    The LifeCycle Project-EU Child Cohort Network : a federated analysis infrastructure and harmonized data of more than 250,000 children and parents

    Early life is an important window of opportunity to improve health across the full lifecycle. An accumulating body of evidence suggests that exposure to adverse stressors during early life leads to developmental adaptations, which subsequently affect disease risk in later life. Also, geographical, socio-economic, and ethnic differences are related to health inequalities from early life onwards. To address these important public health challenges, many European pregnancy and childhood cohorts have been established over the last 30 years. The enormous wealth of data of these cohorts has led to important new biological insights and important impact for health from early life onwards. The impact of these cohorts and their data could be further increased by combining data from different cohorts. Combining data will lead to the possibility of identifying smaller effect estimates, and the opportunity to better identify risk groups and risk factors leading to disease across the lifecycle across countries. Also, it enables research on better causal understanding and modelling of life course health trajectories. The EU Child Cohort Network, established by the Horizon2020-funded LifeCycle Project, brings together nineteen pregnancy and childhood cohorts, together including more than 250,000 children and their parents. A large set of variables has been harmonised and standardized across these cohorts. The harmonized data are kept within each institution and can be accessed by external researchers through a shared federated data analysis platform using the R-based platform DataSHIELD, which takes relevant national and international data regulations into account. The EU Child Cohort Network has an open character. All protocols for data harmonization and setting up the data analysis platform are available online. The EU Child Cohort Network creates great opportunities for researchers to use data from different cohorts, during and beyond the LifeCycle Project duration. It also provides a novel model for collaborative research in large research infrastructures with individual-level data. The LifeCycle Project will translate results from research using the EU Child Cohort Network into recommendations for targeted prevention strategies to improve health trajectories for current and future generations by optimizing their earliest phases of life.Peer reviewe

    COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≀ 18 years: 69, 48, 23; 85%), older adults (≄ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

    Etude de la molécule CD1e et de son trafic intracellulaire

    Au sein de la famille des molĂ©cules prĂ©sentatrices d'antigĂšnes CD1, la molĂ©cule CD1e est trĂšs particuliĂšre. Le CD1e est en effet localisĂ© exclusivement de façon intracellulaire et existe, sous une forme associĂ©e Ă  la membrane, dans le rĂ©ticulum endoplasmique et les compartiments golgiens et sous une forme soluble, dans les endosomes. Nous avons Ă©tudiĂ© d'une part la localisation du CD1e dans les cellules dendritiques (CDs) immatures et en cours de maturation. Dans les CDs immatures, le CD1e est principalement localisĂ© dans le Golgi mais est aussi dĂ©tectĂ© dans les compartiments tardifs multilamellaires. Dans les CDs matures, les molĂ©cules CD1e sont relocalisĂ©es dans les compartiments tardifs matures oĂč se trouvent Ă©galement le CD1b. D'autre part, un modĂšle de cellules transfectĂ©es HLA-DR+ a Ă©tĂ© utilisĂ© pour comprendre quelles parties du CD1e sont impliquĂ©es dans le contrĂŽle de son trafic et pour connaĂźtre les mĂ©canismes cellulaires qui s'y rapportent. Nous avons montrĂ© que le domaine cytoplasmique du CD1e joue un rĂŽle important dans la rĂ©tention intracellulaire du CD1e au niveau des compartiments golgiens et dans l'adressage du CD1e vers les compartiments tardifs. Sa substitution par celui du CD1b empĂȘche la gĂ©nĂ©ration d'une forme soluble. Ces rĂ©sultats suggĂšrent que le trafic du CD1e est destinĂ© Ă  l'obtention d'une forme soluble et que le CD1b est exclu de ce trajet. De plus, en absence des complexes AP-1 et AP-3, le CD1e gagne toujours les endosomes tardifs mais la dĂ©ficience en AP-1 empĂȘche son clivage. Enfin, nous avons montrĂ© que le CD1e possĂšde un propeptide, clivĂ© dans les endosomes. Ces rĂ©sultats suggĂšrent que le CD1e acquiert ses fonctions biologiques dans ces compartiments. Ces travaux pourront ĂȘtre poursuivis sur le plan fonctionnel au moyen de tests de prĂ©sentation antigĂšnique, dans le cadre d'un travail collaboratif, dont les premiers rĂ©sultats mettent en Ă©vidence un rĂŽle du CD1e pour la prĂ©sentation de certains glycolipides par le CD1b.CD1e belongs to the family of antigenic presenting molecules CD1 but CD1e is very particular. Indeed, CD1e remains surprisingly intracellular, accumulating mainly in the Golgi compartments of immature dendritic cells (DCs) into a complete transmembrane associated form and in the late endosomes of mature DCs. In the latter compartments, CD1e is cleaved and becomes soluble due to a cleavage between the luminal and transmembrane domains. We studied the distribution of CD1e in immature and maturing DCs. In immature DCs, CD1e accumulates in Golgi compartments, is not expressed on the plasma membrane but reaches CD1b+ late multilamellar compartments. During maturation, CD1e molecules accumulate in mature endosomes CD1b+. Its cytoplasmic domain does not contain known targeting motifs. CD1e reaches late endosomes in AP-1 and AP-3 deficient cell lines. Interestingly, the cleavage of CD1e occurs in AP-3-, but not in AP-1- cells. Deletion and substitution mutants were engineered and expressed in an HLA-DR+ melanoma cell line. Experiments showed that different parts of the cytoplasmic tail control the localization of CD1e in the golgi compartments and its absence on the cell surface, facilitating the routing of CD1e to late endosomes where it is cleaved. The replacement of the cytoplasmic domain of CD1e by that of CD1b blocked its cleavage. Thus, CD1e reaches late endosomes using a cellular pathway not shared by CD1b. Finally, we show that CD1e contains a propeptide which is cleaved in endosomes. Our data strongly suggest that the function of the intracellular transport of CD1e is the generation of soluble endosomal molecules, which might play a role in the presentation of antigens by other CD1 molecules transiting in CD1e+ endosomes.STRASBOURG-Sc. et Techniques (674822102) / SudocSudocFranceF

