    Disforía de género en la infancia y adolescencia: Un reto para la enfermería

    Trabajo fin de grado en EnfermeríaIntroducción: La disforia de género en la infancia se describe como una marcada incongruencia entre el sexo que uno siente o expresa y el que se le asigna, de una duración mínima de 6 meses, y teniendo su inicio entre los 2 a 3 años de edad. Objetivos: Identificar las necesidades y barreras de los niños/adolescentes con disforia de género y sus familias, así como el papel de enfermería, los cuidados enfermeros, intervenciones y recursos disponibles para lograr una transición sana. Metodología: Se ha llevado a cabo una revisión narrativa mediante la búsqueda bibliográfica en las bases de datos PubMed, PsycInfo, CINAHL, Dialnet, Cuiden, Cochrane y Google Schoolar. Resultados: Se identificaron un total de 114 artículos con títulos relacionados tras realizar la búsqueda en las diferentes bases de datos. Finalmente, fueron seleccionados 20 artículos para la realización de la revisión narrativa identificándose grupos de contenido a través de los cuáles se ha intentado dar respuesta a los objetivos planteados. La valoración enfermera, como pilar fundamental en la identificación precoz; las diversas barreras estructurales, financieras o actitudinales del profesional de la salud con las que se encuentran estos niños y adolescentes para poder acceder al sistema sanitario; la importancia de detectar las necesidades sentidas de ellos y las de sus padres con acompañamiento y soporte para poder dar una calidad máxima en los cuidados enfermeros teniendo en cuenta la disponibilidad de los recursos con los que se cuenta. Conclusiones: El papel de enfermería es fundamental en el acompañamiento a lo largo de todo el proceso de transición social a realizar por los menores, destacando su papel en la educación para la salud y asesoramiento a los padres, así como la colaboración con otros profesionales.Introduction: Gender dysphoria in childhood is described as a marked incongruence between sex one feels or expresses, and which is assigned, for a minimum duration of 6 months and having its onset between 2-3 years old. Nursing assessment, as a fundamental pillar in the early identification; the various structural, financial or attitudinal health professional with these children and adolescents are to access the health system barriers; the importance of identifying the perceived needs of them and their parents with guidance and support to give maximum quality nursing care taking into account the availability of resources with which it counts.     This report has been developed in the context of the project BRIGHTER FUTURE: Innovative tools for developing full potential after early adversity, whose working team consists of the following entities: Comune di Torino (Italy), CORA (Spain), PAC UK/ Family Action (United Kingdom), Pharos Expertise Center on Health Disparities (Netherlands), Universitat Autònoma de Barcelona (Spain), Università di Verona (Italy), University of Groningen (Netherlands). This report aims to identify and raise awareness about the specific challenges children face in school environments so that those challenges can be appropriately addressed. To that end, the team of the Erasmus+ project "BRIGHTER FUTURE: Innovative tools for developing full potential after early adversity" has contrasted what it is known from research with the experiences of stakeholders: youth who were under state guardianship in their childhood, adoptive and foster families, social workers who work in the child protection system, NGOs that work with families and unaccompanied migrant children, and teachers

    Metoprolol in Critically Ill Patients With COVID-19.

    Severe coronavirus disease-2019 (COVID-19) can progress to an acute respiratory distress syndrome (ARDS), which involves alveolar infiltration by activated neutrophils. The beta-blocker metoprolol has been shown to ameliorate exacerbated inflammation in the myocardial infarction setting. The purpose of this study was to evaluate the effects of metoprolol on alveolar inflammation and on respiratory function in patients with COVID-19-associated ARDS. A total of 20 COVID-19 patients with ARDS on invasive mechanical ventilation were randomized to metoprolol (15 mg daily for 3 days) or control (no treatment). All patients underwent bronchoalveolar lavage (BAL) before and after metoprolol/control. The safety of metoprolol administration was evaluated by invasive hemodynamic and electrocardiogram monitoring and echocardiography. Metoprolol administration was without side effects. At baseline, neutrophil content in BAL did not differ between groups. Conversely, patients randomized to metoprolol had significantly fewer neutrophils in BAL on day 4 (median: 14.3 neutrophils/µl [Q1, Q3: 4.63, 265 neutrophils/µl] vs median: 397 neutrophils/µl [Q1, Q3: 222, 1,346 neutrophils/µl] in the metoprolol and control groups, respectively; P = 0.016). Metoprolol also reduced neutrophil extracellular traps content and other markers of lung inflammation. Oxygenation (PaO2:FiO2) significantly improved after 3 days of metoprolol treatment (median: 130 [Q1, Q3: 110, 162] vs median: 267 [Q1, Q3: 199, 298] at baseline and day 4, respectively; P = 0.003), whereas it remained unchanged in control subjects. Metoprolol-treated patients spent fewer days on invasive mechanical ventilation than those in the control group (15.5 ± 7.6 vs 21.9 ± 12.6 days; P = 0.17). In this pilot trial, intravenous metoprolol administration to patients with COVID-19-associated ARDS was safe, reduced exacerbated lung inflammation, and improved oxygenation. Repurposing metoprolol for COVID-19-associated ARDS appears to be a safe and inexpensive strategy that can alleviate the burden of the COVID-19 pandemic.Mr Clemente-Moragón is supported by a fellowship from the Ministerio de Ciencia e Innovación (FPU2017/01932).     Stakeholders' perspectives on the operationalisation of the ecosystem service concept : Results from 27 case studies

    The ecosystem service (ES) concept is becoming mainstream in policy and planning, but operational influence on practice is seldom reported. Here, we report the practitioners' perspectives on the practical implementation of the ES concept in 27 case studies. A standardised anonymous survey (n = 246), was used, focusing on the science-practice interaction process, perceived impact and expected use of the case study assessments. Operationalisation of the concept was shown to achieve a gradual change in practices: 13% of the case studies reported a change in action (e.g. management or policy change), and a further 40% anticipated that a change would result from the work. To a large extent the impact was attributed to a well conducted science-practice interaction process (>70%). The main reported advantages of the concept included: increased concept awareness and communication; enhanced participation and collaboration; production of comprehensive science-based knowledge; and production of spatially referenced knowledge for input to planning (91% indicated they had acquired new knowledge). The limitations were mostly case-specific and centred on methodology, data, and challenges with result implementation. The survey highlighted the crucial role of communication, participation and collaboration across different stakeholders, to implement the ES concept and enhance the democratisation of nature and landscape planning.
