    Acute appendicitis following blunt abdominal trauma. Coincidence or causality?

    La apendicitis aguda es una enfermedad frecuente en la práctica clínica; entre sus causas bien definidas están la obstrucción luminal por fecalitos, la hiperplasia linfoide, los cuerpos extraños y los parásitos intestinales. El trauma abdominal cerrado se ha asociado como un factor etiológico de esta enfermedad; sin embargo, su relación causal aún no es clara. En el presente artículo, se presenta el caso de un paciente con apendicitis posterior a trauma abdominal cerrado.Acute appendicitis is a common disease in clinical practice; some well-defined causes include luminal obstruction by fecoliths, lymphoid hyperplasia, foreign bodies and intestinal parasites.     El trabajo realizado en este documento representa lo que se ha aprendido durante el desarrollo del diplomado de profundización de Linux, en el cual se aprendió a administrar y gestionar usuarios por medio de la estructuración de permisos, administrando los recursos del sistema operativo GNU/Linux. Se realizo un enfoque en todo lo relacionado a la administración de sistemas informáticos. Para la solución de este paso se utilizó software como VirtualBox para la virtualización de las máquinas, la imagen ISO de Zentyal versión 6.2 y una distribución de Linux para conectarse como cliente, en este caso se usó Linux Mint y Ubuntu Desktop. En el presente informe se realiza la administración y control de una distribución, pero enfocada a la implementación de servicios de infraestructura IT de mayo nivel para intranet y extranet en instituciones complejas.The work carried out in this document represents what has been learned during the development of the Linux deepening diploma, in which it was learned to administer and manage users through the structuring of permissions, managing the resources of the GNU / Linux operating system. A focus was made on everything related to the administration of computer systems. For the solution of this step, software such as VirtualBox was used to virtualize the machines, the ISO image of Zentyal version 6.2 and a Linux distribution to connect as a client, in this case Linux Mint and Ubuntu Desktop were used.     Dating historical droughts from religious ceremonies, the international pro pluvia rogation database

    Climate proxy data are required for improved understanding of climate variability and change in the pre-instrumental period. We present the first international initiative to compile and share information on pro pluvia rogation ceremonies, which is a well-studied proxy of agricultural drought. Currently, the database has more than 3500 dates of celebration of rogation ceremonies, providing information for 153 locations across 11 countries spanning the period from 1333 to 1949. This product provides data for better understanding of the pre-instrumental drought variability, validating natural proxies and model simulations, and multi-proxy rainfall reconstructions, amongst other climatic exercises.     A global drought monitoring system and dataset based on ERA5 reanalysis: A focus on crop-growing regions

They are useful for determining the drought onset and development and defining the spatial extent of drought at any time. Effective drought monitoring requires databases with high spatial and temporal resolution and large spatial and temporal coverage. Recent reanalysis datasets meet these requirements and offer an excellent alternative to observational data. In addition, reanalysis data allow better quantification of some variables that affect drought severity and are more seldom observed. This study presents a global drought dataset and a monitoring system based on the Standardized Precipitation Evapotranspiration Index (SPEI) and ERA5 reanalysis data. Computation of the atmospheric evaporative demand for the SPEI follows the FAO-56 Penman-Monteith equation. The system is updated weekly, providing near real-time information at a 0.5° spatial resolution and global coverage. It also contains a historical dataset with the values of the SPEI at different time scales since January 1979. The drought monitoring system includes the assessment of drought severity for dominant crop-growing areas. A comparison between SPEI computed from the ERA5 and CRU datasets shows generally good spatial and temporal agreement, albeit with some important differences originating mainly from the different spatial patterns of SPEI anomalies, as well as from employing long-term climate trends for different regions worldwide. The results show that the ERA5 dataset offers robust results and supports its use for drought monitoring.     Research on heart failure in Colombia 1980-2015: a systematic review

