the Psychiatric Genomics Consortium (n = 39,830). Using the same ROH calling thresholds and procedures as Keller et al. (2012), we were unable to replicate the significant association between ROH burden and schizophrenia in the independent PGC phase II data, although the effect was in the predicted direction, and the combined (original + replication) dataset yielded an attenuated but significant relationship between Froh and schizophrenia (β = 4.86,CI(β) = [0.90,8.83],Z = 2.40,p = 0.02). Since Keller et al. (2012), several studies reported inconsistent association of ROH burden with complex traits, particularly in case-control data. These conflicting results might suggest that the effects of autozygosity are confounded by various factors, such as socioeconomic status, education, urbanicity, and religiosity, which may be associated with both real inbreeding and the outcome measures of interest

    Age at first birth in women is genetically associated with increased risk of schizophrenia

    Prof. Paunio on PGC:n jäsenPrevious studies have shown an increased risk for mental health problems in children born to both younger and older parents compared to children of average-aged parents. We previously used a novel design to reveal a latent mechanism of genetic association between schizophrenia and age at first birth in women (AFB). Here, we use independent data from the UK Biobank (N = 38,892) to replicate the finding of an association between predicted genetic risk of schizophrenia and AFB in women, and to estimate the genetic correlation between schizophrenia and AFB in women stratified into younger and older groups. We find evidence for an association between predicted genetic risk of schizophrenia and AFB in women (P-value = 1.12E-05), and we show genetic heterogeneity between younger and older AFB groups (P-value = 3.45E-03). The genetic correlation between schizophrenia and AFB in the younger AFB group is -0.16 (SE = 0.04) while that between schizophrenia and AFB in the older AFB group is 0.14 (SE = 0.08). Our results suggest that early, and perhaps also late, age at first birth in women is associated with increased genetic risk for schizophrenia in the UK Biobank sample. These findings contribute new insights into factors contributing to the complex bio-social risk architecture underpinning the association between parental age and offspring mental health.Peer reviewe
