    Computerized Analysis of Magnetic Resonance Images to Study Cerebral Anatomy in Developing Neonates

    The study of cerebral anatomy in developing neonates is of great importance for the understanding of brain development during the early period of life. This dissertation therefore focuses on three challenges in the modelling of cerebral anatomy in neonates during brain development. The methods that have been developed all use Magnetic Resonance Images (MRI) as source data. To facilitate study of vascular development in the neonatal period, a set of image analysis algorithms are developed to automatically extract and model cerebral vessel trees. The whole process consists of cerebral vessel tracking from automatically placed seed points, vessel tree generation, and vasculature registration and matching. These algorithms have been tested on clinical Time-of- Flight (TOF) MR angiographic datasets. To facilitate study of the neonatal cortex a complete cerebral cortex segmentation and reconstruction pipeline has been developed. Segmentation of the neonatal cortex is not effectively done by existing algorithms designed for the adult brain because the contrast between grey and white matter is reversed. This causes pixels containing tissue mixtures to be incorrectly labelled by conventional methods. The neonatal cortical segmentation method that has been developed is based on a novel expectation-maximization (EM) method with explicit correction for mislabelled partial volume voxels. Based on the resulting cortical segmentation, an implicit surface evolution technique is adopted for the reconstruction of the cortex in neonates. The performance of the method is investigated by performing a detailed landmark study. To facilitate study of cortical development, a cortical surface registration algorithm for aligning the cortical surface is developed. The method first inflates extracted cortical surfaces and then performs a non-rigid surface registration using free-form deformations (FFDs) to remove residual alignment. Validation experiments using data labelled by an expert observer demonstrate that the method can capture local changes and follow the growth of specific sulcus

    Recent trends, technical concepts and components of computer-assisted orthopedic surgery systems: A comprehensive review

    Computer-assisted orthopedic surgery (CAOS) systems have become one of the most important and challenging types of system in clinical orthopedics, as they enable precise treatment of musculoskeletal diseases, employing modern clinical navigation systems and surgical tools. This paper brings a comprehensive review of recent trends and possibilities of CAOS systems. There are three types of the surgical planning systems, including: systems based on the volumetric images (computer tomography (CT), magnetic resonance imaging (MRI) or ultrasound images), further systems utilize either 2D or 3D fluoroscopic images, and the last one utilizes the kinetic information about the joints and morphological information about the target bones. This complex review is focused on three fundamental aspects of CAOS systems: their essential components, types of CAOS systems, and mechanical tools used in CAOS systems. In this review, we also outline the possibilities for using ultrasound computer-assisted orthopedic surgery (UCAOS) systems as an alternative to conventionally used CAOS systems.Web of Science1923art. no. 519

    Development of an Atlas-Based Segmentation of Cranial Nerves Using Shape-Aware Discrete Deformable Models for Neurosurgical Planning and Simulation

    Twelve pairs of cranial nerves arise from the brain or brainstem and control our sensory functions such as vision, hearing, smell and taste as well as several motor functions to the head and neck including facial expressions and eye movement. Often, these cranial nerves are difficult to detect in MRI data, and thus represent problems in neurosurgery planning and simulation, due to their thin anatomical structure, in the face of low imaging resolution as well as image artifacts. As a result, they may be at risk in neurosurgical procedures around the skull base, which might have dire consequences such as the loss of eyesight or hearing and facial paralysis. Consequently, it is of great importance to clearly delineate cranial nerves in medical images for avoidance in the planning of neurosurgical procedures and for targeting in the treatment of cranial nerve disorders. In this research, we propose to develop a digital atlas methodology that will be used to segment the cranial nerves from patient image data. The atlas will be created from high-resolution MRI data based on a discrete deformable contour model called 1-Simplex mesh. Each of the cranial nerves will be modeled using its centerline and radius information where the centerline is estimated in a semi-automatic approach by finding a shortest path between two user-defined end points. The cranial nerve atlas is then made more robust by integrating a Statistical Shape Model so that the atlas can identify and segment nerves from images characterized by artifacts or low resolution. To the best of our knowledge, no such digital atlas methodology exists for segmenting nerves cranial nerves from MRI data. Therefore, our proposed system has important benefits to the neurosurgical community

    Development of a Surgical Assistance System for Guiding Transcatheter Aortic Valve Implantation

