    Fermented wheat germ extract - nutritional supplement or anticancer drug?

    <p>Abstract</p> <p>Background</p> <p>Fermented wheat germ extract (FWGE) is a multisubstance composition and, besides others, contains 2-methoxy benzoquinone and 2, 6-dimethoxy benzoquinone which are likely to exert some of its biological effects. FWGE interferes with anaerobic glycolysis, pentose cycle and ribonucleotide reductase. It has significant antiproliferative effects and kills tumor cells by the induction of apoptosis via the caspase-poly [ADP-ribose] polymerase-pathway. FWGE interacts synergistically with a variety of different anticancer drugs and exerted antimetastatic properties in mouse models. In addition, FWGE modulates immune response by downregulation of MHC-I complex and the induction of TNF-α and various interleukins. Data in the F-344 rat model provide evidence for a colon cancer preventing effect of FWGE.</p> <p>Clinical data from a randomized phase II trial in melanoma patients indicate a significant benefit for patients treated with dacarbazine in combination with FWGE in terms of progression free survival (PFS) and overall survival (OS). Similarly, data from studies in colorectal cancer suggested a benefit of FWGE treatment. Besides extension of OS and PFS, FWGE improved the quality of life in several studies.</p> <p>Conclusion</p> <p>In conclusion, available data so far, justify the use of FWGE as a non-prescription medical nutriment for cancer patients. Further randomized, controlled and large scale clinical studies are mandatory, to further clarify the value of FWGE as a drug component of future chemotherapy regimens.</p

    Within-trial economic evaluation of diabetes-specific cognitive behaviour therapy in patients with type 2 diabetes and subthreshold depression

    <p>Abstract</p> <p>Background</p> <p>Despite the high prevalence of subthreshold depression in patients with type 2 diabetes, evidence on cost-effectiveness of different therapy options for these patients is currently lacking.</p> <p>Methods/Design</p> <p>Within-trial economic evaluation of the diabetes-specific cognitive behaviour therapy for subthreshold depression. Patients with diabetes and subthreshold depression are randomly assigned to either 2 weeks of diabetes-specific cognitive behaviour group therapy (n = 104) or to standard diabetes education programme only (n = 104). Patients are followed for 12 months. During this period data on total health sector costs, patient costs and societal productivity costs are collected in addition to clinical data. Health related quality of life (the SF-36 and the EQ-5D) is measured at baseline, immediately after the intervention, at 6 and at 12 months after the intervention. Quality adjusted life years (QALYs), and cumulative costs will be estimated for each arm of the trial. Cost-effectiveness of the diabetes-specific cognitive behaviour group therapy will be analysed from the perspective of the German statutory health insurance and from the societal perspective. To this end, incremental cost-effectiveness ratio (ICER) in terms of cost per QALY gained will be calculated.</p> <p>Discussion</p> <p>Some methodological issues of the described economic evaluation are discussed.</p> <p>Trial registration</p> <p>The trial has been registered at the Clinical Trials Register (NCT01009138).</p

    Cost effectiveness of benralizumab for severe, uncontrolled oral corticosteroid-dependent asthma in Sweden

    Aim: We investigated cost effectiveness of benralizumab vs. standard of care (SOC) plus oral corticosteroids (OCS) for patients with severe, eosinophilic OCS-dependent asthma in Sweden. Materials and methods: A three-state, cohort-based Markov model of data from three Phase III benralizumab clinical trials (ZONDA [NCT02075255], SIROCCO [NCT01928771], and CALIMA [NCT01914757]) was used to assess the incremental cost-effectiveness ratio of benralizumab vs. SOC plus OCS. Health outcomes were estimated in terms of quality-adjusted life-years (QALYs). The model included costs and disutilities associated with extrapolated OCS-related adverse events. Patients with severe asthma were defined as those receiving OCS &gt;= 5 mg/day. Results: Benralizumab demonstrated a cost-effectiveness ratio vs. SOC plus OCS of 2018 Swedish Kronor (SEK) 366,855 (euro34,127) per QALY gained, based on increases of 1.33 QALYs and SEK 488,742 (euro45,344) per patient. Benralizumab treatment costs contributed most to incremental costs. The probability of benralizumab's being cost-effective with willingness-to-pay (WTP) thresholds between SEK 429,972 (euro40,000) and SEK 752,452 (euro70,000) ranged from 75% to 99%. Limitations: Potential limitations of these analyses include the use of combined data from three different clinical trials, a one-way sensitivity analysis that did not include mortality and transition estimates, and Observational &amp; Pragmatic Research Institute (OPRI) data from the UK as a proxy of the Swedish health care system. Conclusions: The results of these analyses demonstrate that benralizumab has a high probability of being cost-effective compared with SOC plus OCS for a subgroup of patients with severe, eosinophilic asthma receiving regular OCS treatment and may support clinicians, payers and patients in making treatment decisions

La composante culturelle en langue Ă©trangĂšre », organisĂ©es par l’Equipe de recherche PLIDAM (INALCO), Ă  Paris, les 11 et 12 juin 2009.La distance culturelle qui s’ajoute a la distance linguistique prend un relief tout particulier dans l’enseignement des langues Ă©trangĂšres : les savoirs constitutifs de l’ensemble d’une culture Ă©trangĂšre (relations interpersonnelles, hiĂ©rarchies sociales, expressions de la sagesse populaire, rĂšgles de politesse, conventions de prise de parole et de silence, humour, tabous) peuvent difficilement ĂȘtre catĂ©gorisĂ©s ou codifiĂ©s sous forme de manuels, de grammaires ou de dictionnaires, alors que le monde cible est prĂ©sent dans tous les supports de l’apprentissage des langues vivantes (mots, textes, images, situations). Le fait culturel rĂ©siste fortement Ă  toute opĂ©ration de repĂ©rage, d’interprĂ©tation ou de traduction et constitue un champ d’investigation prometteur en didactique des langues Ă©trangĂšres. Trois axes de rĂ©flexion ont nourri les dĂ©bats :a) Tout systĂšme de rĂ©fĂ©rences culturelles sous-entend une organisation de la rĂ©alitĂ©, une logique ou vision du monde, des savoirs et des aspects du rĂ©el implicites (plus ou moins ostentatoires ou secrets), acceptĂ©s et diffusĂ©s par les membres d’une communautĂ© donnĂ©e.b) Toute situation de communication peut conduire Ă  une pratique langagiĂšre particuliĂšre et dans bien des cas la situation extralinguistique impose un emploi spĂ©cifique et stĂ©rĂ©otypĂ© de la langue : habitudes et rĂšgles discursives, gestes, regard, distance 
c) La compĂ©tence culturelle est constituĂ©e Ă©galement d’un ensemble de reprĂ©sentations partagĂ©es qui sont autant d’images du rĂ©el collectif : visions socialement Ă©laborĂ©es, symboles et mythes fondateurs et prescripteurs, maniĂšres d’interprĂ©ter la rĂ©alitĂ© et l’altĂ©ritĂ©
