Feasibility and acceptability of Lee Silverman Voice Treatment for patients with hereditary ataxia
Objectives: This study aimed to assess the feasibility and acceptability of Lee Silverman Voice Treatment (LSVT) provided via Skype for patients with hereditary ataxia. Background: There are currently no evidence based speech treatments available for people with hereditary ataxia. As a result, these patients are often not offered treatment despite significant impact on communication related quality of life. This study investigated whether LSVT, a speech treatment focusing on increasing loudness, can improve communication in these speakers. Method: 20 patients were recruited to the study. An extended version of LSVT, consisting of 2 sessions a week over 8 weeks was provided via Skype. Two baseline (2 weeks apart) and two post-treatment measures (immediately following and 8 weeks after treatment) were collected. Materials included a range of speech tasks, questionnaires and a patient interview. Pre and post-treatment comparisons focused on acoustic and perceptual speech parameters, including voice quality, breath support, intelligibility and naturalness, as well as impact and communication participation. Results Results indicate improvements in breath support and in voice quality for sustained vowels and connected speech, both for acoustic and perceptual measures. Most of these improvements were maintained longer term. No changes were evident for intelligibility or naturalness, however, patient reported outcomes were positive for most participants, both in the speech and psychosocial domains. There were no negative reports with regard to fatigue, and the majority of patients preferred Skype delivery over face to face provision. Conclusion The study suggests, along with a recent pilot study by Vogel et al. (2019) that speech treatment can have a beneficial impact on communication in patients with hereditary ataxia and should therefore be provided by clinicians
Sustaining family forests in rural landscapes: Rationale, challenges, and an illustration from Oregon, USA
Family forests are critical components of rural landscapes, societies and economies. In Oregon, where nonindustrial private forests comprise only 16% of the forestland base, the ecological, social and economic impact of this ownership category is disproportionately large. This is due to the landscape position these lands occupy, the diversification they contribute to forest cover and local economies, and the political and cultural connections they provide to urban populations. The significance of this ownership category is even greater in the United States as a whole, where nonindustrial private forests comprise nearly two-thirds of the commercial forestland base, dominating rural landscapes in many regions of the country. Despite the important role family forests play, their ability to contribute to the wellbeing of rural areas is challenged by several dynamic factors, including industrial consolidation in global wood markets, loss of family forestland to corporate ownership, and parcelization and fragmentation of family forestland at the urban fringe. Moreover, family forestry does not enjoy a strong social contract with the American public, which is largely ignorant of the existence of this ownership class. A foundation of broad social approval and appreciation for family forestry is a prerequisite to development of policies which can sustain family forestland ownerships and the contributions they make. This paper draws from recent research in Oregon to argue that, whereas most research on nonindustrial private forests has focused on economics and management at the individual producer level, these challenges demand greater attention to the role of family forests in the wider context of landscape, culture and rural economy
The Five Factor Model of personality and evaluation of drug consumption risk
The problem of evaluating an individual's risk of drug consumption and misuse
is highly important. An online survey methodology was employed to collect data
including Big Five personality traits (NEO-FFI-R), impulsivity (BIS-11),
sensation seeking (ImpSS), and demographic information. The data set contained
information on the consumption of 18 central nervous system psychoactive drugs.
Correlation analysis demonstrated the existence of groups of drugs with
strongly correlated consumption patterns. Three correlation pleiades were
identified, named by the central drug in the pleiade: ecstasy, heroin, and
benzodiazepines pleiades. An exhaustive search was performed to select the most
effective subset of input features and data mining methods to classify users
and non-users for each drug and pleiad. A number of classification methods were
employed (decision tree, random forest, -nearest neighbors, linear
discriminant analysis, Gaussian mixture, probability density function
estimation, logistic regression and na{\"i}ve Bayes) and the most effective
classifier was selected for each drug. The quality of classification was
surprisingly high with sensitivity and specificity (evaluated by leave-one-out
cross-validation) being greater than 70\% for almost all classification tasks.
The best results with sensitivity and specificity being greater than 75\% were
achieved for cannabis, crack, ecstasy, legal highs, LSD, and volatile substance

Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries
Background
Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres.
Methods
This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and lowâmiddle-income countries.
Results
In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of âsingle-useâ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for lowâmiddle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia.
Conclusion
This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both highâ and lowâmiddleâincome countries
Crystallization of synthetic haemozoin (ÎČ-haematin) nucleated at the surface of lipid particles
The mechanism of formation of haemozoin, a detoxification by-product of several blood-feeding organisms including malaria parasites, has been a subject of debate; however, recent studies suggest that neutral lipids may serve as a catalyst. In this study, a model system consisting of an emulsion of neutral lipid particles was employed to investigate the formation of ÎČ-haematin, the synthetic counterpart of haemozoin, at the lipid-water interface. A solution of monoglyceride, either monostearoylglycerol (MSG) or monopalmitoylglycerol (MPG), dissolved in acetone and methanol was introduced to an aqueous surface. Fluorescence, confocal and transmission electron microscopic (TEM) imaging and dynamic light scattering analysis of samples obtained from beneath the surface confirmed the presence of homogeneous lipid particles existing in two major populations: one in the low micrometre size range and the other in the hundred nanometre range. The introduction of haem (Fe(iii)PPIX) to this lipid particle system under biomimetic conditions (37 °C, pH 4.8) produced ÎČ-haematin with apparent first-order kinetics and an average half life of 0.5 min. TEM of monoglycerides (MSG or MPG) extruded through a 200 nm filter with haem produced ÎČ-haematin crystals aligned and parallel to the lipid-water interface. These TEM data, together with a model system replacing the lipid with an aqueous organic solvent interface using either methyl laurate or docosane demonstrated that the OH and CO groups are apparently necessary for efficient nucleation. This suggests that ÎČ-haematin crystallizes via epitaxial nucleation at the lipid-water interface through interaction of Fe(iii)PPIX with the polar head group. Once nucleated, the crystal grows parallel to the interface until growth is terminated by the curvature of the lipid particle. The hydrophobic nature of the mature crystal favours an interior transport resulting in crystals aligned parallel to the lipid-water interface and each other, strikingly similar to that seen in malaria parasites. © The Royal Society of Chemistry 2010.Articl
The wild side of a major crop : Soybean's perennial cousins from Down Under
The accumulation of over 30 years of basic research on the biology, genetic variation, and evolution of the wild perennial relatives of soybean (Glycine max) provides a foundation to improve cultivated soybean. The cultivated soybean and its wild progenitor, G. soja, have a center of origin in eastern Asia and are the only two species in the annual subgenus Soja. Systematic and evolutionary studies of the ca. 30 perennial species of subgenus Glycine, native to Australia, have benefited from the availability of the G. max genomic sequence. The perennial species harbor many traits of interest to soybean breeders, among them resistance to major soybean pathogens such as cyst nematode and leaf rust. New species in the Australian subgenus continue to be described, due to the collection of new material and to insights gleaned through systematic studies of accessions in germplasm collections. Ongoing studies in perennial species focus on genomic regions that contain genes for key traits relevant to soybean breeding. These comparisons also include the homoeologous regions that are the result of polyploidy in the common ancestor of all Glycine species. Subgenus Glycine includes a complex of recently formed allopolyploids that are the focus of studies aimed at elucidating genomic, transcriptomic, physiological, taxonomic, morphological, developmental, and ecological processes related to polyploid evolution. Here we review what has been learned over the past 30 years and outline ongoing work on photosynthesis, nitrogen fixation, and floral biology, much of it drawing on new technologies and resources
