    Human mobility and spatial disease dynamics

    The understanding of human mobility and the development of qualitative models as well as quantitative theories for it is of key importance to the research of human infectious disease dynamics on large geographical scales. In our globalized world, mobility and traffic have reached a complexity and volume of unprecedented degree. Long range human mobility is now responsible for the rapid geographical spread of emergent infectious diseases. Multiscale human mobility networks exhibit two prominent features: (1) Networks exhibit a strong heterogeneity, the distribution of weights, traffic fluxes and populations sizes of communities range over many orders of magnitude. (2) Although the interaction magnitude in terms of traffic intensities decreases with distance, the observed power-laws indicate that long range interactions play a significant role in spatial disease dynamics. We will review how the topological features of traffic networks can be incorporated in models for disease dynamics and show, that the way topology is translated into dynamics can have a profound impact on the overall disease dynamics. We will also introduce a class of spatially extended models in which the impact and interplay of both spatial heterogeneity as well as long range spatial interactions can be investigated in a systematic fashion. Our analysis of multiscale human mobility networks is based on a proxy network of dispersing US dollar bills, which we incorporated in a model to produce real-time epidemic forecasts that projected the spatial spread of the recent outbreak of Influenza A(H1N1)

    Diseño de una planta de tratamiento de aguas residuales para la Universidad Politecnica Salesiana, Sede Quito, Campus Sur

    The general objective of the study was to design a treatment plant for the Universidad Politécnica Salesiana Sede Quito Campus Sur (UPS-Q-S), which was the measurement of the flow from the University with a lignigrafo-sampler giving a total of 0,944 l/s. The sample that was taken from the campus was sent to the CICAM (Centre for research and environmental Control) for their respective analysis and characterization. They were developed and presented three different and autonomous processes such as wastewater treatment, to consider different options according to different characteristics such as advantages and disadvantages. Taking into consideration the flow proceeded to a first design which consisted of a facultative lagoons and aeration system. As second design was a reactor blanket of ascending flow (UASB), which takes place in a tank in an anaerobic atmosphere and process that requires that the water income has certain characteristics as a pH slightly acid and low presence of solid materials. As the third design arose as a septic tank, common treatment used for most domestic water treatment. It took into account environmental and topographical characteristics of the land, the costs of construction and operation. . This study concludes that the most effective solution for treating waste effluents from the UPS-Q-S is the UASB that involves a treatment of low-cost maintenance and high removal of contaminants discharges in liquidEl objetivo general del trabajo fue diseñar una planta de tratamiento para la Universidad Politécnica Salesiana Sede Quito Campus Sur (UPS-Q-S), para lo cual se realizó la medición del caudal proveniente de la universidad con un limnígrafomuestreador dando un total de 0,944 l/s. La muestra que se tomó del campus se envió al CICAM (Centro de Investigación y Control Ambiental) para su respectivo análisis y caracterización. Fueron desarrollados y planteados tres procesos diferentes y autónomos, como tratamiento del agua residual, para plantear diferentes opciones según diferentes características como ventajas y desventajas. Tomando en consideración el caudal se procedió a un primer diseño el cual consistió en un sistema de lagunas facultativas y de aireación. Como segundo diseño se realizó un reactor blanket de flujo ascendente (UASB), proceso que se realiza en un tanque en un medio anaerobio y que requiere que el agua de ingreso presente ciertas características como un pH ligeramente ácido y baja presencia de materiales sólidos. Como tercer diseño se planteó un tanque séptico, tratamiento común utilizado para el tratamiento de aguas en su mayoría domésticas. Se tomó en cuenta las características ambientales y topográficas del terreno, los costos de construcción y operación. . Del presente estudio se concluye que la solución más efectiva para tratar los efluentes residuales de la UPS-Q-S es el UASB por que implica un tratamiento de bajo costo de mantenimiento y alta remoción de contaminantes presentes en las descargas líquid

    Enumeration of max-pooling responses with generalized permutohedra

    We investigate the combinatorics of max-pooling layers, which are functions that downsample input arrays by taking the maximum over shifted windows of input coordinates, and which are commonly used in convolutional neural networks. We obtain results on the number of linearity regions of these functions by equivalently counting the number of vertices of certain Minkowski sums of simplices. We characterize the faces of such polytopes and obtain generating functions and closed formulas for the number of vertices and facets in a 1D max-pooling layer depending on the size of the pooling windows and stride, and for the number of vertices in a special case of 2D max-pooling.Comment: 35 pages, 11 figures, 4 tables.     Effects of physical exercise on physical function in older adults in residential care: a systematic review and network meta-analysis of randomised controlled trials

