Cultural Adaptation and Linguistic Validation of the Beliefs about Medicines Questionnaire in Malaysia
BACKGROUND: Low rate of adherence was found strongly associated with patients' beliefs across the studies about chronic diseases with hypertension. A crucial move is needed to bridge the gap between appropriate assessment tools and local hypertensive patients' medication adherence. OBJECTIVE: To produce a translated version in Malay language of Beliefs about Medicines Questionnaire (BMQ) that was "conceptually equivalent" to the original English version for use in local clinical practice and research. METHODS: The forward translation process was conducted by two independent professional translators and back translation was done by two other independent translators. A reliability analysis was conducted on 238 conveniently selected hypertensive patients. Intraclass correlation coefficient (ICC) was used to assess test-retest reliability for the randomly selected 40 patients in a period of 2 weeks. Discriminant validity was tested through Necessity-Concerns differential, BMQ subscales, and other parameters. RESULTS: The overall Cronbach alpha for the internal consistency was good (0.860). The subscales of the BMQ demonstrated adequate internal consistency, with Cronbach alpha value of 0.759 for Specific-Necessity, 0.762 for Specific Concern, 0.624 for General-Overuse, and 0.756 for General-Harm. The ICC was excellent (0.922). Discriminant validity revealed that BMQ Specific-Necessity score was significantly inversely correlated with the systolic blood pressure level. Systolic and diastolic blood pressure levels (P = 0.038; P = 0.05) were reported to be significantly correlated with the Necessity-Concerns differential, with Necessity score equal or exceeding Concerns score. CONCLUSIONS: The Malay-translated version of BMQ is a reliable and valid tool to assess patient belief about medication, especially medication adherence among the hypertensive patients in Malaysia
Asian âGuanâ Parenting and Life Satisfaction Among Adolescents in Malaysia: The Mediating Role of Filial Piety
Literature on adolescent development has shown that parenting practices have positive relationships with adolescentsâ life satisfaction. Adolescentsâ life satisfaction improves when they have parents low in psychological control who uphold reciprocal self-disclosure in their communication. Guan parenting was found to correlate positively with adolescentsâ development. Therefore, it is methodologically important to replicate the investigation on the relationship between adolescentsâ life satisfaction and Guan parenting. Literature suggests that filial piety is shaped by parenting practices and adolescents who perceived intense parental concern, care, and involvement tend to uphold filial piety and express gratitude toward parents which may promote the adolescentsâ life satisfaction. In this study, mediation analysis was done to elucidate the relationship among parentsâ guan parenting style, filial piety, and life satisfaction on 606 adolescents (Mage=15.07; SDage=1.03; 52.1% females) in Malaysia. The adolescents were sampled through cluster sampling, and data were collected using self-administered questionnaires. The results showed positive relationship between paternal and maternal guan parenting with filial piety and adolescentsâ life satisfaction. Greater parentsâ filial piety was linked to higher life satisfaction among adolescents. Findings from the mediation models indicated the association among guan parenting with filial piety, gratitude toward parents, and higher life satisfaction. The findings also offered empirical evidence to the underlying mechanism of how guan parenting could affect adolescent life satisfaction via the mediating role of filial piety. The findings also supported the importance of culture-infused parenting in inculcating adolescentsâ filial piety besides establishing its link to life satisfaction in Asian families
Psychometric Properties of the Training Parenting Style Scale in a Malaysian Sample of Adolescents: Factor Analysis, Internal Consistency, and Measurement Invariance
The importance of parenting styles on childrenâs outcomes, including cognitive, social, academic, and values makes this topic a central concern to social researchers and psychologists. However, past research has reported controversial evidence on the relationship between authoritarian parenting and childrenâs outcomes in non-Western cultural contexts. This raises awareness on the implication of cultural differences in parenting styles. As a result, the training parenting style scale (TPSS) was proposed based on the Confucian concept of âGuanâ and âChiao Shu.â This scale is allegedly more reflective of the Asian parenting style. The present study examined the psychometric properties and measurement invariance of the Malay version of the TPSS across adolescentsâ perceived maternal and paternal training and by adolescent gender. Of the 8 items in the original TPSS, confirmatory factor analysis supported 6-item scale with error correlations was the best-fitting model. Internal consistency was also good for the 6-item scale. Furthermore, support for configural, metric, scalar, residual, and structural invariance emerged across adolescentsâ perceived maternal and paternal training and across adolescent gender. Results of this study supported the psychometric properties of the 6-item TPSS after taking into account several cautiously considered limitations
The impact of meridian balance method electro-acupuncture treatment on chronic pelvic pain in women: a three-armed randomised controlled feasibility study using a mixed methods approach

Chronic pelvic pain (CPP) is estimated to affect 6%â27% of women worldwide. In the United Kingdom, over 1âmillion women suffer from CPP and it has been highlighted as a key area of unmet need. Standard treatments are associated with unacceptable side effects. The meridian balance method electro-acupuncture (BMEA), and traditional Chinese medicine health consultation (TCM HC) (BMEAâ+âTCM HCâ=âBMEA treatment) may be an effective adjunct to standard treatment.\ud

The aim of our study was to evaluate the feasibility of a future trial, to determine the effectiveness of the BMEA treatment for CPP in women. The primary objectives were to determine recruitment and retention rates. The secondary objectives were to assess the effectiveness of the BMEA treatment and acceptability of the studyâs methodology.\ud

Women with CPP were randomised into BMEA treatment (group 1), TCM HC alone (group 2) (each intervention administered twice weekly for 4âweeks) or National Health Service standard care (NHS SC, group 3). Primary outcomes were assessed by the proportion of eligible participants randomised, and the proportion of randomised participants who returned follow-up questionnaires. Interventions were assessed by validated pain/physical/emotional functioning questionnaires at baseline (0), 4, 8 and 12âweeks. Focus groups and semi-structured telephone interviews were embedded in the study.\ud

