    The role of character strengths in the mental health of informal dementia caregivers

    El rol de cuidador informal de una persona con demencia tiene una serie de consecuencias negativas, siendo la sobrecarga del cuidador la más estudiada. Sin embargo, también tiene consecuencias positivas, usándose el término ganancia para referirse a cualquier beneficio afectivo o práctico experimentado como resultado de ser un cuidador. El principal marco teórico para explicar las consecuencias del cuidado es el modelo del proceso del estrés, que tiene en cuenta varios factores que interactúan y determinan como una persona reacciona a este rol: factores contextuales, estresores primarios y secundarios, y mediadores y moderadores. Los valores se han propuesto como un recurso psicosocial que pueden mediar o moderar la relación entre los estresores y la salud mental de los cuidadores. Esos valores podrían estudiarse desde la perspectiva de la psicología positiva, y más en concreto, utilizando la clasificación VIA de las fortalezas psicológicas. Estas fortalezas psicológicas son rasgos positivos que se manifiestan a través del pensamiento, el sentimiento, la voluntad y la acción. El objetivo de la presente tesis es analizar el rol de las fortalezas psicológicas en cuidadores informales de personas con demencia, de acuerdo con el modelo del proceso del estrés, estudiando su relación con la sobrecarga del cuidador, con las ganancias percibidas, y con la satisfacción vital. Los resultados indicaron que la fortaleza esperanza es la que mejor predice la sobrecarga (asociación negativa), las ganancias y la satisfacción vital (ambas con asociación positiva). Además, tiene un efecto mediador entre la relación entre el estrés percibido por el cuidador y estas tres variables. Los resultados indican que esperanza es la principal fortaleza relacionada con el bienestar y la salud mental de los cuidadores informales. El desarrollo de esta fortaleza podría implicar una mejora en el bienestar de los cuidadores y reducir el impacto negativo del rol que desempeñan

We aimed to assess whether the availability of anti‐TNF agents impacts the need for early surgery in Crohn's disease (CD) and ulcerative colitis (UC). Methods Retrospective, cohort study of patients diagnosed within a 6‐year period before and after the licensing of anti‐TNFs (1990‐1995 and 2007‐2012 for CD; 1995‐2000 and 2007‐2012 for UC) were identified in the ENEIDA Registry. Surgery‐free survival curves were compared between cohorts. Results A total of 7,370 CD patients (2,022 in Cohort 1 and 5,348 in Cohort 2) and 8,069 UC patients (2,938 in Cohort 1 and 5,131 in Cohort 2) were included. Immunosuppressants were used significantly earlier and more frequently in both CD and UC post‐biological cohorts. The cumulative probability of surgery was lower in CD following anti‐TNF approval (16% and 11%, 22% and 16%, and 29% and 19%, at 1, 3 and 5 years, respectively p<0.0001), though not in UC (3% and 2%, 4% and 4%, and 6% and 5% at 1, 3 and 5 years, respectively; p=0.2). Ileal involvement, older age at diagnosis and active smoking in CD, and extensive disease in UC, were independent risk factors for surgery, whereas high‐volume IBD centres (in both CD and UC) and immunosuppressant use (in CD) were protective factors.     Non-productive angiogenesis disassembles Aß plaque-associated blood vessels

    The human Alzheimer’s disease (AD) brain accumulates angiogenic markers but paradoxically, the cerebral microvasculature is reduced around Aß plaques. Here we demonstrate that angiogenesis is started near Aß plaques in both AD mouse models and human AD samples. However, endothelial cells express the molecular signature of non-productive angiogenesis (NPA) and accumulate, around Aß plaques, a tip cell marker and IB4 reactive vascular anomalies with reduced NOTCH activity. Notably, NPA induction by endothelial loss of presenilin, whose mutations cause familial AD and which activity has been shown to decrease with age, produced a similar vascular phenotype in the absence of Aß pathology. We also show that Aß plaque-associated NPA locally disassembles blood vessels, leaving behind vascular scars, and that microglial phagocytosis contributes to the local loss of endothelial cells. These results define the role of NPA and microglia in local blood vessel disassembly and highlight the vascular component of presenilin loss of function in AD

