    The Enforcement of the Illinois Felony Marijuana Law in Chicago

    This study looks at the enforcement of the Illinois marijuana law by the police and court having jurisdiction over the City of Chicago. Data was collected in two separate surveys during the summer of 1969, one of the Chicago Police and the other of the Circuit Court of Cook County Criminal Division\u27s Narcotics Court. It should be noted that the purpose of this study was not to follow the same individuals as they move through the Chicago enforcement system. Rather, the purpose here was to see how the system charged with the enforcement of the marijuana law deals with the violator, and what effect the conflict between law and order has on the enforcers

    Does self-monitoring reduce blood pressure? Meta-analysis with meta-regression of randomized controlled trials

    Introduction. Self-monitoring of blood pressure (BP) is an increasingly common part of hypertension management. The objectives of this systematic review were to evaluate the systolic and diastolic BP reduction, and achievement of target BP, associated with self-monitoring. Methods. MEDLINE, Embase, Cochrane database of systematic reviews, database of abstracts of clinical effectiveness, the health technology assessment database, the NHS economic evaluation database, and the TRIP database were searched for studies where the intervention included self-monitoring of BP and the outcome was change in office/ambulatory BP or proportion with controlled BP. Two reviewers independently extracted data. Meta-analysis using a random effects model was combined with meta-regression to investigate heterogeneity in effect sizes. Results. A total of 25 eligible randomized controlled trials (RCTs) (27 comparisons) were identified. Office systolic BP (20 RCTs, 21 comparisons, 5,898 patients) and diastolic BP (23 RCTs, 25 comparisons, 6,038 patients) were significantly reduced in those who self-monitored compared to usual care (weighted mean difference (WMD) systolic −3.82 mmHg (95% confidence interval −5.61 to −2.03), diastolic −1.45 mmHg (−1.95 to −0.94)). Self-monitoring increased the chance of meeting office BP targets (12 RCTs, 13 comparisons, 2,260 patients, relative risk = 1.09 (1.02 to 1.16)). There was significant heterogeneity between studies for all three comparisons, which could be partially accounted for by the use of additional co-interventions. Conclusion. Self-monitoring reduces blood pressure by a small but significant amount. Meta-regression could only account for part of the observed heterogeneity

    Intelligence within BAOR and NATO's Northern Army Group

    During the Cold War the UK's principal military role was its commitment to the North Atlantic Treaty Organisation (NATO) through the British Army of the Rhine (BAOR), together with wartime command of NATO's Northern Army Group. The possibility of a surprise attack by the numerically superior Warsaw Pact forces ensured that great importance was attached to intelligence, warning and rapid mobilisation. As yet we know very little about the intelligence dimension of BAOR and its interface with NATO allies. This article attempts to address these neglected issues, ending with the impact of the 1973 Yom Kippur War upon NATO thinking about warning and surprise in the mid-1970s. It concludes that the arrangements made by Whitehall for support to BAOR from national assets during crisis or transition to war were - at best - improbable. Accordingly, over the years, BAOR developed its own unique assets in the realm of both intelligence collection and special operations in order to prepare for the possible outbreak of conflict

    The observation of nitric acid-containing particles in the tropical lower stratosphere

    Airborne in situ measurements over the eastern Pacific Ocean in January 2004 have revealed a new category of nitric acid (HNO<sub>3</sub>)-containing particles in the tropical lower stratosphere. These particles are most likely composed of nitric acid trihydrate (NAT). They were intermittently observed in a narrow layer above the tropopause (18&plusmn;0.1 km) and over a broad geographic extent (>1100 km). In contrast to the background liquid sulfate aerosol, these particles are solid, much larger (1.7-4.7 &micro;m vs.&nbsp;0.1&micro;m in diameter), and significantly less abundant (<10<sup>-4</sup> cm<sup>-3</sup> vs.&nbsp;10 cm<sup>-3</sup>). Microphysical trajectory models suggest that the NAT particles grow over a 6-14 day period in supersaturated air that remains close to the tropical tropopause and might be a common feature in the tropics. The small number density of these particles implies a highly selective or slow nucleation process. Understanding the formation of solid NAT particles in the tropics could improve our understanding of stratospheric nucleation processes and, therefore, dehydration and denitrification

    Prioritising between direct observation of therapy and case-finding interventions for tuberculosis: use of population impact measures

