    A perspective on the Healthgrid initiative

    This paper presents a perspective on the Healthgrid initiative which involves European projects deploying pioneering applications of grid technology in the health sector. In the last couple of years, several grid projects have been funded on health related issues at national and European levels. A crucial issue is to maximize their cross fertilization in the context of an environment where data of medical interest can be stored and made easily available to the different actors in healthcare, physicians, healthcare centres and administrations, and of course the citizens. The Healthgrid initiative, represented by the Healthgrid association (http://www.healthgrid.org), was initiated to bring the necessary long term continuity, to reinforce and promote awareness of the possibilities and advantages linked to the deployment of GRID technologies in health. Technologies to address the specific requirements for medical applications are under development.     Innovative in silico approaches to address avian flu using grid technology

    The recent years have seen the emergence of diseases which have spread very quickly all around the world either through human travels like SARS or animal migration like avian flu. Among the biggest challenges raised by infectious emerging diseases, one is related to the constant mutation of the viruses which turns them into continuously moving targets for drug and vaccine discovery. Another challenge is related to the early detection and surveillance of the diseases as new cases can appear just anywhere due to the globalization of exchanges and the circulation of people and animals around the earth, as recently demonstrated by the avian flu epidemics. For 3 years now, a collaboration of teams in Europe and Asia has been exploring some innovative in silico approaches to better tackle avian flu taking advantage of the very large computing resources available on international grid infrastructures. Grids were used to study the impact of mutations on the effectiveness of existing drugs against H5N1 and to find potentially new leads active on mutated strains. Grids allow also the integration of distributed data in a completely secured way.     Economic values for ecosystem services:A global synthesis and way forward

    This paper presents a global synthesis of economic values for ecosystem services provided by 15 terrestrial and marine biomes. Information from over 1,300 studies, yielding over 9,400 value estimates in monetary units, has been collected and organised in the Ecosystem Services Valuation Database (ESVD). This is a substantial expansion of data since the de Groot et al. (2012) description of the ESVD and provides an important juncture to explore developments in the use of valuation methods and the contexts in which valuations are conducted. In this paper we provide summary values for 23 ecosystem services from 15 biomes to represent the magnitude, variation and gaps in economic values. To enable the comparison and synthesis of values, estimates in the ESVD are standardised to a common set of units (Int$/ha/year at 2020 price levels). This data provides a basis for value transfers to inform decision-making in current policy contexts but requires due consideration and adjustment for context specific determinants of value. Although the coverage of the ESVD is global, the geographic distribution of data is not even. There is a particularly high representation of European ecosystems and relatively little information for Russia, Central Asia and North Africa. Therefore, the data are not globally representative of biophysical and socio-economic contexts. The distribution of data across ecosystem services is also far from even, with some services very well represented (e.g. recreation, wild fish and wild animals, ecosystem and species appreciation, air filtration and global climate regulation) and others with almost no value estimates (e.g. disease control, water baseflow maintenance, rainfall pattern regulation). In the past decade, there has been a notable increase in demand for information on the economic value of ecosystem services from both public and private institutions to improve the conservation and management of natural capital. The literature is developing to meet this demand but there is a need for targeted and refined valuation research to ensure sufficient certainty, comparability, and representativeness of the data, and to enable transferability and fill knowledge gaps. This paper concludes by identifying avenues for future development to further increase the amount, quality, representativeness and application of data on economic values for ecosystem services.</p

    Learning experiences for the transition to professional work

    A better educated workforce contributes to a more informed and tolerant society with higher economic output, and this is also associated with higher levels of personal health, interpersonal trust and civic and social engagement. Against this backdrop, the role of universities has expanded, as university learning has moved beyond providing an education to preparing students for leadership positions within society. This article examines the effectiveness of final-year learning experiences from the perception of recent graduates. The aim is to improve undergraduate curriculum to facilitate the transition to professional employment. An online quantitative and qualitative survey instrument was developed to investigate graduates’ perceptions of their different learning experiences and assessment types in their senior year. Four hundred and twelve alumni from five universities completed the survey. Our results indicate that graduates value case studies, group work and oral presentations, and that graduates rate lectures and guest lectures from practitioners as the least important in their transition to work. The results validate the use of graduate capability frameworks and mapping the development of the skills over the curriculum. These results are useful for curriculum designers to assist with designing programmes on the transition to professional work

    Characterizing Long COVID: Deep Phenotype of a Complex Condition.