    Treatment of congenital thrombocytopenia and decreased collagen reactivity in G6b-B-deficient mice

    Mice lacking the immunoreceptor tyrosine-based inhibition motif-containing co-inhibitory receptor G6b-B (Mpig6b, G6b knockout, KO) are born with a complex megakaryocyte (MK) per platelet phenotype, characterized by severe macrothrombocytopenia, expansion of the MK population, and focal myelofibrosis in the bone marrow and spleen. Platelets are almost completely devoid of the glycoprotein VI (GPVI)-FcRÎł-chain collagen receptor complex, have reduced collagen integrin α2ÎČ1, elevated Syk tyrosine kinase activity, and a subset has increased surface immunoglobulins. A similar phenotype was recently reported in patients with null and loss-of-function mutations in MPIG6B. To better understand the cause and treatment of this pathology, we used pharmacological- and genetic-based approaches to rescue platelet counts and function in G6b KO mice. Intravenous immunoglobulin resulted in a transient partial recovery of platelet counts, whereas immune deficiency did not affect platelet counts or receptor expression in G6b KO mice. Syk loss-of-function (R41A) rescued macrothrombocytopenia, GPVI and α2ÎČ1 expression in G6b KO mice, whereas treatment with the Syk kinase inhibitor BI1002494 partially rescued platelet count but had no effect on GPVI and α2ÎČ1 expression or bleeding. The Src family kinase inhibitor dasatinib was not beneficial in G6b KO mice. In contrast, treatment with the thrombopoietin mimetic romiplostim rescued thrombocytopenia, GPVI expression, and platelet reactivity to collagen, suggesting that it may be a promising therapeutic option for patients lacking functional G6b-B. Intriguingly, GPVI and α2ÎČ1 expression were significantly downregulated in romiplostim-treated wild-type mice, whereas GPVI was upregulated in romiplostim-treated G6b KO mice, suggesting a cell intrinsic feedback mechanism that autoregulates platelet reactivity depending on physiological needs

    Should 45,X/46,XY boys with no or mild anomaly of external genitalia be investigated and followed up?

    International audienceOBJECTIVE: Few studies of patients with a 45,X/46,XY mosaicism have considered those with normal male phenotype. The purpose of this study was to evaluate the clinical outcome of 45,X/46,XY boys born with normal or minor abnormalities of external genitalia, notably in terms of growth and pubertal development.METHODS: Retrospective longitudinal study of 40 patients followed between 1982 and 2017 in France.RESULTS: Twenty patients had a prenatal diagnosis, whereas 20 patients had a postnatal diagnosis, mainly for short stature. Most patients had stunted growth, with abnormal growth spurt during puberty and a mean adult height of 158 ± 7.6 cm, i.e. -2.3 DS with correction for target height. Seventy percent of patients presented Turner-like syndrome features including cardiac (6/23 patients investigated) and renal malformations (3/19 patients investigated). Twenty-two patients had minor abnormalities of external genitalia. One patient developed a testicular embryonic carcinoma, suggesting evidence of partial gonadal dysgenesis. Moreover, puberty occurred spontaneously in 93% of patients but 71% ( = 5) of those evaluated at the end of puberty presented signs of declined Sertoli cell function (low inhibin B levels and increased FSH levels).CONCLUSION: This study emphasizes the need to identify and follow-up 45,X/46,XY patients born with normal male phenotype until adulthood, as they present similar prognosis than those born with severe genital anomalies. Currently, most patients are diagnosed in adulthood with azoospermia, consistent with our observations of decreased testicular function at the end of puberty. Early management of these patients may lead to fertility preservation strategies.</p

