    Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     Elective cancer surgery in COVID-19-free surgical pathways during the SARS-CoV-2 pandemic: An international, multicenter, comparative cohort study

    PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19鈥揻ree surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19鈥揻ree surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19鈥揻ree surgical pathways. Patients who underwent surgery within COVID-19鈥揻ree surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19鈥揻ree surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score鈥搈atched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19鈥揻ree surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19鈥揻ree surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks

    Aprender a programar (pensamiento computacional) es un vector de innovaci贸n en auge en el uso de las TIC en el aula. En algunos pa铆ses y comunidades aut贸nomas se ha incluido esta competencia en el curr铆culum oficial de Primaria y Secundaria. El dise帽o de aplicaciones multimedia mediante programaci贸n visual resulta asequible para las edades del alumnado y muy transversal a distintas 谩reas curriculares: lengua, ingl茅s, matem谩ticas, naturales, pl谩stica, m煤sica, entre otras. El dise帽o de apps para sus dispositivos m贸viles representa en el alumnado una evoluci贸n natural en la secuencia progresiva de aprendizaje en interacci贸n con estos terminales. Permite que el alumnado supere el rol de simple consumidor de tecnolog铆a para convertirse en un aut茅ntico creador digital. La elaboraci贸n de productos digitales en distintos formatos encuentra su espacio en el marco del dise帽o y puesta en pr谩ctica en el aula de proyectos educativos con metodolog铆a ABP (Aprendizaje Basado en Proyectos).ES

    Reflexiones sobre los aspectos m谩s relevantes del 谩rea de Educaci贸n F铆sica a la luz de los avances y aportaciones m谩s recientes y de la experiencia acumulada durante esta 煤ltima d茅cada de reformas. Se trata la secuenciaci贸n del dise帽o curricular y el dise帽o de unidades did谩cticas, as铆 como las adaptaciones curriculares para la atenci贸n a la diversidad. En cuanto a la metodolog铆a en educaci贸n f铆sica, se propone una alternancia metodol贸gica, y un enfoque descriptivo para la evaluaci贸n. Se presentan las pautas para la optimizaci贸n del tiempo de actividad, la interacci贸n socio-afectiva entre profesor y alumnos, la ense帽anza de temas te贸ricos, la educaci贸n en valores y transversalidad y la iniciaci贸n deportiva. Por 煤ltimo, se trata la investigaci贸n en educaci贸n f铆sica y la integraci贸n de los medios audiovisuales y recursos de internet para la formaci贸n docente.     Resumen basado en la publicaci贸nSe presenta un programa de creaci贸n de materiales multimedia para la autoformaci贸n y aprendizaje en l铆nea del profesorado de Infantil, Primaria y Secundaria de cualquier 谩rea educativa utilizando las Nuevas Tecnolog铆as de la Informaci贸n y la Comunicaci贸n. Tiene como finalidad la iniciaci贸n en el uso del programa Macromedia Flash para el dise帽o de aplicaciones educativas. Los objetivos previstos son:a) Familiarizarse con la tecnolog铆a Flash para aprovechar en la ense帽anza los recursos multimedia SWF existentes en Internet; b) Mejorar la calidad de presentaci贸n de un espacio web. El profesorado participante en este curso podr谩 emplear esta herramienta en el dise帽o de la web de su centro, proyecto, grupo de trabajo, etc.; c) Dise帽ar aplicaciones multimedia en CD-ROM. Esta herramienta, en su versi贸n MX 2004, posibilita una completa integraci贸n de gr谩ficos, texto, sonidos y videos en distintos formatos. Esto profundiza en el sentido de la convergencia de los medios: inform谩ticos y audiovisuales; d) Traducir a formato SWF presentaciones realizadas con otras herramientas. De esa forma se aprovechar铆an las caracter铆sticas de este formato: velocidad y econom铆a de espacio, compatibilidad con todos los sistemas e incluso m谩s interactividad.