    Development of image-guided interventional systems is growing up rapidly in the recent years. These new systems become an essential part of the modern minimally invasive surgical procedures, especially for the cardiac surgery. Transcatheter aortic valve implantation (TAVI) is a recently developed surgical technique to treat severe aortic valve stenosis in elderly and high-risk patients. The placement of stented aortic valve prosthesis is crucial and typically performed under live 2D fluoroscopy guidance. To assist the placement of the prosthesis during the surgical procedure, a new fluoroscopy-based TAVI assistance system has been developed. The developed assistance system integrates a 3D geometrical aortic mesh model and anatomical valve landmarks with live 2D fluoroscopic images. The 3D aortic mesh model and landmarks are reconstructed from interventional angiographic and fluoroscopic C-arm CT system, and a target area of valve implantation is automatically estimated using these aortic mesh models. Based on template-based tracking approach, the overlay of visualized 3D aortic mesh model, landmarks and target area of implantation onto fluoroscopic images is updated by approximating the aortic root motion from a pigtail catheter motion without contrast agent. A rigid intensity-based registration method is also used to track continuously the aortic root motion in the presence of contrast agent. Moreover, the aortic valve prosthesis is tracked in fluoroscopic images to guide the surgeon to perform the appropriate placement of prosthesis into the estimated target area of implantation. An interactive graphical user interface for the surgeon is developed to initialize the system algorithms, control the visualization view of the guidance results, and correct manually overlay errors if needed. Retrospective experiments were carried out on several patient datasets from the clinical routine of the TAVI in a hybrid operating room. The maximum displacement errors were small for both the dynamic overlay of aortic mesh models and tracking the prosthesis, and within the clinically accepted ranges. High success rates of the developed assistance system were obtained for all tested patient datasets. The results show that the developed surgical assistance system provides a helpful tool for the surgeon by automatically defining the desired placement position of the prosthesis during the surgical procedure of the TAVI.Die Entwicklung bildgeführter interventioneller Systeme wächst rasant in den letzten Jahren. Diese neuen Systeme werden zunehmend ein wesentlicher Bestandteil der technischen Ausstattung bei modernen minimal-invasiven chirurgischen Eingriffen. Diese Entwicklung gilt besonders für die Herzchirurgie. Transkatheter Aortenklappen-Implantation (TAKI) ist eine neue entwickelte Operationstechnik zur Behandlung der schweren Aortenklappen-Stenose bei alten und Hochrisiko-Patienten. Die Platzierung der Aortenklappenprothese ist entscheidend und wird in der Regel unter live-2D-fluoroskopischen Bildgebung durchgeführt. Zur Unterstützung der Platzierung der Prothese während des chirurgischen Eingriffs wurde in dieser Arbeit ein neues Fluoroskopie-basiertes TAKI Assistenzsystem entwickelt. Das entwickelte Assistenzsystem überlagert eine 3D-Geometrie des Aorten-Netzmodells und anatomischen Landmarken auf live-2D-fluoroskopische Bilder. Das 3D-Aorten-Netzmodell und die Landmarken werden auf Basis der interventionellen Angiographie und Fluoroskopie mittels eines C-Arm-CT-Systems rekonstruiert. Unter Verwendung dieser Aorten-Netzmodelle wird das Zielgebiet der Klappen-Implantation automatisch geschätzt. Mit Hilfe eines auf Template Matching basierenden Tracking-Ansatzes wird die Überlagerung des visualisierten 3D-Aorten-Netzmodells, der berechneten Landmarken und der Zielbereich der Implantation auf fluoroskopischen Bildern korrekt überlagert. Eine kompensation der Aortenwurzelbewegung erfolgt durch Bewegungsverfolgung eines Pigtail-Katheters in Bildsequenzen ohne Kontrastmittel. Eine starrere Intensitätsbasierte Registrierungsmethode wurde verwendet, um kontinuierlich die Aortenwurzelbewegung in Bildsequenzen mit Kontrastmittelgabe zu detektieren. Die Aortenklappenprothese wird in die fluoroskopischen Bilder eingeblendet und dient dem Chirurg als Leitfaden für die richtige Platzierung der realen Prothese. Eine interaktive Benutzerschnittstelle für den Chirurg wurde zur Initialisierung der Systemsalgorithmen, zur Steuerung der Visualisierung und für manuelle Korrektur eventueller Überlagerungsfehler entwickelt. Retrospektive Experimente wurden an mehreren Patienten-Datensätze aus der klinischen Routine der TAKI in einem Hybrid-OP durchgeführt. Hohe Erfolgsraten des entwickelten Assistenzsystems wurden für alle getesteten Patienten-Datensätze erzielt. Die Ergebnisse zeigen, dass das entwickelte chirurgische Assistenzsystem ein hilfreiches Werkzeug für den Chirurg bei der Platzierung Position der Prothese während des chirurgischen Eingriffs der TAKI bietet

