    Fire Training Fatalities and Firefighter Adherence to National fire Protection Association Standards

    Sudden cardiac arrest continues to be a major cause of firefighter deaths during training due to a lack of individual firefighter adherence to National Fire Protection Association (NFPA) standards. These standards identify requirements for fire departments to create and maintain fitness programs. Existing research has not identified any relationships between training fatalities and individual firefighter adherence to NFPA 1583, Standard on Health-Related Fitness Programs for Fire Department Members. Using self-determination theory as the foundation, the purpose of this cross-sectional correlation study was to investigate whether individual firefighter adherence to NFPA 1583 has a measurable effect on training fatalities. Survey data were collected from 441 paid firefighters from 7 fire departments located in a rural county in a southern U.S. state. Data were analyzed using multiple linear regression. Results indicated that adherence to NFPA 1583 has a statistically significant relationship with reduced firefighter training fatalities (p = .000). Recommendations include examining adherence policies to all elements of the NFPA 1583 standard, not just chapters 5 through 8 in the publication. These include chapter 1 administration, chapter 2 referenced publications, chapter 3 definitions, and chapter 4 program organization specifications. The study results may be used by fire department training divisions to improve the health and safety of firefighters

    Effect of natalizumab on disease progression in secondary progressive multiple sclerosis (ASCEND). a phase 3, randomised, double-blind, placebo-controlled trial with an open-label extension

    Background: Although several disease-modifying treatments are available for relapsing multiple sclerosis, treatment effects have been more modest in progressive multiple sclerosis and have been observed particularly in actively relapsing subgroups or those with lesion activity on imaging. We sought to assess whether natalizumab slows disease progression in secondary progressive multiple sclerosis, independent of relapses. Methods: ASCEND was a phase 3, randomised, double-blind, placebo-controlled trial (part 1) with an optional 2 year open-label extension (part 2). Enrolled patients aged 18–58 years were natalizumab-naive and had secondary progressive multiple sclerosis for 2 years or more, disability progression unrelated to relapses in the previous year, and Expanded Disability Status Scale (EDSS) scores of 3·0–6·5. In part 1, patients from 163 sites in 17 countries were randomly assigned (1:1) to receive 300 mg intravenous natalizumab or placebo every 4 weeks for 2 years. Patients were stratified by site and by EDSS score (3·0–5·5 vs 6·0–6·5). Patients completing part 1 could enrol in part 2, in which all patients received natalizumab every 4 weeks until the end of the study. Throughout both parts, patients and staff were masked to the treatment received in part 1. The primary outcome in part 1 was the proportion of patients with sustained disability progression, assessed by one or more of three measures: the EDSS, Timed 25-Foot Walk (T25FW), and 9-Hole Peg Test (9HPT). The primary outcome in part 2 was the incidence of adverse events and serious adverse events. Efficacy and safety analyses were done in the intention-to-treat population. This trial is registered with ClinicalTrials.gov, number NCT01416181. Findings: Between Sept 13, 2011, and July 16, 2015, 889 patients were randomly assigned (n=440 to the natalizumab group, n=449 to the placebo group). In part 1, 195 (44%) of 439 natalizumab-treated patients and 214 (48%) of 448 placebo-treated patients had confirmed disability progression (odds ratio [OR] 0·86; 95% CI 0·66–1·13; p=0·287). No treatment effect was observed on the EDSS (OR 1·06, 95% CI 0·74–1·53; nominal p=0·753) or the T25FW (0·98, 0·74–1·30; nominal p=0·914) components of the primary outcome. However, natalizumab treatment reduced 9HPT progression (OR 0·56, 95% CI 0·40–0·80; nominal p=0·001). In part 1, 100 (22%) placebo-treated and 90 (20%) natalizumab-treated patients had serious adverse events. In part 2, 291 natalizumab-continuing patients and 274 natalizumab-naive patients received natalizumab (median follow-up 160 weeks [range 108–221]). Serious adverse events occurred in 39 (13%) patients continuing natalizumab and in 24 (9%) patients initiating natalizumab. Two deaths occurred in part 1, neither of which was considered related to study treatment. No progressive multifocal leukoencephalopathy occurred. Interpretation: Natalizumab treatment for secondary progressive multiple sclerosis did not reduce progression on the primary multicomponent disability endpoint in part 1, but it did reduce progression on its upper-limb component. Longer-term trials are needed to assess whether treatment of secondary progressive multiple sclerosis might produce benefits on additional disability components. Funding: Biogen

    Detecting and Characterizing Mg II absorption in DESI Survey Validation Quasar Spectra

    In this paper we will present findings on the detection of Magnesium II (MgII, lambda = 2796 {\AA}, 2803 {\AA}) absorption systems observed in data from the Early Data Release (EDR) of the Dark Energy Spectroscopic Instrument (DESI). DESI is projected to obtain spectroscopy of approximately 3 million quasars (QSOs), of which over 99% are anticipated to be found at redshifts greater than z < 0.3, such that DESI would be able to observe an associated or intervening Mg II absorber illuminated by the background QSO. We have developed an autonomous supplementary spectral pipeline that detects such systems through an initial line-fitting process and then confirms line properties using a Markov Chain Monte Carlo (MCMC) sampler. Based upon both a visual inspection and the reanalysis of coadded observations, we estimate this sample of absorption systems to have a completeness of 82.56% and purity of 99.08%. As the spectra in which Mg II systems are detected are the result of coadding multiple observations, we can determine the sensitivity, and therefore completeness, of the sample by searching for known Mg II systems in coadded data with fewer observations (and therefore lower signal-to-noise). From a parent catalog containing 83,207 quasars, we detect a total of 23,921 Mg II absorption systems following a series of quality cuts. Extrapolating from this occurrence rate of 28.75% implies a catalog at the completion of the five-year DESI survey that contains over eight hundred thousand Mg II absorbers. The cataloging of these systems will enable significant further research as they carry information regarding circumgalactic medium (CGM) environments, the distribution of intervening galaxies, and the growth of metallicity across the redshift range 0.3 < z < 2.5.Comment: 12 pages, 7 figure

