    Generating evidence and understanding the treatment of osteoarthritis in Brazil: a study through Delphi methodology

    OBJECTIVES: This study aimed to provide evidence for understanding how to treat osteoarthritis (OA) in our country. Therefore, it was necessary to match information and investigations related to the treatment of the disease from the three main types of specialists involved: physiatrists, orthopedists and rheumatologists. METHODS: The authors acted as a scientific advisory committee. From the initial discussions, a structured questionnaire was developed for use with a group of specialists on OA using the Delphi technique. The questionnaire was sent to 21 experts appointed by the authors, and the results obtained were critically analyzed and validated. RESULTS: The prevalence of OA was 33% in Brazil, corresponding to one-third of the individuals in the reference population, which included individuals over 25 years of age. Another significant finding was that most patients did not receive any form of treatment in the early stages of OA. CONCLUSION: The committee pointed to the need for early intervention and that the available medicinal resources can fulfil this important role, as is the case with SYSADOA treatments. Glucosamine-based medicinal products with or without chondroitin could also fulfill this need for early treatment.     Com o envelhecimento da população mundial, aumenta a cada dia a prevalência das doenças relacionadas à idade, com destaque para a osteoartrite, forma mais comum de doença articular e que, além da alta prevalência, relaciona-se a altos custos médicos e sociais. Dentre as modalidades de tratamento, a viscosuplementação, injeção intra-articular de derivados do ácido hialurônico, vem ganhando cada vez mais destaque. Trata-se de um polissacarídeo de alto peso molecular que possui, além das funções mecânicas de distribuição de peso e lubrificação, propriedades anti-inflamatórias e ação físico-química sobre diversos aspectos da articulação, sendo que tais efeitos são diretamente proporcionais ao peso molecular, concentração e presença de ligações cruzadas da droga utilizada. A viscosuplementação é procedimento simples e pode ser realizada ambulatorialmente. Traz benefício para dor e função, e também altera favoravelmente o curso da doença, melhorando quantitativamente e qualitativamente a cartilagem articular. Apresenta bom perfil de segurança e favorável relação custo-efetividade, sendo indicada tanto para osteoartrite quanto após um procedimento de artroscopia

    Total knee arthroplasty in valgus knee deformities continues to be a challenge. It comprises only 10% of patients who undergo total knee arthroplasty. The surgeon should be aware of the technical aspects that differentiate it from the varus deformity: surgical exposure, bone cuts, ligament balancing, gap balancing, joint line obliquity, patellar tracking, preserving fibular nerve function, and selection of the implant.     Biomechanical evaluation of orthopaedic cement combined with antibiotic and methylene blue

    OBJETIVO: O cimento acrílico é utilizado há anos nas cirurgias ortopédicas, especialmente nas artroplastias do joelho, merecendo atenção também quando utilizado com o acréscimo de antibióticos (para tratamento de infecções) ou corantes (para facilitação de uma possível retirada). Neste estudo procurou-se avaliar diferenças mecânicas entre o cimento ortopédico puro e quando misturado com antibiótico e/ou corante. MÉTODOS: Foram utilizados cimento acrílico ortopédico Surgical Simplex®P Stryker, vancomicina em pó e azul de metileno e as misturas submetidas a testes físicos e mecânicos de acordo com a norma ABNT NBR ISO 5833. Avaliou-se: tempo para a formação de massa, capacidade de intrusão, resistência à compressão, resistência ao dobramento (tensão) e temperatura máxima atingida pelas misturas. RESULTADOS: As misturas avaliadas foram aprovadas quanto ao tempo de preparação da mistura, temperatura máxima alcançada, profundidade da intrusão e resistência à compressão. Somente aquela contendo apenas cimento puro foi aprovada no ensaio de flexão. CONCLUSÃO: O acréscimo de vancomicina e/ou azul de metileno ao cimento ortopédico Surgical Simplex®P Stryker reduz a resistência da mistura à flexão, sendo reprovada pela norma ABNT NBR ISO 5833.OBJECTIVE: Acrylic cement has been used for years on orthopaedic surgeries, especially on knee arthroplasties, deserving special attention when added to antibiotics (for treatment of deep bone infections) or stains (to facilitate its removal). The present study was conducted in order to evaluate potential mechanical differences between the orthopaedic cement itself and when this is added to antibiotic and/or stains. METHODS: Surgical bone cement Simplex®P Stryker, vancomycin and methylene blue were used, and the mixtures were submitted to physical and mechanical tests according the ABNT NBR ISO 5833 rule. The parameters studied here were: time for mass formation, intrusion capability, resistance to compression, resistance to flexion and maximum temperature reached by the mixtures. RESULTS: The evaluated mixtures were approved as to mass formation, maximum temperature, intrusion capability and resistance to compression. Only the one containing pure cement was approved on the flexion essay. CONCLUSION: The addition of vancomycin and/or methylene blue to Surgical Simplex®P Stryker bone cement reduces its resistance to flexion, being unacceptable by the ABNT NBR ISO 5833 rule

    A survey of the clinicopathological and molecular characteristics of patients with suspected Lynch syndrome in Latin America

    Background: Genetic counselling and testing for Lynch syndrome (LS) have recently been introduced in several Latin America countries. We aimed to characterize the clinical, molecular and mismatch repair (MMR) variants spectrum of patients with suspected LS in Latin America. Methods: Eleven LS hereditary cancer registries and 34 published LS databases were used to identify unrelated families that fulfilled the Amsterdam II (AMSII) criteria and/or the Bethesda guidelines or suggestive of a dominant colorectal (CRC) inheritance syndrome. Results: We performed a thorough investigation of 15 countries and identified 6 countries where germline genetic testing for LS is available and 3 countries where tumor testing is used in the LS diagnosis. The spectrum of pathogenic MMR variants included MLH1 up to 54%, MSH2 up to 43%, MSH6 up to 10%, PMS2 up to 3% and EPCAM up to 0.8%. The Latin America MMR spectrum is broad with a total of 220 different variants which 80% were private and 20% were recurrent. Frequent regions included exons 11 of MLH1 (15%), exon 3 and 7 of MSH2 (17 and 15%, respectively), exon 4 of MSH6 (65%), exons 11 and 13 of PMS2 (31% and 23%, respectively). Sixteen international founder variants in MLH1, MSH2 and MSH6 were identified and 41 (19%) variants have not previously been reported, thus representing novel genetic variants in the MMR genes. The AMSII criteria was the most used clinical criteria to identify pathogenic MMR carriers although microsatellite instability, immunohistochemistry and family history are still the primary methods in several countries where no genetic testing for LS is available yet.     Photography-based taxonomy is inadequate, unnecessary, and potentially harmful for biological sciences

    The question whether taxonomic descriptions naming new animal species without type specimen(s) deposited in collections should be accepted for publication by scientific journals and allowed by the Code has already been discussed in Zootaxa (Dubois & Nemésio 2007; Donegan 2008, 2009; Nemésio 2009a–b; Dubois 2009; Gentile & Snell 2009; Minelli 2009; Cianferoni & Bartolozzi 2016; Amorim et al. 2016). This question was again raised in a letter supported by 35 signatories published in the journal Nature (Pape et al. 2016) on 15 September 2016. On 25 September 2016, the following rebuttal (strictly limited to 300 words as per the editorial rules of Nature) was submitted to Nature, which on 18 October 2016 refused to publish it. As we think this problem is a very important one for zoological taxonomy, this text is published here exactly as submitted to Nature, followed by the list of the 493 taxonomists and collection-based researchers who signed it in the short time span from 20 September to 6 October 2016
