Kidney Disease Management in the Hospital Setting: A Focus on Inappropriate Drug Prescriptions in Older Patients
Aging with multimorbidity and polytherapy are the most significant factors that could led to inappropriate prescribing of contraindicated medications in patients with chronic kidney disease (CKD). The aim of this study was to evaluate the prescriptions of contraindicated drugs in older adults in CKD and to identify their associated factors in a hospital context. An observational retrospective study was carried out considering all patients â„65 years with at least one serum creatinine value recorded into the REPOSI register into 2010â2016 period. The estimated glomerular filtration rate (eGFR) was applied to identify CKD. A descriptive analysis was performed to compare demographic and clinical characteristics; logistic regression models were used to estimate factors of inappropriate and percentage changes of drug use during hospitalization. A total of 4,713 hospitalized patients were recorded, of which 49.8% had an eGFR <60 ml/min/1.73 m2; the 21.9% were in treatment with at least one inappropriate drug at the time of hospital admission with a decrease of 3.0% at discharge (p = 0.010). The probability of using at least one contraindicated drug was significantly higher in patients treated with more several drugs (OR 1.21, 95% CI 1.16â1.25, p <0.001) and with CKD end-stages (G4: 16.90, 11.38â25.12, p < 0.001; G5: 19.38, 11.51â32.64, p < 0.001). Low-dose acetylsalicylic acid was the contraindicated drug mainly used at the time of admission, reducing 1.2% at discharge. An overall increase in therapeutic appropriateness in hospitalized older patients with CKD was observed, despite a small percentage of therapeutic inappropriateness at discharge that underlines the need for a closer collaboration with the pharmacologist to improve the drug management
Beta-Blocker Use in Older Hospitalized Patients Affected by Heart Failure and Chronic Obstructive Pulmonary Disease: An Italian Survey From the REPOSI Register
Beta (ÎČ)-blockers (BB) are useful in reducing morbidity and mortality in patients with heart failure (HF) and concomitant chronic obstructive pulmonary disease (COPD). Nevertheless, the use of BBs could induce bronchoconstriction due to ÎČ2-blockade. For this reason, both the ESC and GOLD guidelines strongly suggest the use of selective ÎČ1-BB in patients with HF and COPD. However, low adherence to guidelines was observed in multiple clinical settings. The aim of the study was to investigate the BBs use in older patients affected by HF and COPD, recorded in the REPOSI register. Of 942 patients affected by HF, 47.1% were treated with BBs. The use of BBs was significantly lower in patients with HF and COPD than in patients affected by HF alone, both at admission and at discharge (admission, 36.9% vs. 51.3%; discharge, 38.0% vs. 51.7%). In addition, no further BB users were found at discharge. The probability to being treated with a BB was significantly lower in patients with HF also affected by COPD (adj. OR, 95% CI: 0.50, 0.37â0.67), while the diagnosis of COPD was not associated with the choice of selective ÎČ1-BB (adj. OR, 95% CI: 1.33, 0.76â2.34). Despite clear recommendations by clinical guidelines, a significant underuse of BBs was also observed after hospital discharge. In COPD affected patients, physicians unreasonably reject BBs use, rather than choosing a ÎČ1-BB. The expected improvement of the BB prescriptions after hospitalization was not observed. A multidisciplinary approach among hospital physicians, general practitioners, and pharmacologists should be carried out for better drug management and adherence to guideline recommendations
A new highly segmented start counter for the CLAS detector
The design, construction and performance of a highly segmented Start Counter are described. The Start Counter is an integral part of the trigger used in photon beam running with CLAS in Hall B at the Thomas Jefferson National Accelerator Facility (TJNAF). The Start Counter is constructed of 24 2.2-mm-thick single-ended scintillation paddles, forming a hermetic hexagon around the target region. This device measures the interaction time of the incoming photon in the target by detecting the outgoing particles. The counter provides complex trigger topologies, shows good efficiency and achieved a time resolution of 350 ps
Late-Proterozoic to Paleozoic history of the peri-Gondwana CalabriaâPeloritani Terrane inferred from a review of zircon chronology
UâPb analyses of zircon from ten samples of augen gneisses, eight mafic and intermediate metaigneous rocks and six metasediments from some tectonic domains along the CalabriaâPeloritani Terrane (Southern Italy) contribute to knowledge of peri-Gondwanan evolution from Late-Proterozoic to Paleozoic times. All samples were equilibrated under amphibolite to granulite facies metamorphism during the Variscan orogeny. The zircon grains of all considered samples preserve a Proterozoic memory suggestive of detrital, metamorphic and igneous origin. The available data fit a frame involving: (1) Neoproterozoic detrital input from cratonic areas of Gondwana; (2) Pan-African/Cadomian assemblage of blocks derived from East and West African Craton; (3) metamorphism and bimodal magmatism between 535 and 579Â Ma, within an active margin setting; (4) rifting and opening of Ordovician basins fed by detrital input from the assembled Cadomian blocks. The Paleozoic basins evolved through sedimentation, metamorphism and magmatism during the Variscan orogeny involving Palaeozoic and pre-Paleozoic blocks. The Proterozoic zircon records decidedly decrease in the high grade metamorphic rocks affected by Variscan pervasive partial melting. A new multianodic large area photomultiplier to be used in underwater neutrino detectors
In this article we describe the properties of a new 10-in. hemispherical photomultiplier manufactured by Hamamatsu. The prototype has a segmented photocathode and four independent amplification stages. The photomultiplier is one of the main components of a newly designed direction-sensitive optical module to be employed in large-scale underwater neutrino telescopes. The R&D activity has been co-funded by the INFN and the KM3NeT Consortium. The prototype performance fully meets with the design specifications
