    Mortality of emergency abdominal surgery in high-, middle- and low-income countries

    Background: Surgical mortality data are collected routinely in high-income countries, yet virtually no low- or middle-income countries have outcome surveillance in place. The aim was prospectively to collect worldwide mortality data following emergency abdominal surgery, comparing findings across countries with a low, middle or high Human Development Index (HDI). Methods: This was a prospective, multicentre, cohort study. Self-selected hospitals performing emergency surgery submitted prespecified data for consecutive patients from at least one 2-week interval during July to December 2014. Postoperative mortality was analysed by hierarchical multivariable logistic regression. Results: Data were obtained for 10 745 patients from 357 centres in 58 countries; 6538 were from high-, 2889 from middle- and 1318 from low-HDI settings. The overall mortality rate was 1⋅6 per cent at 24 h (high 1⋅1 per cent, middle 1⋅9 per cent, low 3⋅4 per cent; P < 0⋅001), increasing to 5⋅4 per cent by 30 days (high 4⋅5 per cent, middle 6⋅0 per cent, low 8⋅6 per cent; P < 0⋅001). Of the 578 patients who died, 404 (69⋅9 per cent) did so between 24 h and 30 days following surgery (high 74⋅2 per cent, middle 68⋅8 per cent, low 60⋅5 per cent). After adjustment, 30-day mortality remained higher in middle-income (odds ratio (OR) 2⋅78, 95 per cent c.i. 1⋅84 to 4⋅20) and low-income (OR 2⋅97, 1⋅84 to 4⋅81) countries. Surgical safety checklist use was less frequent in low- and middle-income countries, but when used was associated with reduced mortality at 30 days. Conclusion: Mortality is three times higher in low- compared with high-HDI countries even when adjusted for prognostic factors. Patient safety factors may have an important role.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Objetivou-se com o presente estudo caracterizar as fraÃÃes de carboidratos e proteÃna e estimar o valor energÃtico e a digestibilidade intestinal da proteÃna nÃo-degradada no rÃmen de alimentos por intermÃdio da tÃcnica de trÃs estÃgios da algaroba (Prosopis juliflora), da canafÃstula (Pithecellobium multiflorum), da flor-de-seda (Calotropis procera), da jitirana (Ipomea sp.), do juazeiro (Zizyphus joazeiro), do mata-pasto (Senna obtusifolia), do sabià (Mimosa caesalpiniaefolia), da palma gigante (Opuntia fÃcus indica) e do xique-xique (Cereus gounellei) e dos subprodutos do abacaxi (Ananas comosus), da acerola (Malpighia emarginata), do caju (Anacardium occidentale), do coco (Cocos nucifera), do melÃo (Cucumis melo), do maracujà (Passiflora eduli), da uva (Vitis labrusca) e do urucum (Bixa orellana). Para obtenÃÃo do fracionamento dos carboidratos, de acordo com o sistema CNCPS, foram calculados os carboidratos totais, as suas fraÃÃes B2, C e os componentes solÃveis em detergente neutro. Foram determinadas as fraÃÃes potencialmente degradÃvel (B2) e nÃo-degradÃvel (C) da fibra em detergente neutro, corrigida para cinzas e proteÃna. Para determinaÃÃo das fraÃÃes nitrogenadas, foram analisados os compostos nitrogenados nÃo-protÃicos, nitrogÃnio solÃvel e insolÃvel em tampÃo borato-fosfato e nitrogÃnio protÃico insolÃvel em detergente neutro e em detergente Ãcido. Para a estimaÃÃo dos Nutrientes DigestÃveis Totais (NDT) foram utilizadas as equaÃÃes propostas pelo NRC (2001). Para a determinaÃÃo da digestibilidade intestinal da proteÃna nÃo-degradada no rÃmen, os alimentos foram inicialmente incubados no rÃmen de fÃmea bovina fistulada, por 16 horas para determinaÃÃo da ProteÃna nÃo-degradada no rÃmen (PNDR), sendo o resÃduo submetido à digestÃo com soluÃÃo de pepsina durante 1 hora, e soluÃÃo de pancreatina a 38ÂC durante 24 horas, cujos resÃduos foram analisados para nitrogÃnio total. Acerola e melÃo apresentaram substancial conteÃdo nitrogenado na fraÃÃo C o que reduziria a disponibilidade de N tanto para microrganismos do rÃmen quanto para o hospedeiro. As forrageiras estudadas apresentaram maiores proporÃÃes do nitrogÃnio nas fraÃÃes A e B2, consequentemente disponibilizando nitrogÃnio para bactÃrias fermentadoras de carboidratos fibrosos. O percentual de fraÃÃo C na acerola e no melÃo em detrimento da fraÃÃo B2 acarretaria maior efeito de repleÃÃo ruminal e diminuiÃÃo da disponibilidade energÃtica, por sua caracterÃstica de indigestibilidade ao longo do trato gastrointestinal, quando submetidos a ensaios de alimentaÃÃo para animais. Nas forrageiras nativas e nas adaptadas ao clima da regiÃo Nordeste foram observados valores de fraÃÃo A + B1 de 35,26 a 73,37%, para fraÃÃo