    Reducing risks associated with medicines and lifestyle in a residential care population with intellectual disabilities: evaluation of a pharmacy review initiative in England.

    OBJECTIVES: A collaborative service initiative involving community pharmacists and a specialist mental health pharmacist was developed to provide pharmacist reviews for care home residents with intellectual disabilities (IDs). This study aimed to characterise the medicines and lifestyle risk outcomes of the service and determine how these align with national priority issues in ID. DESIGN: Descriptive statistical analysis of routinely collected service delivery data. SETTING: Residential care homes in the Wirral, England for people with ID. PARTICIPANTS: 160 residents. INTERVENTIONS: Pharmacist review of residents' medicines and lifestyle risk factors between November 2019 and May 2020. PRIMARY AND SECONDARY OUTCOME MEASURES: Numbers of medicines prescribed, the nature of pharmacists' interventions/recommendations and general practitioner (GP)/psychiatrist acceptance. RESULTS: The 160 residents were prescribed 1207 medicines, 74% were prescribed ≄5 medicines and 507 interventions/recommendations were made, averaging 3.3 per resident. The highest proportion (30.4%) were lifestyle risk related, while changing and stopping medicines accounted for 17.9% and 12.8%, respectively. Of the recommendations discussed with GPs/psychiatrists, 86% were accepted. Medicines with anticholinergic properties were prescribed for 115 (72%) residents, of whom 43 (37%) had a high anticholinergic burden score. Pharmacists recommended anticholinergic discontinuation or dose reduction for 28 (24%) residents. The pharmacists made interventions/recommendations about constipation management for 10% of residents and about respiratory medicines for 17 (81%) of the 21 residents with respiratory diagnoses. CONCLUSIONS: The findings indicate considerable polypharmacy among the residents and a high level of pharmacists' interventions/recommendations about medicines and lifestyle risk, most of which were accepted by GPs/psychiatrists. This included anticholinergic burden reduction and improving respiratory disease and constipation management, which are national priority issues. Wider adoption of collaborative pharmacist review models could have similar benefits for residential populations with ID and potentially reduce pressure on other health services

    Genetic polymorphism among Cryptosporidium parvum isolates: evidence of two distinct human transmission cycles.

    We report the results of molecular analysis of 39 isolates of Cryptosporidium parvum from human and bovine sources in nine human outbreaks and from bovine sources from a wide geographic distribution. All 39 isolates could be divided into either of two genotypes, on the basis of genetic polymorphism observed at the thrombospondin-related adhesion protein (TRAP-C2) locus. Genotype 1 was observed only in isolates from humans. Genotype 2, however, was seen in calf isolates and in isolates from a subset of human patients who reported direct exposure to infected cattle or consumed items thought to be contaminated with cattle faces. Furthermore, experimental infection studies showed that genotype 2 isolates were infective to mice or calves under routine laboratory conditions, whereas genotype 1 isolates were not. These results support the occurrence of two distinct transmission cycles of C. parvum in humans

    Multiwavelength Constraints on the Origin of a Nearby Repeating Fast Radio Burst Source in a Globular Cluster

    Since fast radio bursts (FRBs) were discovered, their precise origins have remained a mystery. Multiwavelength observations of nearby FRB sources provide one of the best ways to make rapid progress in our understanding of the enigmatic FRB phenomenon. We present results from a sensitive, broadband multiwavelength X-ray and radio observational campaign of FRB 20200120E, the closest known extragalactic repeating FRB source. At a distance of 3.63 Mpc, FRB 20200120E resides in an exceptional location, within a ~10 Gyr-old globular cluster in the M81 galactic system. We place deep limits on both the persistent X-ray luminosity and prompt X-ray emission at the time of radio bursts from FRB 20200120E, which we use to constrain possible progenitors for the source. We compare our results to various classes of X-ray sources and transients. In particular, we find that FRB 20200120E is unlikely to be associated with: ultraluminous X-ray bursts (ULXBs), similar to those observed from objects of unknown origin in other extragalactic globular clusters; giant flares, like those observed from Galactic and extragalactic magnetars; or most intermediate flares and very bright short X-ray bursts, similar to those seen from magnetars in the Milky Way. We show that FRB 20200120E is also unlikely to be powered by a persistent or transient ultraluminous X-ray (ULX) source or a young, extragalactic pulsar embedded in a Crab-like nebula. We also provide new constraints on the compatibility of FRB 20200120E with accretion-based FRB models involving X-ray binaries and models that require a synchrotron maser process from relativistic shocks to generate FRB emission. These results highlight the power that multiwavelength observations of nearby FRBs can provide for discriminating between potential FRB progenitor models.Comment: 58 pages, 10 figures, 7 tables, submitte