    Rapid Improvement after Starting Elexacaftor–Tezacaftor–Ivacaftor in Patients with Cystic Fibrosis and Advanced Pulmonary Disease

    International audienceRationale: Elexacaftor-tezacaftor-ivacaftor is a CFTR (cystic fibrosis [CF] transmembrane conductance regulator) modulator combination, developed for patients with CF with at least one Phe508del mutation. Objectives: To evaluate the effects of elexacaftor-tezacaftor- ivacaftor in patients with CF and advanced respiratory disease. Methods: A prospective observational study, including all patients aged â©Ÿ12 years and with a percent-predicted FEV1 (ppFEV1) <40 who initiated elexacaftor-tezacaftor-ivacaftor from December 2019 to August 2020 in France was conducted. Clinical characteristics were collected at initiation and at 1 and 3 months. Safety and effectiveness were evaluated by September 2020. National-level transplantation and mortality figures for 2020 were obtained from the French CF and transplant centers and registries. Measurements and Main Results: Elexacaftor-tezacaftor- ivacaftor was initiated in 245 patients with a median (interquartile range) ppFEV1 = 29 (24-34). The mean (95% confidence interval) absolute increase in the ppFEV1 was +15.1 (+13.8 to +16.4; P < 0.0001), and the mean (95% confidence interval) in weight was +4.2 kg (+3.9 to +4.6; P < 0.0001). The number of patients requiring long-term oxygen, noninvasive ventilation, and/or enteral tube feeding decreased by 50%, 30%, and 50%, respectively (P < 0.01). Although 16 patients were on the transplant waiting list and 37 were undergoing transplantation evaluation at treatment initiation, only 2 received a transplant, and 1 died. By September 2020, only five patients were still on the transplantation path. Compared with the previous 2 years, a twofold decrease in the number of lung transplantations in patients with CF was observed in 2020, whereas the number of deaths without transplantation remained stable. Conclusions: In patients with advanced disease, elexacaftor-tezacaftor-ivacaftor is associated with rapid clinical improvement, often leading to the indication for lung transplantation being suspended

    The association between macrovascular complications and intensive care admission, invasive mechanical ventilation, and mortality in people with diabetes hospitalized for coronavirus disease-2019 (COVID-19)

    International audienceAbstract Background It is not clear whether pre-existing macrovascular complications (ischemic heart disease, stroke or peripheral artery disease) are associated with health outcomes in people with diabetes mellitus hospitalized for COVID-19. Methods We conducted cohort studies of adults with pre-existing diabetes hospitalized for COVID-19 infection in the UK, France, and Spain during the early phase of the pandemic (between March 2020—October 2020). Logistic regression models adjusted for demographic factors and other comorbidities were used to determine associations between previous macrovascular disease and relevant clinical outcomes: mortality, intensive care unit (ICU) admission and use of invasive mechanical ventilation (IMV) during the hospitalization. Output from individual logistic regression models for each cohort was combined in a meta-analysis. Results Complete data were available for 4,106 (60.4%) individuals. Of these, 1,652 (40.2%) had any prior macrovascular disease of whom 28.5% of patients died. Mortality was higher for people with compared to those without previous macrovascular disease (37.7% vs 22.4%). The combined crude odds ratio (OR) for previous macrovascular disease and mortality for all four cohorts was 2.12 (95% CI 1.83–2.45 with an I 2 of 60%, reduced after adjustments for age, sex, type of diabetes, hypertension, microvascular disease, ethnicity, and BMI to adjusted OR 1.53 [95% CI 1.29–1.81]) for the three cohorts. Further analysis revealed that ischemic heart disease and cerebrovascular disease were the main contributors of adverse outcomes. However, proportions of people admitted to ICU (adjOR 0.48 [95% CI 0.31–0.75], I 2 60%) and the use of IMV during hospitalization (adjOR 0.52 [95% CI 0.40–0.68], I 2 37%) were significantly lower for people with previous macrovascular disease. Conclusions This large multinational study of people with diabetes mellitus hospitalized for COVID-19 demonstrates that previous macrovascular disease is associated with higher mortality and lower proportions admitted to ICU and treated with IMV during hospitalization suggesting selective admission criteria. Our findings highlight the importance correctly assess the prognosis and intensive monitoring in this high-risk group of patients and emphasize the need to design specific public health programs aimed to prevent SARS-CoV-2 infection in this subgroup
