    Topical Tacrolimus as an adjunct to Conventional Therapy for Stromal Herpetic Keratitis: a Randomized Clinical Trial

    Purpose: This study investigates the effects of 0.05% topical tacrolimus as an adjunct therapy for patients with non-necrotizing herpetic stromal keratitis (HSK). Methods: Patients with non-necrotizing HSK, referred to the Cornea Clinic at Hospital in Rasht, Iran, between September 2016 and February 2018, were randomly assigned to two groups. The case group (N = 25) and the control group (N = 25) received conventional treatment with systemic acyclovir and topical prednisolone. The case group (N = 25) additionally received 0.05% tacrolimus eye drops four times a day for one month. Complete ocular examinations, including best-corrected visual acuity (BCVA) assessment, intraocular pressure (IOP) measurement, slit lamp biomicroscopy, and photo slit lamp imaging, were performed before treatment, and 3, 7, 14, 21, and 28 days after the intervention. Results: The mean age of the patients was 46.2 ± 12.9 years, and 70% of the patients were male. There was no difference between the groups in terms of age, sex, and baseline ocular measurements (P > 0.05). The case group had a lower mean logarithm of the minimum angle of resolution (LogMAR) for BCVA, lower grading scores, and steeper decreasing trends for corneal haziness, edema, neovascularization, and epitheliopathy compared to the control group after the second week (P < 0.05), while IOP remained unchanged between groups (P > 0.05). Conclusion: The addition of 0.05% topical tacrolimus enhances visual acuity and reduces corneal inflammation, neovascularization, and scarring; thus, it can used as an appropriate adjunct treatment for patients with HSK

    Evaluation of Effects of Zingiber officinale on Salivation in Rats

    There are some herbal plants in Iranian traditional system of medicine which are believed to be excellent remedies to alleviate the symptoms of xerostomia. The aim of the present study was to evaluate the effect of systemic administration of seven different herbal extracts on the rate of salivation in rats. The extracts of 7 herbs; Zingiber officinale Roscoe (Zingiberaceae), Citrus sinensis (L.) Osbeck (Rutaceae), Artemisia absinthium L. (Asteraceae), Cichorium intybus L. (Asteraceae), Pimpinella anisum L.(Apiaceae), Portulaca oleracea L.(Portulacaceae), Tribulus terrestris L. (Zygophyllaceae) were prepared. Nine groups of animals (including negative and positive control groups) were used and seven rats were tested in each group. After the injection of extracts, saliva volume was measured gravimetrically in four continuous seven-minute intervals. The results showed that after injection of ginger extracts salivation was significantly higher as compared to the negative control group and other herbal extracts in all of the four intervals (P<0.01). The peak action of the ginger was during the first 7-minute interval and following this, salivation decreased to some extent. The present study suggests that the extract of Zingiber offiicianle can increase the rate of salivation significantly in animal model. Further investigations on different constituents of ginger seem to be essential to identify the responsible constituent for stimulation of saliva secretion

    Effect of friction hardening pre-treatment on increasing cytocompatibility of alkali heat-treated Ti-6Al-4V alloy

    Alkali heat treatment (AHT) has been widely used as an effective surface chemical modification to improve the bioactivity and biocompatibility of titanium and its alloys; however, this method suffers from long process time. In this study, friction hardening (FH) as a cost-effective and straightforward severe plastic deformation (SPD) technique prior to AHT was applied in order to reduce the time-consumption and the manufacturing costs of AHT method. The results showed that the chemically modified FHed (FAHT) samples could prepare finer hierarchical nano/meso-structured network, with an average size range of 20–130 nm, on the surface of Ti-6Al-4V alloy at significant lower treating time. According to XPS results, this surface nanostructure possessed the toxic-element-free oxide layers. In-vitro cell culture showed that FAHT substrates, especially FAHT600-12 sample could remarkably enhance the cell-substrate interactions including: cell adhesion, spreading, proliferation as well as ALP activity. This can be attributed to both finer topography and better wettability

    The association between death anxiety with spiritual experiences and life satisfaction in elderly people

    Introduction: Death anxiety is a concept with greater importance among the elderly as they approach inevitability of death. Identifying the correlates of death anxiety among old people is important in order to reduce the burden of this problem. Therefore, the present study was performed with the aim to examine the association between spiritual experiences and life satisfaction with death anxiety in this stage of life. Method: This cross-sectional study with descriptive-analytical design included 190 elderly people visiting the health and medical centers of Neyshabur city, Iran, during fall and winter, 2016. Participants were asked to complete three questionnaires including a 16-item spiritual experiences scale, life satisfaction index proposed by Wood and Shifor with 13 items, and a 27-item death anxiety scale developed by Aminpour. Analytical statistics (Spearman’s correlation coefficient, Pearson’s correlation coefficient) were conducted using SPSS software version 22. Results: Fifty-eight percent of participants were in younger elderly age group with mean age of 68.18±7.13 years and the number of men and women was the same (95). A significant positive association between spiritual experiences and life satisfaction (r=0.2, p<0.05), a significant negative association between spiritual experiences and death anxiety (r=-0.184, p< 0.05) and a significant negative relationship between life satisfaction and death anxiety (r=-0.2, p<0.05) was found. Conclusion: Based on results, it seems that reducing stressors in this stage of life including reduction of death anxiety, is possible through use of spiritual experiences and increasing life satisfaction

    Microstructure, Texture, Electrical and Mechanical Properties of AA-6063 Processed by Multi Directional Forging

    In current research, the effect of the multi-directional forging (MDF) process on the microstructure, texture, mechanical and electrical properties of AA-6063 under different heat treatment conditions at various MDF temperatures was studied. The annealed AA-6063 alloy was processed up to three passes of MDF at ambient temperature. Three passes of this process were also applied to the solution-treated AA-6063 at ambient temperature and 177 &#176;C. Microstructural investigations demonstrated that the MDF process led to a significant reduction in the average grain size as well as a considerable increase in the fraction of low angle grain boundaries. Texture analysis revealed that copper and Goss textures were mainly developed within the annealed and solution-treated samples of AA-6063, respectively. The hardness and shear strength values of all processed samples also showed a sizeable improvement compared to the initial heat-treated samples. For example, the hardness and shear yield strength value of the solution-treated sample MDFed for three passes showed more than 100 and 70% increase, respectively. The effect of the MDF process on the electrical conductivity of AA-6063 under different heat treatment conditions at various temperatures was negligible. So, it can be concluded that the MDF process increased the mechanical properties without an appreciable decrease in electrical conductivity
