Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

Evaluación de la comodidad auditiva en ambientes pedagógicos de la Universidad Católica de Manizales, Colombia

The listening comfort is an element that affects people’s health and their quality of life. The listening comfort may or may not benefit the teaching and learning processes. For that reason, this research shows the studies done regarding ambient noise and time of reverberation in the basic pedagogical internal environments located at the Catholic University of Manizales, which have been classified and analyzed according to the Colombian regulation NTC 4595 for the planning and design of facilities and educational settings. There have been studied twenty-twoareas, which represent the general acoustic conditions in the campus. The results showed a compliance rate of 5% in both ambient noise and reverberation time, proving the need to implement policies and guidelines to improve soundproofing and acoustic conditioning, towards generate optimal learning spaces.La comodidad auditiva es un elemento que afecta la calidad de vida y salud de las personas y puede favorecer o no los procesos de enseñanza y aprendizaje. Esta investigación muestra los estudios sobre ruido ambiental y tiempo de reverberación en los ambientes pedagógicos internos básicos de la UniversidadCatólica de Manizales, clasificados y analizados según la norma técnica colombiana NTC 4595 para el planeamiento y diseño de instalaciones y ambientes escolares. Se estudiaron en total 22 espacios que representan las condicionesgenerales acústicas del campus. Los resultados arrojaron un cumplimiento del 5% tanto en el ruido ambiental, como en el tiempo de reverberación, reflejando la necesidad de implementar políticas y lineamientos para mejorar el aislamiento y acondicionamiento acústico, en pro de generar espacio óptimos para el aprendizaje

Evaluación lumínica de los ambientes pedagógicos de la Universidad Católica de Manizales.

This work evaluated lighting parameters of 23 basic internal pedagogic environments (APIB) at Catholic University of Manizales city, through a diagnostic methodology under in situ measurements, and according to recommendations of Colombian Technical Standard for Planning and Design of School Installations and Environments (NTC 4595 for its Spanish abbreviation). Results yielded a compliance percentage between 17% and 56%, according to the variable, and it was conclude that in 65% of the APIB, the cause of average illuminance noncompliance is due to design factors of artificial lighting installations. Finally, a methodology was established which may be copied at any education institution in order to evaluate luminous conditions of their APIB.Este trabajo evaluó los parámetros lumínicos de 23 ambientes pedagógicos internos básicos (APIB) de la Universidad Católica de Manizales, mediante una metodología diagnóstica con mediciones in situ y acorde a las recomendaciones de la Norma Técnica Colombiana para el Planeamiento y Diseño de Instalaciones y Ambientes Escolares (NTC 4595). Los resultados arrojaron porcentajes de cumplimiento entre el 17% y 56% según la variable y se pudo concluir que en el 65% de los APIB, la causa de incumplimiento de la iluminancia media se debe a factores de diseño en las instalaciones de iluminación artificial. Finalmente, se logró establecer una metodología que puede ser replicada en cualquier institución educativa que busque evaluar las condiciones lumínicas de sus APIB

Sustainable university campus

La sustentabilidad en universidades tiene como reto proporcionar conocimientos que permitan el desarrollo institucional desde sus funciones sustantivas. De ahí que el estudio buscó construir un concepto de campus universitario sustentable identificando variables y dinámicas propias de las universidades. Para ello se planteó una metodología descriptiva con dos fases; la primera exploró teóricamente el término “desarrollo sostenible” en la historia, para entender el concepto de campus universitario sustentable; la segunda fase, de tipo cualitativo, indaga y reconoce la realidad de la Universidad Católica de Manizales (UCM) frente al ejercicio de la sustentabilidad en el campus. Se identificaron particularidades en las esferas del desarrollo sustentable ampliando el rango de las esferas a la académica y físicoespacial de los ambientes de aprendizaje internos y externos, encontrando su aplicación en los campus universitarios; por último, fue posible reconocer estrategias y acciones implementadas en la UCM, encaminadas a integrar conceptos de campus sustentables.Sustainability in universities faces the challenge of providing knowledge that allows institutional development based on its substantive functions. Hence, the study sought to develop a concept of sustainable university campus by identifying variables and dynamics proper to the university. For this purpose, a descriptive methodology was proposed with two phases. The first phase theoretically explores the term “sustainable development” in history, in order to understand the concept of sustainable university campus. The second phase, of qualitative type, investigates and acknowledges the reality of the Universidad Católica de Manizales (UCM) regarding the exercise of sustainability on campus. Distinctive features were identified in the fields of sustainable development, expanding their range to the academic and physical-spatial areas of internal and external learning environments, finding their application on university campuses. Finally, it was possible to evidence strategies and actions implemented at the UCM, aimed at integrating concepts of sustainable campus

La Extensión Universitaria como una herramienta para la apropiación social del conocimiento

ENGIU: Encuentro Nacional de Grupos de Investigación de UNIMINUTO.

