    Isolation and characterisation of human gingival margin-derived STRO-1/MACS+ and MACS− cell populations

    Recently, gingival margin-derived stem/progenitor cells isolated via STRO-1/magnetic activated cell sorting (MACS) showed remarkable periodontal regenerative potential in vivo. As a second-stage investigation, the present study's aim was to perform in vitro characterisation and comparison of the stem/progenitor cell characteristics of sorted STRO-1-positive (MACS+) and STRO-1-negative (MACS−) cell populations from the human free gingival margin. Cells were isolated from the free gingiva using a minimally invasive technique and were magnetically sorted using anti-STRO-1 antibodies. Subsequently, the MACS+ and MACS− cell fractions were characterized by flow cytometry for expression of CD14, CD34, CD45, CD73, CD90, CD105, CD146/MUC18 and STRO-1. Colony-forming unit (CFU) and multilineage differentiation potential were assayed for both cell fractions. Mineralisation marker expression was examined using real-time polymerase chain reaction (PCR). MACS+ and MACS− cell fractions showed plastic adherence. MACS+ cells, in contrast to MACS− cells, showed all of the predefined mesenchymal stem/progenitor cell characteristics and a significantly higher number of CFUs (P<0.01). More than 95% of MACS+ cells expressed CD105, CD90 and CD73; lacked the haematopoietic markers CD45, CD34 and CD14, and expressed STRO-1 and CD146/MUC18. MACS− cells showed a different surface marker expression profile, with almost no expression of CD14 or STRO-1, and more than 95% of these cells expressed CD73, CD90 and CD146/MUC18, as well as the haematopoietic markers CD34 and CD45 and CD105. MACS+ cells could be differentiated along osteoblastic, adipocytic and chondroblastic lineages. In contrast, MACS− cells demonstrated slight osteogenic potential. Unstimulated MACS+ cells showed significantly higher expression of collagen I (P<0.05) and collagen III (P<0.01), whereas MACS− cells demonstrated higher expression of osteonectin (P<0.05; Mann–Whitney). The present study is the first to compare gingival MACS+ and MACS− cell populations demonstrating that MACS+ cells, in contrast to MACS− cells, harbour stem/progenitor cell characteristics. This study also validates the effectiveness of the STRO-1/MACS+ technique for the isolation of gingival stem/progenitor cells.     The discontinuous nature of chromospheric activity evolution

    Chromospheric activity has been thought to decay smoothly with time and, hence, to be a viable age indicator.     Circulating Cell-Free DNA in Dogs with Mammary Tumors: Short and Long Fragments and Integrity Index

    Circulating cell-free DNA (cfDNA) has been considered an interesting diagnostic/prognostic plasma biomarker in tumor-bearing subjects. In cancer patients, cfDNA can hypothetically derive from tumor necrosis/apoptosis, lysed circulating cells, and some yet unrevealed mechanisms of active release. This study aimed to preliminarily analyze cfDNA in dogs with canine mammary tumors (CMTs). Forty-four neoplastic, 17 non-neoplastic disease-bearing, and 15 healthy dogs were recruited. Necrosis and apoptosis were also assessed as potential source of cfDNA on 78 CMTs diagnosed from the 44 dogs. The cfDNA fragments and integrity index significantly differentiated neoplastic versus non-neoplastic dogs (P<0.05), and allowed the distinction between benign and malignant lesions (P<0.05). Even if without statistical significance, the amount of cfDNA was also affected by tumor necrosis and correlated with tumor size and apoptotic markers expression. A significant (P<0.01) increase of Bcl-2 in malignant tumors was observed, and in metastatic CMTs the evasion of apoptosis was also suggested. This study, therefore, provides evidence that cfDNA could be a diagnostic marker in dogs carrying mammary nodules suggesting that its potential application in early diagnostic procedures should be further investigated

    Posterior Reversible Encephalopathy Syndrome in a Child with Steroid Sensitive Nephrotic Syndrome

    Introduction: Posterior reversible encephalopathy syndrome (PRES) was described in adults more than children. It was reported in cases of nephrotic syndrome which were mainly on immunosuppressant medicationsor had severe hypertension.Case report: We report the case of a 9-years old girl who presented with nephrotic syndrome and moderate hypertension. Nine days after the initiation of steroid therapy she developed disturbed level of consciousnessand generalized tonic clonic seizures. Her blood pressure was 145/90 mm Hg and she had normal serum creatinine and electrolytes levels. She was treated with furosemide and convulsions were controlled. After regainingconsciousness, the patient complained of loss of vision. Fundal&#160; examination was normal. Magnetic resonant&#160; imaging, axial FLAIR images and diffusion weighted imaging showed hyper-intensity signal in the parieto-occipital areas. Magnetic resonant arteriography and spectroscopy excluded ischemic insults and neoplastic process. She regained full consciousness and normal vision and was discharged from the ICU four days later. PRES was diagnosed based on the typical pattern of brainimaging and the reversibility of symptoms. .Conclusion: Nephrotic syndrome in children should be considered a risk factor for developing PRES even without the use of immunosuppressant agents or high doses of steroids