    Characteristics of patients with type 2 diabetes mellitus newly treated with GLP-1 receptor agonists (CHADIG Study): a cross-sectional multicentre study in Spain

    Objective: Several glucagon-like peptide-1 (GLP-1) receptor agonists (GLP-1Ra) have been made recently available in Spain for type 2 diabetes mellitus (DM2) treatment. There are no published data on the clinical and sociodemographic profile of patients initiating treatment with GLP-1Ra in Spain. Our objective was to understand these patients' characteristics in a real-world clinical practice setting. Design: Cross-sectional observational study. Setting: Spanish specialist outpatient clinics. Participants: 403 adults with DM2 initiating GLP-1Ra treatment were included. Primary and secondary outcome measures: Sociodemographic and DM2-related clinical data, including treatment at and after GLP-1Ra initiation and comorbidities, were collected. Results: Evaluable patients (n=403; 50.9% female) were included ( July 2013 to March 2014) at 24 centres by 53 specialists (47 endocrinology, 6 internal medicine), with the following profile (value±SD): age (58.3±10.4 years), diabetes duration (9.9±7 years), body mass index (BMI; 36.2±5.5) and glycated haemoglobin (HbA1c; 8.4±1.4%); 14% had HbA1c≤7%. Previous antidiabetic treatment: 53.8% only oral antidiabetic drugs (OADs), 5.2% insulin and 40% insulin and OAD; of those receiving OAD, 35% single drug, 38.2% 2 drugs and 24% 3 drugs. Concomitant to GLP-1Ra, 55.3% were only on OAD, 36.2% on insulin and OAD, and 7.2% only on insulin. Of those receiving OAD, the GLP-1Ra was mainly associated with 1 drug (65%) or 2 drugs (31.8%). GLP-1Ra are frequently added to existing antidiabetic drugs, with dipeptidyl peptidase-4 inhibitors being the OAD most frequently switched (45% receiving 1 before starting GLP-1Ra, only 2.7% receiving it concomitantly). Conclusions: In Spain, GLP-1Ra therapy is usually started in combination with OADs or OADs and insulin.     RET Fusion Testing in Patients With NSCLC: The RETING Study

    Introduction: RET inhibitors with impressive overall response rates are now available for patients with NSCLC, yet the identi fication of RET fusions remains a dif ficult challenge. Most guidelines encourage the upfront use of next -generation sequencing (NGS), or alternatively, fluorescence in situ hybridization (FISH) or reverse transcriptase-polymerase chain reaction (RT-PCR) when NGS is not possible or available. Taken together, the suboptimal performance of single-analyte assays to detect RET fusions, although consistent with the notion of encouraging universal NGS, is currently widening some of the clinical practice gaps in the implementation of predictive biomarkers in patients with advanced NSCLC. Methods: This situation prompted us to evaluate several RET assays in a large multicenter cohort of RET fusion -positive NSCLC (n 1 / 4 38) to obtain real -world data. In addition to RNA -based NGS (the criterion standard method), all positive specimens underwent break -apart RET FISH with two different assays and were also tested by an RT-PCR assay. Results: The most common RET partners were KIF5B (78.9%), followed by CCDC6 (15.8%). The two RET NGSpositive but FISH -negative samples contained a KIF5B(15)RET(12) fusion. The three RET fusions not identi fied with RT-PCR were AKAP13(35)-RET(12) , KIF5B(24)-RET(9) and KIF5B(24)-RET(11) . All three false -negative RT-PCR cases were FISH -positive, exhibited a typical break -apart pattern, and contained a very high number of positive tumor cells with both FISH assays. Signet ring cells, psammoma bodies, and pleomorphic features were frequently observed (in 34.2%, 39.5%, and 39.5% of tumors, respectively). Conclusions: In-depth knowledge of the advantages and disadvantages of the different RET testing methodologies could help clinical and molecular tumor boards implement and maintain sensible algorithms for the rapid and effective detection of RET fusions in patients with NSCLC. The likelihood of RET false -negative results with both FISH and RT-PCR reinforces the need for upfront NGS in patients with NSCLC. (c) 2024 The Authors.     All Roads Lead to Rome: Results of Non-Invasive Respiratory Therapies Applied in a Tertiary-Care Hospital Without an Intermediate Care Unit During the COVID-19 Pandemic

