    The prevalence and adequacy of antibiotic prescriptions in Hospital de Clínicas de Porto Alegre

    Durante o dia 30 de julho de 1987, foram estudados alguns aspectos envolvendo o uso de antibióticos no Hospital de Clínicas de Porto Alegre. De 432 pacientes internados, 182 (42%) estavam utilizando antibióticos,e destes, 42% recebiam a medicação de forma inadequada. Dos antibióticos usados terapeuticarhente, os mais prescritos foram ampicilina, penicilina, cefalosporinas e amicacina. Entre os usados profilaticamente, destacaram-se cefa/osporinas, gentamicina e ampicilina. O trabalho sugere que o controle do uso de antibióticos pode reduzir a adoção inapropriada destes agentes.On July 30th 1987 there was made a survey to evaluate the antibiotic use in the Hospital de Clínicas de Porto Alegre. 432 patients were investigated and 182 (42%) were under antimicrobial therapy. This group showed 42% o f patients receiving antimicrobians inappropriately. The antibiotics most used therapeuticaly were ampicillin, penicillin, cefalosporin and amikacin. For prophylaxis the most used were cefalosporin, gentamicin and ampicillin. This paper suggests that contra/ procedures can reduce the inappropriate use of antimicrobial agents in our hospital

    DIABETES MELLITUS GESTACIONAL: ALTERAÇÕES HISTOPATOLÓGICAS EM PLACENTAS HUMANAS E SUAS CONSEQUÊNCIAS PERINATAIS

    O diabetes gestacional (DMG) é uma condição metabólica diagnosticada no segundo ou terceiro trimestre da gravidez, afetando cerca de uma em cada seis gestações. A prevalência do DMG varia de 1% a 37,7%, com uma média global de aproximadamente 16,2%. No Brasil, cerca de 18% das gestações acompanhadas pelo Sistema Único de Saúde (SUS) são afetadas pelo DMG. O DMG mal controlado durante a gestação pode resultar em aborto espontâneo, anomalias congênitas, pré-eclâmpsia, natimortalidade, macrossomia fetal, hipoglicemia e níveis elevados de bilirrubina neonatal. Além disso, pode causar poliúria fetal, levando ao polidrâmnio e aumentando o risco de ruptura prematura das membranas placentárias e parto prematuro. As alterações histopatológicas na placenta de mulheres com DMG incluem edema, imaturidade e fibrose de vilosidades, aumento da fibrina peri vilositária, aumento de nós sinciciais e espessamento da decídua. Lesões vasculares podem ser observadas nos troncos das vilosidades e capilares, com hipervascularização das vilosidades. As manifestações clínicas incluem síndrome do desconforto respiratório agudo, hipoglicemia, icterícia neonatal, hipocalcemia, hiperbilirrubinemia, doença da membrana hialina e malformações congênitas. A macrossomia fetal frequentemente leva a partos por cesariana, aumentando o risco de complicações puerperais, como hemorragias e infecções. Portanto, o rastreio e controle adequado do DMG são essenciais para prevenir complicações tanto para a mãe quanto para o feto, destacando a importância da atenção pré-natal rigorosa em gestantes com fatores de risco. A conscientização sobre o DMG é fundamental para melhorar os resultados da gravidez e a saúde de mães e bebês

    O uso do paracetamol na gestação associado ao risco de desenvolvimento do espectro autista em crianças: uma revisão de literatura

    Popularmente conhecido como autismo, Transtorno do Espectro Autista (TEA), é uma síndrome que tem diversas origens que afetam o comportamento e tem influência direta na evolução da criança, afetando o seu desenvolvimento neurológico. Por décadas, o TEA tinha como principal diagnóstico um tipo de ‘’esquizofrenia infantil’’, porém como o avanço da medicina, começou a diferir este transtorno entre distúrbios metais. Utilizam-se também os termos ASDs ou DEA para referenciar as Desordens do Espectro Autista, onde estes são empregados para referir 3 de 5 transtornos do desenvolvimento que são invasivos. Nos últimos anos, tem sido notado um aumento considerável de nascimentos de crianças com TEA ao redor de todo o mundo. O paracetamol (acetaminofeno), como é conhecido popularmente, são responsáveis por inibir a síntese de prostaglandinas por meio do ácido araquidônico, inibindo a cicloxigenase (COX), gerando ação anestésica e antipirética, por ocorrerem a nível do sistema nervoso central, este medicamento ao ser ingerido por gestantes, ultrapassa a barreira placentária, é muito utilizado no período gestacional para o combate de febre, desta forma, tornou-se o objeto importante de estudo, quando relacionado à pesquisa de TEA. O objetivo do trabalho é apresentar uma possível hipótese do uso do paracetamol durante a gestação, onde este uso pode interferir no desenvolvimento neurológico da criança, gerando o Transtorno do Espectro Autista, em crianças com predisposições genéticas e imunológicas

