    Evolucion de la enzima ACC oxidasa durante almacenaje de manzanas y peras

    Resumen (Spanish, English)84 p.Frutos de dos cvs. de manzanas (Braeburn y Fuji) y peras (Packham´s Triumph y Beurre Bosc) fueron cosechados en dos épocas de cosecha (temprana y tardía) y almacenados en frío convencional (FC) y atmósfera controlada (AC) durante seis meses en cámaras pertenecientes al Centro de Pomáceas de la Universidad de Talca. Se utilizó FC y dos condiciones de AC para manzanas, mientras que para peras fueron FC y una AC. Frutos de cada especie y variedad fueron removidos cada mes de almacenaje para evaluar sus índices de madurez (TPE ul/kg.h; CIE ppm; Firmeza Lb.; S.Solubles ºBrix y Almidón) y la actividad de la enzima ACC oxidasa, responsable de la biosíntesis de etileno. Las evaluaciones se realizaron a salida de almacenaje y luego de 7 y 15 días a Tº ambiente para manzanas y de 3 días para peras, como simulación del periodo de comercialización. Los resultados arrojaron que en el caso de manzanas los cultivares Braeburn y Fuji producen su alza climatérica en almacenaje y en periodo de comercialización, presentando alza en la TPE y CIE, disminución en firmeza, principalmente en FC y E2 de cosecha. Además se correlacionó positivamente la actividad de la enzima ACC oxidada con los índices de madurez TPE y CIE, encontrándose que la mayor actividad de la enzima coincide con el alza de TPE y disminución de firmeza, para ambas variedades de manzanas, efecto más marcado para condición FC. En el caso de los cultivares de peras, mostraron que el uso de AC retrasa la entrada al climacterio de la fruta, mientras que en FC pierden calidad en almacenaje y de manera acelerada durante periodo de comercialización. Se incluye también en el presente trabajo, tratamientos en manzanas cv. Braeburn y peras cv. Packham´s Triumph con etileno exógeno (Ethrel) y un inhibidor de la acción del etileno (1-MCP). Se observó que tanto en manzanas como peras se puede adelantar en 1 día el alza climatérica en peras y 2 días en manzanas con el uso de Ethrel, con respecto a Control (No tratada), mientras que el inhibidor de la acción del etileno (1MCP) retrasa el climacterio en ambas especies por periodo de tiempo mayor al utilizado en comercialización (17 días en peras y 29 días en manzanas)

    Increased quality of life in patients with breakthrough cancer pain after individualized therapy : the CAVIDIOM study

    Aim: To evaluate the quality of life (QoL) in patients with breakthrough cancer pain (BTcP) in Spanish medical oncology departments. Patients & methods: In a prospective, observational, multicenter study, we assessed QoL using the EQ-5D-5L instrument at baseline and after 15 and 30 days of individualized BTcP therapy, as well as BTcP characteristics and treatment. Results: Patients (n = 118) were mainly women, over 64 years old and with advanced cancer. QoL improved at 15 (p = 0.013) and 30 days (p = 0.011) versus baseline. Individualized BTcP therapy consisted mostly of rapid-onset opioids (transmucosal fentanyl at doses of 67-800 μg) according to the physician evaluation. BTcP improved, including statistically significant reductions in intensity, duration, number of episodes in the last 24 h and time to onset of BTcP relief. Conclusion: QoL increased after individualized pain therapy in patients with advanced cancer and BTcP in medical oncology departments.     Structure, Process, and Mortality Associated with Acute Coronary Syndrome Management in Guatemala’s National Healthcare System: The ACS-GT Registry

    Background: Acute coronary syndromes (ACS) include ST-segment elevation myocardial infarction (STEMI), non-ST-segment elevation myocardial infarction (NSTEMI), and unstable angina (UA). The leading cause of mortality in Guatemala is acute myocardial infarction (AMI) and there is no established national policy nor current standard of care. Objective: Describe the factors that influence ACS outcome, evaluating the national healthcare system’s quality of care based on the Donabedian health model. Methods: The ACS-Gt study is an observational, multicentre, and prospective national registry. A total of 109 ACS adult patients admitted at six hospitals from Guatemala’s National Healthcare System were included. These represent six out of the country’s eight geographic regions. Data enrolment took place from February 2020 to January 2021. Data was assessed using chi-square test, Student’s t-test, or Mann-Whitney U test, whichever applied. A p-value < 0.05 was considered statistically significant. Results: One hundred and nine patients met inclusion criteria (80.7% STEMI, 19.3% NSTEMI/UA). The population was predominantly male, (68%) hypertensive (49.5%), and diabetic (45.9%). Fifty-nine percent of STEMI patients received fibrinolysis (alteplase 65.4%) and none for primary Percutaneous Coronary Intervention (pPCI). Reperfusion success rate was 65%, and none were taken to PCI afterwards in the recommended time period (2–24 hours). Prognostic delays in STEMI were significantly prolonged in comparison with European guidelines goals. Optimal in-hospital medical therapy was 8.3%, and in-hospital mortality was 20.4%. Conclusions: There is poor access to ACS pharmacological treatment, low reperfusion rate, and no primary, urgent, or rescue PCI available. No patient fulfilled the recommended time period between successful fibrinolysis and PCI. Resources are limited and inefficiently used

    Evolving trends in the management of acute appendicitis during COVID-19 waves. The ACIE appy II study

    Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide

    A identidade maya-poqomchi’ guatemalteca em suas manifestações espirituais e religiosas

    Guatemala es un país caracterizado por una diversidad cultural considerable. Entre los más de veinte grupos mayas que pueblan su geografía, los poqomchi´es que habitan en Alta y Baja Verapaz han mantenido gran parte de su idiosincrasia particular, heredada de los antiguos mayas. El principal objeto de este artículo es analizar los aspectos más destacados de la espiritualidad y religiosidad poqomchi´, como herencia del pueblo maya del que proceden y producto tanto de la interacción con otros colectivos mayas cercanos como del sincretismo como respuesta de supervivencia. Para ello, nos basaremos en la observación sistemática y participante y la entrevista, para llegar a la conclusión de cómo aún en la actualidad los principales rasgos de la espiritualidad poqomchi´ continúan vigentes, no exentos de la influencia de los pueblos con los que comparte espacio.Guatemala is a country of considerable cultural diversity. Among the more than twenty Mayan groups that populate its territory, the Poqomchi´ who inhabit Alta and Baja Verapaz have preserved a great deal of their particular idiosyncrasy inherited from the ancient Maya. The main purpose of this article is to analyze the most important aspects of Poqomchi´ spirituality and religiosity as the legacy of the ancient Maya from whom they descend, as well as a product both of their constant interaction with other Mayan groups and syncretism as a survival mechanism. For this purpose, it is based on systematic participant observation and interviews which have led to conclude that the main features of Poqomchi´ spirituality remain alive even today, although not free from the influence of the peoples with whom they share space.A Guatemala é um país caracterizado por uma diversidade cultural considerável.     Taking the pulse of Earth's tropical forests using networks of highly distributed plots

    Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science.
