    Make Changes Early! High Stress Levels Predict Lower Academic Performance in First-Year Physical Therapy Students: A Pilot Study

    Purpose: Evidence exists in the literature that allows admissions committees to predict which students will be most successful in entry-level physical therapy educational programs. Evidence linking modifiable predictors of academic performance to currently enrolled students allowing enhancement of their performance during enrollment is lacking. Previous healthcare education literature indicates that stress has a negative predictive relationship with academic performance, and that stress is the highest in the beginning of an academic program. Research studies with medical students report exercise assists in minimizing stress and improving academic performance, while the role of leisure time has also been investigated. No study has explored the role that stress, exercise, and leisure time may have in predicting academic performance in physical therapy students. Method: Physical therapy students (N = 37) who had successfully completed the first year of the program were recruited. Participants completed a questionnaire measuring their stress level and the time spent exercising and participating in leisure activities. Grade point average was used to measure academic performance. A hierarchical multiple regression was performed with each of the three predictor variables and the one outcome variable. Results: A significant negative predictive relationship was observed between stress (M = 43.73, s = 15.90) and GPA (M = 3.75, s = .31), p = .042. The stress predictor [r = .293, r2 = .086], explained only 8.6% of the variance in GPA. Conclusions: Higher stress predicts decreased academic performance in first-year physical therapy students. Future research on stress modification could be beneficial in improving performance in students struggling in an academic program

    Can the Forward-Step-Down Test Be Used Reliably in the Clinical Setting to Assess Movement Changes Resulting from Maximal Exertion? A Pilot Study

    Introduction: Visual assessment of lower extremity mechanics is used frequently in clinical practice, and objective scoring of the visual assessment is beneficial to improve objectivity of patient evaluation. In addition, lower extremity mechanics change with fatigue and these changes may increase the risk of lower extremity injury. The Forward-Step-Down Test (FSDT) is one such objective tool, but its ability to detect changes in movement quality in response to exertion are not known. Methods: This study utilized a repeated-measures design, where the participants were scored on the FSDT before performing the Bruce protocol for an exertion stimulus. The participants were re-scored on the FSDT at one, five, and ten minutes after completing the Bruce protocol. Results: Wilcoxon signed-rank tests showed a significant change in FSDT score between baseline and five minutes post-exertion (a\u3c .017). Friedman’s ANOVA was non-significant across all four assessments. In addition, despite testing healthy young adults, 50% of participants scored as “poor” movement quality on the initial test. This number increased to 75% at significant five-minute post-exertion mark. Discussion: Median scores on the FSDT were significantly different at five minutes post-exertion. However, this statistically significant change is of questionable clinical relevance because the median score changed by 0.5. This small change from 3.5 to 4.0 may not represent a change in overall movement quality from “moderate” to “poor.” Results do indicate that the participants in this study overall had poorer than expected movement quality throughout the testing. These results suggest larger data collection and analysis may be warranted for this population and the general population prior to partaking in exercise. Conclusion: The FSDT detects changes in lower extremity mechanics five minutes following a single exertion stimulus. Participants’ scores returned to baseline by ten minutes post-exertion. The FSDT may be a viable tool to assess changes in lower extremity movement quality following a single bout of exertion, and may help determine when participants have recovered back to baseline movement quality

    The Effect of the Di-Tertiary Butyl Peroxide (DTBP) additive on HCCI Combustion of Fuel Blends of Ethanol and Diethyl Ether

    The influence of the small amounts (1-3%) of the additive di-tertiary butyl peroxide (DTBP) on the combustion event of Homogeneous Charge Compression Ignition (HCCI) engines was investigated using engine experiments, numerical modeling, and carbon-14 isotope tracing. DTBP was added to neat ethanol and diethyl ether (DEE) in ethanol fuel blends for a range of combustion timings and engine loads. The addition of DTBP to the fuel advanced combustion timing in each instance, with the DEE-in-ethanol mixture advancing more than the ethanol alone. A numerical model reproduced the experimental results. Carbon-14 isotope tracing showed that more ethanol burns to completion in DEE-in-ethanol blends with a DTBP additive when compared to results for DEE-in-ethanol without the additive. However, the addition of DTBP did not elongate the heat release in either case. The additive advances combustion timing for both pure ethanol and for DEE-in-ethanol mixtures, but the additive results in more of an advance in timing for the DEE-in-ethanol mixture. This suggests that although there are both thermal and kinetic influences from the addition of DTBP, the thermal effects are more important

    AbstractThe Ancillary Therapy and Supportive Care Working Group had 3 goals: (1) to establish guidelines for ancillary therapy and supportive care in chronic graft-versus-host disease (GVHD), including treatment for symptoms and recommendations for patient education, preventive measures, and appropriate follow-up; (2) to provide guidelines for the prevention and management of infections and other common complications of treatment for chronic GVHD; and (3) to highlight the areas with the greatest need for clinical research. The definition of “ancillary therapy and supportive care” embraces the most frequent immunosuppressive or anti-inflammatory interventions used with topical intent and any other interventions directed at organ-specific control of symptoms or complications resulting from GVHD and its therapy. Also included in the definition are educational, preventive, and psychosocial interventions with this same objective. Recommendations are organized according to the strength and quality of evidence supporting them and cover the most commonly involved organs, including the skin, mouth, female genital tract, eyes, gastrointestinal tract, and lungs. Recommendations are provided for prevention of infections, osteoporosis, and steroid myopathy and management of neurocognitive and psychosocial adverse effects related to chronic GVHD. Optimal care of patients with chronic GVHD often requires a multidisciplinary approach

    Search for supersymmetry in events with b-quark jets and missing transverse energy in pp collisions at 7 TeV