    Long-term outcomes of the global tuberculosis and COVID-19 co-infection cohort

    Background: Longitudinal cohort data of patients with tuberculosis (TB) and coronavirus disease 2019 (COVID-19) are lacking. In our global study, we describe long-term outcomes of patients affected by TB and COVID-19. Methods: We collected data from 174 centres in 31 countries on all patients affected by COVID-19 and TB between 1 March 2020 and 30 September 2022. Patients were followed-up until cure, death or end of cohort time. All patients had TB and COVID-19; for analysis purposes, deaths were attributed to TB, COVID-19 or both. Survival analysis was performed using Cox proportional risk-regression models, and the log-rank test was used to compare survival and mortality attributed to TB, COVID-19 or both. Results: Overall, 788 patients with COVID-19 and TB (active or sequelae) were recruited from 31 countries, and 10.8% (n=85) died during the observation period. Survival was significantly lower among patients whose death was attributed to TB and COVID-19 versus those dying because of either TB or COVID-19 alone (p<0.001). Significant adjusted risk factors for TB mortality were higher age (hazard ratio (HR) 1.05, 95% CI 1.03-1.07), HIV infection (HR 2.29, 95% CI 1.02-5.16) and invasive ventilation (HR 4.28, 95% CI 2.34-7.83). For COVID-19 mortality, the adjusted risks were higher age (HR 1.03, 95% CI 1.02-1.04), male sex (HR 2.21, 95% CI 1.24-3.91), oxygen requirement (HR 7.93, 95% CI 3.44-18.26) and invasive ventilation (HR 2.19, 95% CI 1.36-3.53). Conclusions: In our global cohort, death was the outcome in >10% of patients with TB and COVID-19. A range of demographic and clinical predictors are associated with adverse outcomes

    The European Reference Genome Atlas: piloting a decentralised approach to equitable biodiversity genomics

    A genomic database of all Earth’s eukaryotic species could contribute to many scientific discoveries; however, only a tiny fraction of species have genomic information available. In 2018, scientists across the world united under the Earth BioGenome Project (EBP), aiming to produce a database of high-quality reference genomes containing all ~1.5 million recognized eukaryotic species. As the European node of the EBP, the European Reference Genome Atlas (ERGA) sought to implement a new decentralised, equitable and inclusive model for producing reference genomes. For this, ERGA launched a Pilot Project establishing the first distributed reference genome production infrastructure and testing it on 98 eukaryotic species from 33 European countries. Here we outline the infrastructure and explore its effectiveness for scaling high-quality reference genome production, whilst considering equity and inclusion. The outcomes and lessons learned provide a solid foundation for ERGA while offering key learnings to other transnational, national genomic resource projects and the EBP.info:eu-repo/semantics/publishedVersio

    Consistent patterns of common species across tropical tree communities

    Trees structure the Earth’s most biodiverse ecosystem, tropical forests. The vast number of tree species presents a formidable challenge to understanding these forests, including their response to environmental change, as very little is known about most tropical tree species. A focus on the common species may circumvent this challenge. Here we investigate abundance patterns of common tree species using inventory data on 1,003,805 trees with trunk diameters of at least 10 cm across 1,568 locations1,2,3,4,5,6 in closed-canopy, structurally intact old-growth tropical forests in Africa, Amazonia and Southeast Asia. We estimate that 2.2%, 2.2% and 2.3% of species comprise 50% of the tropical trees in these regions, respectively. Extrapolating across all closed-canopy tropical forests, we estimate that just 1,053 species comprise half of Earth’s 800 billion tropical trees with trunk diameters of at least 10 cm. Despite differing biogeographic, climatic and anthropogenic histories7, we find notably consistent patterns of common species and species abundance distributions across the continents. This suggests that fundamental mechanisms of tree community assembly may apply to all tropical forests. Resampling analyses show that the most common species are likely to belong to a manageable list of known species, enabling targeted efforts to understand their ecology. Although they do not detract from the importance of rare species, our results open new opportunities to understand the world’s most diverse forests, including modelling their response to environmental change, by focusing on the common species that constitute the majority of their trees.Publisher PDFPeer reviewe

    Atrasentan and renal events in patients with type 2 diabetes and chronic kidney disease (SONAR): a double-blind, randomised, placebo-controlled trial

