    RAMSTRONG: AN EMPLOYEE WELLNESS INITIATIVE

    The RAMSTRONG project’s mission is to create a mobile website accessible online and through the VCU Mobile app that provides VCU employees with user-friendly, accessible resources to support their holistic well-being. The RAMSTRONG project seeks to meet three basic needs. First, while VCU and the Greater Richmond area offer a plethora of resources to promote health, information about these resources is not readily accessible, and especially not accessible from one website or mobile app. RAMSTRONG aims to provide an accessible means for employees to learn about and take advantage of these resources. Second, while a growing body of scientific literature indicates employer sponsored health promotion programs increase job satisfaction, productivity, and retention, these programs are only effective if they are utilized. RAMSTRONG aims to increase their utilization by promoting awareness of their availability. Third, our society invests a substantial sum of resources to the care of those suffering from injury and illness and less to promoting our health and well-being. The RAMSTRONG project is motivated by a vision of a society that invests significantly in the promotion of wellness so as to reduce the incidence of injury and illness and to increase the prevalence of personal and social satisfaction at work and in life. Our model for the RAMSTRONG app draws from the public health concept of the Wheel of Wellness, which specifies eight interrelated and interdependent dimensions of health: emotional, environmental, financial, social, spiritual, occupational, physical, and intellectual. When a person can demonstrate strength and well-being in each of these areas, they are more productive and receive greater satisfaction in life. Universities, including Princeton University, that have implemented similar website resources and the National Wellness Institute define wellness as “an active process through which people become aware of, and make choices toward, a more successful existence”. Our RAMSTRONG website and mobile app will provide employees with an efficient, friendly means for becoming aware of campus and community resources and making choices that actively contribute to individual and community well-being in each of the eight dimensions. It is our hope that with the implementation of this project, VCU employees will have the resources to take charge of their wellness in each dimension and become RAMSTRONG

    A two-dimensional Kolmogorov-Smirnov test for crowded field source detection: ROSAT sources in NGC 6397

    We present a two-dimensional version of the classical one-dimensional Kolmogorov-Smirnov (K-S) test, extending an earlier idea due to Peacock (1983) and an implementation proposed by Fasano & Franceschini (1987). The two-dimensional K-S test is used to optimise the goodness of fit in an iterative source-detection scheme for astronomical images. The method is applied to a ROSAT/HRI x-ray image of the post core-collapse globular cluster NGC 6397 to determine the most probable source distribution in the cluster core. Comparisons to other widely-used source detection methods, and to a Chandra image of the same field, show that our iteration scheme is superior in measuring statistics-limited sources in severely crowded fields.Comment: 12 pages, 6 figures, 6 tables.     Individual differences in self-affirmation: distinguishing self-affirmation from positive self-regard

    Research into self-affirmation has almost exclusively employed experimental manipulations. In this paper we address individual differences in the tendency to respond to threats with self-affirming cognitions and distinguish this from two overlapping constructs: habitual positive self-thought and trait self-esteem. Items we designed to measure self-affirmation were represented by three first-order factors and loaded on a higher-order factor, creating the Spontaneous Self-Affirmation Measure (SSAM). The SSAM correlated moderately with self-esteem and habitual positive self-thought. In competitive analyses, the SSAM was an independent predictor of a large number of outcomes. The studies provide evidence about the correlates of individual differences in reported spontaneous self-affirmation in response to threat and the contribution made to this response by habitual positive self-thought and trait self-esteem

    White dwarfs with hydrogen-deficient atmospheres and the dark matter content of the Galaxy

    The nature of the several microlensing events observed by the MACHO team towards the Large Magellanic Cloud (LMC) is still a subject of debate. Low-mass substellar objects and stars with masses larger than ~M_{sun} have been ruled out as major components of a Massive Astrophysical Compact Halo Object (MACHO) Galactic halo, while stars of half a solar mass seem to be viable candidates. Main sequence stars have been already discarded, and there are tight restrictions on the role played by white dwarfs with hydrogen-dominated atmospheres. In this paper we evaluate the contribution to the dark matter content of the Galaxy of white dwarfs with hydrogen-deficient atmospheres. For this purpose we use a Monte Carlo simulator which incorporates up-to-date evolutionary sequences of white dwarfs with hydrogen-rich and hydrogen-deficient atmospheres. We also take into account detailed descriptions of the thick disk and the halo of our Galaxy as well as of a reliable model of the LMC. We find that the contribution of white dwarfs with hydrogen-deficient atmospheres moderately increases the theoretical estimate of the optical depth with respect to the value obtained when only hydrogen-rich white dwarfs are considered. We also find that the contribuiton of the thick disk population of white dwarfs is comparable to the halo contribution. However, the contributions of both the halo and the thick disk white-dwarf populations are still insufficient to explain the number of events observed by the MACHO team. Finally, we find that the contribution to the halo dark matter of the entire population under study is less than 10% at the 95% conficence level.Comment: 12 pages, 5 figures, accepted for publication in A&

