Reduced model for the thermo-fluid dynamic analysis of a power transformer radiator working in ONAF mode
This work introduces an extension of the semi-analytical reduced model presented in G. Ríos Rodriguez et.al., App Thermal Eng, 112:1271–1280 (2016); with the aim of analyzing the thermo-fluid dynamic behavior of a power transformer radiator working in ONAF (Oil Natural Air Forced) mode, considering vertical blowing of the fans. The objective of this research paper is the development of a numerical tool to be used during the design process of power transformers in order to obtain more detailed information about the radiators performance, either they are working in ONAF or ONAN (Oil Natural Air Natural) mode. Since it is observed both in the experimental measurements and in the numerical simulations that not all the radiator panels are blown by a single fan, the reduced model here presented considers a mixed situation wherein the heat is removed by natural convection in a fraction of the radiator while in the rest the heat is removed by forced convection. As a consequence, besides adding to the set of conservation equations introduced in G. Ríos Rodriguez et.al., App Thermal Eng, 112:1271– 1280 (2016); those corresponding to the momentum and energy balance for considering forced heat convection transfer, there appears the need of introducing additional equations for coupling the natural and forced heat convection transfer models. The reduced model developed in this work is applied to estimate different characteristic parameters of a power transformer radiator working in ONAF mode, like the oil flow rate, dissipated power, outlet and inlet oil temperatures, among others. The computed results are compared to those obtained by CFD simulations and experimental measurements carried out on an ad-hoc workbench for validation purposes. It is found that the reduced model reproduces with acceptable accuracy the values of the most important design variables at a very low computation cost.

Una estrategia de acoplamiento conservativa y monótona para mallas no coincidentes en problemas multifísica particionados
Las simulaciones numéricas de problemas multifísica se han vuelto populares en una gran cantidad de aplicaciones ingenieriles, tales como las simulaciones de problemas de interacción fluidoestructura. Como la complejidad de tales simulaciones ha aumentado debido a la introducción de otros fenómenos físicos, como los termodinámicos y los acústicos, es conveniente considerar estrategias de solución particionadas con el fin de reutilizar algoritmos (denominados solvers) específicos son capaces de resolver tales problemas independientemente, sin introducir mayores modificaciones a los códigos. En éste escenario, es deseable poder utilizar diferentes discretizaciones para cada subdominio del problema. Sin embargo, esto introduce la necesidad de proyectar soluciones desde la interfaz de un subdominio a otro, para transferir los desplazamientos y velocidades de los contornos de los subdominios y sincronizar los solvers. Por lo tanto, en este trabajo un esquema de proyección monótono y conservativo es introducido y utilizado para acoplar un solver computacional de dinámica de fluidos y uno de dinámica de estructuras. La precisión y consistencia, así como también las propiedades de conservación y monotonicidad, serán evaluadas sobre dominios 2D cuyas interfaces de interacción son 1D. National Prevalence and Trends of HIV Transmitted Drug Resistance in Mexico
BACKGROUND: Transmitted drug resistance (TDR) remains an important concern for the management of HIV infection, especially in countries that have recently scaled-up antiretroviral treatment (ART) access. METHODOLOGY/PRINCIPAL FINDINGS: We designed a study to assess HIV diversity and transmitted drug resistance (TDR) prevalence and trends in Mexico. 1655 ART-naïve patients from 12 Mexican states were enrolled from 2005 to 2010. TDR was assessed from plasma HIV pol sequences using Stanford scores and the WHO TDR surveillance mutation list. TDR prevalence fluctuations over back-projected dates of infection were tested. HIV subtype B was highly prevalent in Mexico (99.9%). TDR prevalence (Stanford score>15) in the country for the study period was 7.4% (95% CI, 6.2∶8.8) and 6.8% (95% CI, 5.7∶8.2) based on the WHO TDR surveillance mutation list. NRTI TDR was the highest (4.2%), followed by NNRTI (2.5%) and PI (1.7%) TDR. Increasing trends for NNRTI (p = 0.0456) and PI (p = 0.0061) major TDR mutations were observed at the national level. Clustering of viruses containing minor TDR mutations was observed with some apparent transmission pairs and geographical effects. CONCLUSIONS: TDR prevalence in Mexico remains at the intermediate level and is slightly lower than that observed in industrialized countries. Whether regional variations in TDR trends are associated with differences in antiretroviral drug usage/ART efficacy or with local features of viral evolution remains to be further addressed
Use of rituximab in pediatric patients with steroid-resistant nephrotic syndrome. A single center study
ABSTRACT: Steroids are the cornerstone of therapy for nephrotic syndrome (NS) with a remission rate as high as 90%. In patients who do not respond to them or are steroid dependent, other immunosuppressive drugs have been used. Although rituximab use in NS is off-label, many authors have published their experience with it.