    Background: Physical exercise is effective at attenuating ageing-related physical decline in general, but evidence of its benefits for older adults in residential care, who often have functional dependency, multimorbidity, and polypharmacy, is inconclusive. We aimed to establish the effects of exercise interventions on the physical function of this population. Methods: For this systematic review and network meta-analysis, we searched PubMed, Web of Science, Cochrane Library, Rehabilitation & Sports Medicine Source, and SPORTDiscus to identify randomised controlled trials assessing the effects of exercise interventions (vs usual care) on physical function (ie, functional independence, physical performance, and other related measures, such as muscle strength, balance, or flexibility) in adults aged 60 years or older living in residential care. Relevant studies published in English or Spanish up to Jan 12, 2023, were included in the systematic review. The quality of studies was assessed using the Tool for the Assessment of Study Quality and Reporting in Exercise (TESTEX) score. A network meta-analysis was performed for physical function-related outcomes reported in at least ten studies, with subanalyses for specific intervention (ie, exercise type, training volume, and study duration) and participant (eg, having cognitive impairment or dementia, pre-frail or frail status, and being functionally dependent) characteristics. The study protocol was registered on PROSPERO (CRD42021247809). Findings: 147 studies (11 609 participants, with mean ages ranging from 67 years [SD 9] to 92 years [2]) were included in the systematic review, and were rated as having overall good quality (median TESTEX score 9 [range 3–14]). In the meta-analysis (including 105 studies, n=7759 participants), exercise interventions were associated with significantly improved overall physical function, with a standardised mean difference [SMD] of 0·13 (95% credible interval [CrI] 0·04–0·21), which was confirmed in all analysed subpopulations. The strongest association was observed with 110–225 min per week of exercise, and the greatest improvements were observed with 170 min per week (SMD 0·36 [95% CrI 0·20–0·52]). No significant differences were found between exercise types. Subanalyses showed significant improvements for almost all analysed physical function-related outcomes (Barthel index, five-times sit-to-stand test, 30-s sit-to-stand test, knee extension, hand grip strength, bicep curl strength, Short Physical Performance Battery, 6-min walking test, walking speed, Berg balance scale, and sit-and-reach test). Large heterogeneity was found between and within studies in terms of population and intervention characteristics. Interpretation: Exercise interventions are associated with improved physical function in older adults in residential care, and should, therefore, be routinely promoted in long-term care facilities. The European Journalism Observatory (EJO) is a network of 14 non-profit media research institutes in 11 countries where Spain continue to be the missing element. All researchers in the EJO network actively strive to transfer their knowledge to the media industry as well as interested publics outside the scientific community, as a platform to enable online availability of at least a portion of many publications in several languages

    Introducción: conocer la seguridad de las drogas actualmente disponibles para el tratamiento de las enfermedades reumáticas es muy importante al momento de tomar decisiones terapéuticas objetivas e individualizadas en la consulta médica diaria. Asimismo, datos de la vida real amplían el conocimiento revelado por los ensayos clínicos. Objetivos: describir los eventos adversos (EA) reportados, estimar su frecuencia e identificar los factores relacionados con su desarrollo. Materiales y métodos: se utilizaron datos BIOBADASAR, un registro voluntario y prospectivo de seguimiento de EA de tratamientos biológicos y sintéticos dirigidos en pacientes con enfermedades reumáticas inmunomediadas. Los pacientes son seguidos hasta la muerte, pérdida de seguimiento o retiro del consentimiento informado. Para este análisis se extrajeron datos recopilados hasta el 31 de enero de 2023. Resultados: se incluyó un total de 6253 pacientes, los cuales aportaron 9533 ciclos de tratamiento, incluyendo 3647 (38,3%) ciclos sin drogas modificadoras de la enfermedad biológicas y sintéticas dirigidas (DME-b/sd) y 5886 (61,7%) con DME-b/sd. Dentro de estos últimos, los más utilizados fueron los inhibidores de TNF y abatacept. Se reportaron 5890 EA en un total de 2701 tratamientos (844 y 1857 sin y con DME-b/sd, respectivamente), con una incidencia de 53,9 eventos cada 1000 pacientes/año (IC 95% 51,9-55,9). La misma fue mayor en los ciclos con DME-b/sd (71,1 eventos cada 1000 pacientes/año, IC 95% 70,7-77,5 versus 33,7, IC 95% 31,5-36,1; p<0,001). Las infecciones, particularmente las de la vía aérea superior, fueron los EA más frecuentes en ambos grupos. El 10,9% fue serio y el 1,1% provocó la muerte del paciente. El 18,7% de los ciclos con DME-b/sd fue discontinuado a causa de un EA, significativamente mayor a lo reportado en el otro grupo (11,5%; p<0,001). En el análisis ajustado, las DME-b/sd se asociaron a mayor riesgo de presentar al menos un EA (HR 1,82, IC 95% 1,64-1,96). De igual manera, la mayor edad, el mayor tiempo de evolución, el antecedente de enfermedad pulmonar obstructiva crónica, el diagnóstico de lupus eritematoso sistémico y el uso de corticoides se asociaron a mayor riesgo de EA.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio
