    Developing, delivering and documenting rehabilitation in a multi-centre randomised controlled surgical trial: experiences from the ProFHER trial

    OBJECTIVES: A rigorous approach to developing, delivering and documenting rehabilitation within randomised controlled trials of surgical interventions is required to underpin the generation of reliable and usable evidence. This article describes the key processes used to ensure provision of good quality and comparable rehabilitation to all participants of a multi-centre randomised controlled trial comparing surgery with conservative treatment of proximal humeral fractures in adults. METHODS: These processes included the development of a patient information leaflet on self-care during sling immobilisation, the development of a basic treatment physiotherapy protocol that received input and endorsement by specialist physiotherapists providing patient care, and establishing an expectation for the provision of home exercises. Specially designed forms were also developed to facilitate reliable reporting of the physiotherapy care that patients received. RESULTS: All three initiatives were successfully implemented, alongside the measures to optimise the documentation of physiotherapy. Thus, all participating sites that recruited patients provided the sling immobilisation leaflet, all adhered to the physiotherapy protocol and all provided home exercises. There was exemplary completion of the physiotherapy forms that often reflected a complex patient care pathway. These data demonstrated equal and high access to and implementation of physiotherapy between groups, including the performance of home exercises. CONCLUSION: In order to increase the validity and relevance of the evidence from trials of surgical interventions and meet international reporting standards, careful attention to study design, conduct and reporting of the intrinsic rehabilitation components is required.     Water Managers' Boundary Judgments and Adaptive Water Governance. An Analysis of the Dutch Haringvliet Sluices Case

    __Abstract__ In this paper, we explore how managing actors' boundary judgments influence the adaptability of water governance. We approach this question by examining the relationship between the way water managers frame, and act in, complex water issues on the one hand and develop adaptive water governance strategies on the other. We define four categories of boundary judgments made by water managers in order to deal with the complexities in water governance issues. An in-depth case study analysis of an attempt to adjust the management of the water regime in the south-west Delta of the Netherlands is provided in order to reconstruct the water managers' boundary judgments and their impact upon governance strategies used. We found that, most of the time, the water managers involved predominantly made tight boundary judgments. These tight boundary judgments seemed to hamper the mutual learning process among a variety of stakeholders that is needed to realize adaptive water governance. We argue that wide boundary judgments enhance the chance of realizing adaptive practices and build upon exploration, learning, and connection

    Business model innovation and transition to a sustainable food system: A case study in the Lisbon metropolitan area

    The food systems’ transition towards a sustainable involves structural changes, namely the emphasis on local production, short supply chains, and the preference for organic products. The shift in the agri-food system is taking place through the creation of entirely new businesses and individual farms moving towards organic production. In both cases, the enterprises use a combination of well-established agricultural knowledge and techniques, new scientific knowledge on productive methods and new technological platforms for commercialization. These mixed sources permit the creation of innovative business models (BMs). They exemplify how traditional industries can absorb/generate innovation at technological and organizational levels, and become part of the new knowledge-based era. The study has three objectives: to analyse the emerging agri-food businesses in the Lisbon Metropolitan Area (LMA); to characterize innovative sustainable BMs within the transition dynamics; to reflect on the challenges that the characteristics of the food system pose for the emergence of these BMs. The study is part of an interdisciplinary project on Spatial Planning for Change (SPLACH). The analysis addresses the food system transition in a specific territory, namely the LMA.     ‘Demand pull’ government policies to support Product-Service System activity: the case of Energy Service Companies (ESCos) in the UK

    Product-Service Systems (PSSs) constitute a family of service-based business models designed to satisfy our societal needs in an economically and environmentally sustainable manner. To date however PSS application has remained niche due to a variety of critical barriers. This paper explores how ‘demand pull’ national government policies could support PSS activity by addressing these barriers and cultivating market demand. Lessons are drawn from a case study of how regulatory, economic incentive, informative and procurement policies have supported Energy Service Company (ESCo) activity in the UK; a sub-set of the PSS family focused on energy service provision. Subsequently five policy recommendations are presented to support PSS activity: (1) balancing economic incentives and regulatory disincentives; (2) promoting indirect policy support; (3) redesigning existing market structures; (4) promoting locally-led PSS activity; and (5) creating stable policy frameworks. The paper warns however that national government policy cannot easily address all PSS barriers, such as customer preferences, international developments, technological progress and inherent business model weaknesses, pointing to the need for other complementary solutions.     The effect of chemotherapy on health-related quality of life in mesothelioma: Results from the SWAMP trial