Objective and methods: To describe retrospectively a group of seven children with nephrotic syndrome, either steroid-dependent (SDNS) or steroid-resistant (SRNS), treated with rituximab and mycophenolate, at Pablo Tobón Uribe Hospital, in Medellín, Colombia.
Results: Two patients with SDNS and five with SRNS were evaluated; median age at diagnosis was 2 years (p25-75: 1-5); six months after treatment with rituximab there was reduction in proteinuria (93%), in the steroid dose (100%) and in the relapse episodes. However, proteinuria reappeared 12 months after treatment.
Conclusion: During the first year after rituximab treatment of NS there is reduction in proteinuria and in the steroid dose, but thereafter there is relapse. It is suggested to carry out another study using a second dose of rituximab one year after the first one.RESUMEN: Los esteroides son el tratamiento de elección del síndrome nefrótico con tasas de éxito hasta del 90%. En pacientes que no responden adecuadamente a ellos, se han usado diferentes esquemas de inmunosupresión.
Objetivo y métodos: describir la respuesta terapéutica en un grupo de siete niños con síndrome nefrótico córtico-dependiente (SNCD) o córtico-resistente (SNCR) que recibieron tratamiento con rituximab y micofenolato mofetil en un hospital universitario de la ciudad de Medellín durante los años 2010-2012.
Resultados: dos pacientes tenían SNCD y cinco, SNCR; la mediana de edad en el momento del diagnóstico fue de 2 años (p25-75: 1-5); seis meses después de la aplicación del rituximab se encontró disminución de la proteinuria en el 93% de los pacientes; los esteroides se lograron suspender en el 100%; además, disminuyó el número de recaídas; sin embargo, la proteinuria reapareció un año después de dicho tratamiento.
Conclusión: con el rituximab disminuyen la proteinuria y la dosis de esteroides, pero la enfermedad recurre 12 meses después de usarlo. Se sugiere hacer otro estudio evaluando el efecto de una segunda dosis de rituximab al año de la primera
Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world
Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic.
Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality.
Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States.
Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis.
Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection
Taking the pulse of Earth's tropical forests using networks of highly distributed plots
Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science. H-adaptivity of unstructured finite element meshes in steady and unsteady compressible flow problems
La presente tesis trata sobre el desarrollo de una estrategia de adaptación de mallas no estructuradas para resolver problemas estacionarios y transitorios de flujos compresibles utilizando el método de elementos finitos. La misma se concibe atendiendo a la calidad de la malla, la que es analizada bajo un enfoque estadístico de la calidad de sus elementos, empleando métricas de calidad algebraicas y geométricas. Las mallas adaptadas son no conformes. Esto obliga a imponer restricciones al refinamiento para que no exista una diferencia arbitraria entre los tamaños de elementos vecinos. Se utilizan técnicas de estimación de error para identificar las regiones de la malla que deben ser refinadas. Se describen dos métodos para la estimación a posteriori del error en sistemas de ecuaciones hiperbólicos. La adaptación se ejecuta secuencialmente en un nodo cualquiera de un cluster Beowulf, mientras que el flujo se resuelve en paralelo empleando el método de elementos finitos SUPG y técnicas de Shock Capturing. Esta implementación es parte del solver PETSc-FEM. El algoritmo de adaptación se implementó en lenguaje C++ utilizando contenedores y algoritmos de las librerías STL y Boost. Los problemas que se resuelven permiten evalur la precisión en el cálculo de la solución, la calidad de las mallas adaptadas y la performance del código adaptativo. Los resultados numéricos son comparados con soluciones teóricas de la dinámica de gases y con resultados experimentales. Finalmente se elaboran conclusiones sobre la estrategia desarrollada y se proponen actividades a futuro, destinadas a mejorar las capacidades de dicha estrategia.An adaptive refinement strategy to solve steady and unsteady compressible non- viscous flows by the finite element method over unstructured meshes is developed. The quality of the adapted mesh is the main driving force to devise the refinement strategy. The quality of the mesh elements is evaluated using well known quality measures. The adapted meshes are non-conforming, ie there exists hanging or green nodes in the adapted mesh. Refinement constraints are enforced in order to guarantee a smooth size distribution amongst neighbour elements. The refinement strategy and constraints are implemented by a recursive algorithm. On the other hand, a posteriori error estimators are used to steer the adaptivity process. Common a posteriori error estimators for hyperbolic systems of partial differential equations are introduced. The adaptivity code is partially parallelized, ie the mesh adaption task is executed on a single node of a Beowulf cluster of PC while the fluid dynamic solver is executed in parallel. The SUPG finite element scheme is used to solve the governing equations together with Shock-Capturing techniques. This scheme is implemented in a code that is part of the PETSc-FEM multiphysics solver. The adaption algorithm is written in the C++ programming language together with algorithms and containers provided by the Standard Template Library (STL) and the Boost libraries. Mesh adaption code performance, adapted mesh quality and accuracy improvements in the solution are evaluated whenever possible. Conclusions are drawn based on these results and research activities for the near future are finally proposed.Consejo Nacional de Investigaciones Científicas y Técnica

Implementation and clinical effectiveness of a community-based non-communicable disease treatment programme in rural Mexico: a difference-in-differences analysis
Abstract Non-communicable diseases (NCDs) account for the five largest contributors to burden of disease in Mexico, with diabetes representing the greatest contributor. However, evidence supporting chronic disease programmes in Mexico is limited, especially in rural communities. Compañeros En Salud (CES) partnered with the Secretariat of Health of Chiapas, Mexico to implement a novel community-based NCD treatment programme. We describe the implementation of this programme and conducted a population-based, retrospective analysis, using a difference-in-differences regression approach to estimate the impact of the programme. Specifically, we examined changes in diabetes and hypertension control rates between 2014 and 2016, comparing CES intervention clinics (n = 9) to care-as-usual at non-CES clinics (n = 806), adjusting for differences in facility-level characteristics. In 2014, the percent of diabetes patients with this condition under control was 36.9% at non-CES facilities, compared with 41.3% at CES facilities (P > 0.05). For hypertension patients, these figures were 45.2% at non-CES facilities compared with 56.2% at CES facilities (P = 0.02). From 2014 to 2016, the percent of patients with diabetes under control declined by 9.2% at non-CES facilities, while improving by 11.3% at non-CES facilities where the Compañeros En Salud Programa de Enfermedades Crónicas intervention was implemented (P 0.05). Introduction of the CES model of NCD care was associated with significantly greater improvements in diabetes management between 2014 and 2016, compared with care-as-usual. Hypertension control measures were already greater at CES facilities in 2014, a difference that was maintained through 2016. These findings highlight the successful implementation of a framework for providing NCD care in rural Mexico, where a rapidly increasing NCD disease burden exists