    Group cognitive rehabilitation to reduce the psychological impact of multiple sclerosis on quality of life: the CRAMMS RCT

    AbstractBackgroundPeople with multiple sclerosis have problems with memory and attention. The effectiveness of cognitive rehabilitation has not been established.ObjectivesThe objectives were to assess the clinical effectiveness and cost-effectiveness of a cognitive rehabilitation programme for people with multiple sclerosis.DesignThis was a multicentre, randomised controlled trial in which participants were randomised in a ratio of 6 : 5 to receive cognitive rehabilitation plus usual care or usual care alone. Participants were assessed at 6 and 12 months after randomisation.SettingThe trial was set in hospital neurology clinics and community services.ParticipantsParticipants were people with multiple sclerosis who had cognitive problems, were aged 18–69 years, could travel to attend group sessions and gave informed consent.InterventionThe intervention was a group cognitive rehabilitation programme delivered weekly by an assistant psychologist to between four and six participants for 10 weeks.Main outcome measuresThe primary outcome was the Multiple Sclerosis Impact Scale – Psychological subscale at 12 months. Secondary outcomes included results from the Everyday Memory Questionnaire, the 30-Item General Health Questionnaire, the EuroQol-5 Dimensions, five-level version and a service use questionnaire from participants, and the Everyday Memory Questionnaire – relative version and the Modified Carer Strain Index from a relative or friend of the participant.ResultsOf the 449 participants randomised, 245 were allocated to cognitive rehabilitation (intervention group) and 204 were allocated to usual care (control group). Of these, 214 in the intervention group and 173 in the control group were included in the primary analysis. There was no clinically important difference in the Multiple Sclerosis Impact Scale – Psychological subscale score between the two groups at the 12-month follow-up (adjusted difference in means –0.6, 95% confidence interval –1.5 to 0.3; p = 0.20). There were no important differences between the groups in relation to cognitive abilities, fatigue, employment, or carer strain at follow-up. However, there were differences, although small, between the groups in the Multiple Sclerosis Impact Scale – Psychological subscale score at 6 months (adjusted difference in means –0.9, 95% confidence interval –1.7 to –0.1; p = 0.03) and in everyday memory on the Everyday Memory Questionnaire as reported by participants at 6 (adjusted difference in means –5.3, 95% confidence interval –8.7 to –1.9) and 12 months (adjusted difference in means –4.4, 95% confidence interval –7.8 to –0.9) and by relatives at 6 (adjusted difference in means –5.4, 95% confidence interval –9.1 to –1.7) and 12 months (adjusted difference in means –5.5, 95% confidence interval –9.6 to –1.5) in favour of the cognitive rehabilitation group. There were also differences in mood on the 30-Item General Health Questionnaire at 6 (adjusted difference in means –3.4, 95% confidence interval –5.9 to –0.8) and 12 months (adjusted difference in means –3.4, 95% confidence interval –6.2 to –0.6) in favour of the cognitive rehabilitation group. A qualitative analysis indicated perceived benefits of the intervention. There was no evidence of a difference in costs (adjusted difference in means –£574.93, 95% confidence interval –£1878.93 to £729.07) or quality-adjusted life-year gain (adjusted difference in means 0.00, 95% confidence interval –0.02 to 0.02). No safety concerns were raised and no deaths were reported

    The Search for High-Mass X-ray Binaries in the Phoenix Dwarf Galaxy

    We report on the first X-ray images of the Phoenix dwarf galaxy, taken with \emph{XMM-Newton} in July 2009. This local group dwarf galaxy shares similarities with the Small Magellanic Cloud (SMC) including a burst of star formation \sim50 Myr ago. The SMC has an abundance of High Mass X-ray Binaries (HMXBs) and so we have investigated the possibility of an HMXB population in Phoenix with the intention of furthering the understanding of the HMXB-star formation rate relation. The data from the combined European Photon Imaging Cameras (EPIC) were used to distinguish between different source classes (foreground stars, background galaxies, AGN and supernova remnants) using EPIC hardness ratios and correlations with optical and radio catalogues. Of the 81 X-ray sources in the field of view, six are foreground stars, four are galaxies and one is an AGN. The remaining sources with optical counterparts have log(fXfopt\frac{f_X}{f_{opt}}) consistent with AGN in the local universe. Further investigation of five sources in the field of view suggests they are all background AGN. Their position behind the gas cloud associated with Phoenix makes them a possible tool for further probing the metallicity of this region. We find no evidence for any HMXBs in Phoenix at this time. This rules out the existence of the X-ray persistent supergiant X-ray binary systems. However the transient nature of the Be/X-ray binaries means we cannot rule out a population of these sources but can conclude that it is not extensive.Comment: 13 pages, 4 figures, 4 tables, Accepted for publication in MNRA

    The Solution Structures of Two Human IgG1 Antibodies Show Conformational Stability and Accommodate Their C1q and FcγR Ligands.

