Robotic-Assisted Surgery Improves the Quality of Total Mesorectal Excision for Rectal Cancer Compared to Laparoscopy: Results of a Case-Controlled Analysis.
BACKGROUND: The use of a robotic surgical system is claimed to allow precise traction and counter-traction, especially in a narrow pelvis. Whether this translates to improvement of the quality of the resected specimen is not yet clear. The aim of the study was to compare the quality of the TME and the short-term oncological outcome between robotic and laparoscopic rectal cancer resections.
METHODS: 20 consecutive robotic TME performed in a single institution for rectal cancer (Rob group) were matched 1:2 to 40 laparoscopic resections (Lap group) for gender, body mass index (BMI), and distance from anal verge on rigid proctoscopy. The quality of TME was assessed by 2 blinded and independent pathologists and reported according to international standardized guidelines.
RESULTS: Both samples were well matched for gender, BMI (median 25.9 vs. 24.2 kg/m(2), p = 0.24), and level of the tumor (4.1 vs. 4.8 cm, p = 0.20). The quality of the TME was better in the Robotic group (complete TME: 95 vs. 55 %; p = 0.0003, nearly complete TME 5 vs. 37 %; p = 0.04, incomplete TME 0 vs. 8 %, p = 0.09). A trend for lower positive circumferential margin was observed in the Robotic group (10 vs. 25 %, p = 0.1).
CONCLUSIONS: These results suggest that robotic-assisted surgery improves the quality of TME for rectal cancer. Whether this translates to better oncological outcome needs to be further investigated
End-colostomy diverticulitis with parastomal phlegmon: A case report.
Acute colonic diverticulitis is a well-known surgical emergency, which occurs in about 10 percent of patients known for diverticulosis.
The case of a 77-year-old woman is reported, with past history of abdominoperineal resection with end-colostomy for low rectal adenocarcinoma, and who developed an acute colonic diverticulitis in a subcutaneous portion of colostomy with parastomal phlegmon.
Initial computed tomography imaging demonstrated a significant submucosal parietal edema with local fat tissues infiltration in regard of 3 diverticula.
A two-step treatment was decided: first a nonoperative treatment was initiated with 2 weeks antibiotics administration, followed by, 6 weeks after, a segmental resection of the terminal portion of the colon with redo of a new colostomy by direct open approach.
Patient was discharged on the second postoperative day without complications. Follow-up at 2 weeks revealed centimetric dehiscence of the stoma, which was managed conservatively until sixth postoperative week by stomatherapists.
Treatment of acute diverticulitis with parastomal phlegmon in a patient with end-colostomy could primary be nonoperative. Delayed surgical treatment with segmental colonic resection was proposed to avoid recurrence and potential associated complications

The impact of discrete modes of spinal cord injury on bladder muscle contractility
Background: Prior studies have compared the effect of spinal cord injury elicited using distinct approaches on motor and visceral function. However, the impact of such discrete modes of injury specifically on bladder muscle contractility has not been explored in detail. The goal of this study is to compare the impact of complete spinal cord transection versus clip compression at thoracic vertebra eight (T8) on bladder muscle contractility. Methods: Rats underwent no treatment (Control), laminectomy (Sham, SH); complete extradural transection (TX); or cord compression with an aneurysm clip (CX). Bladders and spinal cords were harvested at 6 wk for contractility studies or histological analysis. Results: Detrusor strips from TX and CX rats showed higher spontaneous activity than those from SH rats. Furthermore, the duration of the neurally-mediated contractile response was longer in TX and CX rats compared to controls and showed attenuated relaxation. No significant differences were observed between muscle strips from SH, TX or CX rats in response to KCl, ATP or phenylephrine. However, tissues from TX and CX rats showed a higher sensitivity to carbachol compared to that from SH animals. Conclusions: Complete SCI in rats either by cord transection or compression elicits qualitatively similar changes in bladder muscle contractility. Whereas cord transection is arguably easier to perform experimentally, cord compression better models the situation observed clinically, such that each approach has clear advantages and limitations
Encoding simplicial quantum geometry in group field theories
We show that a new symmetry requirement on the GFT field, in the context of
an extended GFT formalism, involving both Lie algebra and group elements,
leads, in 3d, to Feynman amplitudes with a simplicial path integral form based
on the Regge action, to a proper relation between the discrete connection and
the triad vectors appearing in it, and to a much more satisfactory and
transparent encoding of simplicial geometry already at the level of the GFT

Effective Hamiltonian Constraint from Group Field Theory
Spinfoam models provide a covariant formulation of the dynamics of loop
quantum gravity. They are non-perturbatively defined in the group field theory
(GFT) framework: the GFT partition function defines the sum of spinfoam
transition amplitudes over all possible (discretized) geometries and
topologies. The issue remains, however, of explicitly relating the specific
form of the group field theory action and the canonical Hamiltonian constraint.
Here, we suggest an avenue for addressing this issue. Our strategy is to expand
group field theories around non-trivial classical solutions and to interpret
the induced quadratic kinematical term as defining a Hamiltonian constraint on
the group field and thus on spin network wave functions. We apply our procedure
to Boulatov group field theory for 3d Riemannian gravity. Finally, we discuss
the relevance of understanding the spectrum of this Hamiltonian operator for
the renormalization of group field theories.Comment: 14 page

Group field theory and simplicial quantum gravity
We present a new Group Field Theory for 4d quantum gravity. It incorporates
the constraints that give gravity from BF theory, and has quantum amplitudes
with the explicit form of simplicial path integrals for 1st order gravity. The
geometric interpretation of the variables and of the contributions to the
quantum amplitudes is manifest. This allows a direct link with other simplicial
gravity approaches, like quantum Regge calculus, in the form of the amplitudes
of the model, and dynamical triangulations, which we show to correspond to a

Strange particle production in proton-proton collisions at TeV with ALICE at the LHC
The production of mesons containing strange quarks (K, ) and both
singly and doubly strange baryons (, Anti-, and
+Anti-) are measured at central rapidity in pp collisions at
= 0.9 TeV with the ALICE experiment at the LHC. The results are
obtained from the analysis of about 250 k minimum bias events recorded in 2009.
Measurements of yields (dN/dy) and transverse momentum spectra at central
rapidities for inelastic pp collisions are presented. For mesons, we report
yields () of 0.184 0.002 stat. 0.006 syst. for K and
0.021 0.004 stat. 0.003 syst. for . For baryons, we find
= 0.048 0.001 stat. 0.004 syst. for , 0.047
0.002 stat. 0.005 syst. for Anti- and 0.0101 0.0020 stat.
0.0009 syst. for +Anti-. The results are also compared with
predictions for identified particle spectra from QCD-inspired models and
provide a baseline for comparisons with both future pp measurements at higher

Two-pion Bose-Einstein correlations in central Pb-Pb collisions at = 2.76 TeV
The first measurement of two-pion Bose-Einstein correlations in central Pb-Pb
collisions at TeV at the Large Hadron Collider is
presented. We observe a growing trend with energy now not only for the
longitudinal and the outward but also for the sideward pion source radius. The
pion homogeneity volume and the decoupling time are significantly larger than
