    Resting tachycardia, a warning sign in anorexia nervosa: case report

    BACKGROUND: Among psychiatric disorders, anorexia nervosa has the highest mortality rate. During an exacerbation of this illness, patients frequently present with nonspecific symptoms. Upon hospitalization, anorexia nervosa patients are often markedly bradycardic, which may be an adaptive response to progressive weight loss and negative energy balance. When anorexia nervosa patients manifest tachycardia, even heart rates in the 80–90 bpm range, a supervening acute illness should be suspected. CASE PRESENTATION: A 52-year old woman with longstanding anorexia nervosa was hospitalized due to progressive leg pain, weakness, and fatigue accompanied by marked weight loss. On physical examination she was cachectic but in no apparent distress. She had fine lanugo-type hair over her face and arms with an erythematous rash noted on her palms and left lower extremity. Her blood pressure was 96/50 mm Hg and resting heart rate was 106 bpm though she appeared euvolemic. Laboratory tests revealed anemia, mild leukocytosis, and hypoalbuminemia. She was initially treated with enteral feedings for an exacerbation of anorexia nervosa, but increasing leukocytosis without fever and worsening left leg pain prompted the diagnosis of an indolent left lower extremity cellulitis. With antibiotic therapy her heart rate decreased to 45 bpm despite minimal restoration of body weight. CONCLUSIONS: Bradycardia is a characteristic feature of anorexia nervosa particularly with significant weight loss. When anorexia nervosa patients present with nonspecific symptoms, resting tachycardia should prompt a search for potentially life-threatening conditions

    Interpreting genotype-by-environment interaction for biomass production in hybrid poplars under short-rotation coppice in Mediterranean environments

    Understanding genotype × environment interaction (GEI) is crucial to optimize the deployment of clonal material to field conditions in short‐rotation coppice poplar plantations. Hybrid poplars are grown for biomass production under a wide range of climatic and edaphic conditions, but their adaptive performance in Mediterranean areas remains poorly characterized. In this work, site regression (SREG) and factorial regression mixed models are combined to gain insight into the nature and causes underlying GEI for biomass production of hybrid poplar clones. SREG addresses the issue of clonal recommendation in multi‐environment trials through a biplot representation that visually identifies superior genotypes. Factorial regression, alternatively, involves a description of clonal reaction to the environment in terms of physical variables that directly affect productivity. Initially, SREG aided in identifying cross‐over interactions that often involved hybrids of different taxonomic background. Factorial regression then selected latitude, mean temperature of the vegetative period (MTVP) and soil sand content as main site factors responsible for differential clonal adaptation. Genotypic responses depended strongly on taxonomic background: P. deltoides Bartr. ex Marsh. × P. nigra L. clones showed an overall positive sensitivity to increased MTVP and negative sensitivity to increased sand content, whereas the opposite occurred for P. trichocarpa Torr. & Gray × P. deltoides clones; the three‐cross hybrid [(P. deltoides × P. trichocarpa) × P. nigra] often displayed an intermediate performance. This information can contribute toward the identification and biological understanding of adaptive characteristics relevant for poplar breeding in Mediterranean conditions and facilitate clonal recommendation at eco‐regional level.This research was funded by MINECO (Spain) throughout the project RTA2008-00025-C02-01 and RTA2011-00006-00-00.     Candidate Tidal Dwarf Galaxies in Arp 305: Lessons on Dwarf Detachment and Globular Cluster Formation

    To search for Tidal Dwarf Galaxies (TDGs) and to study star formation in tidal features, we are conducting a large UV imaging survey of interacting galaxies selected from the Arp (1996) Atlas using the GALEX telescope. As part of that study, we present a GALEX UV and SDSS and SARA optical study of the gas-rich interacting galaxy pair Arp 305 (NGC 4016/7). The GALEX UV data reveal much extended diffuse UV emission and star formation outside the disks. This includes a luminous star forming region between the two galaxies, and a number of such regions in tidal tails. We have identified 45 young star forming clumps in Arp 305, including several TDG candidates. By comparing the UV and optical colors to population synthesis models, we determined that the clumps are very young, with several having ages of about 6 Myr. We do not find many intermediate age clumps in spite of the fact that the last closest encounter was about 300 Myr ago. We have used a smooth particle hydrodynamics code to model the interaction and determine the fate of the star clusters and candidate TDGs.Comment: 12 pages, 6 figures, and 5 tables.     CIRPASS near-infrared integral-field spectroscopy of massive star clusters in the starburst galaxy NGC 1140

    [ABRIDGED] We analyse near-infrared integral field spectroscopy of the central starburst region of NGC 1140, obtained at the Gemini-South telescope equipped with CIRPASS. Our ~1.45-1.67 um wavelength coverage includes the bright [Fe II] emission line, as well as high-order Brackett (hydrogen) lines. While strong [Fe II] emission, thought to originate in the thermal shocks associated with supernova remnants, is found throughout the galaxy, both Br 12-4 and Br 14-4 emission, and weak CO(6,3) absorption, is predominantly associated with the northern starburst region. The Brackett lines originate from recombination processes occurring on smaller scales in (young) HII regions. The time-scale associated with strong [Fe II] emission implies that most of the recent star-formation activity in NGC 1140 was induced in the past 35-55 Myr. Based on the spatial distributions of the [Fe II] versus Brackett line emission, we conclude that a galaxy-wide starburst was induced several tens of Myr ago, with more recent starburst activity concentrated around the northern starburst region. This scenario is (provisionally) confirmed by our analysis of the spectral energy distributions of the compact, young massive star clusters (YMCs) detected in new and archival broad-band HST images. The YMC ages in NGC 1140 are all <= 20 Myr, consistent with independently determined estimates of the galaxy's starburst age, while there appears to be an age difference between the northern and southern YMC complexes in the sense expected from our CIRPASS analysis. Our photometric mass estimates of the NGC 1140 YMCs, likely upper limits, are comparable to those of the highest-mass Galactic globular clusters and to spectroscopically confirmed masses of (compact) YMCs in other starburst galaxies.Comment: 16 pages LaTeX, incl. 6 postscript figures; accepted for publication in MNRA