    Evaluating Spatial Normalization Methods for the Human Brain

    Cortical stimulation mapping (CSM) studies have shown cortical locations for language function are highly variable from one subject to the next. Because no two cortical surfaces are alike and language is a higher order cognitive function, observed variability is attributable to a combination of functional and anatomical variation. If individual variation can be normalized, patterns of language organization may emerge that were heretofore hidden. In order to discover whether or not such patterns exist, computer-aided spatial normalization is required. Because CSM data has been collected on the cortical surface, we believe that a surface-based normalization method will provide more accurate results than will a volume-based method. To investigate this hypothesis, we evaluate a surface-based (Caret) and volume-based method (SPM2). For our application, the "ideal" method would i) minimize variation as measured by spread reduction between cortical language sites across subjects while also ii) preserving anatomical localization of sites. Evaluation technique: Eleven MR image volumes and corresponding CSM site coordinates were selected. Images were segmented to create left hemisphere surface reconstruction for each patient. Individual surfaces were registered to the colin27 human brain atlas using each method. Deformation parameters from each method were applied to CSM coordinates to obtain post-normalization coordinates in 2D space and 3D ICBM152 space. Accuracy metrics were calculated i) as mean distance between language sites across subjects in both 2D and 3D space and ii) by visual inspection of post-normalization site locations on a 2D map. Results: Globally, we found no statistically significant difference between CARET (surface-based method) and SPM2 (volume-based method) as measured by both spread reduction and anatomical localization. Local analysis showed that more than twenty percent of total mapping errors were mapped incorrectly by both methods. There was a statistically significant difference between Caret and SPM2 mapping of type 2 errors

    Facial soft tissue segmentation

    The importance of the face for socio-ecological interaction is the cause for a high demand on any surgical intervention on the facial musculo-skeletal system. Bones and soft-tissues are of major importance for any facial surgical treatment to guarantee an optimal, functional and aesthetical result. For this reason, surgeons want to pre-operatively plan, simulate and predict the outcome of the surgery allowing for shorter operation times and improved quality. Accurate simulation requires exact segmentation knowledge of the facial tissues. Thus semi-automatic segmentation techniques are required. This thesis proposes semi-automatic methods for segmentation of the facial soft-tissues, such as muscles, skin and fat, from CT and MRI datasets, using a Markov Random Fields (MRF) framework. Due to image noise, artifacts, weak edges and multiple objects of similar appearance in close proximity, it is difficult to segment the object of interest by using image information alone. Segmentations would leak at weak edges into neighboring structures that have a similar intensity profile. To overcome this problem, additional shape knowledge is incorporated in the energy function which can then be minimized using Graph-Cuts (GC). Incremental approaches by incorporating additional prior shape knowledge are presented. The proposed approaches are not object specific and can be applied to segment any class of objects be that anatomical or non-anatomical from medical or non-medical image datasets, whenever a statistical model is present. In the first approach a 3D mean shape template is used as shape prior, which is integrated into the MRF based energy function. Here, the shape knowledge is encoded into the data and the smoothness terms of the energy function that constrains the segmented parts to a reasonable shape. In the second approach, to improve handling of shape variations naturally found in the population, the fixed shape template is replaced by a more robust 3D statistical shape model based on Probabilistic Principal Component Analysis (PPCA). The advantages of using the Probabilistic PCA are that it allows reconstructing the optimal shape and computing the remaining variance of the statistical model from partial information. By using an iterative method, the statistical shape model is then refined using image based cues to get a better fitting of the statistical model to the patient's muscle anatomy. These image cues are based on the segmented muscle, edge information and intensity likelihood of the muscle. Here, a linear shape update mechanism is used to fit the statistical model to the image based cues. In the third approach, the shape refinement step is further improved by using a non-linear shape update mechanism where vertices of the 3D mesh of the statistical model incur the non-linear penalty depending on the remaining variability of the vertex. The non-linear shape update mechanism provides a more accurate shape update and helps in a finer shape fitting of the statistical model to the image based cues in areas where the shape variability is high. Finally, a unified approach is presented to segment the relevant facial muscles and the remaining facial soft-tissues (skin and fat). One soft-tissue layer is removed at a time such as the head and non-head regions followed by the skin. In the next step, bones are removed from the dataset, followed by the separation of the brain and non-brain regions as well as the removal of air cavities. Afterwards, facial fat is segmented using the standard Graph-Cuts approach. After separating the important anatomical structures, finally, a 3D fixed shape template mesh of the facial muscles is used to segment the relevant facial muscles. The proposed methods are tested on the challenging example of segmenting the masseter muscle. The datasets were noisy with almost all possessing mild to severe imaging artifacts such as high-density artifacts caused by e.g. dental fillings and dental implants. Qualitative and quantitative experimental results show that by incorporating prior shape knowledge leaking can be effectively constrained to obtain better segmentation results