ANTARES: the first undersea neutrino telescope
The ANTARES Neutrino Telescope was completed in May 2008 and is the first
operational Neutrino Telescope in the Mediterranean Sea. The main purpose of
the detector is to perform neutrino astronomy and the apparatus also offers
facilities for marine and Earth sciences. This paper describes the design, the
construction and the installation of the telescope in the deep sea, offshore
from Toulon in France. An illustration of the detector performance is given
Relative contributions of crust and mantle to generation of Campanian high-K calc-alkaline I-type granitoids in a subduction setting, with special reference to the Harsit Pluton, Eastern Turkey
We present elemental and Sr-Nd-Pb isotopic data for the magmatic suite (similar to 79 Ma) of the Harsit pluton, from the Eastern Pontides (NE Turkey), with the aim of determining its magma source and geodynamic evolution. The pluton comprises granite, granodiorite, tonalite and minor diorite (SiO(2) = 59.43-76.95 wt%), with only minor gabbroic diorite mafic microgranular enclaves in composition (SiO(2) = 54.95-56.32 wt%), and exhibits low Mg# (<46). All samples show a high-K calc-alkaline differentiation trend and I-type features. The chondrite-normalized REE patterns are fractionated [(La/Yb)(n) = 2.40-12.44] and display weak Eu anomalies (Eu/Eu* = 0.30-0.76). The rocks are characterized by enrichment of LILE and depletion of HFSE. The Harsit host rocks have weak concave-upward REE patterns, suggesting that amphibole and garnet played a significant role in their generation during magma segregation. The host rocks and their enclaves are isotopically indistinguishable. Sr-Nd isotopic data for all of the samples display I(Sr) = 0.70676-0.70708, epsilon(Nd)(79 Ma) = -4.4 to -3.3, with T(DM) = 1.09-1.36 Ga. The lead isotopic ratios are ((206)Pb/(204)pb) = 18.79-18.87, ((207)Pb/(204)Pb) = 15.59-15.61 and ((208)Pb/(204)Pb) = 38.71-38.83. These geochemical data rule out pure crustal-derived magma genesis in a post-collision extensional stage and suggest mixed-origin magma generation in a subduction setting. The melting that generated these high-K granitoidic rocks may have resulted from the upper Cretaceous subduction of the Izmir-Ankara-Erzincan oceanic slab beneath the Eurasian block in the region. The back-arc extensional events would have caused melting of the enriched subcontinental lithospheric mantle and formed mafic magma. The underplating of the lower crust by mafic magmas would have played a significant role in the generation of high-K magma. Thus, a thermal anomaly induced by underplated basic magma into a hot crust would have caused partial melting in the lower part of the crust. In this scenario, the lithospheric mantle-derived basaltic melt first mixed with granitic magma of crustal origin at depth. Then, the melts, which subsequently underwent a fractional crystallization and crustal assimilation processes, could ascend to shallower crustal levels to generate a variety of rock types ranging from diorite to granite. Sr-Nd isotope modeling shows that the generation of these magmas involved similar to 65-75% of the lower crustal-derived melt and similar to 25-35% of subcontinental lithospheric mantle. Further, geochemical data and the Ar-Ar plateau age on hornblende, combined with regional studies, imply that the Harsit pluton formed in a subduction setting and that the back-arc extensional period started by least similar to 79 Ma in the Eastern Pontides.Geochemistry & GeophysicsMineralogySCI(E)33ARTICLE4467-48716
Thermal history vs. fabric anisotropy in granitoids emplaced at different crustal levels: an example from Calabria, southern Italy
The Sila and Serre granitoids of Calabria were emplaced in the late Carboniferous at depths ranging from 6 to 23 km in a postcollisional extensional regime. Their fabric, which developed during and after final crystallization up to the solid state, strongly increases in intensity with emplacement depth, This relationship is attributed to the thermal history of the Calabrian basement. Cooling histories of granitoids, constrained by geological data and Rb-Sr cooling ages on micas, demonstrate that residence times of rocks at temperatures greater than those of the brittleductile transition vary greatly as a function of initial emplacement depth. This explains why shallow-level granitoids, that remained for about 10 Myr at temperatures above those of the brittle domain, retain their original magmatic fabric, By contrast, the strong fabrics of the deep-seated granitoids are explained by solid-state strain overprint that lasted more than 100 Myr at temperatures above those of the brittle domain
Dike magmatism in the Sila Grande (Calabria, southern Italy): Evidence of PennsylvanianâEarly Permian exhumation
A dike network transecting a basement of intrusive and metamorphic rocks
related to the Hercynian orogeny is exposed in the Sila Grande (southern
Italy). Dike magmatism, similarly to other regions of the western
Mediterranean, such as Sardinia, Corsica, and Catalonia, is of
calc-alkaline to alkali-calcic affinity. Zircon U-Pb geochronology
indicates that dike magmatism took place between 295 +/- 1 to 277 +/- 1
Ma, after the main late Hercynian emplacement of granitoids (306 +/- 1
Ma). Barometry indicates that the basement underwent exhumation of 8 +/-
3 km before dike injection. The dike network has a geometrical
arrangement consistent with a transtensional stress regime that resulted
in ductile thinning of the lower crust during the late stage of the
Hercynian orogeny and concurrent fracturing of the upper crust that made
possible magma ascent through dikes. The proposed tectonic evolution is
related to dismemberment of the southern Hercynian belt in the central
Mediterranean area as a result of dextral transtension of Gondwana in
relation to Laurasia during the Pennsylvanian-Early Permian
