    The Use of Intraosseous Needles for Injection of Contrast Media for Computed Tomographic Angiography of the Thoracic Aorta

    Background The objective of this study is to evaluate the safety and quality of computed tomographic angiography of the thoracic aorta (CTA-TA) exams performed using intraosseous needle intravenous access (ION-IVA) for contrast media injection (CMI). Methods All CTA-TA exams at the study institution performed between 1/1/2013 and 8/14/2015 were reviewed retrospectively to identify those exams which had been performed using ION-IVA (ION-exams). ION-exams were then analyzed to determine aortic attenuation and contrast-to-noise ratio (CNR). Linear regression was used to determine how injection rate and other variables affected image quality for ION-exams. Patient electronic medical records were reviewed to identify any adverse events related to CTA-TA or ION-IVA. Results 17 (∟0.2%) of 7401 exams were ION-exams. ION-exam CMI rates varied between 2.5 and 4 ml/s. Mean attenuation was 312 HU (SD 88 HU) and mean CNR was 25 (SD 9.9). A strong positive linear association between attenuation and injection rate was found. No immediate or delayed complications related to the ION-exams, or intraosseous needle use in general, occurred. Conclusion For CTA-TA, ION-IVA appears to be a safe and effective route for CMI at rates up to 4 ml/s

    Multistable Solitons in the Cubic-Quintic Discrete Nonlinear Schr\"odinger Equation

    We analyze the existence and stability of localized solutions in the one-dimensional discrete nonlinear Schr\"{o}dinger (DNLS) equation with a combination of competing self-focusing cubic and defocusing quintic onsite nonlinearities. We produce a stability diagram for different families of soliton solutions, that suggests the (co)existence of infinitely many branches of stable localized solutions. Bifurcations which occur with the increase of the coupling constant are studied in a numerical form. A variational approximation is developed for accurate prediction of the most fundamental and next-order solitons together with their bifurcations. Salient properties of the model, which distinguish it from the well-known cubic DNLS equation, are the existence of two different types of symmetric solitons and stable asymmetric soliton solutions that are found in narrow regions of the parameter space. The asymmetric solutions appear from and disappear back into the symmetric ones via loops of forward and backward pitchfork bifurcations.Comment: To appear Physica D. 23 pages, 13 figure

    Ocean circulation and Tropical Variability in the Coupled Model ECHAM5/MPI-OM

    This paper describes the mean ocean circulation and the tropical variability simulated by the Max Planck Institute for Meteorology (MPI-M) coupled atmosphere–ocean general circulation model (AOGCM). Results are presented from a version of the coupled model that served as a prototype for the Intergovernmental Panel on Climate Change (IPCC) Fourth Assessment Report (AR4) simulations. The model does not require flux adjustment to maintain a stable climate. A control simulation with present-day greenhouse gases is analyzed, and the simulation of key oceanic features, such as sea surface temperatures (SSTs), large-scale circulation, meridional heat and freshwater transports, and sea ice are compared with observations. A parameterization that accounts for the effect of ocean currents on surface wind stress is implemented in the model. The largest impact of this parameterization is in the tropical Pacific, where the mean state is significantly improved: the strength of the trade winds and the associated equatorial upwelling weaken, and there is a reduction of the model’s equatorial cold SST bias by more than 1 K. Equatorial SST variability also becomes more realistic. The strength of the variability is reduced by about 30% in the eastern equatorial Pacific and the extension of SST variability into the warm pool is significantly reduced. The dominant El Niño–Southern Oscillation (ENSO) period shifts from 3 to 4 yr. Without the parameterization an unrealistically strong westward propagation of SST anomalies is simulated. The reasons for the changes in variability are linked to changes in both the mean state and to a reduction in atmospheric sensitivity to SST changes and oceanic sensitivity to wind anomalies

    Involvement of the Cav3.2 T-type calcium channel in thalamic neuron discharge patterns

    <p>Abstract</p> <p>Background</p> <p>Mice that have defects in their low-threshold T-type calcium channel (T-channel) genes show altered pain behaviors. The changes in the ratio of nociceptive neurons and the burst firing property of reticular thalamic (RT) and ventroposterior (VP) neurons in Cav3.2 knockout (KO) mice were studied to test the involvement of thalamic T-channel and burst firing activity in pain function.</p> <p>Results</p> <p>Under pentobarbital or urethane anesthesia, the patterns of tonic and burst firings were recorded in functionally characterized RT and VPL neurons of Cav3.2 KO mice. Many RT neurons were nociceptive (64% under pentobarbital anesthesia and 50% under urethane anesthesia). Compared to their wild-type (WT) controls, fewer nociceptive RT neurons were found in Cav3.2 KO mice. Both nociceptive and tactile RT neurons showed fewer bursts in Cav3.2 KO mice. Within a burst, RT neurons of Cav3.2 KO mice had a lower spike frequency and less-prominent accelerando-decelerando change. In contrast, VP neurons of Cav3.2 KO mice showed a higher ratio of bursts and a higher discharge rate within a burst than those of the WT control. In addition, the long-lasting tonic firing episodes in RT neurons of the Cav3.2 KO had less stereotypic regularity than their counterparts in WT mice.</p> <p>Conclusions</p> <p>RT might be important in nociception of the mouse. In addition, we showed an important role of Cav3.2 subtype of T-channel in RT burst firing pattern. The decreased occurrence and slowing of the bursts in RT neurons might cause the increased VP bursts. These changes would be factors contributing to alternation of pain behavior in the Cav3.2 KO mice.</p