    BACKGROUND: Numerous publications describe the clinical manifestations of post-acute sequelae of SARS-CoV-2 (PASC or long COVID ), but they are difficult to integrate because of heterogeneous methods and the lack of a standard for denoting the many phenotypic manifestations. Patient-led studies are of particular importance for understanding the natural history of COVID-19, but integration is hampered because they often use different terms to describe the same symptom or condition. This significant disparity in patient versus clinical characterization motivated the proposed ontological approach to specifying manifestations, which will improve capture and integration of future long COVID studies. METHODS: The Human Phenotype Ontology (HPO) is a widely used standard for exchange and analysis of phenotypic abnormalities in human disease but has not yet been applied to the analysis of COVID-19. FINDINGS: We identified 303 articles published before April 29, 2021, curated 59 relevant manuscripts that described clinical manifestations in 81 cohorts three weeks or more following acute COVID-19, and mapped 287 unique clinical findings to HPO terms. We present layperson synonyms and definitions that can be used to link patient self-report questionnaires to standard medical terminology. Long COVID clinical manifestations are not assessed consistently across studies, and most manifestations have been reported with a wide range of synonyms by different authors. Across at least 10 cohorts, authors reported 31 unique clinical features corresponding to HPO terms; the most commonly reported feature was Fatigue (median 45.1%) and the least commonly reported was Nausea (median 3.9%), but the reported percentages varied widely between studies. INTERPRETATION: Translating long COVID manifestations into computable HPO terms will improve analysis, data capture, and classification of long COVID patients. If researchers, clinicians, and patients share a common language, then studies can be compared/pooled more effectively. Furthermore, mapping lay terminology to HPO will help patients assist clinicians and researchers in creating phenotypic characterizations that are computationally accessible, thereby improving the stratification, diagnosis, and treatment of long COVID.     Design students' experience of engagement and creativity

    The terms 'creativity' and 'engagement' are used broadly throughout the literature and are in common usage in our education and design language. Students, however, understand these terms in unexpected ways and this can sometimes cause a disjunction between the teacher's intention for the learning activity and the ways in which the student may go about it. In this article we tease out the relationships between engagement and creativity for student learning in design. Our data suggest that engagement relies on certain conditions and attributes that have to be met before a student makes a personally meaningful commitment to study. Engagement here means some form of interlocking between the student and the task, or some form of attentiveness facilitated by the teacher or the environment. By creativity we mean the meshing between person, process and product which is then appreciated by the broader design community. We suggest that understanding creativity as a complex attribute contributes to the nature and quality of student engagement with their learning and the profession.15 page(s

    Considering something ELSE: ethical, legal and socio-economic factors in medical imaging and medical informatics

    The focus on the use of existing and new technologies to facilitate advances in medical imaging and medical informatics (MIMI) is often directed to the technical capabilities and possibilities that these technologies bring. The technologies, though, in acting as a mediating agent alter the dynamics and context of information delivery in subtle ways. While these changes bring benefits in more efficient information transfer and offer the potential of better healthcare, they also disrupt traditional processes and practices which have been formulated for a different setting. The governance processes that underpin core ethical principles, such as patient confidentiality and informed consent, may no longer be appropriate in a new technological context. Therefore, in addition to discussing new methodologies, techniques and applications, there is need for a discussion of ethical, legal and socio-economic (ELSE) issues surrounding the use and application of technologies in MIMI. Consideration of these issues is especially important for the area of medical informatics which after all exists to support patients, healthcare practitioners and inform science. This paper brings to light some important ethical, legal and socio-economic issues related to MIMI with the aim of furthering an interdisciplinary approach to the increasing use of Information and Communication Technologies (ICT) in healthcare
