    Community based rehabilitation by paraplegic - a case report

    A journey of a young man started from day one in the hospital. When he came with the traumatic paraplegia, no doubt a paraplegic has to face a lot of challenges in his life when he came to know that his both legs are not working. He became bedridden and his moral is also become down for his future planning. But we accept the challenge, a team work by an orthopaedician and a physio-occupational therapist make it little easier. Postoperatively we prefer to offer rehabilitation through community base programme. Inspiration of hospital environment and equipment are going to help him for such kind of programme he accepts the challenge, basic aim of providing this programme is to reduce duration as well as incurred monitor losses. Special attraction of this case report is homemade bicycle, which served the purpose of providing repetitive rhythmic mobilisation of bilateral lower limbs which facilitates normal pattern of ambulation. Now he is ambulatory and goes back to his work. So, a person with spinal cord injury involved with their community with a little modification in his activity and equipment is worth for him. Traumatic paraplegia offers a big challenge to physio- occupational therapist due to long term recovery it takes.  For Such patients on regular follow up till complete recovery, we preferred community based rehabilitation. This made him independent and incurred reduced monitory losses

It demands high clinical judgment to diagnose the case as most of the times patient is erroneously diagnosed as tracheoesophageal fistula and recurrent esophageal intubation or slipping of endotacheal tube in the esophagus should arise suspicion of LTEC. It has ben divided into four types by Benjamin and Inglis and severity depends upon type. Type III and IV present with severe symptoms in the neonatal period. Knowledge of different approaches to secure and maintain airway and preparedness for the same in such a case is most important to prevent catastrophic events

Australian medicinal plants collected from the Kuuku I’yu (Northern Kaanju) homelands, Cape York Peninsula, Queensland, Australia were investigated to determine their therapeutic potential. Extracts were tested for inhibition of protein glycation and key enzymes relevant to the management of hyperglycaemia and hypertension. The inhibitory activities were further correlated with the antioxidant activities. Methods Extracts of five selected plant species were investigated: Petalostigma pubescens, Petalostigma banksii, Memecylon pauciflorum, Millettia pinnata and Grewia mesomischa. Enzyme inhibitory activity of the plant extracts was assessed against α-amylase, α-glucosidase and angiotensin converting enzyme (ACE). Antiglycation activity was determined using glucose-induced protein glycation models and formation of protein-bound fluorescent advanced glycation endproducts (AGEs). Antioxidant activity was determined by measuring the scavenging effect of plant extracts against 1, 1-diphenyl-2-picryl hydrazyl (DPPH) and using the ferric reducing anti-oxidant potential assay (FRAP). Total phenolic and flavonoid contents were also determined. Results Extracts of the leaves of Petalostigma banksii and P. pubescens showed the strongest inhibition of α-amylase with IC50 values of 166.50 ± 5.50 ÎŒg/mL and 160.20 ± 27.92 ÎŒg/mL, respectively. The P. pubescens leaf extract was also the strongest inhibitor of α-glucosidase with an IC50 of 167.83 ± 23.82 ÎŒg/mL. Testing for the antiglycation potential of the extracts, measured as inhibition of formation of protein-bound fluorescent AGEs, showed that P. banksii root and fruit extracts had IC50 values of 34.49 ± 4.31 ÎŒg/mL and 47.72 ± 1.65 ÎŒg/mL, respectively, which were significantly lower (p < 0.05) than other extracts. The inhibitory effect on α-amylase, α-glucosidase and the antiglycation potential of the extracts did not correlate with the total phenolic, total flavonoid, FRAP or DPPH. For ACE inhibition, IC50 values ranged between 266.27 ± 6.91 to 695.17 ± 15.38 ÎŒg/mL. Conclusions The tested Australian medicinal plant extracts inhibit glucose-induced fluorescent AGEs, α-amylase, α-glucosidase and ACE with extracts of Petalostigma species showing the most promising activity. These medicinal plants could potentially be further developed as therapeutic agents in the treatment of hyperglycaemia and hypertension

    Furanoflavones pongapin and lanceolatin B blocks the cell cycle and induce senescence in CYP1A1-overexpressing breast cancer cells

    © 2018 Elsevier Ltd Expression of cytochrome P450-1A1 (CYP1A1) is suppressed under physiologic conditions but is induced (a) by polycyclic aromatic hydrocarbons (PAHs) which can be metabolized by CYP1A1 to carcinogens, and (b) in majority of breast cancers. Hence, phytochemicals or dietary flavonoids, if identified as CYP1A1 inhibitors, may help in preventing PAH-mediated carcinogenesis and breast cancer. Herein, we have investigated the cancer chemopreventive potential of a flavonoid-rich Indian medicinal plant, Pongamia pinnata (L.) Pierre. Methanolic extract of its seeds inhibits CYP1A1 in CYP1A1-overexpressing normal human HEK293 cells, with IC50 of 0.6 ”g/mL. Its secondary metabolites, the furanoflavonoids pongapin/lanceolatin B, inhibit CYP1A1 with IC50 of 20 nM. Although the furanochalcone pongamol inhibits CYP1A1 with IC50 of only 4.4 ”M, a semisynthetic pyrazole-derivative P5b, has ∌10-fold improved potency (IC50, 0.49 ÎŒM). Pongapin/lanceolatin B and the methanolic extract of P. pinnata seeds protect CYP1A1-overexpressing HEK293 cells from B[a]P-mediated toxicity. Remarkably, they also block the cell cycle of CYP1A1-overexpressing MCF-7 breast cancer cells, at the G0-G1 phase, repress cyclin D1 levels and induce cellular-senescence. Molecular modeling studies demonstrate the interaction pattern of pongapin/lanceolatin B with CYP1A1. The results strongly indicate the potential of methanolic seed-extract and pongapin/lanceolatin B for further development as cancer chemopreventive agents

