    Einfluss von Diclofenac auf die Frakturheilung bei alten Mäusen

    Nichtsteroidale Antiphlogistika (NSAIDs) gehören zu den meistrezeptierten Medikamenten weltweit. Zur Schmerzbehandlung finden sie auch im Alter und insbesondere nach Frakturen breite Anwendung. Mittlerweile gibt es jedoch Studien, die zeigen konnten, dass NSAIDs die Frakturheilung negativ beeinflussen können. In dieser Studie wurde der Effekt von Diclofenac auf die Frakturheilung im Alter untersucht. Dazu wurde bei 62 Mäusen eine diaphysäre Femurfraktur mit einer intramedullären Zugschraube versorgt. 33 Mäusen wurde über einen Zeitraum von zwei beziehungsweise fünf Wochen täglich 5mg/kg KG Diclofenac subkutan in die Nackenfalte appliziert. Die Kontrollgruppe (n=29) erhielt eine äquivalente Menge Kochsalzlösung. Die Frakturheilung wurde anschließend biomechanisch, radiologisch, histologisch sowie proteinbiochemisch untersucht. Die biomechanische Testung zeigte eine signifikant verminderte Drei-Punkt-Biegesteifigkeit fünf Wochen postoperativ bei den mit Diclofenac behandelten Tieren im Vergleich zur Kontrollgruppe. In der computertomographischen Untersuchung war der Knochenanteil innerhalb des Frakturkallus nach fünf Wochen vermindert. In der histologischen Untersuchung zeigte sich ein verminderter Knochenanteil sowie vermehrt Bindegewebe innerhalb des Frakturkallus in der Diclofenac-Gruppe. Zudem war die Umwandlung des neugebildeten Gewebes hin zu reifem Lamellenknochen verzögert. Die Osteoklastenzahl war nach zwei Wochen verringert, nach fünf Wochen jedoch erhöht. Der Osteoklasteninhibitor Osteoprotegerin zeigte sich in der Western-Blot-Untersuchung erhöht. Dagegen wurden die Osteogenesemarker Bone morphogenetic protein 2 und Cystein-rich protein 61 unter Diclofenac-Therapie nur etwa halb so stark exprimiert. Diese Ergebnisse sprechen dafür, dass Diclofenac die Frakturheilung bei alten Mäusen durch eine Hemmung des Knochenauf- und umbaus verzögert.Non-steroidal anti-inflammatory drugs (NSAIDs) are among the most widely prescribed medications worldwide. They are also widely used to treat pain in the elderly and especially after fractures. However, recent studies have shown that NSAIDs can have a negative impact on fracture healing. In this study, the effect of diclofenac on fracture healing in the elderly was investigated. For this purpose, a diaphyseal femur fracture was treated with an intramedullary screw in 62 mice. 33 mice were given 5mg/kg bw diclofenac subcutaneously daily into the nuchal fold for a period of two and five weeks, respectively. The control group (n=29) received an equivalent amount of saline. The outcome of fracture healing was subsequently studied biomechanically, radiologically, histologically, and protein biochemically. Biomechanical testing revealed significantly decreased three-point bending stiffness five weeks postoperatively in diclofenac-treated animals compared with the control group. In the computed tomography examination, the amount of bone within the fracture callus was reduced after five weeks. Histologic examination showed decreased bone content and increased connective tissue within the fracture callus of the diclofenac group. Moreover, the transformation of the newly formed tissue into mature lamellar bone was delayed. Osteoclast numbers were decreased after two weeks but increased after five weeks. The osteoclast inhibitor osteoprotegerin was found to be increased in the Western blot assay. In contrast, the osteogenesis markers bone morphogenetic protein 2 and cystein-rich protein 61 were expressed only about half as much with diclofenac therapy. These results suggest that diclofenac delays fracture healing in aged mice by inhibiting bone formation and remodeling

    Nonsteroidal anti-inflammatory drugs (NSAIDs), such as diclofenac, belong to the most prescribed analgesic medication after traumatic injuries. However, there is accumulating evidence that NSAIDs impair fracture healing. Because bone regeneration in aged patients is subject to significant changes in cell differentiation and proliferation as well as a markedly altered pharmacological action of drugs, we herein analyzed the effects of diclofenac on bone healing in aged mice using a stable closed femoral facture model. Thirty-three mice (male n = 14, female n = 19) received a daily intraperitoneal injection of diclofenac (5 mg/kg body weight). Vehicletreated mice (n = 29; male n = 13, female n = 16) served as controls. Fractured mice femora were analyzed by means of X-ray, biomechanics, micro computed tomography (μCT), histology and Western blotting. Biomechanical analyses revealed a significantly reduced bending stiffness in diclofenac-treated animals at 5 weeks after fracture when compared to vehicle-treated controls. Moreover, the callus tissue in diclofenac-treated aged animals exhibited a significantly reduced amount of bone tissue and higher amounts of fibrous tissue. Further histological analyses demonstrated less lamellar bone after diclofenac treatment, indicating a delay in callus remodeling. This was associated with a decreased number of osteoclasts and an increased expression of osteoprotegerin (OPG) during the early phase of fracture healing. These findings indicate that diclofenac delays fracture healing in aged mice by affecting osteogenic growth factor expression and bone formation as well as osteoclast activity and callus remodeling

