Brief advice and active referral for smoking cessation services among community smokers: a study protocol for randomized controlled trial

Abstract Background Most smokers do not use smoking cessation (SC) services although it increases successful quits. Passive referral providing SC information to smokers is commonly used in SC studies. Little was known about active referral in the community setting. This study aims to motivate community smokers to quit by brief SC advice using a validated AWARD model (Ask, Warn, Advise, Refer and Do-it-again) that adjunct with active referral of smokers to various SC services in Hong Kong. Methods/Design This is a single-blinded, parallel three-armed cluster randomized controlled trial (RCT) with two treatment groups of (1) brief SC advice using the AWARD model, active referral to SC services plus a referral card and a health warning leaflet (active referral group) and (2) brief SC advice using AWARD model and health warning leaflet (brief advice group) and a control group receives general very brief advice with a self-help booklet. A total of 1291 smokers will be recruited from 66 clusters (recruitment sessions) with 22 will be allocated to each of the two intervention and one control groups. SC ambassadors will be trained for delivering the interventions and conducting telephone follow-up. The primary outcomes are self-reported 7-days point prevalence (PP) abstinence at 3 and 6 months follow-up. Intention-to-treat principle and multi-level regressions will be used for data analysis. Discussion This is the first RCT on assessing a model combining brief advice and active referral to SC services among community smokers. The results will inform the practices of SC services and intervention studies. Trial registration NCT02539875 (ClinicalTrials.gov registry; registered retrospectively on 22 July 2015

Cosmological Constraints on Decaying Dark Matter

We present a complete analysis of the cosmological constraints on decaying dark matter. Previous analyses have used the cosmic microwave background and Type Ia supernova. We have updated them with the latest data as well as extended the analysis with the inclusion of Lyman-$\alpha$ forest, large scale structure and weak lensing observations. Astrophysical constraints are not considered in the present paper. The bounds on the lifetime of decaying dark matter are dominated by either the late-time integrated Sachs-Wolfe effect for the scenario with weak reionization, or CMB polarization observations when there is significant reionization. For the respective scenarios, the lifetimes for decaying dark matter are $\Gamma^{-1} \gtrsim 100$ Gyr and $(f \Gamma) ^{-1} \gtrsim 5.3 \times 10^8$ Gyr (at 95.4% confidence level), where the phenomenological parameter $f$ is the fraction of the decay energy deposited in baryonic gas. This allows us to constrain particle physics models with dark matter candidates through investigation of dark matter decays into Standard Model particles via effective operators. For decaying dark matter of $\sim 100$ GeV mass, we found that the size of the coupling constant in the effective dimension-4 operators responsible for dark matter decay has to generically be $\lesssim 10^{-22}$. We have also explored the implications of our analysis for representative models in theories of gauge-mediated supersymmetry breaking, minimal supergravity and little Higgs.Comment: 29 pages, 6 figures. Robust estimation of bacterial cell count from optical density

Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data

Surgical site infection after gastrointestinal surgery in high-income, middle-income, and low-income countries: a prospective, international, multicentre cohort study

Background: Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world. Methods: This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials.gov, number NCT02662231. Findings: Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58·5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31·2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (10·2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12·3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9·4%] of 7339 patients), middle (549 [14·0%] of 3918 patients), and low (298 [23·2%] of 1282) HDI (p < 0·001). The highest SSI incidence in each HDI group was after dirty surgery (102 [17·8%] of 574 patients in high-HDI countries; 74 [31·4%] of 236 patients in middle-HDI countries; 72 [39·8%] of 181 patients in low-HDI countries). Following risk factor adjustment, patients in low-HDI countries were at greatest risk of SSI (adjusted odds ratio 1·60, 95% credible interval 1·05–2·37; p=0·030). 132 (21·6%) of 610 patients with an SSI and a microbiology culture result had an infection that was resistant to the prophylactic antibiotic used. Resistant infections were detected in 49 (16·6%) of 295 patients in high-HDI countries, in 37 (19·8%) of 187 patients in middle-HDI countries, and in 46 (35·9%) of 128 patients in low-HDI countries (p < 0·001). Interpretation: Countries with a low HDI carry a disproportionately greater burden of SSI than countries with a middle or high HDI and might have higher rates of antibiotic resistance. In view of WHO recommendations on SSI prevention that highlight the absence of high-quality interventional research, urgent, pragmatic, randomised trials based in LMICs are needed to assess measures aiming to reduce this preventable complication