    Results are presented from a search for physics beyond the standard model based on events with large missing transverse energy, at least three jets, and at least one, two, or three b-quark jets. The study is performed using a sample of proton-proton collision data collected at sqrt(s) = 7 TeV with the CMS detector at the LHC in 2011. The integrated luminosity of the sample is 4.98 inverse femtobarns. The observed number of events is found to be consistent with the standard model expectation, which is evaluated using control samples in the data. The results are used to constrain cross sections for the production of supersymmetric particles decaying to b-quark-enriched final states in the context of simplified model spectra.Comment: Submitted to Physical Review

    Mechanism of antibody-specific deglycosylation and immune evasion by Streptococcal IgG-specific endoglycosidases

    Bacterial pathogens have evolved intricate mechanisms to evade the human immune system, including the production of immunomodulatory enzymes. Streptococcus pyogenes serotypes secrete two multi-modular endo--N-acetylglucosaminidases, EndoS and EndoS2, that specifically deglycosylate the conserved N-glycan at Asn297 on IgG Fc, disabling antibody-mediated effector functions. Amongst thousands of known carbohydrate-active enzymes, EndoS and EndoS2 represent just a handful of enzymes that are specific to the protein portion of the glycoprotein substrate, not just the glycan component. Here, we present the cryoEM structure of EndoS in complex with the IgG1 Fc fragment. In combination with small-angle X-ray scattering, alanine scanning mutagenesis, hydrolytic activity measurements, enzyme kinetics, nuclear magnetic resonance and molecular dynamics analyses, we establish the mechanisms of recognition and specific deglycosylation of IgG antibodies by EndoS and EndoS2. Our results provide a rational basis from which to engineer novel enzymes with antibody and glycan selectivity for clinical and biotechnological applications

    Susceptibility of Pancreatic Beta Cells to Fatty Acids Is Regulated by LXR/PPARα-Dependent Stearoyl-Coenzyme A Desaturase

    Chronically elevated levels of fatty acids-FA can cause beta cell death in vitro. Beta cells vary in their individual susceptibility to FA-toxicity. Rat beta cells were previously shown to better resist FA-toxicity in conditions that increased triglyceride formation or mitochondrial and peroxisomal FA-oxidation, possibly reducing cytoplasmic levels of toxic FA-moieties. We now show that stearoyl-CoA desaturase-SCD is involved in this cytoprotective mechanism through its ability to transfer saturated FA into monounsaturated FA that are incorporated in lipids. In purified beta cells, SCD expression was induced by LXR- and PPARα-agonists, which were found to protect rat, mouse and human beta cells against palmitate toxicity. When their SCD was inhibited or silenced, the agonist-induced protection was also suppressed. A correlation between beta cell-SCD expression and susceptibility to palmitate was also found in beta cell preparations isolated from different rodent models. In mice with LXR-deletion (LXRÎČ-/- and LXRαÎČ-/-), beta cells presented a reduced SCD-expression as well as an increased susceptibility to palmitate-toxicity, which could not be counteracted by LXR or PPARα agonists. In Zucker fatty rats and in rats treated with the LXR-agonist TO1317, beta cells show an increased SCD-expression and lower palmitate-toxicity. In the normal rat beta cell population, the subpopulation with lower metabolic responsiveness to glucose exhibits a lower SCD1 expression and a higher susceptibility to palmitate toxicity. These data demonstrate that the beta cell susceptibility to saturated fatty acids can be reduced by stearoyl-coA desaturase, which upon stimulation by LXR and PPARα agonists favors their desaturation and subsequent incorporation in neutral lipids

    The Multiplanet System TOI-421: A Warm Neptune and a Super Puffy Mini-Neptune Transiting a G9 V Star in a Visual Binary

    We report the discovery of a warm Neptune and a hot sub-Neptune transiting TOI-421 (BD-14 1137, TIC 94986319), a bright (V = 9.9) G9 dwarf star in a visual binary system observed by the Transiting Exoplanet Survey Satellite (TESS) space mission in Sectors 5 and 6. We performed ground-based follow-up observations—comprised of Las Cumbres Observatory Global Telescope transit photometry, NIRC2 adaptive optics imaging, and FIbre-fed EchellĂ© Spectrograph, CORALIE, High Accuracy Radial velocity Planet Searcher, High Resolution Échelle Spectrometer, and Planet Finder Spectrograph high-precision Doppler measurements—and confirmed the planetary nature of the 16 day transiting candidate announced by the TESS team. We discovered an additional radial velocity signal with a period of five days induced by the presence of a second planet in the system, which we also found to transit its host star. We found that the inner mini-Neptune, TOI-421 b, has an orbital period of P_b = 5.19672 ± 0.00049 days, a mass of M_b = 7.17 ± 0.66 M⊕, and a radius of R_b = 2.68^(+0.19)_(-0.18) R⊕, whereas the outer warm Neptune, TOI-421 c, has a period of Pc = 16.06819 ± 0.00035 days, a mass of M_c = 16.42^(+1.06)_(-1.04) M⊕, a radius of R_c = 5.09^(+0.16)_(-0.15) R⊕ and a density of ρ_c = 0.685^(+0.080)_(-0.072) g cm⁻³. With its characteristics, the outer planet (ρ_c = 0.685^(+0.080)_(-0.072) g cm⁻³) is placed in the intriguing class of the super-puffy mini-Neptunes. TOI-421 b and TOI-421 c are found to be well-suited for atmospheric characterization. Our atmospheric simulations predict significant Lyα transit absorption, due to strong hydrogen escape in both planets, as well as the presence of detectable CH4 in the atmosphere of TOI-421 c if equilibrium chemistry is assumed
