    Chemical ionization tandem mass spectrometer for the in situ measurement of methyl hydrogen peroxide

    A new approach for measuring gas-phase methyl hydrogen peroxide [(MHP) CH_3OOH] utilizing chemical ionization mass spectrometry is presented. Tandem mass spectrometry is used to avoid mass interferences that hindered previous attempts to measure atmospheric CH_3OOH with CF_3O− clustering chemistry. CH_3OOH has been successfully measured in situ using this technique during both airborne and ground-based campaigns. The accuracy and precision for the MHP measurement are a function of water vapor mixing ratio. Typical precision at 500 pptv MHP and 100 ppmv H_2O is ±80 pptv (2 sigma) for a 1 s integration period. The accuracy at 100 ppmv H_2O is estimated to be better than ±40%. Chemical ionization tandem mass spectrometry shows considerable promise for the determination of in situ atmospheric trace gas mixing ratios where isobaric compounds or mass interferences impede accurate measurements

    Persistent Intersection Homology for the Analysis of Discrete Data

    Topological data analysis is becoming increasingly relevant to support the analysis of unstructured data sets. A common assumption in data analysis is that the data set is a sample---not necessarily a uniform one---of some high-dimensional manifold. In such cases, persistent homology can be successfully employed to extract features, remove noise, and compare data sets. The underlying problems in some application domains, however, turn out to represent multiple manifolds with different dimensions. Algebraic topology typically analyzes such problems using intersection homology, an extension of homology that is capable of handling configurations with singularities. In this paper, we describe how the persistent variant of intersection homology can be used to assist data analysis in visualization. We point out potential pitfalls in approximating data sets with singularities and give strategies for resolving them.Comment: Topology-based Methods in Visualization 201

    Susceptibility of hamsters to clostridium difficile isolates of differing toxinotype

    Clostridium difficile is the most commonly associated cause of antibiotic associated disease (AAD), which caused ~21,000 cases of AAD in 2011 in the U.K. alone. The golden Syrian hamster model of CDI is an acute model displaying many of the clinical features of C. difficile disease. Using this model we characterised three clinical strains of C. difficile, all differing in toxinotype; CD1342 (PaLoc negative), M68 (toxinotype VIII) and BI-7 (toxinotype III). The naturally occurring non-toxic strain colonised all hamsters within 1-day post challenge (d.p.c.) with high-levels of spores being shed in the faeces of animals that appeared well throughout the entire experiment. However, some changes including increased neutrophil influx and unclotted red blood cells were observed at early time points despite the fact that the known C. difficile toxins (TcdA, TcdB and CDT) are absent from the genome. In contrast, hamsters challenged with strain M68 resulted in a 45% mortality rate, with those that survived challenge remaining highly colonised. It is currently unclear why some hamsters survive infection, as bacterial and toxin levels and histology scores were similar to those culled at a similar time-point. Hamsters challenged with strain BI-7 resulted in a rapid fatal infection in 100% of the hamsters approximately 26 hr post challenge. Severe caecal pathology, including transmural neutrophil infiltrates and extensive submucosal damage correlated with high levels of toxin measured in gut filtrates ex vivo. These data describes the infection kinetics and disease outcomes of 3 clinical C. difficile isolates differing in toxin carriage and provides additional insights to the role of each toxin in disease progression

    Platinum Complexes with a Phosphino-Oxime/Oximate Ligand

    The platinum(II) complex [PtCl2(COD)] (2; COD = 1,5- cyclooctadiene) reacted with 1 and 2 equiv. of 2-(diphenylphosphanyl) benzaldehyde oxime (1) to generate [PtCl2{¿2-(P,N)-2- Ph2PC6H4CH=NOH}] (3) and [Pt{¿2-(P,N)-2-Ph2PC6H4CH=NOH}2]- [Cl]2 (4), respectively. Deprotonation of the oxime hydroxyl group of 3 with Na2CO3 led to the selective formation of the dinuclear species (¿-O)-[PtCl{¿2-(P,N)-2-Ph2PC6H4CH=NO}]2 (5), while the related methylated derivative (¿-O)-[PtMe{¿2-(P,N)-2- Ph2PC6H4CH=NO}]2 (7) could be obtained from the direct reaction of [PtMe2(COD)] (6) with the phosphino-oxime ligand 1. In the case of 4, its treatment with Na2CO3 yielded complex [Pt({¿2-(P,N)-2-Ph2PC6H4CH=NO}2H)][Cl] (8), as a result of the deprotonation of only one of the OH groups of 4. On the other hand, contrary to what was observed with 6, no deprotonation of the oxime occurred in the reaction of [PtMe3I]4 (9) with 1, from which the mononuclear PtIV derivative fac-[PtIMe3{¿2-(P,N)- 2-Ph2PC6H4CH=NOH}] (10) was isolated. The solid-state structures of compounds 3, 4, 7 and 10 were determined by X-ray crystallography. In addition, the potential of all the synthesized complexes as catalysts for the dehydrogenative coupling of hydrosilanes with alcohols is also briefly discussed.Peer Reviewe

    Headache service quality evaluation : implementation of quality indicators in primary care in Europe

Publisher Copyright: © 2021, The Author(s).Background: Lifting The Burden (LTB) and European Headache Federation (EHF) have developed a set of headache service quality indicators, successfully tested in specialist headache centres. Their intended application includes all levels of care. Here we assess their implementation in primary care. Methods: We included 28 primary-care clinics in Germany (4), Turkey (4), Latvia (5) and Portugal (15). To implement the indicators, we interviewed 111 doctors, 92 nurses and medical assistants, 70 secretaries, 27 service managers and 493 patients, using the questionnaires developed by LTB and EHF. In addition, we evaluated 675 patients’ records. Enquiries were in nine domains: diagnosis, individualized management, referral pathways, patient education and reassurance, convenience and comfort, patient satisfaction, equity and efficiency of headache care, outcome assessment and safety. Results: The principal finding was that Implementation proved feasible and practical in primary care. In the process, we identified significant quality deficits. Almost everywhere, histories of headache, especially temporal profiles, were captured and/or assessed inaccurately. A substantial proportion (20%) of patients received non-specific ICD codes such as R51 (“headache”) rather than specific headache diagnoses. Headache-related disability and quality of life were not part of routine clinical enquiry. Headache diaries and calendars were not in use. Waiting times were long (e.g., about 60 min in Germany). Nevertheless, most patients (> 85%) expressed satisfaction with their care. Almost all the participating clinics provided equitable and easy access to treatment, and follow-up for most headache patients, without unnecessary barriers. Conclusions: The study demonstrated that headache service quality indicators can be used in primary care, proving both practical and fit for purpose. It also uncovered quality deficits leading to suboptimal treatment, often due to a lack of knowledge among the general practitioners. There were failures of process also. These findings signal the need for additional training in headache diagnosis and management in primary care, where most headache patients are necessarily treated. More generally, they underline the importance of headache service quality evaluation in primary care, not only to identify-quality failings but also to guide improvements. This study also demonstrated that patients’ satisfaction is not, on its own, a good indicator of service quality.publishersversionPeer reviewe

From saved output of an Arctic tides model, we introduce parameterizations for (1) enhanced ocean mixing associated with tides and (2) the role of tides fracturing and mobilizing sea ice. Results show tides enhancing loss of heat from Atlantic waters. The impact of tides on sea ice is more subtle as thinning due to enhanced ocean heat flux competes with net ice growth during rapid openings and closings of tidal leads. Present model results are compared with an ensemble of nine models under the Arctic Ocean Model Intercomparison Project (AOMIP). Among results from AOMIP is a tendency for models to accumulate excessive Arctic Ocean heat throughout the intercomparison period 1950 to 2000 which is contrary to observations.     Prevention of haematoma progression by tranexamic acid in intracerebral haemorrhage patients with and without spot sign on admission scan: a statistical analysis plan of a pre-specified sub-study of the TICH-2 trial

    Objective We present the statistical analysis plan of a prespecified Tranexamic Acid for Hyperacute Primary Intracerebral Haemorrhage (TICH)-2 sub-study aiming to investigate, if tranexamic acid has a different effect in intracerebral haemorrhage patients with the spot sign on admission compared to spot sign negative patients. The TICH-2 trial recruited above 2000 participants with intracerebral haemorrhage arriving in hospital within 8 h after symptom onset. They were included irrespective of radiological signs of on-going haematoma expansion. Participants were randomised to tranexamic acid versus matching placebo. In this subgroup analysis, we will include all participants in TICH-2 with a computed tomography angiography on admission allowing adjudication of the participants’ spot sign status. Results Primary outcome will be the ability of tranexamic acid to limit absolute haematoma volume on computed tomography at 24 h (± 12 h) after randomisation among spot sign positive and spot sign negative participants, respectively. Within all outcome measures, the effect of tranexamic acid in spot sign positive/negative participants will be compared using tests of interaction. This sub-study will investigate the important clinical hypothesis that spot sign positive patients might benefit more from administration of tranexamic acid compared to spot sign negative patients

    Measurement of the Proton and Deuteron Spin Structure Function g_1 in the Resonance Region

    We have measured the proton and deuteron spin structure functions g_1^p and g_1^d in the region of the nucleon resonances for W^2 < 5 GeV^2 and Q20.5Q^2\simeq 0.5 and Q21.2Q^2\simeq 1.2 GeV^2 by inelastically scattering 9.7 GeV polarized electrons off polarized 15NH3^{15}NH_3 and 15ND3^{15}ND_3 targets. We observe significant structure in g_1^p in the resonance region. We have used the present results, together with the deep-inelastic data at higher W^2, to extract Γ(Q2)01g1(x,Q2)dx\Gamma(Q^2)\equiv\int_0^1 g_1(x,Q^2) dx. This is the first information on the low-Q^2 evolution of Gamma toward the Gerasimov-Drell-Hearn limit at Q^2 = 0.Comment: 7 pages, 2 figure