    The human IgG1 antibody subclass shows distinct properties compared with the IgG2, IgG3, and IgG4 subclasses and is the most exploited subclass in therapeutic antibodies. It is the most abundant subclass, has a half-life as long as that of IgG2 and IgG4, binds the FcγR receptor, and activates complement. There is limited structural information on full-length human IgG1 because of the challenges of crystallization. To rectify this, we have studied the solution structures of two human IgG1 6a and 19a monoclonal antibodies in different buffers at different temperatures. Analytical ultracentrifugation showed that both antibodies were predominantly monomeric, with sedimentation coefficients s20,w (0) of 6.3-6.4 S. Only a minor dimer peak was observed, and the amount was not dependent on buffer conditions. Solution scattering showed that the x-ray radius of gyration Rg increased with salt concentration, whereas the neutron Rg values remained unchanged with temperature. The x-ray and neutron distance distribution curves P(r) revealed two peaks, M1 and M2, whose positions were unchanged in different buffers to indicate conformational stability. Constrained atomistic scattering modeling revealed predominantly asymmetric solution structures for both antibodies with extended hinge structures. Both structures were similar to the only known crystal structure of full-length human IgG1. The Fab conformations in both structures were suitably positioned to permit the Fc region to bind readily to its FcγR and C1q ligands without steric clashes, unlike human IgG4. Our molecular models for human IgG1 explain its immune activities, and we discuss its stability and function for therapeutic applications

    The clinical and cost-effectiveness of rehabilitation of memory in brain injury: the ReMemBrIn RCT

    Background People with traumatic brain injuries (TBIs) commonly report memory impairments. These are persistent, debilitating, and reduce quality of life, but patients do not routinely receive memory rehabilitation after discharge from hospital. Objectives To assess the clinical and cost-effectiveness of a group memory rehabilitation programme for people with TBI. Design Multi-centre, pragmatic, cluster randomised controlled trial. Qualitative and health economic evaluations were also undertaken. Setting Community settings in nine sites in England. Participants Participants were aged 18-69 years, with TBI more than 3 months prior to recruitment, reported memory problems, who were able to travel to a site to attend group sessions, communicate in English, and gave informed consent. Randomisation and blinding Clusters of 4 to 6 participants were randomised to intervention or control on a 1:1 ratio. Randomisation was based on a computer generated pseudo-random code using random permuted blocks of randomly varying size, stratified by study site. Participants and therapists were aware of the treatment allocation; outcome assessors were blinded. Interventions Ten weekly sessions of a manualised memory rehabilitation programme were provided in addition to usual care. Participants were taught restitution strategies to retrain impaired memory functions and compensation strategies to enable them to cope with memory problems. The control arm received usual care only. Outcomes Outcomes were assessed at 6 and 12 months after randomisation. Primary: Patient-completed Everyday Memory Questionnaire (EMQ-p) at 6-month follow-up. Secondary: Rivermead Behavioural Memory Test-3, General Health Questionnaire-30, European Brain Injury Questionnaire, Everyday Memory Questionnaire-relative version, individual goal attainment. Costs (based on a UK NHS and PSS perspective) were collected using a service use questionnaire, with the EQ-5D 5L used to derive Quality Adjusted Life Years. A Markov model was developed to explore cost-effectiveness at 5 and 10 years, with 3.5% discount applied. Results We randomised 328 participants (intervention: n=171; control: n=157), with 129 in the intervention arm and 122 in the control arm included in the primary analysis. We found no clinically important difference on the EMQ-p between the two arms at 6-month follow-up (adjusted difference in means -2.1, 95% CI -6.7 to 2.5, p=0.37). For secondary outcomes, differences favouring the intervention arm were observed at 6-month follow-up for RBMT and goal attainment, but remained only for goal attainment at 12-month follow-up. There were no differences between arms in mood or quality of life. Qualitative results suggested positive experiences of participating in the trial and of attending the groups. Participants reported that memory rehabilitation was not routinely accessible in usual care. The primary health economics outcome at 12-months found memory rehabilitation to be £26.89 cheaper than usual care but less effective with an incremental QALY loss of 0.007. Differences in costs and effects were not statistically significant and non-parametric bootstrapping demonstrated considerable uncertainty in these findings. No safety concerns were raised and no deaths reported. Limitations As a pragmatic trial, we had broad inclusion criteria, therefore there was considerable heterogeneity within the sample. The study was not powered to perform further sub-group analyses. Participants and therapists could not be blinded to treatment allocation. Conclusions The group memory rehabilitation delivered in this trial is very unlikely to lead to clinical benefits or to be a cost-effective treatment for people with TBI in the community. Future work Future studies should examine the inclusion and selection of participants who may benefit most from memory rehabilitation.