    Atmospheric Heating and Wind Acceleration: Results for Cool Evolved Stars based on Proposed Processes

    A chromosphere is a universal attribute of stars of spectral type later than ~F5. Evolved (K and M) giants and supergiants (including the zeta Aurigae binaries) show extended and highly turbulent chromospheres, which develop into slow massive winds. The associated continuous mass loss has a significant impact on stellar evolution, and thence on the chemical evolution of galaxies. Yet despite the fundamental importance of those winds in astrophysics, the question of their origin(s) remains unsolved. What sources heat a chromosphere? What is the role of the chromosphere in the formation of stellar winds? This chapter provides a review of the observational requirements and theoretical approaches for modeling chromospheric heating and the acceleration of winds in single cool, evolved stars and in eclipsing binary stars, including physical models that have recently been proposed.     Evaluation of IR Spectral Analysis and Dyeing Parameters for Plasma and /or Nano-Silver Treatments of Polyester and Nylon Fabrics

    In our work of this paper, we study the effect of surface modification of polyester and nylon fabrics induced by DC plasma discharge and/ or nano-siliver treatments .DC plasma discharge was employed at first, as a function of plasma device parameters including different time, different current and different hydrostatic pressure using chemically inert working gas: argon or nitrogen. Optimization of the performance of the applied DC plasma discharge with various applied conditions were performed using Fourier Transform Infra-Red (FTIR) Spectroscopy spectral analysis, by following up the changes in the peak intensity values of the characteristic functional groups that characterize polyester fabric. Then the dyeing properties of different pretreated fabrics with plasma by the best conditions are subjected to nano-silver treatment by concentration 50 ppm under the effect of different dye concentrations, different dyeing temperature and different dyeing time. Finally, the fastness properties to light and washing for the treated samples were studied. The results obtained showed that both of the dyeing parameters and fastness properties were highly improved by the treatment of fabrics by either individual plasma treatment or combined DC cold plasma and nano-silver treatments

    Surgical site infection after gastrointestinal surgery in high-income, middle-income, and low-income countries: a prospective, international, multicentre cohort study

    Background: Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world. Methods: This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials.gov, number NCT02662231. Findings: Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58·5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31·2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (10·2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12·3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9·4%] of 7339 patients), middle (549 [14·0%] of 3918 patients), and low (298 [23·2%] of 1282) HDI (p < 0·001). The highest SSI incidence in each HDI group was after dirty surgery (102 [17·8%] of 574 patients in high-HDI countries; 74 [31·4%] of 236 patients in middle-HDI countries; 72 [39·8%] of 181 patients in low-HDI countries). Following risk factor adjustment, patients in low-HDI countries were at greatest risk of SSI (adjusted odds ratio 1·60, 95% credible interval 1·05–2·37; p=0·030). 132 (21·6%) of 610 patients with an SSI and a microbiology culture result had an infection that was resistant to the prophylactic antibiotic used. Resistant infections were detected in 49 (16·6%) of 295 patients in high-HDI countries, in 37 (19·8%) of 187 patients in middle-HDI countries, and in 46 (35·9%) of 128 patients in low-HDI countries (p < 0·001). Interpretation: Countries with a low HDI carry a disproportionately greater burden of SSI than countries with a middle or high HDI and might have higher rates of antibiotic resistance. In view of WHO recommendations on SSI prevention that highlight the absence of high-quality interventional research, urgent, pragmatic, randomised trials based in LMICs are needed to assess measures aiming to reduce this preventable complication

    Solar Magnetic Flux Tube Simulations with Time-Dependent Ionization

    In the present work we expand the study of time-dependent ionization previously identified to be of pivotal importance for acoustic waves in solar magnetic flux tube simulations. We focus on longitudinal tube waves (LTW) known to be an important heating agent of solar magnetic regions. Our models also consider new results of wave energy generation as well as an updated determination of the mixing length of convection now identified as 1.8 scale heights in the upper solar convective layers. We present 1-D wave simulations for the solar chromosphere by studying tubes of different spreading as function of height aimed at representing tubes in environments of different magnetic filling factors. Multi-level radiative transfer has been applied to correctly represent the total chromospheric emission function. The effects of time-dependent ionization are significant in all models studied. They are most pronounced behind strong shocks and in low density regions, i.e., the middle and high chromosphere. Concerning our models of different tube spreading, we attained pronounced differences between the various types of models, which were largely initiated by different degrees of dilution of the wave energy flux as well as the density structure partially shaped by strong shocks, if existing. Models showing a quasi-steady rise of temperature with height are obtained via monochromatic waves akin to previous acoustic simulations. However, longitudinal flux tube waves are identified as insufficient to heat the solar transition region and corona in agreement with previous studies.Comment: 13 pages, 9 figures, 4 tables; Mon.     ВИКЛАДАННЯ ДИСЦИПЛІНИ “ОСНОВИ ПСИХОЛОГІЇ”: СУЧАСНИЙ СТАН ТА ПЕРСПЕКТИВИ

    The aim of the work – assessment of the students’ needs in the discipline of Bases of Psychology and the search for ways to restore the teaching of this discipline in the context of the strategy of medical education development in Ukraine. The main body. Ukraine’s medical education has to prepare high-level specialists to provide quality medical care. The list of educational disciplines and competencies is approved by the standards of medical education. The current standard had moved the discipline of Basics of Psychology to students in the field of knowledge Health 22, in specialty of Medicine 222, Stomatology 221 from compulsory disciplines to disciplines that should be selected. The article provides data on the survey of 482 students of the III and IV&nbsp;courses trained in the specialty of Medicine, in particular 113 students – citizens of other countries who studied this discipline according to the previous standard. Most of the polled students (98.5 %) believe that in the higher medical school needs to study the discipline of Basics of Psychology, and 81.3 % of the respondents claim that this discipline should be mandatory. Conclusions. The study concluded that there was a need for a timely revision of the standards of higher education, in particular taking into account the views of students, and proposed to return the discipline of Basics of Psychology to compulsory disciplines.Мета роботи – оцінка потреби студентів у викладанні дисципліни “Основи психології” та пошук шляхів відновлення викладання цієї дисципліни в розрізі стратегії розвитку медичної освіти в Україні. Основна частина. Медична освіта України має готувати фахівців високого рівня заради забезпечення громадян якісною медичною допомогою. Перелік навчальних дисциплін і, відповідно, компетентностей затверджується стандартами медичної освіти. Діючий наразі стандарт перевів дисципліну “Основи психології” для студентів у галузі знань 22 “Охорона здоров’я”, за фахом 222 “Медицина”, 221 “Стоматологія” з обов’язкової до дисциплін за вибором. У статті наводяться дані опитування 482 студентів ІІІ та ІV курсів, які навчаються за спеціальністю “Лікувальна справа”, зокрема 113 студентів – громадян інших країн, які вивчали цю дисципліну ще за попереднім стандартом. Переважна більшість опитаних (98,5 %) вважає, що у вищому медичному навчальному закладі потрібно вивчати дисципліну “Основи психології”, а 81,3 % опитаних стверджують, що ця дисципліна має бути обов’язковою до вивчення. Висновок. За результатами дослідження зроблено висновок про необхідність вчасного перегляду стандартів вищої освіти, зокре­ма з урахуванням думок студентів, та запропоновано повернути дисципліну “Основи психології” до обов’язкових дисциплін

    Validating the predictive ability of the 2MACE score for major adverse cardiovascular events in patients with atrial fibrillation: results from phase II/III of the GLORIA-AF registry

    The 2MACE score was specifically developed as a risk-stratification tool in atrial fibrillation (AF) to predict cardiovascular outcomes. We evaluated the predictive ability of the 2MACE score in the GLORIA-AF registry. All eligible patients from phase II/III of the prospective global GLORIA-AF registry were included. Major adverse cardiac events (MACEs) were defined as the composite outcome of stroke, myocardial infarction and cardiovascular death. Cox proportional hazards were used to examine the relationship between the 2MACE score and study outcomes. Predictive capability of the 2MACE score was investigated using receiver-operating characteristic curves. A total of 25,696 patients were included (mean age 71 years, female 44.9%). Over 3 years, 1583 MACEs were recorded. Patients who had MACE were older, with more cardiovascular risk factors and were less likely to be managed using a rhythm-control strategy. The median 2MACE score in the MACE and non-MACE groups were 2 (IQR 1–3) and 1 (IQR 0–2), respectively (p &lt; 0.001). The 2MACE score was positively associated with an increase in the risk of MACE, with a score of ≥ 2 providing the best combination of sensitivity (69.6%) and specificity (51.6%), HR 2.47 (95% CI, 2.21–2.77). The 2MACE score had modest predictive performance for MACE in patients with AF (AUC 0.655 (95% CI, 0.641–0.669)). Our analysis in this prospective global registry demonstrates that the 2MACE score can adequately predict the risk of MACE (defined as myocardial infarction, CV death and stroke) in patients with AF.