    Observation of an Excited Bc+ State

    Using pp collision data corresponding to an integrated luminosity of 8.5 fb-1 recorded by the LHCb experiment at center-of-mass energies of s=7, 8, and 13 TeV, the observation of an excited Bc+ state in the Bc+π+π- invariant-mass spectrum is reported. The observed peak has a mass of 6841.2±0.6(stat)±0.1(syst)±0.8(Bc+) MeV/c2, where the last uncertainty is due to the limited knowledge of the Bc+ mass. It is consistent with expectations of the Bc∗(2S31)+ state reconstructed without the low-energy photon from the Bc∗(1S31)+→Bc+Îł decay following Bc∗(2S31)+→Bc∗(1S31)+π+π-. A second state is seen with a global (local) statistical significance of 2.2σ (3.2σ) and a mass of 6872.1±1.3(stat)±0.1(syst)±0.8(Bc+) MeV/c2, and is consistent with the Bc(2S10)+ state.     First observation and amplitude analysis of the B−→D+K−π−B^{-}\to D^{+}K^{-}\pi^{-} decay

    The B−→D+K−π−B^{-}\to D^{+}K^{-}\pi^{-} decay is observed in a data sample corresponding to 3.0 fb−13.0~\rm{fb}^{-1} of pppp collision data recorded by the LHCb experiment during 2011 and 2012. Its branching fraction is measured to be B(B−→D+K−π−)=(7.31±0.19±0.22±0.39)×10−5{\cal B}(B^{-}\to D^{+}K^{-}\pi^{-}) = (7.31 \pm 0.19 \pm 0.22 \pm 0.39) \times 10^{-5} where the uncertainties are statistical, systematic and from the branching fraction of the normalisation channel B−→D+π−π−B^{-}\to D^{+}\pi^{-}\pi^{-}, respectively. An amplitude analysis of the resonant structure of the B−→D+K−π−B^{-}\to D^{+}K^{-}\pi^{-} decay is used to measure the contributions from quasi-two-body B−→D0∗(2400)0K−B^{-}\to D_{0}^{*}(2400)^{0}K^{-}, B−→D2∗(2460)0K−B^{-}\to D_{2}^{*}(2460)^{0}K^{-}, and B−→DJ∗(2760)0K−B^{-}\to D_{J}^{*}(2760)^{0}K^{-} decays, as well as from nonresonant sources.     Observation of the Bs0→ηâ€Čηâ€ČB^0_s\to\eta'\eta' decay

    The first observation of the Bs0→ηâ€Čηâ€ČB^0_s\to\eta'\eta' decay is reported. The study is based on a sample of proton-proton collisions corresponding to 3.03.0 fb−1{\rm fb^{-1}} of integrated luminosity collected with the LHCb detector. The significance of the signal is 6.46.4 standard deviations. The branching fraction is measured to be [3.31±0.64 (stat)±0.28 (syst)±0.12 (norm)]×10−5[3.31 \pm 0.64\,{\rm (stat)} \pm 0.28\,{\rm (syst)} \pm 0.12\,{\rm (norm)}]\times10^{-5}, where the third uncertainty comes from the B±→ηâ€ČK±B^{\pm}\to\eta' K^{\pm} branching fraction that is used as a normalisation. In addition, the charge asymmetries of B±→ηâ€ČK±B^{\pm}\to\eta' K^{\pm} and B±→ϕK±B^{\pm}\to\phi K^{\pm}, which are control channels, are measured to be (−0.2±1.3)%(-0.2 \pm1.3)\% and (+1.7±1.3)%(+1.7\pm1.3)\%, respectively.     Differential branching fraction and angular analysis of Λb0→ΛΌ+Ό−\Lambda^{0}_{b} \rightarrow \Lambda \mu^+\mu^- decays

    The differential branching fraction of the rare decay Λb0→ΛΌ+Ό−\Lambda^{0}_{b} \rightarrow \Lambda \mu^+\mu^- is measured as a function of q2q^{2}, the square of the dimuon invariant mass. The analysis is performed using proton-proton collision data, corresponding to an integrated luminosity of 3.0 \mbox{ fb}^{-1}, collected by the LHCb experiment. Evidence of signal is observed in the q2q^2 region below the square of the J/ψJ/\psi mass. Integrating over 15 < q^{2} < 20 \mbox{ GeV}^2/c^4 the branching fraction is measured as d\mathcal{B}(\Lambda^{0}_{b} \rightarrow \Lambda \mu^+\mu^-)/dq^2 = (1.18 ^{+ 0.09} _{-0.08} \pm 0.03 \pm 0.27) \times 10^{-7} ( \mbox{GeV}^{2}/c^{4})^{-1}, where the uncertainties are statistical, systematic and due to the normalisation mode, Λb0→J/ψΛ\Lambda^{0}_{b} \rightarrow J/\psi \Lambda, respectively. In the q2q^2 intervals where the signal is observed, angular distributions are studied and the forward-backward asymmetries in the dimuon (AFBlA^{l}_{\rm FB}) and hadron (AFBhA^{h}_{\rm FB}) systems are measured for the first time. In the range 15 < q^2 < 20 \mbox{ GeV}^2/c^4 they are found to be A^{l}_{\rm FB} = -0.05 \pm 0.09 \mbox{ (stat)} \pm 0.03 \mbox{ (syst)} and A^{h}_{\rm FB} = -0.29 \pm 0.07 \mbox{ (stat)} \pm 0.03 \mbox{ (syst)}.Comment: 27 pages, 10 figures, Erratum adde