    Introduction: Heart failure is one of the most prevalent diseases worldwide. In Colombia, the state of research on the subject is unknown.Objective: To describe the original publications on heart failure in Colombia.Materials and methods: Systematic review. Digital search in Embase, PubMed, LILACS and Scielo, using the MeSH terms: “heart failure”, “Colombian”, “Colombia”, “Latin America”, “developing countries”. Manual search of 58 journals identified in Publindex. Original research that evaluated adult Colombians with heart failure and published between 1980 and 2015 were included.Results: 2 684 articles were identified, of which 35 met the inclusion criteria. 30 (85.7%) were published since 2009, 30 (85.7%) were conducted in Bogotá and Medellín, 11 (31.4%) had n200, 19 (54.2%) were descriptive and 5 (14.2%) quasi-experimental. Moreover, 9 (25.7%) described general populations, 9 (25.7%) addressed the issue of self-care, 3 (8.8%) cardiac rehabilitation, 3 (8.8%) perception of the disease and 3 (8.8%) prognostic factors.Conclusions: The amount of published original research on heart failure is low, and most of them were carried out recently. Descriptive design was the most frequent, while the most frequently addressed topics were self-care and population descriptions.Introducción. La falla cardíaca es una de las enfermedades con mayor prevalencia a nivel mundial.     Prescripción de analgésicos y reacciones adversas en pacientes hospitalizados

    Justificación. El dolor tiene alta prevalencia en la población general, siendo mayor en pacientes hospitalizados, con cifras cercanas al 80%. El control del dolor no siempre es adecuado debido a varios factores, siendo uno de los más importantes, el escasoconocimiento en el manejo de los analgésicos. Como una de las primeras medidasde mejoramiento asistencial para reducir el dolor en un hospital, debe conocerse la prevalencia de este síntoma y, asimismo, debe analizarse el abordaje terapéutico realizado.Es por esto por lo que este estudio tuvo como objetivo analizar el uso de analgésicos en pacientes de un hospital de cuarto nivel en Bogotá, Colombia, identificar los analgésicos más utilizados, el rango de dosis, las posibles interacciones y reacciones adversas a medicamentos (RAM) existentes. Resultados. El 78,9% de los pacienteshospitalizados presentaban dolor. Se prescribió analgésicos en 561 pacientes hospitalizados.Entre los más utilizados se encuentran: el acetaminofén (33,3%) y eltramadol (29,7%). El rango de duración de la terapia analgésica más común fue de 1 a 5 días (81,7%). Se identificaron 37 casos de sobredosificación y tres de terapia duplicada. Se presentaron 28 casos de reacciones adversas a medicamento (RAM).Discusión. El adecuado tratamiento del dolor cobra vital importancia al evaluar la satisfacción del paciente. Se encuentran diferentes tendencias de uso de analgésicos en pacientes hospitalizados, siendo los más utilizados el acetaminofén y los antiinflamatoriosno esteroideos (AINE). Se debe seguir investigando en este tema para tener evidencias científicas más firmes que permitan dictar unos lineamientos de aplicación universal para un correcto alivio del dolor.Justification. Pain has a high prevalence in the general population, being higher in inpatient, with figures close to 80%. Pain management is not always suitable due to several factors, one of the most important the lack of knowledge in the managementof analgesics. One of the first measures of improving care to reduce pain in a hospital should be determine the prevalence of this symptom and analyze the therapeutical proach performed. That is why the aim of this study was to analyze the use of analgesics in patients of a fourth-level hospital in Bogotá, Colombia, identify the mostcommonly used analgesics, range of doses, possible interactions and adverse drugs reactions. Results. The 78.9% of the patients were pain. Analgesics were prescribed in 561 hospitalized patients. The most used were acetaminophen (33.3%) and tramadol(29.7%). The most common range of duration of analgesic therapy was 1-5 days (81.7%). 37 cases of overdosing and 3 cases of duplicate therapy were identified. 28 cases of adverse drug reactions (ADRs) were presented. Discussion. Proper treatmentof pain plays a vital role in assessing patient satisfaction. There are different trends analgesic use in inpatient, being the most used acetaminophen and nonsteroidal anti-inflammatory drugs (NSAIDs). It should further investigate on this issueto have stronger scientific evidence to dictate a universal application guidelines forproper pain relief

    El avance acelerado de la tecnología en la sociedad actual hace que, continuamente, se esté repensando la forma de llevar a cabo las actividades formativas en los diferentes campos del saber con el fin de obtener métodos más eficientes, eficaces e innovadores. Esta avalancha de cambios también invade al entorno educativo y obliga a que la sociedad se plantee una serie de interrogantes que cuestionan todos los ámbitos de la labor docente. La influencia en el ámbito educativo no solamente se limita al impacto que tiene el desarrollo de nuevas aplicaciones y programas en la labor del proceso de enseñanza-aprendizaje, sino que también debe reconocerse el impacto que tiene sobre el currículo la incursión de nuevas técnicas, tecnologías y softwares, dado que modifican las competencias requeridas para el ejercicio profesional.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio
