    Analysis of the incidence of acute respiratory diseases in the paediatric population in Poland in the light of the “Health Needs Map”

    Introduction: Statistical data on the structure of acute respiratory diseases incidence in the paediatric population are still scarce. The demand for such data results mainly from the need to constantly implement new systemic and economic solutions. The aim of the study was to attempt to use reported data for an assessment of the incidence of acute respiratory diseases in various age groups.Material and methods: An analysis of selected acute respiratory diseases was conducted in relation to diagnoses reported from 1 January to 31 December 2014 to the National Health Fund (NFZ, Narodowy Fundusz Zdrowia) in accordance with the codes of the International Statistical Classification of Diseases and Related Health Problems, 10th Revision. The study was conducted under the Knowledge Education Development operational programme co-funded by the European Social Fund.Results: A total of 101,000 children were hospitalised due to acute respiratory diseases, which amounted to 1,554 hospitalisa-tions per 100.000. The most common causes of hospitalisation were pneumonia and bronchitis/bronchiolitis. Boys were hospital-ised more often in each age group. The shortest average length of stay (ALOS) was 5.21 days and concerned hospitalisation due to bronchitis. The longest length of stay for children was due to tuberculosis (14.3 days). The highest age average of a child was recorded in pleural diseases (10.51 years) and the lowest in bronchitis (2.93 years). Rehospitalisation was necessary in children in whom tuberculosis or pleural diseases were diagnosed (1.43 vs 1.34). A total of 67 inpatient deaths were recorded, of which 19 were due to pneumonia or its complications.Conclusions: Epidemiological data reported to the National Health Fund (NFZ) seem quite reliable and do not differ significantly from those reported in other European countries. The analysed data may be useful in estimating health needs in paediatrics

    Neuroendocrine cell hyperplasia of infancy is a rare form of children’s interstitial lung disease recognised usually in infancy and in children younger than two years old. The typical clinical scenario, such as chest retractions, tachypnoea, hypoxaemia, crackles, characteristic changes in high-resolution computed tomography and histological examination of the lung parenchyma, is the cornerstone for diagnosis. In the article, the authors describe clinical manifestation of neuroendocrine cell hyperplasia and a present case of an infant with this rare interstitial lung disease.Neuroendokrynna hiperplazja wieku niemowlęcego jest chorobą śródmiąższową płuc rozpoznawaną przede wszystkim u niemowląt i dzieci do drugiego roku życia.     Recommendations for the management of tuberculosis in children — KOMPASS TB. Part 1: Tuberculosis prevention

    Since the second half of the 20th century the incidence of tuberculosis has been declining in Poland. Despite this, current epidemiological data still support the need for the continued mass BCG vaccination in Poland in the near future. Apart from the protection against severe hematogenous forms of tuberculosis, vaccination lowers the risk of infection with Mycobacterium tuberculosis. Primary and acquired immunodeficiency, including immunity disorders associated with an ongoing treatment, are contraindications to BCG vaccination. The most common adverse effects following BCG vaccination are reactions at the site of injection and in regional lymph nodes, which usually does not require treatment. Methods of tuberculosis prevention, particularly recommended in low-incidence countries, include: diagnostic investigations of patients who had contacts with pulmonary tuberculosis as well as an active detection and treatment of latent Mycobacterium tuberculosis infection. Latent tuberculosis infection can be identified on the basis of positive results of the tuberculin skin test or interferon-gamma release assays after the active disease has been ruled out. This condition does require prophylactic treatment

    Sustainable planning of cross-border cooperation: a strategy for alliances in border cities

    In recent years, cooperation among nations has become a critical issue towards sustainable development of neighbor cities in border areas. In this regard, sustainable common planning approaches and policies are an increasing reality, particularly in European territories. Considering the significant amount of cross-border cooperation (CBC) projects and strategies within Europe, it is crucial to promote research approaches that are able to identify the most positive approaches towards the establishment of alliances in border territories, serving as pivotal methodologies for achieving success. Contextually, the present study considered direct and indirect research methods and tools, literature reviews, data collection, computer-assisted telephone interviewing (CATI) and computer-assisted web interview (CAWI), all applied over two European border cities: Cieszyn (Poland) and Cesky Tesin (Czech Republic). These methods enabled the assembly of perspectives of local authorities, public and private institutions, non-governmental organizations, and entrepreneurs from the cities under study. Through the analysis of the collected data, five conditions have been identified for the success of strategic alliances in CBC projects: (i) well defining the alliance goals; (ii) ensuring participation in the alliance of various groups of stakeholders; (iii) involvement of both partners with extensive experience in CBC; (iv) ensuring the coherence of the key objective; and (v) guaranteeing the alliance benefits both sides. These conditions might effectively contribute to achieve more successful outputs in CBC projects, highlighting the relevance of previously developed strategies on the definition of future approaches.info:eu-repo/semantics/publishedVersio

    A model for double notches and bifurcated components in radio profiles of pulsars and magnetars - Evidence for the parallel acceleration maser in pulsar magnetosphere

    Averaged pulse profiles of three nearby pulsars: B1929+10, J0437-4715 and B0950+08 exhibit unusual `double notches'. These W-like looking features consist of two adjacent V-shaped dips that approach each other at increasing observation frequency nuobs roughly at a rate sep \propto nuobs^{-1/2}, where sep is the separation between the notches' minima. We show that basic properties of the notches, namely their W-like look and the rate of their converging can be understood within a narrow class of models of coherent radio emission from pulsars: the free electron maser models based on coherent inverse Compton scattering of parallel oscillations of ambient electric field. The observed properties of the pulsars imply that the Fourier spectrum of the wiggler-like oscillations is narrow and that the broad-band character of the radio emission reflects the width of the electron energy distribution. Such a model provides a natural explanation for the frequency-independent separation between the main pulse and interpulse of B0950+08 as well as for the lack of radius to frequency mapping in the conal-like emission of J0437-4715. The frequency behaviour of the main pulse in the profile of the first radio magnetar XTE J1810-197 can also be explained within this model.Comment: 15 pages, 10 figures, accepted by A&A after minor change

    Healthcare resource utilization and medical costs for children with interstitial lung diseases (chILD) in Europe

    Background No data on healthcare utilisation and associated costs for the many rare entities of children's interstitial lung diseases (chILD) exist. This paper portrays healthcare utilisation structures among individuals with chILD, provides a pan-European estimate of a 3-month interval per-capita costs and delineates crucial cost drivers. Methods Based on longitudinal healthcare resource utilisation pattern of 445 children included in the Kids Lung Register diagnosed with chILD across 10 European countries, we delineated direct medical and non-medical costs of care per 3-month interval. Country-specific utilisation patterns were assessed with a children-tailored modification of the validated FIMA questionnaire and valued by German unit costs. Costs of care and their drivers were subsequently identified via gamma-distributed generalised linear regression models. Results During the 3 months prior to inclusion into the registry (baseline), the rate of hospital admissions and inpatient days was high. Unadjusted direct medical per capita costs (euro19 818) exceeded indirect (euro1 907) and direct non-medical costs (euro1 125) by far. Country-specific total costs ranged from euro8 713 in Italy to euro28 788 in Poland. Highest expenses were caused by the disease categories 'diffuse parenchymal lung disease (DPLD)-diffuse developmental disorders' (euro45 536) and 'DPLD-unclear in the non-neonate' (euro47 011). During a follow-up time of up to 5 years, direct medical costs dropped, whereas indirect costs and non-medical costs remained stable. Conclusions This is the first prospective, longitudinal study analysing healthcare resource utilisation and costs for chILD across different European countries. Our results indicate that chILD is associated with high utilisation of healthcare services, placing a substantial economic burden on health systems

    Measurement of health-related quality of life is mostly used with advanced measurement tools, such as scales. The use of a certain scale in different cultural settings or in age groups other than those it was designed for needs conducting the adaptation process and assessment of psychometric properties of the adapted scale. This paper presents the results of theoretical validity and reliability analysis of the Polish adaptation of the VQLQ scale for adolescents with Hymenoptera venom allergy. The study sample consisted of 78 adolescents aged 14-19 years, who were treated with venom immunotherapy in Polish allergological centers in 2008 year. Theoretical validity of the scale was analyzed with exploratory factor analysis using the principal components method. Reliability analysis was assessed in terms of internal consistency with Cronbach's α coefficient and by testing Kline's criterion. The results showed satisfactory validity of the scale: factor analysis revealed a 3-factor structure of the scale - extracted factors were described as anxiety, caution and discomfort. All the scale items contributed to unique factors, except for one item concerning limitation in summer due to allergy, which was identified as a separate dimension of health-related quality of life of Polish adolescents with Hymenoptera venom allergy. All the extracted subscales were characterized by values of α coefficient equal or higher than 0.8, what is usually considered as a high-level reliability coefficient. The adapted scale is a valid and reliable tool measuring health-related quality of life in Polish adolescents treated with venom-specific immunotherapy

    Bacteriophages encode a wide variety of cell wall disrupting enzymes that aid the viral escape in the final stages of infection. These lytic enzymes have accumulated notable interest due to their potential as novel antibacterials for infection treatment caused by multiple-drug resistant bacteria. Here, the detailed functional and structural characterization of Thermus parvatiensis prophage peptidoglycan lytic amidase AmiP, a globular Amidase_3 type lytic enzyme adapted to high temperatures is presented. The sequence and structure comparison with homologous lytic amidases reveals the key adaptation traits that ensure the activity and stability of AmiP at high temperatures. The crystal structure determined at a resolution of 1.8 Å displays a compact α/β-fold with multiple secondary structure elements omitted or shortened compared to protein structures of similar proteins. The functional characterisation of AmiP demonstrates high efficiency of catalytic activity and broad substrate specificity towards thermophilic and mesophilic bacteria strains containing Orn-type or DAP-type peptidoglycan. The here presented AmiP constitutes the most thermoactive and ultrathermostable Amidase_3 type lytic enzyme biochemically characterised with a temperature optimum at 85 °C. The extraordinary high melting temperature Tm 102.6 °C confirms fold stability up to approximately 100 °C. Furthermore, AmiP is shown to be more active over the alkaline pH range with pH optimum at pH 8.5 and tolerates NaCl up to 300 mM with the activity optimum at 25 mM NaCl. This set of beneficial characteristics suggests that AmiP can be further exploited in biotechnology