    Sub-arcsecond imaging with the International LOFAR Telescope I. Foundational calibration strategy and pipeline

    The International LOFAR Telescope is an interferometer with stations spread across Europe. With baselines of up to ~2000 km, LOFAR has the unique capability of achieving sub-arcsecond resolution at frequencies below 200 MHz. However, it is technically and logistically challenging to process LOFAR data at this resolution. To date only a handful of publications have exploited this capability. Here we present a calibration strategy that builds on previous high-resolution work with LOFAR. It is implemented in a pipeline using mostly dedicated LOFAR software tools and the same processing framework as the LOFAR Two-metre Sky Survey (LoTSS). We give an overview of the calibration strategy and discuss the special challenges inherent to enacting high-resolution imaging with LOFAR, and describe the pipeline, which is publicly available, in detail. We demonstrate the calibration strategy by using the pipeline on P205+55, a typical LoTSS pointing with an 8 h observation and 13 international stations. We perform in-field delay calibration, solution referencing to other calibrators in the field, self-calibration of these calibrators, and imaging of example directions of interest in the field. We find that for this specific field and these ionospheric conditions, dispersive delay solutions can be transferred between calibrators up to ~1.5° away, while phase solution transferral works well over ~1°. We also demonstrate a check of the astrometry and flux density scale with the in-field delay calibrator source. Imaging in 17 directions, we find the restoring beam is typically ~0.3â€Čâ€Č ×0.2â€Čâ€Č although this varies slightly over the entire 5 deg2 field of view. We find we can achieve ~80–300 ÎŒJy bm−1 image rms noise, which is dependent on the distance from the phase centre; typical values are ~90 ÎŒJy bm−1 for the 8 h observation with 48 MHz of bandwidth. Seventy percent of processed sources are detected, and from this we estimate that we should be able to image roughly 900 sources per LoTSS pointing. This equates to ~ 3 million sources in the northern sky, which LoTSS will entirely cover in the next several years. Future optimisation of the calibration strategy for efficient post-processing of LoTSS at high resolution makes this estimate a lower limit

    Objective: To evaluate the antihyperglycemic activity of aqueous extract of leaves of Cocculus hirsutus (L.) Diels in alloxan-induced diabetic mice. Materials and Methods: Alloxan-induced (70 mg/kg, i.v.) diabetic mice were given aqueous leaf extract (250, 500, and 1000 mg/kg, p.o., n= 6) of C. hirsutus or vehicle (distilled water, 10 ml/kg, p.o.) or standard drug glyburide (10 mg/kg, p.o.) for 28 days. Blood samples were withdrawn by retro-orbital puncture and were analyzed for serum glucose on 0th, 7th, 14th, 21st, and 28th days by glucose oxidase/peroxidase method. In oral glucose tolerance test, glucose (2.5 g/kg, p.o.) was administered to nondiabetic control, glyburide (10 mg/kg, p.o.), and aqueous extract of C. hirsutus (1000 mg/kg, p.o.) treated mice. The serum glucose level was analyzed at 0, 30, 60, and 120 min after drug administration. Results: The aqueous leaf extract of C. hirsutus (250, 500, and 1000 mg/kg, p.o.) showed significant ( P <0.01) reduction of serum glucose level in alloxan-induced diabetic mice at 28th day. In oral glucose tolerance test, aqueous extract of C. hirsutus increased the glucose tolerance. Conclusion: It is concluded that C. hirsutus has significant antihyperglycemic activity as it lowers serum glucose level in diabetic mice and significantly increases glucose tolerance

    Using strong lensing to understand the microJy radio emission in two radio quiet quasars at redshift 1.7

    peer reviewedThe radio quasar luminosity function exhibits an upturn around L6 GHz=1023L_{6\rm \:GHz}=10^{23} W Hz[SUP]-1[/SUP] that is well-modelled by a star-forming host galaxy population. This distribution leads some authors to cite star formation as the main radio emission mechanism in so-called radio-quiet quasars (RQQs). Understanding the origin of RQQ radio emission is crucial for our understanding of quasar feedback mechanisms - responsible for the regulation of star formation in the host galaxy - and for understanding galaxy evolution as a whole. By observing RQQs that have been magnified by strong gravitational lensing, we have direct access to the RQQ population out to cosmic noon, where evidence for twin mini-jets has recently been found in a sub-ÎŒ\muJy RQQ. Here we present radio observations of two lensed RQQs using the VLA at 5 GHz, the latest objects to be observed in a sample of quadruply-imaged RQQs above -30°. In SDSS J1004+4112 we find strong evidence for AGN-related radio emission in the variability of the source. In PG 1115+080 we find tentative evidence for AGN-related emission, determined by comparing the radio luminosity with modelled dust components. If confirmed in the case of PG 1115+080, which lies on the radio-FIR correlation, the result would reinforce the need for caution when applying the correlation to rule out jet activity and when assuming no AGN heating of FIR-emitting dust when calculating star formation rates. Our programme so far has shown that the two of the faintest radio sources ever imaged show strong evidence for AGN-dominated radio emission