    Exploring the genetics of irritable bowel syndrome: A GWA study in the general population and replication in multinational case-control cohorts

    OBJECTIVE: IBS shows genetic predisposition, but adequately powered gene-hunting efforts have been scarce so far. We sought to identify true IBS genetic risk factors by means of genome-wide association (GWA) and independent replication studies. DESIGN: We conducted a GWA study (GWAS) of IBS in a general population sample of 11\u2005326 Swedish twins. IBS cases (N=534) and asymptomatic controls (N=4932) were identified based on questionnaire data. Suggestive association signals were followed-up in 3511 individuals from six case-control cohorts. We sought genotype-gene expression correlations through single nucleotide polymorphism (SNP)-expression quantitative trait loci interactions testing, and performed in silico prediction of gene function. We compared candidate gene expression by real-time qPCR in rectal mucosal biopsies of patients with IBS and controls. RESULTS: One locus at 7p22.1, which includes the genes KDELR2 (KDEL endoplasmic reticulum protein retention receptor 2) and GRID2IP (glutamate receptor, ionotropic, delta 2 (Grid2) interacting protein), showed consistent IBS risk effects in the index GWAS and all replication cohorts and reached p=9.31 710(-6) in a meta-analysis of all datasets. Several SNPs in this region are associated with cis effects on KDELR2 expression, and a trend for increased mucosal KDLER2 mRNA expression was observed in IBS cases compared with controls. CONCLUSIONS: Our results demonstrate that general population-based studies combined with analyses of patient cohorts provide good opportunities for gene discovery in IBS. The 7p22.1 and other risk signals detected in this study constitute a good starting platform for hypothesis testing in future functional investigations. Published by the BMJ Publishing Group Limited.     Cloning and Characterization of Voltage-Gated Calcium Channel Alpha1 Subunits in Xenopus laevis During Development

    Voltage-gated calcium channels play a critical role in regulating the Ca(2+) activity that mediates many aspects of neural development, including neural induction, neurotransmitter phenotype specification, and neurite outgrowth. Using Xenopus laevis embryos, we describe the spatial and temporal expression patterns during development of the 10 pore-forming alphal subunits that define the channels\u27 kinetic properties. In situ hybridization indicates that Ca(V)1.2, Ca(V)2.1, Ca(V)2.2, and Ca(V)3.2 are expressed during neurula stages throughout the neural tube. These, along with Ca(V)1.3 and Ca(V)2.3, beginning at early tail bud stages, and Ca(V)3.1 at late tail bud stages, are detected in complex patterns within the brain and spinal cord through swimming tadpole stages. Additional expression of various alphal subunits was observed in the cranial ganglia, retina, olfactory epithelium, pineal gland, and heart. The unique expression patterns for the different alphal subunits suggests they are under precise spatial and temporal regulation and are serving specific functions during embryonic development.     Post-Infectious Irritable Bowel Syndrome, an Inflammation-Immunological Model with Relevance for Other IBS and Functional Dyspepsia

    This review presents studies that support an inflammation-immunological model for the pathogenesis of post-infectious irritable bowel syndrome (IBS), and highlights recent studies that support a similar disease model in non-post-infectious IBS, in particular, diarrhoea-predominant IBS, as well as in post-infectious functional dyspepsia. These recent studies are highlighted to demonstrate that one line of research in functional gastrointestinal disorders has moved away from the old psychosomatic concepts. It is hoped that this will encourage future students of this field to explore the role of immunological events

    Cost-effectiveness of six strategies for Helicobacter pylori diagnosis and management in uninvestigated dyspepsia assuming a high resource intensity practice pattern

    <p>Abstract</p> <p>Background</p> <p>Initial assessment of dyspepsia often includes noninvasive testing for <it>Helicobacter pylori </it>infection. Commercially available tests vary widely in cost and accuracy. Although there is extensive literature on the cost-effectiveness of <it>H. pylori </it>treatment, there is little information comparing the cost-effectiveness of various currently used, noninvasive testing strategies.</p> <p>Methods</p> <p>A Markov simulation was used to calculate cost per symptom-free year and cost per correct diagnosis. Uncertainty in outcomes was estimated using probabilistic sensitivity analysis.</p> <p>Results</p> <p>Under the baseline assumptions, cost per symptom-free year was 122forempiricprotonpumpinhibitor(PPI)trial,andcostsforthenoninvasiveteststrategiesrangedfrom122 for empiric proton pump inhibitor (PPI) trial, and costs for the noninvasive test strategies ranged from 123 (stool antigen) to $129 (IgG/IgA combined serology). Confidence intervals had significant overlap.</p> <p>Conclusions</p> <p>Under our assumptions for how testing for <it>H. pylori </it>infection is employed in United States medical practice, the available noninvasive tests all have similar cost-effectiveness between one another as well as with empiric PPI trial.</p
