This study examined the influence of surgery upon cognition in the elderly and investigated potential intervention agents that might prevent cognitive decline and dementia within that population. The study found strong support for a mechanism of action, as well as multiple avenues for better interventions
The Disinfecting Efficacy of Root Canals with Laser Photodynamic Therapy
Introduction: Infecting microorganisms of the root canals are difficult to eliminate during endodontic treatment. In this study the effect of root canal disinfection with photodynamic therapy (PDT) at different time intervals in comparison to 2.5% sodium hypochlorite (NaOCl) irrigation and passive ultrasonic irrigation (PUI) in extracted teeth colonized with Enterococcus faecalis and Candida albicans was tested to assess which treatment reaches the best disinfection rate.Methods: One hundred and fifty-six extracted single-rooted teeth were collected, sterilized, and incubated with Enterococcus faecalis (ATCC 29212) and Candida albicans (ATCC 60193). The two groups were further divided into 6 groups depending on the treatment mode; HELBO®Endo Blue photosensitizer dye application followed by HELBO laser irradiation, with the output power 100 mW and emission of 660 nm, for a 1, 3 and 5 minutes, irrigation with 2.5% NaOCl, 10 second PUI with 2.5% NaOCl and control group. Flow cytometry and scanning electron microscopic (SEM) analysis were used to determine the effectiveness of the different disinfecting methods.Results: The different disinfecting methods had a significantly different effect on the percent of dead cells (p<0.001). A statistical significance of dead cells between organisms (p<0.001) was observed. Interaction between the disinfecting method and both of organisms had shown the statistical significance (p=0.045). Percent of dead cells in treatment groups were significantly higher compared to control group for both organisms (p<0.001).Conclusions: PUI still remains the most effective method for disinfection of infected root canalsin endodontics compared to hand instrumentation for both microorganisms. SEM analysis only confirmed the results. Other results ex vivo suggested that prolonging the time from 1 to 5 minutes of PDT increased the number of killed microorganisms significantly, therefore longer times of photodynamic therapy were recommended. Irrigation with 2.5% NaOCl showed similar results to 5 min irradiation
Diagnostic Utility of Broad Range Bacterial 16S rRNA Gene PCR with Degradation of Human and Free Bacterial DNA in Bloodstream Infection Is More Sensitive Than an In-House Developed PCR without Degradation of Human and Free Bacterial DNA
We compared a commercial broad range 16S rRNA gene PCR assay (SepsiTest) to an in-house developed assay (IHP). We assessed whether CD64 index, a biomarker of bacterial infection, can be used to exclude patients with a low probability of systemic bacterial infection. From January to March 2010, 23 patients with suspected sepsis were enrolled. CD64 index, procalcitonin, and C-reactive protein were measured on admission. Broad range 16S rRNA gene PCR was performed from whole blood (SepsiTest) or blood plasma (IHP) and compared to blood culture results. Blood samples spiked with Staphylococcus aureus were used to assess sensitivity of the molecular assays in vitro. CD64 index was lower in patients where possible sepsis was excluded than in patients with microbiologically confirmed sepsis (P=0.004). SepsiTest identified more relevant pathogens than blood cultures (P=0.008); in three patients (13%) results from blood culture and SepsiTest were congruent, whereas in four cases (17.4%) relevant pathogens were detected by SepsiTest only. In vitro spiking experiments suggested equal sensitivity of SepsiTest and IHP. A diagnostic algorithm using CD64 index as a decision maker to perform SepsiTest shows improved detection of pathogens in patients with suspected blood stream infection and may enable earlier targeted antibiotic therapy

Peri-Operative Risk Factors Associated with Post-Operative Cognitive Dysfunction (POCD): An Umbrella Review of Meta-Analyses of Observational Studies
This umbrella review aimed to systematically identify the peri-operative risk factors associated with post-operative cognitive dysfunction (POCD) using meta-analyses of observational studies. To date, no review has synthesised nor assessed the strength of the available evidence examining risk factors for POCD. Database searches from journal inception to December 2022 consisted of systematic reviews with meta-analyses that included observational studies examining pre-, intra- and post-operative risk factors for POCD. A total of 330 papers were initially screened. Eleven meta-analyses were included in this umbrella review, which consisted of 73 risk factors in a total population of 67,622 participants. Most pertained to pre-operative risk factors (74%) that were predominantly examined using prospective designs and in cardiac-related surgeries (71%). Overall, 31 of the 73 factors (42%) were associated with a higher risk of POCD. However, there was no convincing (class I) or highly suggestive (class II) evidence for associations between risk factors and POCD, and suggestive evidence (class III) was limited to two risk factors (pre-operative age and pre-operative diabetes). Given that the overall strength of the evidence is limited, further large-scale studies that examine risk factors across various surgery types are recommended
A Spectroscopic Study of Type Ibc Supernova Host Galaxies from Untargeted Surveys
We present the largest spectroscopic study of the host environments of Type
Ibc supernovae (SN Ibc) discovered exclusively by untargeted SN searches. Past
studies of SN Ibc host environments have been biased towards high-mass,
high-metallicity galaxies by focusing on SNe discovered in galaxy-targeted SN
searches. Our new observations more than double the total number of
spectroscopic stellar population age and metallicity measurements published for
untargeted SN Ibc host environments, and extend to a median redshift about
twice as large as previous statistical studies (z = 0.04). For the 12 SNe Ib
and 21 SNe Ic in our metallicity sample, we find median metallicities of
log(O/H)+12 = 8.48 and 8.61, respectively, but determine that the discrepancy
in the full distribution of metallicities is not statistically significant.
This median difference would correspond to only a small difference in the mass
loss via metal-line driven winds (<30%), suggesting this does not play the
dominant role in distinguishing SN Ib and Ic progenitors. However, the median
metallicity of the 7 broad-lined SN Ic (SN Ic-BL) in our sample is
significantly lower, log(O/H)+12 = 8.34. The age of the young stellar
population of SN Ic-BL host environments also seems to be lower than for SN Ib
and Ic, but our age sample is small. A synthesis of SN Ibc host environment
spectroscopy to date does not reveal a significant difference in SN Ib and Ic
metallicities, but reinforces the significance of the lower metallicities for
SN Ic-BL. This combined sample demonstrates that galaxy-targeted SN searches
introduce a significant bias for studies seeking to infer the metallicity
distribution of SN progenitors, and we identify and discuss other systematic
effects that play smaller roles. We discuss the path forward for making

