    El impacto de la deslaboralización en la cultura organizacional de las empresas floricultoras del Oriente Antioqueño

    Actualmente las empresas viven una continua división de sus formaciones sociales, pues la dinámica tiende a fraccionarlas en grupos humanos, pequeños y diversos, a consecuencia de las tendencias globalizantes. En este sentido, las instituciones pueden ser vistas como unidades productivas de grupos humanos, donde existen culturas heterogenias resultantes de micro-construcciones y procesos sociales con poca identidad. Desde el punto de vista teórico, es posible considerar esas organizaciones como tipologías sociales específicas o construcciones visibles que pueden ser planeadas, coordinadas y supervisadas de acuerdo con las características de los sistemas económico, social y político de la región y de la actividad que desarrollan, por lo que se hizo necesario indagar por la conformación de su cultura organizacional, especialmente en aquéllas que se catalogan como empresas productoras y comercializadoras de flores a nivel nacional e internacional En este contexto la cultura es un conjunto de acciones orientadoras de las interacciones entre los individuos que las conforman, la cual es susceptible de reproducirse superando conflictos, tensiones y contradicciones, de orden técnico, económico, legislativo y social constituyéndose en un conjunto de hábitos rutinas y costumbres inherentes a las formación, ejecución y proyección, para garantizar así su permanencia. Lo anterior significa que la cultura es un conjunto de prácticas, actitudes, roles, comportamiento concurrentes y recurrentes, que permiten reanudar las relaciones entre los individuos que conformen las empresas al interior y, en algunos casos en su exterior, a partir de la codificación, la difusión y la aceptación de políticas y normas compartidas por los individuos y los conglomerados humanos. Sin embargo, a través de la evolución de la empresa dedicada a esta actividad y de la sociedad se generan múltiples niveles de complejidad y diferenciación generando identidad y desarrollando estrategias de competitividad y comparación, tanto en lo social como en lo económico, lo cual permite contextualizar y conceptuar al respecto de la cultura y de las subculturas, mas aun ahora que las reformas laborales y la deslaboralizacion de las relaciones a impactado a este tipo de organizaciones. Aunque hay múltiples escritos sobre el tema de cultura organizacional es necesario elaborar uno que se centre en empresas dedicada a la producción de flores para mercados nacionales e internacionales las cuales adicionalmente están condicionadas a factores no solo económicos sino también climáticos o estacionales a consecuencia de que se constituyen en el punto de referencia para otras más pequeñas y de diferentes sectores al ser dominantes en la economía de la región por el número de personas que vinculan directa o indirectamente, la cantidad de productos generados y el aporte económico que hacen a la región y al país en general. Se puede recordar que el tema de cultura organizacional tiene el apoyo de algunos autores, pero de igual manera tiene opositores Sí se tiene muy en claro que se presenta una coincidencia de propósitos, intereses y saberes, lo que establece niveles de participación y apoyo. Dado que el ser humano en general es la base a partir de sus comportamientos, reacciones, actitudes y toma de posición a partir del cual se genera la cultura se propicia la estabilidad de las organizaciones. Se asume entonces que la cultura está compuesta por tres dimensiones: la intelectual, la afectiva y la social, al igual que la conducta de las persona permitiendo los proceso de conformación y transformación. Al investigar las empresas se hace necesario indagar y estudiar los espacios donde conviven y se desarrollan los seres humanos para satisfacer sus necesidades y, por lo tanto, también obliga a referirse al direccionamiento. Al aceptar que empresa, cultura, direccionamiento y necesidades humanas son elaboraciones sociales, sin olvidar que estas evolucionan a través del tiempo, tienen que estar vinculadas entre sí para generar bienes y servicios que satisfagan a las colectividades.Nowadays companies face a continuous division of its social formations, because the dynamic of globalisation tends to fracture them into small and diverse human groups. In this sense, institutions are seen as productive units of human groups, where heterogeneous cultures exist as a result of micro constructions and social processes with little identity. From a theoretical point of view, it is possible to consider those organisations as specific social typologies or visible constructions that can be planned, coordinated and monitored according to the characteristics of the economical, social and political systems of the region and the activity they develop, that’s the reason why inquiring on the formation of its organisational culture became necessary, especially for those companies that are catalogued in flowers production and commercialisation internationally. In this context, culture is a set of guiding actions for the interactions among its members which is susceptible of reproducing itself over conflicts, tensions and contradictions from a technical, economical, legal and social order creating a set of manners, routines and customs related to form, execution and projection to guarantee its survival in time. This means that culture is a set of practices, attitudes, roles and concurrent and recurrent behaviours that allow rebuilding relations between individuals that integrate enterprises from its interior and, in some cases its exterior, starting from coding, diffusion and acceptation of norms and policies shared by individuals and human conglomerates. Nevertheless, during societies and companies evolution in the business of flower trading many levels of complexities and differentiation are generated which creates identity and develops strategies of competitiveness and benchmarking in the social and economical realm, allowing the contextualization and definition of culture and its subcultures, even more now that labour changes and the trend to eliminate labour from the relations with the employee has impacted this type of organisations. Although there are many articles on organisational culture, it is necessary to appoint one centred on flower trading companies for national and international markets because these are dependent on economical, social, stationary and climatic factors and build a reference point for smaller companies and for different sectors since they are dominant in the region according to their employment rates, generated products and economical impact in the country. The topic of organisational culture has the support of several authors, but is has also its critics if you have cleared that many coincidences of purposes, interests and specific knowledge is present, establishing levels of participation and support. The human being in general is the sum of its behaviours, reactions, attitudes, among others, which generate the culture that propitiates the stability of organisations. When it comes to research on companies, it is necessary to inquiry and study the space where humans are developed and established to satisfy their needs and, forces to appoint the company’s vision.     Safety and efficacy of tirofiban in acute ischemic stroke due to tandem lesions undergoing mechanical thrombectomy: A multicenter randomized clinical trial (ATILA) protocol