B2, de 3,87% a 28,45% e para fraÃÃo C de 17,34 e 40,63%. As fraÃÃes A, B1, B2, B3 e C dos compostos nitrogenados apresentaram variaÃÃo de 9,84 a 42,33 %; 1,58 a 11,47%; 48,63 a 80,10%; 0,70 a 6,13% e 0,43 a 2,86%, respectivamente. As fraÃÃes de carboidratos dos subprodutos analisados variaram de 19,35 a 58,52%; 9,95 a 61,44% e 15,35 a 70,06 para A+B1, B2 e C, respectivamente; enquanto para os compostos nitrogenados variaram de 5,01 a 33,02%; 1,69 a 15,23%; 2,26 a 75,60%; 1,53 a 40,26% e 0,08 a 43,84% para fraÃÃes A, B1 B2, B3 e C, respectivamente. O NDT estimado das forrageiras variou de 48,30 a 65,42%, enquanto o dos subprodutos variou de 31,41 a 128,90%. A estimativa da PNDR das forragens variou de 13,37 a 83,6%, e dos subprodutos variou de 39,14 a 89,06%. A digestibilidade intestinal da PNDR das forragens variou de 26,09 a 80,68%, enquanto para os subprodutos variou de 22,26 a 76,82%. To obtain the carbohydrates fractions, according with Cornell Net Carbohydrate and Protein System (CNCPS) system, were calculated the total carbohydrates, their fractions B2, C and the soluble components in neutral detergent. It was determinated the potentially degradable (B2) and undegradable (C)fractions of the neutral detergent fiber, corrected for ashes and protein. The non-protein nitrogenous compounds, soluble and insoluble nitrogen in borate-phosphate buffer, nitrogenous protein insoluble in neutral and acid detergent were analyzed for the determination of the nitrogen fractions. To estimate Total Digestible Nutrients (TDN) had been used the equations proposals by National Research Council - NRC (2001). The feeds were incubated in rumen during 16 hours to determine the rumen-undegradable protein (RUDP), then the residue was incubated with pepsin solution during 1 hour, and pancreatic solution during 24 hours at 38ÂC, whose residues were analyzed for total nitrogen. The Malpighia emarginata and Cucumis melo showed substantial nitrogen content in fraction C which would reduce the disponibility of nitrogen for the rumen microorganisms and its host. The evaluated forages showed higher proportions of nitrogen in fractions A and B2, providing nitrogen for the rumen microorganisms. The percentage of fraction C in Malpighia emarginata and Cucumis melo at the expense of the B2 fraction would entail greater effect of ruminal fill and decreased availability of energy, for its characteristic of indigestibility along the gastrointestinal tract, when being tested for feeding animals. In the evaluated forages had been observed values of A+B1 fraction from 35.26 to 73.37%, for B2 fraction, from 3.87% to 28.45% and for C fraction from 17.34 to 40.63%. The fractions, A, B1, B2, B3 and C of nitrogenous compounds had presented variation from 9.84 to 42.33%; 1.58 to 11.47%; 48.63 to 80.10%; 0.70 to 6.13% and 0.43 to 2.86%, respectively. The byproducts carbohydrate fractions analyzed varied from 19.35 to 58.52%; 9.95 to 61.44% and 15.35 to 70.06, for A+B1, B2 and C, respectively. In turn, the nitrogenous compounds varied from 5.01 to 33.02%; from 1.69 to 15.23%; from 2.26 to 75.60%; from 1.53 to 40.26% and from 0.08 to 43.84% for fractions, A, B1 B2, B3 and C, respectively. The forages TDN values varied from 48.30 to 65.42%, while of by-products it varied from 31.41 to 128.9%. The estimate of RUDP in the forages ranged from 13.37% to 83.6%, and the byproducts RUDP ranged from 39,14 to 89.06%. The intestinal digestion of RUDP ranged from 26.09 to 80.68%, %, while for byproducts varied of 22,26 76.82%. The Mimosa caesalpiniaefolia Benth was the forage that presented better ID and better RUDPd, and the Calotropis procera, presented lowest values for these variables.     Entende-se pelo termo clientelismo político, a utilização de órgãos públicos com a finalidade de prestação de favores, em detrimento da grande maioria da população, tendo como objetivo amarrar politicamente, à custa de votos ou dinheiro, o beneficiado.  É pratica comum na política Brasileira desde o século XIX, apresentando-se como uma porta da corrupção política, das irregularidades e do uso da "máquina administrativa" com finalidades perversas e os prejudicados são a maioria dos cidadãos e cidadãs que cumprem com seus deveres. No contexto atual, é importante reconsiderar esse conceito, sugerindo-se sistemas de intermediação de interesse entre a sociedade e gestores públicos que possam ser provenientes de processos que conjuguem cooptação e representação provenientes de ambas as partes, objetivando que modernos arranjos político-institucionais, assim como novas formas de participação política, convivem com formas igualmente novas de clientelismo político, desvinculando-o, assim, da caracterização comum enquanto forma tradicional de dominação política