    © 2015 Cancer Research UK. All rights reserved. Background: The effect of chemotherapy on health-related quality of life (HRQoL) in malignant pleural mesothelioma (MPM) is poorly understood. Patient-individualised prognostication and prediction of treatment response from chemotherapy is useful but little evidence exists to guide practice. Method: Consecutive patients with MPM who were fit for first-line chemotherapy with pemetrexed and cisplatin\carboplatin were recruited and followed up for a minimum of 12 months. This study focussed on the HRQoL outcomes of these patients using the EQ-5D, EORTC QLQ-C30 and LC13. Results: Seventy-three patients were recruited of which 58 received chemotherapy and 15 opted for best supportive care (BSC). Compliance with HRQoL questionnaires was 98% at baseline. The chemotherapy group maintained HRQoL compared with the BSC group whose overall HRQoL fell (P=0.006) with worsening dyspnoea and pain. The impact of chemotherapy was irrespective of histological subtype although those with non-epithelioid disease had worse HRQoL at later time points (P=0.012). Additionally, those with a falling mesothelin or improvement on modified-RECIST CT at early follow-up had a better HRQoL at 16 weeks. Conclusions: HRQoL was maintained following chemotherapy compared with a self-selected BSC group. Once chemotherapy is initiated, a falling mesothelin or improved RECIST CT findings infer a quality-of-life advantage

    Selective Exoenzymatic Labeling of Lipooligosaccharides of Neisseria gonorrhoeae with α2,6-Sialoside Analogues

    The interactions between bacteria and their host often rely on recognition processes that involve host or bacterial glycans. Glycoengineering techniques make it possible to modify and study the glycans on the host's eukaryotic cells, but only a few are available for the study of bacterial glycans. Here, we have adapted selective exoenzymatic labeling (SEEL), a chemical reporter strategy, to label the lipooligosaccharides of the bacterial pathogen Neisseria gonorrhoeae, using the recombinant glycosyltransferase ST6Gal1, and three synthetic CMP-sialic acid derivatives. We show that SEEL treatment does not affect cell viability and can introduce an α2,6-linked sialic acid with a reporter group on the lipooligosaccharides by Western blot, flow cytometry and fluorescent microscopy. This new bacterial glycoengineering technique allows for the precise modification, here with α2,6-sialoside derivatives, and direct detection of specific surface glycans on live bacteria, which will aid in further unravelling the precise biological functions of bacterial glycans

    Sustainable development discourse in Smart Specialization Strategies

    Smart Specialization Strategies have been used to implement European Union Cohesion Policy in recent years, aiming to achieve economic, social and territorial development by reducing disparities between regions. The European Union often declares an alignment between Cohesion Policy, Smart Specialization Strategies and the United Nations Agenda 2030. However, and despite the recurrent Sustainable Development discourse present in the European political agenda, it is not evident that Smart Specialization Strategies integrate Sustainable Development goals and practices. Hence, this paper aims to explore and discuss the degree of embeddedness of the Sustainable Development discourse in the Research and Innovation Strategies for Smart Specialization, using the Portuguese Centro Region as an exploratory study. In this research a content analysis of the Centro Region Smart Specialization Strategy was used for data collection and analysis, framed by the 17 goals of Agenda 2030 and their targets. Results show a good embeddedness of the overall goals of Agenda 2030, but a lower integration of the targets, suggesting that this strategy needs to be revisited to further integrate Agenda 2030. Moreover, the Centro Smart Specialization Strategy has a strong alignment with the goals related with economic and environmental issues, almost disregarding social aspects. These findings are amplified by the fact that this strategy is mainly being implemented through projects financed under the economy pillar. Finally, there is evidence about the need of developing regional innovation policies towards sustainable development from the regional to the national level. Overall, it is critical to enlarge the debate on how Smart Specialization Strategies can be an effective engine for a new development trajectory for European regions assuming the Agenda 2030 as a framework for the necessary change.publishe