The same neuron may play different functional roles in the neural circuits to which it belongs. For example, neurons in the Tritonia pedal ganglia may participate in variable phases of the swim motor rhythms [1]. While such neuronal functional variability is likely to play a major role the delivery of the functionality of neural systems, it is difficult to study it in most nervous systems. We work on the pyloric rhythm network of the crustacean stomatogastric ganglion (STG) [2]. Typically network models of the STG treat neurons of the same functional type as a single model neuron (e.g. PD neurons), assuming the same conductance parameters for these neurons and implying their synchronous firing [3, 4]. However, simultaneous recording of PD neurons shows differences between the timings of spikes of these neurons. This may indicate functional variability of these neurons. Here we modelled separately the two PD neurons of the STG in a multi-neuron model of the pyloric network. Our neuron models comply with known correlations between conductance parameters of ionic currents. Our results reproduce the experimental finding of increasing spike time distance between spikes originating from the two model PD neurons during their synchronised burst phase. The PD neuron with the larger calcium conductance generates its spikes before the other PD neuron. Larger potassium conductance values in the follower neuron imply longer delays between spikes, see Fig. 17.Neuromodulators change the conductance parameters of neurons and maintain the ratios of these parameters [5]. Our results show that such changes may shift the individual contribution of two PD neurons to the PD-phase of the pyloric rhythm altering their functionality within this rhythm. Our work paves the way towards an accessible experimental and computational framework for the analysis of the mechanisms and impact of functional variability of neurons within the neural circuits to which they belong

Ultralight vector dark matter search using data from the KAGRA O3GK run

Among the various candidates for dark matter (DM), ultralight vector DM can be probed by laser interferometric gravitational wave detectors through the measurement of oscillating length changes in the arm cavities. In this context, KAGRA has a unique feature due to differing compositions of its mirrors, enhancing the signal of vector DM in the length change in the auxiliary channels. Here we present the result of a search for U(1)B−L gauge boson DM using the KAGRA data from auxiliary length channels during the first joint observation run together with GEO600. By applying our search pipeline, which takes into account the stochastic nature of ultralight DM, upper bounds on the coupling strength between the U(1)B−L gauge boson and ordinary matter are obtained for a range of DM masses. While our constraints are less stringent than those derived from previous experiments, this study demonstrates the applicability of our method to the lower-mass vector DM search, which is made difficult in this measurement by the short observation time compared to the auto-correlation time scale of DM

Evaluation of an artificial intelligence literacy course for university students with diverse study backgrounds

A few limited efforts have been made to promote artificial intelligence (AI) literacy for citizens. The objective of this study was to design, implement and evaluate an AI literacy course for university students. One of the study's research questions was whether university students from a variety of disciplines could develop a conceptual understanding of AI through a literacy course. We promoted this course to 4000 students and recruited 120 volunteer participants to attend and complete the 7-h course. The results of our pre-course and post-course surveys indicated that the participants made significant progress in understanding AI concepts, felt empowered to work with AI. These findings indicated that the participants of diverse study backgrounds, and of both genders, could understand the concepts of machine learning, supervised learning, regression, classification, unsupervised learning, and clustering. Prior knowledge of programming was not necessary for AI concepts development, and the flipped classroom learning approach enabled more flexible learning by the participants. In the future, this AI literacy course could be extended to include AI application projects and discussions of related ethical issues regarding the wider use of AI in society. We are planning to introduce this literacy course to senior secondary school students

Evaluating an Artificial Intelligence Literacy Programme for Developing University Students’ Conceptual Understanding, Literacy, Empowerment and Ethical Awareness

Emerging research is highlighting the importance of fostering artificial intelligence (AI) literacy among educated citizens of diverse academic backgrounds. However, what to include in such literacy programmes and how to teach literacy is still under-explored. To fill this gap, this study designed and evaluated an AI literacy programme based on a multi-dimensional conceptual framework, which developed participants’ conceptual understanding, literacy, empowerment and ethical awareness. It emphasised conceptual building, highlighted project work in application development and initiated teaching ethics through application development. Thirty-six university students with diverse academic backgrounds joined and completed this programme, which included 7 hours on machine learning, 9 hours on deep learning and 14 hours on application development. Together with the project work, the results of the tests, surveys and reflective writings completed before and after these courses indicate that the programme successfully enhanced participants’ conceptual understanding, literacy, empowerment and ethical awareness. The programme will be extended to include more participants, such as senior secondary school students and the general public. This study initiates a pathway to lower the barrier to entry for AI literacy and addresses a public need. It can guide and inspire future empirical and design research on fostering AI literacy among educated citizens of diverse backgrounds