    It is understood by the term political patronage, use of public agencies with the aim of providing favors to the detriment of the vast majority of the population, aiming to tie politically, at the expense of votes or money, benefited. It is common practice in Brazilian politics since the nineteenth century, presenting itself as a gateway to political corruption, irregularities and the use of "administrative machinery" with evil purposes and the losers are the majority of citizens who fulfill their duties.     The present work had as main objective to identify and to characterize rural producers as well as it productive systems. The characterization of the small producing native of northeastern Brazil, supported in a scientific knowledge about the local agricultural reality, is fundamental to elaborate politics of diffusion of technologies, programs and projects of agricultural development for the northeast. The disorganization of the productive chain and the little articulation among the different segments of this chain, allied the lack of zootecnic control, to the genetic standard bass of the flocks and the technical, managerial less information and of market of the producers it has been resulting in low productive indexes dedicated to the exploration of the cattle raising in Brazil. For initiative of the Clerkship of Agriculture of Quixeramobim a questionnaire was elaborated in partnership with IBAMA, SEBRAE and EMATERCE. The same was applied in the 11 districts of the municipal district in an eclectic and including way, in order to obtain a sampling the most representative possible. The great totality of the producers maintains a typical profile of the family agriculture, with emphasis in the subsistence, little technological progress and little specialization.     It was aimed to estimate the intestinal digestibility (ID) of rumen-undegradable protein (RUDP) of several feeds by a three-steps procedure. The evaluated forages were algaroba (Prosopis juliflora), canafístula (Pithecellobium multiflorum), flor-de-seda (Calotropis procera), jitirana (Ipomea sp.), juazeiro (Ziziphus joazeiro), mata-pasto (Senna obtusifolia), sabiá (Mimosa caesalpiniaefolia Benth), palma gigante (Opuntia ficus indica) and xique-xique (Cereus gounellei), and the agroindustry byproducts were pineapple (Ananas comosus L.), barbados cherry (Malpighia emarginata), cashew (Anacardium occidentale), coconut (Cocos nucifera L.), melon (Cucumis melo), passion fruit (Passiflora eduli), grape (Vitis labrusca) and anatto seeds (Bixa orellana L.). The feeds were incubated in rumen during 16 hours to determine the RUDP, and the residue was submitted to the digestion with pepsin solution during one hour, and pancreatic solution during 24 hours at 38ºC, those residues were analyzed for total nitrogen. The estimative of RUDP forage ranged from 13.37 to 83.6%, and the RUDP by-product ranged from 39.14 to 89.06%. The intestinal digestion of RUDP of the forages ranged from 26.09 to 80.68%, while for by-products varied from 22.26 to 76.82%. The sabiá was the forage that presented the highest intestinal digestibility and digestive rumen-undegradable protein (RUDPd), and the flor-de-seda, the lowest digestibility; while for by-products, melon and cashew presented, respectively, the highest values for DI and RUDP. The coconut presented the lowest values for ID and RUDPd. Although, some formulation systems of diets for ruminant consider that the RUDP present constant ID, the data obtained in this work suggest variation among the different feeds.A pesquisa objetivou estimar a digestibilidade intestinal (DI) da proteína não-degradada no rúmen (PNDR) de alimentos por intermédio da técnica de três estágios.     The present study aimed to characterize the carbohydrates and proteins fractions and to estimate the energetic value of native and adapted forages and agro industry by-products produced in Brazilian Northeast. For the determination of nitrogenous fractions, were analyzed the non-protein nitrogenous compounds, soluble and insoluble nitrogen in borate-phosphate buffer, protein nitrogen insoluble in neutral detergent and acid detergent. To the estimative of TDN, were used the equations proposed by NRC (2001). The by-products analyzed carbohydrates fractions varied from 19.35 to 58.52%; 9.95 to 61.44% and 15.35 to 70.06, for A+B1, B2 and C, respectively. The fractionation of nitrogen compounds and carbohydrates are simple methodologies and should be common practices in laboratories of feed evaluation.