    Background: Short-term treatment for people with type 2 diabetes using a low dose of the selective endothelin A receptor antagonist atrasentan reduces albuminuria without causing significant sodium retention. We report the long-term effects of treatment with atrasentan on major renal outcomes. Methods: We did this double-blind, randomised, placebo-controlled trial at 689 sites in 41 countries. We enrolled adults aged 18–85 years with type 2 diabetes, estimated glomerular filtration rate (eGFR)25–75 mL/min per 1·73 m 2 of body surface area, and a urine albumin-to-creatinine ratio (UACR)of 300–5000 mg/g who had received maximum labelled or tolerated renin–angiotensin system inhibition for at least 4 weeks. Participants were given atrasentan 0·75 mg orally daily during an enrichment period before random group assignment. Those with a UACR decrease of at least 30% with no substantial fluid retention during the enrichment period (responders)were included in the double-blind treatment period. Responders were randomly assigned to receive either atrasentan 0·75 mg orally daily or placebo. All patients and investigators were masked to treatment assignment. The primary endpoint was a composite of doubling of serum creatinine (sustained for ≥30 days)or end-stage kidney disease (eGFR <15 mL/min per 1·73 m 2 sustained for ≥90 days, chronic dialysis for ≥90 days, kidney transplantation, or death from kidney failure)in the intention-to-treat population of all responders. Safety was assessed in all patients who received at least one dose of their assigned study treatment. The study is registered with ClinicalTrials.gov, number NCT01858532. Findings: Between May 17, 2013, and July 13, 2017, 11 087 patients were screened; 5117 entered the enrichment period, and 4711 completed the enrichment period. Of these, 2648 patients were responders and were randomly assigned to the atrasentan group (n=1325)or placebo group (n=1323). Median follow-up was 2·2 years (IQR 1·4–2·9). 79 (6·0%)of 1325 patients in the atrasentan group and 105 (7·9%)of 1323 in the placebo group had a primary composite renal endpoint event (hazard ratio [HR]0·65 [95% CI 0·49–0·88]; p=0·0047). Fluid retention and anaemia adverse events, which have been previously attributed to endothelin receptor antagonists, were more frequent in the atrasentan group than in the placebo group. Hospital admission for heart failure occurred in 47 (3·5%)of 1325 patients in the atrasentan group and 34 (2·6%)of 1323 patients in the placebo group (HR 1·33 [95% CI 0·85–2·07]; p=0·208). 58 (4·4%)patients in the atrasentan group and 52 (3·9%)in the placebo group died (HR 1·09 [95% CI 0·75–1·59]; p=0·65). Interpretation: Atrasentan reduced the risk of renal events in patients with diabetes and chronic kidney disease who were selected to optimise efficacy and safety. These data support a potential role for selective endothelin receptor antagonists in protecting renal function in patients with type 2 diabetes at high risk of developing end-stage kidney disease.     Safety and efficacy of the ChAdOx1 nCoV-19 vaccine (AZD1222) against SARS-CoV-2: an interim analysis of four randomised controlled trials in Brazil, South Africa, and the UK.

    BACKGROUND: A safe and efficacious vaccine against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), if deployed with high coverage, could contribute to the control of the COVID-19 pandemic. We evaluated the safety and efficacy of the ChAdOx1 nCoV-19 vaccine in a pooled interim analysis of four trials. METHODS: This analysis includes data from four ongoing blinded, randomised, controlled trials done across the UK, Brazil, and South Africa. Participants aged 18 years and older were randomly assigned (1:1) to ChAdOx1 nCoV-19 vaccine or control (meningococcal group A, C, W, and Y conjugate vaccine or saline). Participants in the ChAdOx1 nCoV-19 group received two doses containing 5 × 1010 viral particles (standard dose; SD/SD cohort); a subset in the UK trial received a half dose as their first dose (low dose) and a standard dose as their second dose (LD/SD cohort). The primary efficacy analysis included symptomatic COVID-19 in seronegative participants with a nucleic acid amplification test-positive swab more than 14 days after a second dose of vaccine. Participants were analysed according to treatment received, with data cutoff on Nov 4, 2020. Vaccine efficacy was calculated as 1 - relative risk derived from a robust Poisson regression model adjusted for age. Studies are registered at ISRCTN89951424 and ClinicalTrials.gov, NCT04324606, NCT04400838, and NCT04444674. FINDINGS: Between April 23 and Nov 4, 2020, 23 848 participants were enrolled and 11 636 participants (7548 in the UK, 4088 in Brazil) were included in the interim primary efficacy analysis. In participants who received two standard doses, vaccine efficacy was 62·1% (95% CI 41·0-75·7; 27 [0·6%] of 4440 in the ChAdOx1 nCoV-19 group vs71 [1·6%] of 4455 in the control group) and in participants who received a low dose followed by a standard dose, efficacy was 90·0% (67·4-97·0; three [0·2%] of 1367 vs 30 [2·2%] of 1374; pinteraction=0·010). Overall vaccine efficacy across both groups was 70·4% (95·8% CI 54·8-80·6; 30 [0·5%] of 5807 vs 101 [1·7%] of 5829). From 21 days after the first dose, there were ten cases hospitalised for COVID-19, all in the control arm; two were classified as severe COVID-19, including one death. There were 74 341 person-months of safety follow-up (median 3·4 months, IQR 1·3-4·8): 175 severe adverse events occurred in 168 participants, 84 events in the ChAdOx1 nCoV-19 group and 91 in the control group. Three events were classified as possibly related to a vaccine: one in the ChAdOx1 nCoV-19 group, one in the control group, and one in a participant who remains masked to group allocation. INTERPRETATION: ChAdOx1 nCoV-19 has an acceptable safety profile and has been found to be efficacious against symptomatic COVID-19 in this interim analysis of ongoing clinical trials.