Dust and Metal Column Densities in Gamma-Ray Burst Host Galaxies
In this paper we present the results from the analysis of a sample of 28
gamma-ray burst (GRB) afterglow spectral energy distributions, spanning the
X-ray through to near-infrared wavelengths. This is the largest sample of GRB
afterglow spectral energy distributions thus far studied, providing a strong
handle on the optical depth distribution of soft X-ray absorption and
dust-extinction systems in GRB host galaxies. We detect an absorption system
within the GRB host galaxy in 79% of the sample, and an extinction system in
71% of the sample, and find the Small Magellanic Cloud (SMC) extinction law to
provide an acceptable fit to the host galaxy extinction profile for the
majority of cases, consistent with previous findings. The range in the soft
X-ray absorption to dust-extinction ratio, N_{H,X}/Av, in GRB host galaxies
spans almost two orders of magnitude, and the typical ratios are significantly
larger than those of the Magellanic Clouds or Milky Way. Although dust
destruction could be a cause, at least in part, for the large N_{H,X}/Av
ratios, the good fit provided by the SMC extinction law for the majority of our
sample suggests that there is an abundance of small dust grains in the GRB
environment, which we would expect to have been destroyed if dust destruction
were responsible for the large N_{H,X}/Av ratios. Instead, our analysis
suggests that the distribution of N_{H,X}/Av in GRB host galaxies may be mostly
intrinsic to these galaxies, and this is further substantiated by evidence for
a strong negative correlation between N_{H,X}/Av and metallicity for a
subsample of GRB hosts with known metallicity. Furthermore, we find the
N_{H,X}/Av ratio and metallicity for this subsample of GRBs to be comparable to

The Post-Anaesthesia N-acetylcysteine Cognitive Evaluation (PANACEA) trial: study protocol for a randomised controlled trial
BACKGROUND: Some degree of cognitive decline after surgery occurs in as many as one quarter of elderly surgical patients, and this decline is associated with increased morbidity and mortality. Cognition may be affected across a range of domains, including memory, psychomotor skills, and executive function. Whilst the exact mechanisms of cognitive change after surgery are not precisely known, oxidative stress and subsequent neuroinflammation have been implicated. N-acetylcysteine (NAC) acts via multiple interrelated mechanisms to influence oxidative homeostasis, neuronal transmission, and inflammation. NAC has been shown to reduce oxidative stress and inflammation in both human and animal models. There is clinical evidence to suggest that NAC may be beneficial in preventing the cognitive decline associated with both acute physiological insults and dementia-related disorders. To date, no trials have examined perioperative NAC as a potential moderator of postoperative cognitive changes in the noncardiac surgery setting.METHODS AND DESIGN: This is a single-centre, randomised, double-blind, placebo-controlled clinical trial, with a between-group, repeated-measures, longitudinal design. The study will recruit 370 noncardiac surgical patients at the University Hospital Geelong, aged 60 years or older. Participants are randomly assigned to receive either NAC or placebo (1:1 ratio), and groups are stratified by age and surgery type. Participants undergo a series of neuropsychological tests prior to surgery, 7 days, 3 months, and 12 months post surgery. It is hypothesised that the perioperative administration of NAC will reduce the degree of postoperative cognitive changes at early and long-term follow-up, as measured by changes on individual measures of the neurocognitive battery, when compared with placebo. Serum samples are taken on the day of surgery and on day 2 post surgery to quantitate any changes in levels of biomarkers of inflammation and oxidative stress.DISCUSSION: The PANACEA trial aims to examine the potential efficacy of perioperative NAC to reduce the severity of postoperative cognitive dysfunction in an elderly, noncardiac surgery population. This is an entirely novel approach to the prevention of postoperative cognitive dysfunction and will have high impact and translatable outcomes if NAC is found to be beneficial