<p><p>O experimento foi realizado com o objetivo de avaliar as características agronômicas e químico-bromatológica de diferentes cultivares de milho para ensilagem. Foram avaliados doze cultivares de milho em delineamento experimental em blocos casualizados, com três blocos, 12 tratamentos e três repetições. As parcelas foram constituídas por três linhas de seis metros de comprimento e espaçamento de 0,8 m entre linhas. O ciclo das plantas, do plantio à colheita, variou de 105 a 114 dias. A altura média diferiu entre os cultivares (P < 0,05), variando de 182,6 (AGN-3150) a 247,0 cm (AG-122). Não houve diferença (P > 0,05) entre os cultivares para número de espigas por planta (0,9 a 1,1), teores de matéria seca (33,2 a 38,2 %), de extrato etéreo (1,9 a 2,5 %) e de fibra em detergente neutro (49,1 a 56,2 %). A produção de matéria seca diferiu (P < 0,05) entre o cultivar TORK (20,6 t/ha) e os cultivares CD-302 (16,4 t/ha) e TRAKTOR (15,7 t/ha). O AGN-3150 apresentou o menor teor de fibra em detergente ácido (24,2 %) e maior digestibilidade (80,7 %). Todas as cultivares apresentaram características agronômicas e qualitativas adequadas para produção de silagem, destacando-se os cultivares TORK e AGN-3150

    Estimative of the nutritional value of agroindustrial byproducts by using in vitro gas production technique

    Os objetivos deste trabalho foram determinar as frações de carboidratos totais e estimar a taxa de digestão dos carboidratos não-fibrosos de subprodutos agroindustriais, utilizando a técnica de produção de gás. Os subprodutos agroindustriais avaliados foram: caju, maracujá, abacaxi, acerola, urucum e melão, sendo cada amostra incubada em quadriplicata. Após mistura minuciosa de todos os conteúdos, as garrafas foram colocadas em estufa a 39ºC e foram conectadas a um interruptor sensível à pressão, com uma válvula solenóide e uma haste de ventilação. As medições de pressão do gás foram feitas a 0, 3, 6, 12, 15, 18, 22, 26, 30, 34, 40, 48, 60, 72, 96 e 120 horas após a incubação. Espaços em branco e um padrão interno foram incluídos para permitir ajustes na variação entre as leituras. Houve grande variação na composição nutricional dos subprodutos agroindustriais, sendo que caju e urucum apresentaram os maiores teores de proteína (159.3 e 135.3 g/kg de matéria seca, respectivamente). Melão e caju apresentaram os menores valores de produção total de gás (9,60 e 12,85 mL, respectivamente); e urucum, abacaxi e maracujá apresentaram maiores volumes (32,80; 28,16 e 22,54 mL, respectivamente), sendo os maiores percentuais da fração B2 (64,27; 81,25 e 67,49%, respectivamente). Entre os subprodutos agroindustriais, abacaxi e urucum destacaram-se pela maior contribuição dos carboidratos fibrosos para produção de gás total, sendo a fração B2 a principal fonte de energia para o crescimento microbiano, que é degradada em um ritmo mais lento do que os carboidratos não-fibrosos.The objective of this work was to determine the total carbohydrates fractions and to estimate the digestion rate of non-fibrous carbohydrates of agroindustrial byproducts by using the gas production technique. The agroindustrial byproducts evaluated were the following: cashew, passion fruit, pineapple, barbados cherry, annatto and melon, and each sample was incubated in quadruplicate. After mixing all contents thoroughly, the bottles were placed in an incubator at 39ºC, and connecting the lid of each bottle to a pressure sensitive switch, a solenoid valve and a vent stem connector. Gas pressure measurements were made at 0, 3, 6, 12, 15, 18, 22, 26, 30, 34, 40, 48, 60, 72, 96 e 120 hours post-inoculation. Blanks and an internal standard were included to enable adjustments in variation among readings. There was a high variation in nutritional composition of agroindustry byproducts, that cashew and annato presented the highest protein contents (159.3 and 135.3 g/kg of dry matter, respectively). Mellon and cashew presented the lowest values of total gas production (9.60 and 12.85 mL, respectively); and annato, pineapple and passion fruit presented highest gas volume (32.80; 28.16 and 22.54 mL, respectively), being the greatest percentages of phases from B2 fraction (64.27; 81.25 and 67.49%, respectively). Among the byproducts, pineapple and annatto stand out for the highest contribution of fibrous carbohydrate for total gas production, being B2 the main energy source for microbial growth, which is degraded at a slower rate than non-fibrous carbohydrate