    Introducción. Las terapias respiratorias no invasivas (TRNI) fueron ampliamente utilizadas en la primera ola de la pandemia de COVID-19, en escenarios distintos según los medios disponibles. El objetivo fue presentar la supervivencia a 90 días y los factores asociados a esta de los pacientes tratados con TRNI en un centro de tercer nivel sin Unidad de Cuidados Respiratorios Intermedios. Como objetivo secundario comparar los resultados obtenidos de las distintas terapias. Métodos. Estudio observacional de pacientes tratados con TRNI fuera de un ambiente de Cuidados Intensivos o Unidad de Cuidados Respiratorios Intermedios, diagnosticados de COVID-19 y con síndrome de distrés respiratorio agudo por criterios radiológicos y de ratio SpO2/FiO2. Se desarrolló un modelo multivariante de regresión logística para determinar las variables independientemente asociadas, y se compararon los resultados de la terapia de alto flujo con cánula nasal y la presión positiva continua en la vía aérea. Resultados. Se trataron 107 pacientes y sobrevivieron 85 (79,4%) a los 90 días. Antes de iniciar la TRNI el ratio medio de SpO2/FiO2 fue de 119,8±59,4. Un mayor score de SOFA se asoció significativamente a la mortalidad (OR 2,09; IC95% 1,34 – 3,27), mientras que la autopronación fue un factor protector (OR 0,23; IC95% 0,06 – 0,91). La terapia de alto flujo con cánula nasal fue utilizada en 63 sujetos (58,9%), y la presión positiva continua en la vía aérea en 41 (38,3%). No se encontraron diferencias entre ellas. Conclusión. Aproximadamente cuatro de cada cinco pacientes tratados con TRNI sobrevivieron a los 90 días, y no se encontraron diferencias significativas entre la terapia de alto flujo con cánula nasal y la presión positiva continua en la vía aérea.S

    La hipercolesterolemia familiar (HF) es una alteración de origen genético que clínicamente se puede manifestar desde el nacimiento y que se caracteriza por niveles plasmáticos anormalmente altos de colesterol LDL (cLDL) y por una elevada tasa de morbimortalidad cardiovascular prematura. Tiene dos formas de presentación: la HF heterocigótica (HFHe) y la HF homocigótica (HFHo); esta última más severa y de aparición clínica en los primeros años de vida. Históricamente, la prevalencia para la HFHe es de un caso en 500 personas y para la HFHo de un caso por cada millón de personas; sin embargo, los datos reales probablemente son superiores porque hay evidencia de que ambas condiciones están subdiagnosticadas. La terapia recomendada, además de los cambios en el estilo de vida, son las estatinas; sin embargo, con estos fármacos es difícil lograr en muchos casos reducciones aceptables del cLDL, por lo que se requiere asociar otras modalidades terapéuticas, algunas de ellas recientemente aprobadas. Dado que en Colombia no se ha publicado ningún documento de revisión sobre HF, la Sociedad Colombiana de Cardiología y Cirugía Cardiovascular convocó a diferentes especialidades de la medicina para elaborar un documento sobre el tema, que resumiera, de manera práctica y actualizada, aspectos clínicos, genéticos, diagnósticos y de tratamiento.Q44-26Familial hypercholesterolemia (FH) is a genetic disorder that may clinically manifest since birth and is characterized by abnormally high plasma LDL cholesterol (LDLc) levels and a high early cardiovascular morbidity and mortality rate. FH has two presentation forms: heterozygous FH (HeFH) and homozygous FH (HoFH), the latter being more severe and with a clinical onset during the first few years of life. Historically, HeFH prevalence is of 1:500 and HoFH of 1:1 million; however, real data are probably higher because evidence indicated that both conditions are underdiagnosed. Recommended therapy, besides lifestyle changes, are statins; nevertheless, these drugs make it difficult in many cases to achieve reasonable cLDL reductions, therefore an association with other therapeutic models, some of which have recently been approved, is required. Since no review papers have been published in Colombia regarding FH, the Colombian Cardiology and Cardiovascular Surgery Society invited several medical specialties to draft a document on the subject that would sum up, in a practical and updated way, clinical, genetics, diagnostics and therapeutic aspects