A total of 30 women (51% of those referred) were randomised over 8âmonths. Retention rates were 80% (95% confidence interval (CI): 74â96), 53% (95% CI: 36â70) and 87% (95% CI: 63â90), in groups 1, 2, and 3, respectively. Qualitative data suggested a favourable trial experience in groups 1 and 3.\ud

No evidence for a common blood microbiome based on a population study of 9,770 healthy humans
Human blood is conventionally considered sterile but recent studies suggest the presence of a blood microbiome in healthy individuals. Here we characterized the DNA signatures of microbes in the blood of 9,770 healthy individuals using sequencing data from multiple cohorts. After filtering for contaminants, we identified 117 microbial species in blood, some of which had DNA signatures of microbial replication. They were primarily commensals associated with the gut (nâ=â40), mouth (nâ=â32) and genitourinary tract (nâ=â18), and were distinct from pathogens detected in hospital blood cultures. No species were detected in 84% of individuals, while the remainder only had a median of one species. Less than 5% of individuals shared the same species, no co-occurrence patterns between different species were observed and no associations between host phenotypes and microbes were found. Overall, these results do not support the hypothesis of a consistent core microbiome endogenous to human blood. Rather, our findings support the transient and sporadic translocation of commensal microbes from other body sites into the bloodstream
Associations of hippocampal subfields in the progression of cognitive decline related to Parkinson's disease.
OBJECTIVE: Hippocampal atrophy has been associated with mild cognitive impairment (MCI) in Parkinson's disease (PD). However, literature on how hippocampal atrophy affects the pathophysiology of cognitive impairment in PD has been limited. Previous studies assessed the hippocampus as an entire entity instead of their individual subregions. We studied the progression of cognitive status in PD subjects over 18 in relation to hippocampal subfields atrophy. METHODS: 65 PD subjects were included. Using the MDS task force criteria, PD subjects were classified as either having no cognitive impairment (PD-NCI) or PD-MCI. We extended the study by investigating the hippocampal subfields atrophy patterns in those who converted from PD-NCI to PD-MCI (PD-converters) compared to those who remained cognitively stable (PD-stable) over 18Â months. Freesurfer 6.0 was used to perform the automated segmentation of the hippocampus into thirteen subregions. RESULTS: PD-MCI showed lower baseline volumes in the left fimbria, right CA1, and right HATA; and lower global cognition scores compared to PD-NCI. Baseline right CA1 was also correlated with baseline attention. Over 18Â months, decline in volumes of CA2-3 and episodic memory were also seen in PD-converters compared to PD-stable. Baseline volumes of GC-DG, right CA4, left parasubiculum, and left HATA were predictive of the conversion from PD-NCI to PD-MCI. CONCLUSION: The findings from this study add to the anatomical knowledge of hippocampal subregions in PD, allowing us to understand the unique functional contribution of each subfield. Structural changes in the hippocampus subfields could be early biomarkers to detect cognitive impairment in PD
The actin-myosin regulatory MRCK kinases: regulation, biological functions and associations with human cancer
The contractile actin-myosin cytoskeleton provides much of the force required for numerous cellular activities such as motility, adhesion, cytokinesis and changes in morphology. Key elements that respond to various signal pathways are the myosin II regulatory light chains (MLC), which participate in actin-myosin contraction by modulating the ATPase activity and consequent contractile force generation mediated by myosin heavy chain heads. Considerable effort has focussed on the role of MLC kinases, and yet the contributions of the myotonic dystrophy-related Cdc42-binding kinases (MRCK) proteins in MLC phosphorylation and cytoskeleton regulation have not been well characterized. In contrast to the closely related ROCK1 and ROCK2 kinases that are regulated by the RhoA and RhoC GTPases, there is relatively little information about the CDC42-regulated MRCKα, MRCKÎČ and MRCKÎł members of the AGC (PKA, PKG and PKC) kinase family. As well as differences in upstream activation pathways, MRCK and ROCK kinases apparently differ in the way that they spatially regulate MLC phosphorylation, which ultimately affects their influence on the organization and dynamics of the actin-myosin cytoskeleton. Silicon Atomic Quantum Dots Enable Beyond-CMOS Electronics
We review our recent efforts in building atom-scale quantum-dot cellular
automata circuits on a silicon surface. Our building block consists of silicon
dangling bond on a H-Si(001) surface, which has been shown to act as a quantum
dot. First the fabrication, experimental imaging, and charging character of the
dangling bond are discussed. We then show how precise assemblies of such dots
can be created to form artificial molecules. Such complex structures can be
used as systems with custom optical properties, circuit elements for
quantum-dot cellular automata, and quantum computing. Considerations on

The impact of hyperhidrosis on patients' daily life and quality of life : A qualitative investigation
Background: An understanding of the daily life impacts of hyperhidrosis and how patients deal with them, based on qualitative research, is lacking. This study investigated the impact of hyperhidrosis on the daily life of patients using a mix of qualitative research methods. Methods: Participants were recruited through hyperhidrosis patient support groups such as the Hyperhidrosis Support Group UK. Data were collected using focus groups, interviews and online surveys. A grounded theory approach was used in the analysis of data transcripts. Data were collected from 71 participants, out of an initial 100 individuals recruited. Results: Seventeen major themes capturing the impacts of hyperhidrosis were identified; these covered all areas of life including daily life, psychological well-being, social life, professional /school life, dealing with hyperhidrosis, unmet health care needs and physical impact. Conclusions: Psychosocial impacts are central to the overall impacts of hyperhidrosis, cutting across and underlying the limitations experienced in other areas of life.Peer reviewe
