    RadonPotential: An interactive web application for radon potential prediction under different climates and soil textures

    The presence of radon in soil poses a significant health risk when it enters and concentrates indoors. The primary health problem associated with radon exposure is lung cancer, but it can also contribute to other respiratory issues. Estimating radon potential is a challenging task caused by the interaction of various environmental, geological, and variability factors. Although efforts are ongoing to improve radon potential assessment methodologies, there is a lack of software tools that estimate and model radon potential in different scenarios. The paper aims to develop a novel web-based software tool, RadonPotential, that predicts Geogenic Radon Potential by considering variations in climate and soil textures. The program runs using a constant radon concentration or estimates its concentration from the radium activity. RadonPotential calculates the transport of radon through a soil profile based on water content and soil texture. It also determines the dynamics of soil water content in different climates, incorporating long-term weather data under various climatic scenarios and local weather time series. The web-based format of the program increases its dissemination, usage, and manageability among a larger user base compared to an installable computer program. The program aims to provide a simplified and effective characterization of radon potential levels accessible to a wide range of scientists, technical experts and policymakers in developing strategies not only for radon measurement and mitigation buildings but also for developing more reliable large-scale radon potential maps.Open Access funding provided thanks to the CRUE-CSIC agreement with Springer Nature.     Metatranscriptomic Approach to Analyze the Functional Human Gut Microbiota

    The human gut is the natural habitat for a large and dynamic bacterial community that has a great relevance for health. Metagenomics is increasing our knowledge of gene content as well as of functional and genetic variability in this microbiome. However, little is known about the active bacteria and their function(s) in the gastrointestinal tract. We performed a metatranscriptomic study on ten healthy volunteers to elucidate the active members of the gut microbiome and their functionality under conditions of health. First, the microbial cDNAs obtained from each sample were sequenced using 454 technology. The analysis of 16S transcripts showed the phylogenetic structure of the active microbial community. Lachnospiraceae, Ruminococcaceae, Bacteroidaceae, Prevotellaceae, and Rickenellaceae were the predominant families detected in the active microbiota. The characterization of mRNAs revealed a uniform functional pattern in healthy individuals. The main functional roles of the gut microbiota were carbohydrate metabolism, energy production and synthesis of cellular components. In contrast, housekeeping activities such as amino acid and lipid metabolism were underrepresented in the metatranscriptome. Our results provide new insights into the functionality of the complex gut microbiota in healthy individuals. In this RNA-based survey, we also detected small RNAs, which are important regulatory elements in prokaryotic physiology and pathogenicity

    Nonreferral of Possible Soft Tissue Sarcomas in Adults: A Dangerous Omission in Policy

    Introduction. The aim of this study is to compare outcomes in three groups of STS patients treated in our specialist centre: patients referred immediately after an inadequate initial treatment, patients referred after a local recurrence, and patients referred directly, prior to any treatment. Patients and methods. We reviewed all our nonmetastatic extremity-STS patients with a minimum follow-up of 2 years. We compared three patient groups: those referred directly to our centre (group A), those referred after an inadequate initial excision (group B), and patients with local recurrence (group C). Results. The study included 174 patients. Disease-free survival was 73%, 76%, and 28% in groups A, B, and C, respectively (P < .001). Depth, size, and histologic grade influenced the outcome in groups A and B, but not in C. Conclusion. Initial wide surgical treatment is the main factor that determines local control, being even more important than the known intrinsic prognostic factors of tumour size, depth, and histologic grade. The influence on outcome of initial wide local excision (WLE), which is made possible by referral to a specialist centre, is paramount

    A Semiautomated Classification System for Producing Service Directories in Social and Health Care (DESDE-AND): Maturity Assessment Study

    Background: DESDE-LTC (Description and Evaluation of Services and DirectoriEs for Long-Term Care) is an international classification system that allows standardized coding and comparisons between different territories and care sectors, such as health and social care, in defined geographic areas. We adapted DESDE-LTC into a computer tool (DESDE-AND) for compiling a directory of care services in Andalucia, Spain. Objective: The aim of this study was to evaluate the maturity of DESDE-AND. A secondary objective of this study is to show the practicality of a new combined set of standard evaluation tools for measuring the maturity of health technology products. Methods: A system for semiautomated coding of service provision has been co-designed. A panel of 23 domain experts and a group of 68 end users participated in its maturity assessment that included its technology readiness level (TRL), usability, validity, adoption (Adoption Impact Ladder [AIL]), and overall degree of maturity [implementation maturity model [IMM]). We piloted the prototype in an urban environment (Seville, Spain). Results: The prototype was demonstrated in an operational environment (TRL 7). Sixty-eight different care services were coded, generating fact sheets for each service and its geolocation map. The observed agreement was 90%, with moderate reliability. The tool was partially adopted by the regional government of Andalucia (Spain), reaching a level 5 in adoption (AIL) and a level 4 in maturity (IMM) and is ready for full implementation. Conclusions: DESDE-AND is a usable and manageable system for coding and compiling service directories and it can be used as a core module of decision support systems to guide planning in complex cross-sectoral areas such as combined social and health care

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Análisis del tiempo empleado por el alumnado en la realización de exámenes oficiales

    En la educación universitaria, una parte de las actividades sometidas a evaluación consiste en una prueba escrita, que tiene una duración limitada. La presente red tiene como fin analizar la afección del tiempo disponible y el tipo de prueba en la calificación el alumno. Para ello, se propone una metodología de recogida y análisis de datos. La metodología propuesta se ha aplicado a seis asignaturas del Grado de Ingeniería Civil y del Máster en Ingeniería de Caminos, Canales y Puertos de la Universidad de Alicante. Los resultados se organizan según herramientas estadísticas (diagramas de dispersión y de cajas). El análisis de resultados indica que en los exámenes tipo test no se ha encontrado correlación entre el tiempo de entrega y la calificación. Sin embargo, en los exámenes de problemas sí que aparece cierto patrón. Esta metodología permite la identificación de calificaciones poco razonables de algunos alumnos, además de ser una herramienta que permite una autocrítica del profesorado para establecer el tiempo disponible para la realización de estas pruebas escritas