    Optical techniques for 3D surface reconstruction in computer-assisted laparoscopic surgery

    One of the main challenges for computer-assisted surgery (CAS) is to determine the intra-opera- tive morphology and motion of soft-tissues. This information is prerequisite to the registration of multi-modal patient-specific data for enhancing the surgeon’s navigation capabilites by observ- ing beyond exposed tissue surfaces and for providing intelligent control of robotic-assisted in- struments. In minimally invasive surgery (MIS), optical techniques are an increasingly attractive approach for in vivo 3D reconstruction of the soft-tissue surface geometry. This paper reviews the state-of-the-art methods for optical intra-operative 3D reconstruction in laparoscopic surgery and discusses the technical challenges and future perspectives towards clinical translation. With the recent paradigm shift of surgical practice towards MIS and new developments in 3D opti- cal imaging, this is a timely discussion about technologies that could facilitate complex CAS procedures in dynamic and deformable anatomical regions

    A Survey on Deep Learning in Medical Image Analysis

    Deep learning algorithms, in particular convolutional networks, have rapidly become a methodology of choice for analyzing medical images. This paper reviews the major deep learning concepts pertinent to medical image analysis and summarizes over 300 contributions to the field, most of which appeared in the last year. We survey the use of deep learning for image classification, object detection, segmentation, registration, and other tasks and provide concise overviews of studies per application area.     A biomechanical approach for real-time tracking of lung tumors during External Beam Radiation Therapy (EBRT)

    Lung cancer is the most common cause of cancer related death in both men and women. Radiation therapy is widely used for lung cancer treatment. However, this method can be challenging due to respiratory motion. Motion modeling is a popular method for respiratory motion compensation, while biomechanics-based motion models are believed to be more robust and accurate as they are based on the physics of motion. In this study, we aim to develop a biomechanics-based lung tumor tracking algorithm which can be used during External Beam Radiation Therapy (EBRT). An accelerated lung biomechanical model can be used during EBRT only if its boundary conditions (BCs) are defined in a way that they can be updated in real-time. As such, we have developed a lung finite element (FE) model in conjunction with a Neural Networks (NNs) based method for predicting the BCs of the lung model from chest surface motion data. To develop the lung FE model for tumor motion prediction, thoracic 4D CT images of lung cancer patients were processed to capture the lung and diaphragm geometry, trans-pulmonary pressure, and diaphragm motion. Next, the chest surface motion was obtained through tracking the motion of the ribcage in 4D CT images. This was performed to simulate surface motion data that can be acquired using optical tracking systems. Finally, two feedforward NNs were developed, one for estimating the trans-pulmonary pressure and another for estimating the diaphragm motion from chest surface motion data. The algorithm development consists of four steps of: 1) Automatic segmentation of the lungs and diaphragm, 2) diaphragm motion modelling using Principal Component Analysis (PCA), 3) Developing the lung FE model, and 4) Using two NNs to estimate the trans-pulmonary pressure values and diaphragm motion from chest surface motion data. The results indicate that the Dice similarity coefficient between actual and simulated tumor volumes ranges from 0.76±0.04 to 0.91±0.01, which is favorable. As such, real-time lung tumor tracking during EBRT using the proposed algorithm is feasible. Hence, further clinical studies involving lung cancer patients to assess the algorithm performance are justified
