PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks

    Perception and Perspective

    No full text
Your perception of reality is not reality itself. The lens through which you view the world―your unique perspective―shapes everything you experience. But how do perception and perspective interact? And how can understanding their subtle differences empower you to live a more examined and meaningful life? In this book, we'll explore the cognitive processes forming the human experience's foundation. Through compelling case studies and exercises, we'll illustrate how our perceptions are sculpted beginning in childhood by innate biases and environmental influences outside our control, and how perspective, the position from which we observe, retains potential for growth if we cultivate mindfulness and open-mindedness. Learning to distinguish perception from perspective is the first step to recognizing unconscious patterns and enlarging our sphere of thought. This journey of self-discovery is challenging but rewarding. Within these pages, you will find a map to reclaiming ownership over your internal and external worlds. The choice to see more clearly has never been more urgent or liberating

International audienceAustralian cinema has a magnificent history, as well as an excellent and distinctive style that draws viewers in and makes them fall more and more in love with it.Australian cinema has a long and illustrious history, beginning in October 1896 when a French pharmacist named Marius Sestier, an agent for Lumière et Compagnie, and a trained user of the Lumière cinematographe, captured footage of people getting off a ship in Manly, Sydney. The following month, Sestier photographed thirteen films at Flemington Racecourse, including the first images of the Melbourne Cup, known as "the race that stops the nation."The next year, four different filmmaking companies decided to capture the Cup on camera due to the popularity of the occasion and the rumoured Australian passion of sports. In the middle of the nineteenth century, the Edison Kinetoscope was used to project the first motion movies in Australia.James McMahon, a businessman in the theatre industry, built a Kinetoscope Parlour at 148 Pitt Street in Sydney on November 30, 1894.Customers used eyepieces on succeeding Kinetoscope machines to view the films for a shilling. Each device had a distinct film that could only be seen by one person at once. The first short films made in Australia for display focused on actual events like the Queen Victoria Jubilee Procession and the Melbourne Cup.Beginning in 1898, the Limelight Department of the Salvation Army also created short narrative films that were shown as part of their public performances that also included songs, instrumental solos, and lectures. With admission fees going towards funding the Salvation Army's social work, the shows served as a platform for the dissemination of religious propaganda.Australia's film industry has its beginnings in the year 1900.The majority of European firms had Australian sales representatives, there were established production facilities, and show methods were pioneered. The Story of the Kelly Gang, perhaps the first feature film ever made, marked the commencement of Australian cinema in 1906.Since then, several films have been made by Australian crews, some of which have gained attention abroad. Many performers and directors with international reputations got their start in Australian films, and many of them went on to build successful careers in countries with more established film industries, such the United States.Crocodile Dundee, George Miller's Mad Max: Fury Road, Baz Luhrmann's Moulin Rouge! and Chris Noonan's Babe are a few examples of Australian movies that have been financially successful.Picnic at Hanging Rock, Gallipoli, The Tracker, Shine, and Ten Canoes are a few productions that have won awards