    [Background] In-stent thrombosis after mechanical thrombectomy (MT) worsen outcomes in acute ischemic stroke (AIS) due to tandem lesions (TL). Although an optimal antiplatelet therapy is needed, the best approach to avoid in-stent thrombosis is yet to be elucidated.[Hypothesis] Low-dose intravenous tirofiban is superior to intravenous aspirin in avoiding in-stent thrombosis in patients undergoing MT plus carotid stenting in the setting of AIS due to TL.[Methods] The ATILA-trial is a multicenter, prospective, phase IV, randomized, controlled (aspirin group as control), assessor-blinded clinical trial. Patients fulfilling inclusion criteria (AIS due to TL, ASPECTS ⩾ 6, pre-stroke modified Rankin Scale ⩽2 and onset <24 h) will be randomized (1:1) at MT onset to experimental (intravenous tirofiban) or control group (intravenous aspirin). Intravenous aspirin will be administered at a 500 mg single dose and tirofiban at a 500 µg bolus followed by a 200 µg/h infusion during first 22 h. All patients will be followed up to 3 months. Sample size estimated is 240 patients.[Outcomes] The primary efficacy outcome is the proportion of patients with carotid in-stent thrombosis within the first 24 h after MT. The primary safety outcome is the rate of symptomatic intracranial hemorrhage. Secondary outcomes include functional independence defined as modified Rankin Scale 0–2, proportion of patients undergoing rescue therapy due to in-stent aggregation during MT and carotid reocclusion at 30 days.[Discussion] ATILA-trial will be the first clinical trial regarding the best antiplatelet therapy to avoid in-stent thrombosis after MT in patients with TL.[Trial registration] NCT0522596.This project was funded by the Instituto de Salud Carlos III (ISCIII) through the project PI21/01322 and co-funded by the European Union.     Newer generations of multi-target CAR and STAb-T immunotherapeutics: NEXT CART Consortium as a cooperative effort to overcome current limitations

    Adoptive T cellular immunotherapies have emerged as relevant approaches for treating cancer patients who have relapsed or become refractory (R/R) to traditional cancer treatments. Chimeric antigen receptor (CAR) T-cell therapy has improved survival in various hematological malignancies. However, significant limitations still impede the widespread adoption of these therapies in most cancers. To advance in this field, six research groups have created the “NEXT Generation CART MAD Consortium” (NEXT CART) in Madrid’s Community, which aims to develop novel cell-based immunotherapies for R/R and poor prognosis cancers. At NEXT CART, various basic and translational research groups and hospitals in Madrid concur to share and synergize their basic expertise in immunotherapy, gene therapy, and immunological synapse, and clinical expertise in pediatric and adult oncology. NEXT CART goal is to develop new cell engineering approaches and treatments for R/R adult and pediatric neoplasms to evaluate in multicenter clinical trials. Here, we discuss the current limitations of T cell-based therapies and introduce our perspective on future developments. Advancement opportunities include developing allogeneic products, optimizing CAR signaling domains, combining cellular immunotherapies, multi-targeting strategies, and improving tumor-infiltrating lymphocytes (TILs)/T cell receptor (TCR) therapy. Furthermore, basic studies aim to identify novel tumor targets, tumor molecules in the tumor microenvironment that impact CAR efficacy, and strategies to enhance the efficiency of the immunological synapse between immune and tumor cells. Our perspective of current cellular immunotherapy underscores the potential of these treatments while acknowledging the existing hurdles that demand innovative solutions to develop their potential for cancer treatment fully