    Constraints on dark matter to dark radiation conversion in the late universe with DES-Y1 and external data

    84siWe study a class of decaying dark matter models as a possible resolution to the observed discrepancies between early- and late-time probes of the universe. This class of models, dubbed DDM, characterizes the evolution of comoving dark matter density with two extra parameters. We investigate how DDM affects key cosmological observables such as the CMB temperature and matter power spectra. Combining 3x2pt data from Year 1 of the Dark Energy Survey,Planck-2018 CMB temperature and polarization data, Supernova (SN) Type Ia data from Pantheon, and BAO data from BOSS DR12, MGS and 6dFGS, we place new constraints on the amount of dark matter that has decayed and the rate with which it converts to dark radiation. The fraction of the decayed dark matter in units of the current amount of dark matter, zetazeta, is constrained at 68% confidence level to be <0.32 for DES-Y1 3x2pt data, <0.030 for CMB+SN+BAO data, and <0.037 for the combined dataset. The probability that the DES and CMB+SN+BAO datasets are concordant increases from 4% for the LambdaLambdaCDM model to 8% (less tension) for DDM. Moreover, tension in S8=sigma8sqrtOmegam/0.3S_8=sigma_8sqrt{Omega_m/0.3} between DES-Y1 3x2pt and CMB+SN+BAO is reduced from 2.3sigmasigma to 1.9sigmasigma. We find no reduction in the Hubble tension when the combined data is compared to distance-ladder measurements in the DDM model.     Nature doesn't judge you – how urban nature supports young people's mental health and wellbeing in a diverse UK city

    Reviewed research reveals a lack of young people's voices articulating if and how urban nature supports their mental health and wellbeing. This paper presents qualitative research with young multi-ethnic urban residents living in a northern UK city and offers an important counter-narrative to the pervasive notion of childhood nature-deficit disorder. Using interviews and creative arts workshops, we explored the value of urban nature for the mental health and wellbeing of 24 young people aged 17–27 years, 9 of whom had lived experience of mental health difficulties. Trees, water, open spaces and views were frequently experienced nature typologies offering benefits. Deteriorating landscapes, young people's shifting identities and perceived time pressures disrupted support. Young people expressed how urban nature encounters were experienced as accepting and relational, offering a: stronger sense of self; feelings of escape; connection and care with the human and non-human world

    Diving into the vertical dimension of elasmobranch movement ecology

    Knowledge of the three-dimensional movement patterns of elasmobranchs is vital to understand their ecological roles and exposure to anthropogenic pressures. To date, comparative studies among species at global scales have mostly focused on horizontal movements. Our study addresses the knowledge gap of vertical movements by compiling the first global synthesis of vertical habitat use by elasmobranchs from data obtained by deployment of 989 biotelemetry tags on 38 elasmobranch species. Elasmobranchs displayed high intra- and interspecific variability in vertical movement patterns. Substantial vertical overlap was observed for many epipelagic elasmobranchs, indicating an increased likelihood to display spatial overlap, biologically interact, and share similar risk to anthropogenic threats that vary on a vertical gradient. We highlight the critical next steps toward incorporating vertical movement into global management and monitoring strategies for elasmobranchs, emphasizing the need to address geographic and taxonomic biases in deployments and to concurrently consider both horizontal and vertical movements

    Variation in Childhood Diarrheal Morbidity and Mortality in Africa, 2000-2015.

    BACKGROUND: Diarrheal diseases are the third leading cause of disease and death in children younger than 5 years of age in Africa and were responsible for an estimated 30 million cases of severe diarrhea (95% credible interval, 27 million to 33 million) and 330,000 deaths (95% credible interval, 270,000 to 380,000) in 2015. The development of targeted approaches to address this burden has been hampered by a paucity of comprehensive, fine-scale estimates of diarrhea-related disease and death among and within countries. METHODS: We produced annual estimates of the prevalence and incidence of diarrhea and diarrhea-related mortality with high geographic detail (5 km2) across Africa from 2000 through 2015. Estimates were created with the use of Bayesian geostatistical techniques and were calibrated to the results from the Global Burden of Diseases, Injuries, and Risk Factors Study 2016. RESULTS: The results revealed geographic inequality with regard to diarrhea risk in Africa. Of the estimated 330,000 childhood deaths that were attributable to diarrhea in 2015, more than 50% occurred in 55 of the 782 first-level administrative subdivisions (e.g., states). In 2015, mortality rates among first-level administrative subdivisions in Nigeria differed by up to a factor of 6. The case fatality rates were highly varied at the national level across Africa, with the highest values observed in Benin, Lesotho, Mali, Nigeria, and Sierra Leone. CONCLUSIONS: Our findings showed concentrated areas of diarrheal disease and diarrhea-related death in countries that had a consistently high burden as well as in countries that had considerable national-level reductions in diarrhea burden.