    BACKGROUND: Population impact measures (PIMs) have been developed as tools to help policy-makers with locally relevant decisions over health risks and benefits. This involves estimating and prioritising potential benefits of interventions in specific populations. Using tuberculosis (TB) in India as an example, we examined the population impact of two interventions: direct observation of therapy and increasing case-finding. METHODS: PIMs were calculated using published literature and national data for India, and applied to a notional population of 100 000 people. Data included the incidence or prevalence of smear-positive TB and the relative risk reduction from increasing case finding and the use of direct observation of therapy (applied to the baseline risks over the next year), and the incremental proportion of the population eligible for the proposed interventions. RESULTS: In a population of 100 000 people in India, the directly observed component of the Directly Observed Treatment, Short-course (DOTS) programme may prevent 0.188 deaths from TB in the next year compared with 1.79 deaths by increasing TB case finding. The costs of direct observation are (in international dollars) I5960andofcasefindingareI5960 and of case finding are I4839 or I31702andI31702 and I2703 per life saved respectively. CONCLUSION: Increasing case-finding for TB will save nearly 10 times more lives than will the use of the directly observed component of DOTS in India, at a smaller cost per life saved. The demonstration of the population impact, using simple and explicit numbers, may be of value to policy-makers as they prioritise interventions for their populations

    In-pandemic development of an application ontology for COVID-19 surveillance in a primary care sentinel network

    Background: Creating an ontology for coronavirus disease 2019 (COVID-19) surveillance should help ensure transparency and consistency. Ontologies formalise conceptualisations at either domain or application level. Application ontologies cross domains and are specified through testable use cases. Our use case was extension of the role of the Oxford Royal College of General Practitioners (RCGP) Research and Surveillance Centre (RSC) to monitor the current pandemic and become an in-pandemic research platform. Objective: To develop an application ontology for COVID-19 which can be deployed across the various use case domains of the Oxford- RCGP RSC research and surveillance activities. Methods: We described our domain-specific use case. The actor was the RCGP RSC sentinel network; the system the course of the COVID-19 pandemic; the outcomes the spread and effect of mitigation measures. We used our established three-step method to develop the ontology, separating ontological concept development from code mapping and data extract validation. We developed a coding system–independent COVID-19 case identification algorithm. As there were no gold standard pandemic surveillance ontologies, we conducted a rapid Delphi consensus exercise through the International Medical Informatics Association (IMIA) Primary Health Care Informatics working group and extended networks. Results: Our use case domains included primary care, public health, virology, clinical research and clinical informatics. Our ontology supported: (1) Case identification, microbiological sampling and health outcomes at both an individual practice and national level; (2) Feedback through a dashboard; (3) A national observatory, (4) Regular updates for Public Health England, and (5) Transformation of the sentinel network to be a trial platform. We have identified a total of 8,627 people with a definite COVID-19 status, 4,240 with probable, and 59,147 people with possible COVID-19, within the RCGP RSC network (N=5,056,075). Conclusions: The underpinning structure of our ontological approach has coped with multiple clinical coding challenges. At a time when there is uncertainty about international comparisons, clarity about the basis on which case definitions and outcomes are made from routine data is essential

    Long-term Trends from Ecosystem Research at the Hubbard Brook Experimental Forest

    The Hubbard Brook Experimental Forest was established by the U.S. Forest Service in 1955 as a major center for hydrologic research in the Northeast. The Hubbard Brook Ecosystem Study originated 8 years later with the idea of using the small watershed approach to study element flux and cycling and the response of forest ecosystems to disturbance. Since that time, the research program at Hubbard Brook has expanded to include various physical, chemical and biological measurements collected by researchers from a number of cooperating institutions. Collaborative, long-term data are the keystone of the Hubbard Brook Ecosystem Study and have provided invaluable insight into how ecosystems respond to disturbances such as air pollution, climate change, forest disturbance, and forest management practices. This report highlights long- term ecological trends at Hubbard Brook, provides explanations for some of the trends, and lists references from the scientific literature for further reading

    Mapping Migratory Bird Prevalence Using Remote Sensing Data Fusion

Recent advances in lidar and radar remote sensing have shown considerable potential for mapping forest structure and habitat characteristics across landscapes. However, their relative efficacies and integrated use in habitat mapping remain largely unexplored. We evaluated the use of lidar, radar and multispectral remote sensing data in predicting multi-year bird detections or prevalence for 8 migratory songbird species in the unfragmented temperate deciduous forests of New Hampshire, USA. \ud \ud Methodology and Principal Findings: A set of 104 predictor variables describing vegetation vertical structure and variability from lidar, phenology from multispectral data and backscatter properties from radar data were derived. We tested the accuracies of these variables in predicting prevalence using Random Forests regression models. All data sets showed more than 30% predictive power with radar models having the lowest and multi-sensor synergy ("fusion") models having highest accuracies. Fusion explained between 54% and 75% variance in prevalence for all the birds considered. Stem density from discrete return lidar and phenology from multispectral data were among the best predictors. Further analysis revealed different relationships between the remote sensing metrics and bird prevalence. Spatial maps of prevalence were consistent with known habitat preferences for the bird species. \ud \ud Conclusion and Significance: Our results highlight the potential of integrating multiple remote sensing data sets using machine-learning methods to improve habitat mapping. Multi-dimensional habitat structure maps such as those generated from this study can significantly advance forest management and ecological research by facilitating fine-scale studies at both stand and landscape level