    Detection level and pattern of positive lesions using PSMA PET/CT for staging prior to radiation therapy

    Background: To determine the potential role of Ga-68-PSMA positron emission tomography/computed tomography (PET/CT) in radiotherapy (RT) planning for prostate cancer (PCa). Methods: One hundred twenty-nine patients (pts) with Ga-68-PSMA PET/CT were retrospectively analysed. Potentially influencing factors (androgen deprivation therapy, amount of Ga-68-PSMA-HBED-CC, PSA doubling time 10 months, PSA before PET/CT, T-/N-category and Gleason score) were evaluated by logistic regression analysis. The detection rate of PSMA PET/CT was compared to contrast enhanced CT and its impact on RT management analysed. Results: One hundred twenty-nine patients (pts) (20 at initial diagnosis, 49 with PSA relapse and 60 with PSA persistence after radical prostatectomy) received PSMA PET/CT prior to RT. The majority of pts. (71.3%) had PET-positive findings (55.1% of pts. with PSA recurrence, 75% of pts. with PSA persistence and 100% of newly diagnosed pts). Median PSA before PET/CT in pts. with pathological findings (n = 92) was 1.90 ng/ml and without (n = 37) 0. 30 ng/ml. PSA level at time of PET/CT was the only factor associated with PET-positivity. In pts. with a PSA <= 0.2 ng/ml, the detection rate of any lesion was 33.3%, with a PSA of 0.21-0.5 ng/ml 41.2% and with a PSA of 0.51-1.0 ng/ml 69. 2%, respectively. Regarding the anatomic distribution of lesions, 42.2% and 14.7% of pts. with relapse or persistence had pelvic lymph node and distant metastases. In pts. at initial diagnosis the detection rate of pelvic lymph nodes and distant metastases was 20% and 10%. 68Ga-PSMA PET/CT had a high detection rate of PCa recurrence outside the prostatic fossa in pts. being considered for salvage RT (22.4% PET-positive pelvic lymph nodes and 4.1% distant metastases). Compared to CT, PSMA PET/CT had a significantly higher sensitivity in diagnosing rates of local recurrence/primary tumour (10.1% vs. 38%), lymph nodes (15.5% vs. 38.8%) and distant metastases (5.4% vs. 14.0%). This resulted in a modification of RT treatment in 56.6% of pts. Conclusions: The detection of PCa is strongly associated with PSA level at time of Ga-68-PSMA PET/CT. PSMA PET/CT differentiates between local, regional and distant metastatic disease with implications for disease management. PSMA PET/CT allows for tumour detection in post-prostatectomy pts. with PSA <= 0.5 ng/ml considered for salvage RT

    Integrative clinical transcriptome analysis reveals TMPRSS2‐ERG dependency of prognostic biomarkers in prostate adenocarcinoma

    In prostate adenocarcinoma (PCa), distinction between indolent and aggressive disease is challenging. Around 50% of PCa are characterized by TMPRSS2‐ERG (T2E)‐fusion oncoproteins defining two molecular subtypes (T2E‐positive/negative). However, current prognostic tests do not differ between both molecular subtypes, which might affect outcome prediction. To investigate gene‐signatures associated with metastasis in T2E‐positive and T2E‐negative PCa independently, we integrated tumor transcriptomes and clinicopathological data of two cohorts (total n = 783), and analyzed metastasis‐associated gene‐signatures regarding the T2E‐status. Here, we show that the prognostic value of biomarkers in PCa critically depends on the T2E‐status. Using gene‐set enrichment analyses, we uncovered that metastatic T2E‐positive and T2E‐negative PCa are characterized by distinct gene‐signatures. In addition, by testing genes shared by several functional gene‐signatures for their association with event‐free survival in a validation cohort (n = 272), we identified five genes (ASPN, BGN, COL1A1, RRM2 and TYMS)—three of which are included in commercially available prognostic tests—whose high expression was significantly associated with worse outcome exclusively in T2E‐negative PCa. Among these genes, RRM2 and TYMS were validated by immunohistochemistry in another validation cohort (n = 135), and several of them proved to add prognostic information to current clinicopathological predictors, such as Gleason score, exclusively for T2E‐negative patients. No prognostic biomarkers were identified exclusively for T2E‐positive tumors. Collectively, our study discovers that the T2E‐status, which is per se not a strong prognostic biomarker, crucially determines the prognostic value of other biomarkers. Our data suggest that the molecular subtype needs to be considered when applying prognostic biomarkers for outcome prediction in PCa

    Prediction of Locally Advanced Urothelial Carcinoma of the Bladder Using Clinical Parameters before Radical Cystectomy - A Prospective Multicenter Study

    Introduction: We aimed at developing and validating a pre-cystectomy nomogram for the prediction of locally advanced urothelial carcinoma of the bladder (UCB) using clinicopathological parameters. Materials and Methods: Multicenter data from 337 patients who underwent radical cystectomy (RC) for UCB were prospectively collected and eligible for final analysis. Univariate and multivariate logistic regression models were applied to identify significant predictors of locally advanced tumor stage (pT3/4 and/or pN+) at RC. Internal validation was performed by bootstrapping. The decision curve analysis (DCA) was done to evaluate the clinical value. Results: The distribution of tumor stages pT3/4, pN+ and pT3/4 and/or pN+ at RC was 44.2, 27.6 and 50.4%, respectively. Age (odds ratio (OR) 0.980; p < 0.001), advanced clinical tumor stage (cT3 vs. cTa, cTis, cT1; OR 3.367; p < 0.001), presence of hydronephrosis (OR 1.844; p = 0.043) and advanced tumor stage T3 and/or N+ at CT imaging (OR 4.378; p < 0.001) were independent predictors for pT3/4 and/or pN+ tumor stage. The predictive accuracy of our nomogram for pT3/4 and/or pN+ at RC was 77.5%. DCA for predicting pT3/4 and/or pN+ at RC showed a clinical net benefit across all probability thresholds. Conclusion: We developed a nomogram for the prediction of locally advanced tumor stage pT3/4 and/or pN+ before RC using established clinicopathological parameters

    The first myriapod genome sequence reveals conservative arthropod gene content and genome organisation in the centipede Strigamia maritima.

    Myriapods (e.g., centipedes and millipedes) display a simple homonomous body plan relative to other arthropods. All members of the class are terrestrial, but they attained terrestriality independently of insects. Myriapoda is the only arthropod class not represented by a sequenced genome. We present an analysis of the genome of the centipede Strigamia maritima. It retains a compact genome that has undergone less gene loss and shuffling than previously sequenced arthropods, and many orthologues of genes conserved from the bilaterian ancestor that have been lost in insects. Our analysis locates many genes in conserved macro-synteny contexts, and many small-scale examples of gene clustering. We describe several examples where S. maritima shows different solutions from insects to similar problems. The insect olfactory receptor gene family is absent from S. maritima, and olfaction in air is likely effected by expansion of other receptor gene families. For some genes S. maritima has evolved paralogues to generate coding sequence diversity, where insects use alternate splicing. This is most striking for the Dscam gene, which in Drosophila generates more than 100,000 alternate splice forms, but in S. maritima is encoded by over 100 paralogues. We see an intriguing linkage between the absence of any known photosensory proteins in a blind organism and the additional absence of canonical circadian clock genes. The phylogenetic position of myriapods allows us to identify where in arthropod phylogeny several particular molecular mechanisms and traits emerged. For example, we conclude that juvenile hormone signalling evolved with the emergence of the exoskeleton in the arthropods and that RR-1 containing cuticle proteins evolved in the lineage leading to Mandibulata. We also identify when various gene expansions and losses occurred.     Improved prediction of nephron-sparing surgery versus radical nephrectomy by the optimized R.E.N.A.L. Score in patients undergoing surgery for renal masses

    BACKGROUND: One major objective of currently available morphometric scores (MS) for renal masses, i.e., R.E.N.A.L., PADUA classification, Centrality-Index, is the prediction of type of surgery (nephron-sparin surgery [NSS] or radical nephrectomy [RN]). METHODS: Based on a prospective study protocol, various MS were assigned and calculated for 108 patients undergoing surgical treatment for renal masses at a single academic center. MS calculation was based on preoperative computed-tomography or magnet-resonance-imaging and performed by two independent readers blinded for surgical approach and outcome. Multivariable logistic-regression- and ROC-analyses were performed to assess the predictive value of various MS for surgical approach and the correlation of clinical parameters with nephrectomy type. Furthermore, the association with perioperative outcome parameters was evaluated. RESULTS: None of the tested MS was significantly superior to tumor size alone (area under the curve [AUC]=0.82) in predicting RN, with Centrality-Index showing the best association (AUC-0.88). Based on these findings, a simplified and optimized R.E.N.A.L. Score (optR.E.N.A.L.) was developed with different weightings of included parameters, which did not only show a significantly enhanced association with surgery type (AUC=0.93) than tumor size, but also outperformed all 1st and 2nd generation MS tested in the study cohort. Besides a modest correlation with postoperative change in renal function, no association with perioperative outcome variables was found for all MS including optR.E.N.A.L.. CONCLUSIONS: optR.E.N.A.L. represents a promising improvement of the preexisting R.E.N.A.L. Score with higher predictive ability for nephrectomy type than established MS and may serve as a benchmarking tool for nephrectomy assessment and comparison of surgical strategies
