    Trends In The Alignment And Harmonization Of Reproductive, Maternal, Newborn, And Child Health Funding, 2008-13.

    Donor financing to low- and middle-income countries for reproductive, maternal, newborn, and child health increased substantially from 2008 to 2013. However, increased spending by donors might not improve outcomes, if funds are delivered in ways that undermine countries' public financial management systems and incur high transaction costs for project implementation. We combined quantitative and qualitative methods to examine the quality of funding for reproductive, maternal, newborn, and child health globally and in Tanzania, based on two principles of aid effectiveness: the alignment of donor financing with the recipient country's public health financial management systems, and donor harmonization for coordinated, transparent, and collectively effective actions. We found that alignment of donor financing deteriorated throughout the period, with the proportion of funds channeled through governments decreasing from 47 percent to 39 percent. Tanzania-based donors attributed the change to the pressure donors were under to achieve and show results. Donor harmonization was low overall and remained relatively constant, although it increased in sub-Saharan Africa and decreased in South Asia. Bilateral funding agencies were the most harmonized donors. We recommend that future assessments of Sustainable Development Goals financing include measures of harmonization and alignment of funding

    Burden on family carers and care-related financial strain at the end of life:A cross-national population-based study

    Background: The rising number of deaths from cancer and other life-limiting illnesses is accompanied by a growing number of family carers who provide long-lasting care, including end-of-life care. This population-based epidemiological study aimed to describe and compare in four European countries the prevalence of and factors associated with physical or emotional overburden and difficulties in covering care-related costs among family carers of people at the end of life. Methods: A cross-national retrospective study was conducted via nationwide representative sentinel networks of general practitioners (GPs). Using a standardized form, GPs in Belgium, The Netherlands, Italy and Spain recorded information on the last 3 months of life of every deceased adult practice patient (1 January 2009-31 December 2010). Sudden deaths were excluded. Results: We studied 4466 deaths. GPs judged family carers of 28% (Belgium), 30% (The Netherlands), 35% (Spain) and 71% (Italy) of patients as physically/emotionally overburdened (P < 0.001). For 8% (Spain), 14% (Belgium), 36% (The Netherlands) and 43% (Italy) patients, GPs reported difficulties in covering care-related costs (P < 0.001). Patients <85 years of age (Belgium, Italy) had higher odds of having physically/emotionally overburdened family carers and financial burden. Death from non-malignant illness (vs. cancer) (Belgium and Italy) and dying at home compared with other locations (The Netherlands and Italy) were associated with higher odds of difficulties in covering care-related costs. Conclusion: In all countries studied, and particularly in Italy, GPs observed a considerable extent of physical/emotional overburden as well as difficulties in covering care-related costs among family carers of people at the end of life. Implications for health- and social care policies are discussed

    15 pagesInternational audienceHabitat destruction due to human land‐use activities is well recognized as a central threat to biodiversity. However, there is still debate about the relative influence of its two components, habitat loss and habitat fragmentation, mostly because few studies have been able to disentangle their respective effects. We studied mechanisms by which habitat destruction might influence the prevalence of vector‐transmitted haemosporidian blood parasites of the genera Plasmodium and Haemoproteus infecting the Lesser Antillean bullfinch Loxigilla noctis on the Caribbean islands of Guadeloupe and Martinique. Starting from a large set of environmental descriptors (including metrics reflecting habitat loss, habitat fragmentation and landscape heterogeneity; as well as other variables not linked to habitat destruction, such as climatic conditions), we used PLS regression analyses to determine which variables were driving parasite prevalence on the islands. Our results showed that variables related to forest destruction were much more influential than other factors for all parasites analyzed on both islands. Remarkably, the effects documented were almost exclusively due to forest fragmentation, as opposed to habitat loss. This positive effect of forest fragmentation on blood parasite prevalence is proposed to happen through its effects on insect vectors and/or host biology. Increased understanding of the role of habitat fragmentation as a driver of parasitic diseases can help limiting the risk of emergence and proliferation of wildlife pathogenic outbreaks and zoonosis through informed landscape planning

    The database of the PREDICTS (Projecting Responses of Ecological Diversity In Changing Terrestrial Systems) project

    The PREDICTS project-Projecting Responses of Ecological Diversity In Changing Terrestrial Systems (www.predicts.org.uk)-has collated from published studies a large, reasonably representative database of comparable samples of biodiversity from multiple sites that differ in the nature or intensity of human impacts relating to land use. We have used this evidence base to develop global and regional statistical models of how local biodiversity responds to these measures. We describe and make freely available this 2016 release of the database, containing more than 3.2 million records sampled at over 26,000 locations and representing over 47,000 species. We outline how the database can help in answering a range of questions in ecology and conservation biology. To our knowledge, this is the largest and most geographically and taxonomically representative database of spatial comparisons of biodiversity that has been collated to date; it will be useful to researchers and international efforts wishing to model and understand the global status of biodiversity