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    (1) Aims: To assess the incidence of inflammatory bowel disease (IBD) in Spain, to describe the main epidemiological and clinical characteristics at diagnosis and the evolution of the disease, and to explore the use of drug treatments. (2) Methods: Prospective, population-based nationwide registry. Adult patients diagnosed with IBD-Crohn's disease (CD), ulcerative colitis (UC) or IBD unclassified (IBD-U)-during 2017 in Spain were included and were followed-up for 1 year. (3) Results: We identified 3611 incident cases of IBD diagnosed during 2017 in 108 hospitals covering over 22 million inhabitants. The overall incidence (cases/100,000 person-years) was 16 for IBD, 7.5 for CD, 8 for UC, and 0.5 for IBD-U; 53% of patients were male and median age was 43 years (interquartile range = 31-56 years). During a median 12-month follow-up, 34% of patients were treated with systemic steroids, 25% with immunomodulators, 15% with biologics and 5.6% underwent surgery. The percentage of patients under these treatments was significantly higher in CD than UC and IBD-U. Use of systemic steroids and biologics was significantly higher in hospitals with high resources. In total, 28% of patients were hospitalized (35% CD and 22% UC patients, p < 0.01). (4) Conclusion: The incidence of IBD in Spain is rather high and similar to that reported in Northern Europe. IBD patients require substantial therapeutic resources, which are greater in CD and in hospitals with high resources, and much higher than previously reported. One third of patients are hospitalized in the first year after diagnosis and a relevant proportion undergo surgery

    Albiglutide and cardiovascular outcomes in patients with type 2 diabetes and cardiovascular disease (Harmony Outcomes): a double-blind, randomised placebo-controlled trial

    Background: Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke. Methods: We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. This study is registered with ClinicalTrials.gov, number NCT02465515. Findings: Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. On Nov 8, 2017, it was determined that 611 primary endpoints and a median follow-up of at least 1·5 years had accrued, and participants returned for a final visit and discontinuation from study treatment; the last patient visit was on March 12, 2018. These 9463 patients, the intention-to-treat population, were evaluated for a median duration of 1·6 years and were assessed for the primary outcome. The primary composite outcome occurred in 338 (7%) of 4731 patients at an incidence rate of 4·6 events per 100 person-years in the albiglutide group and in 428 (9%) of 4732 patients at an incidence rate of 5·9 events per 100 person-years in the placebo group (hazard ratio 0·78, 95% CI 0·68–0·90), which indicated that albiglutide was superior to placebo (p&lt;0·0001 for non-inferiority; p=0·0006 for superiority). The incidence of acute pancreatitis (ten patients in the albiglutide group and seven patients in the placebo group), pancreatic cancer (six patients in the albiglutide group and five patients in the placebo group), medullary thyroid carcinoma (zero patients in both groups), and other serious adverse events did not differ between the two groups. There were three (&lt;1%) deaths in the placebo group that were assessed by investigators, who were masked to study drug assignment, to be treatment-related and two (&lt;1%) deaths in the albiglutide group. Interpretation: In patients with type 2 diabetes and cardiovascular disease, albiglutide was superior to placebo with respect to major adverse cardiovascular events. Evidence-based glucagon-like peptide 1 receptor agonists should therefore be considered as part of a comprehensive strategy to reduce the risk of cardiovascular events in patients with type 2 diabetes.     Este volumen acoge la memoria académica de la Cuarta edición del Congreso Internacional de Ciencia, Tecnología e Innovación para la Sociedad, CITIS 2017, desarrollado entre el 29 de noviembre y el 1 de diciembre de 2017 y organizado por la Universidad Politécnica Salesiana (UPS) en su sede de Guayaquil. El Congreso ofreció un espacio para la presentación, difusión e intercambio de importantes investigaciones nacionales e internacionales ante la comunidad universitaria que se dio cita en el encuentro. El uso de herramientas tecnológicas para la gestión de los trabajos de investigación como la plataforma Open Conference Systems y la web de presentación del Congreso http://citis.blog.ups.edu.ec/, hicieron de CITIS 2017 un verdadero referente entre los congresos que se desarrollaron en el país. La preocupación de nuestra Universidad, de presentar espacios que ayuden a generar nuevos y mejores cambios en la dimensión humana y social de nuestro entorno, hace que se persiga en cada edición del evento la presentación de trabajos con calidad creciente en cuanto a su producción científica. Quienes estuvimos al frente de la organización, dejamos plasmado en estas memorias académicas el intenso y prolífico trabajo de los días de realización del Congreso Internacional de Ciencia, Tecnología e Innovación para la Sociedad al alcance de todos y todas