El desarrollo del prototipo para el sistema de detección de Mina Antipersona (MAP), inicia desde el semillero ADSSOF perteneciente al programa de Administración en Seguridad y Salud en el trabajo de la UNIMINUTO, se realiza a partir de un detector de metales que emite una señal audible, que el usuario puede interpretar como aviso de presencia de un objeto metálico, en este caso una MAP. La señal audible se interpreta como un dato, como ese dato no es perceptible a 5 metros de distancia, se implementa el transmisor de Frecuencia Modulada FM por la facilidad de modulación y la escogencia de frecuencia de transmisión de acuerdo con las normas y resolución del Ministerio de Comunicaciones; de manera que esta sea la plataforma base para enviar los datos obtenidos a una frecuencia establecida. La idea es que el ser humano no explore zonas peligrosas y buscar la forma de crear un sistema que permita eliminar ese riesgo, por otro lado, buscar la facilidad de uso de elementos ya disponibles en el mercado

Prospective observational cohort study on grading the severity of postoperative complications in global surgery research

Background The Clavien–Dindo classification is perhaps the most widely used approach for reporting postoperative complications in clinical trials. This system classifies complication severity by the treatment provided. However, it is unclear whether the Clavien–Dindo system can be used internationally in studies across differing healthcare systems in high- (HICs) and low- and middle-income countries (LMICs). Methods This was a secondary analysis of the International Surgical Outcomes Study (ISOS), a prospective observational cohort study of elective surgery in adults. Data collection occurred over a 7-day period. Severity of complications was graded using Clavien–Dindo and the simpler ISOS grading (mild, moderate or severe, based on guided investigator judgement). Severity grading was compared using the intraclass correlation coefficient (ICC). Data are presented as frequencies and ICC values (with 95 per cent c.i.). The analysis was stratified by income status of the country, comparing HICs with LMICs. Results A total of 44 814 patients were recruited from 474 hospitals in 27 countries (19 HICs and 8 LMICs). Some 7508 patients (16·8 per cent) experienced at least one postoperative complication, equivalent to 11 664 complications in total. Using the ISOS classification, 5504 of 11 664 complications (47·2 per cent) were graded as mild, 4244 (36·4 per cent) as moderate and 1916 (16·4 per cent) as severe. Using Clavien–Dindo, 6781 of 11 664 complications (58·1 per cent) were graded as I or II, 1740 (14·9 per cent) as III, 2408 (20·6 per cent) as IV and 735 (6·3 per cent) as V. Agreement between classification systems was poor overall (ICC 0·41, 95 per cent c.i. 0·20 to 0·55), and in LMICs (ICC 0·23, 0·05 to 0·38) and HICs (ICC 0·46, 0·25 to 0·59). Conclusion Caution is recommended when using a treatment approach to grade complications in global surgery studies, as this may introduce bias unintentionally

The surgical safety checklist and patient outcomes after surgery: a prospective observational cohort study, systematic review and meta-analysis

© 2017 British Journal of Anaesthesia Background: The surgical safety checklist is widely used to improve the quality of perioperative care. However, clinicians continue to debate the clinical effectiveness of this tool. Methods: Prospective analysis of data from the International Surgical Outcomes Study (ISOS), an international observational study of elective in-patient surgery, accompanied by a systematic review and meta-analysis of published literature. The exposure was surgical safety checklist use. The primary outcome was in-hospital mortality and the secondary outcome was postoperative complications. In the ISOS cohort, a multivariable multi-level generalized linear model was used to test associations. To further contextualise these findings, we included the results from the ISOS cohort in a meta-analysis. Results are reported as odds ratios (OR) with 95% confidence intervals. Results: We included 44 814 patients from 497 hospitals in 27 countries in the ISOS analysis. There were 40 245 (89.8%) patients exposed to the checklist, whilst 7508 (16.8%) sustained ≥1 postoperative complications and 207 (0.5%) died before hospital discharge. Checklist exposure was associated with reduced mortality [odds ratio (OR) 0.49 (0.32–0.77); P\u3c0.01], but no difference in complication rates [OR 1.02 (0.88–1.19); P=0.75]. In a systematic review, we screened 3732 records and identified 11 eligible studies of 453 292 patients including the ISOS cohort. Checklist exposure was associated with both reduced postoperative mortality [OR 0.75 (0.62–0.92); P\u3c0.01; I2=87%] and reduced complication rates [OR 0.73 (0.61–0.88); P\u3c0.01; I2=89%). Conclusions: Patients exposed to a surgical safety checklist experience better postoperative outcomes, but this could simply reflect wider quality of care in hospitals where checklist use is routine