    Host-microbe-drug-nutrient screen identifies bacterial effectors of metformin therapy

    Metformin is the first-line therapy for treating type-2 diabetes and a promising anti-aging drug. We set out to address the fundamental question of how gut microbes and nutrition, key regulators of host physiology, impact the effects of metformin. Combining two tractable genetic models, the bacterium E. coli and the nematode C. elegans, we developed a high-throughput four-way screen to define the underlying host-microbe-drug-nutrient interactions. We show that microbes integrate cues from metformin and the diet through the phosphotransferase signalling pathway that converges on the transcriptional regulator Crp. A detailed experimental characterization of metformin effects downstream of Crp in combination with metabolic modelling of the microbiota in metformin-treated type-2 diabetic patients predicts the production of microbial agmatine, a regulator of metformin effects on host lipid metabolism and lifespan. Our high-throughput screening platform paves the way for identifying exploitable drug-nutrient-microbiome interactions to improve host health and longevity through targeted microbiome therapie

    Phenotypic insights into ADCY5-associated disease

    BACKGROUND: Adenylyl cyclase 5 (ADCY5) mutations is associated with heterogenous syndromes: familial dyskinesia and facial myokymia; paroxysmal chorea and dystonia; autosomal-dominant chorea and dystonia; and benign hereditary chorea. We provide detailed clinical data on 7 patients from six new kindreds with mutations in the ADCY5 gene, in order to expand and define the phenotypic spectrum of ADCY5 mutations. METHODS: In 5 of the 7 patients, followed over a period of 9 to 32 years, ADCY5 was sequenced by Sanger sequencing. The other 2 unrelated patients participated in studies for undiagnosed pediatric hyperkinetic movement disorders and underwent whole-exome sequencing. RESULTS: Five patients had the previously reported p.R418W ADCY5 mutation; we also identified two novel mutations at p.R418G and p.R418Q. All patients presented with motor milestone delay, infantile-onset action-induced generalized choreoathetosis, dystonia, or myoclonus, with episodic exacerbations during drowsiness being a characteristic feature. Axial hypotonia, impaired upward saccades, and intellectual disability were variable features. The p.R418G and p.R418Q mutation patients had a milder phenotype. Six of seven patients had mild functional gain with clonazepam or clobazam. One patient had bilateral globus pallidal DBS at the age of 33 with marked reduction in dyskinesia, which resulted in mild functional improvement. CONCLUSION: We further delineate the clinical features of ADCY5 gene mutations and illustrate its wide phenotypic expression. We describe mild improvement after treatment with clonazepam, clobazam, and bilateral pallidal DBS. ADCY5-associated dyskinesia may be under-recognized, and its diagnosis has important prognostic, genetic, and therapeutic implications. © 2016 The Authors.     Localization length in a random magnetic field

    Kubo formula is used to get the d.c conductance of a statistical ensemble of two dimensional clusters of the square lattice in the presence of random magnetic fluxes. Fluxes traversing lattice plaquettes are distributed uniformly between minus one half and plus one half of the flux quantum. The localization length is obtained from the exponential decay of the averaged conductance as a function of the cluster side. Standard results are recovered when this numerical approach is applied to Anderson model of diagonal disorder. The localization length of the complex non-diagonal model of disorder remains well below 10 000 (in units of the lattice constant) in the main part of the band in spite of its exponential increase near the band edges.Comment: 12 two-column pages including 10 figures (epsfig), revtex, to appear in PR

    Heat Safety in the Workplace:Modified Delphi Consensus to Establish Strategies and Resources to Protect U.S Workers

    The purpose of this consensus document was to develop feasible, evidence‐based occupational heat safety recommendations to protect the US workers that experience heat stress. Heat safety recommendations were created to protect worker health and to avoid productivity losses associated with occupational heat stress. Recommendations were tailored to be utilized by safety managers, industrial hygienists, and the employers who bear responsibility for implementing heat safety plans. An interdisciplinary roundtable comprised of 51 experts was assembled to create a narrative review summarizing current data and gaps in knowledge within eight heat safety topics: (a) heat hygiene, (b) hydration, (c) heat acclimatization, (d) environmental monitoring, (e) physiological monitoring, (f) body cooling, (g) textiles and personal protective gear, and (h) emergency action plan implementation. The consensus‐based recommendations for each topic were created using the Delphi method and evaluated based on scientific evidence, feasibility, and clarity. The current document presents 40 occupational heat safety recommendations across all eight topics. Establishing these recommendations will help organizations and employers create effective heat safety plans for their workplaces, address factors that limit the implementation of heat safety best‐practices and protect worker health and productivity
