    Amniotic fluid volume: Rapid MR-based assessment at 28-32 weeks gestation

    Objectives: This work evaluates rapid magnetic resonance projection hydrography (PH) based amniotic fluid volume (AFV) estimates against established routine ultrasound single deepest vertical pocket (SDVP) and amniotic fluid index (AFI) measurements, in utero, at 28-32 weeks gestation. Manual multi-section planimetry (MSP) based measurement of AFV is used as a proxy reference standard. Methods: 35 women with a healthy singleton pregnancy (20-41 years) attending routine antenatal ultrasound were recruited. SDVP and AFI were measured using ultrasound, with same day MRI assessing AFV with PH and MSP. The relationships between the respective techniques were assessed using linear regression analysis and Bland-Altman method comparison statistics. Results: When comparing estimated AFV, a highly significant relationship was observed between PH and the reference standard MSP (R2=0.802, p<0.001). For the US measurements, SDVP measurement related most closely to amniotic fluid volume, (R2=0.470, p<0.001), with AFI demonstrating a weaker relationship (R2=0.208, p=0.007). Conclusion: This study shows that rapid MRI based PH measurement is a better predictor of AFV, relating more closely to our proxy standard than established US techniques. Although larger validation studies across a range of gestational ages are required this approach could form part of MR fetal assessment, particularly where poly or oligohydramnios is suspected.This study was supported by the National Institute of Health Research, Cambridge Biomedical Research Centre.     A porcine model of acute, haematogenous, localized osteomyelitis due to Staphylococcus aureus: a pathomorphological study

    A porcine model of acute, haematogenous, localized osteomyelitis was established. Serial dilutions of Staphylococcus aureus [5–50–500–5000–50 000 CFU/kg body weight (BW) suspended in saline or saline alone] were inoculated into the right brachial artery of pigs (BW 15 kg) separated into six groups of two animals. During the infection, blood was collected for cultivation, and after the animals were killed from day 5 to 15, they were necropsied and tissues were sampled for histopathology. Animals receiving ≀500 CFU/kg BW were free of lesions. Pigs inoculated with 5000 and 50 000 CFU/kg BW only developed microabscesses in bones of the infected legs. In the centre of microabscesses, S. aureus was regularly demonstrated together with necrotic neutrophils. Often, bone lesions resulted in trabecular osteonecrosis. The present localized model of acute haematogenous osteomyelitis revealed a pattern of development and presence of lesions similar to the situation in children. Therefore, this model should be reliably applied in studies of this disease with respect to e.g. pathophysiology and pathomorphology. Moreover, because of the regional containment of the infection to a defined number of bones, the model should be applicable also for screening of new therapy strategies

    Diophantine approximation in Kleinian groups : singular, extremal, and bad limit points

    The overall aim of this note is to initiate a ‘manifold’ theory for metric Diophantine approximation on the limit sets of Kleinian groups. We investigate the notions of singular and extremal limit points within the geometrically finite Kleinian group framework. Also, we consider the natural analogue of Davenport's problem regarding badly approximable limit points in a given subset of the limit set. Beyond extremality, we discuss potential Khintchine-type statements for subsets of the limit set. These can be interpreted as the conjectural ‘manifold’ strengthening of Sullivan's logarithmic law for geodesics

    Removing rician bias in diffusional kurtosis of the prostate using real-data reconstruction.

    Purpose To compare prostate diffusional kurtosis imaging (DKI) metrics generated using phase-corrected real data with those generated using magnitude data with and without noise compensation (NC).Methods Diffusion-weighted images were acquired at 3T in 16 prostate cancer patients, measuring 6 b-values (0-1500 s/mm2 ), each acquired with 6 signal averages along 3 diffusion directions, with noise-only images acquired to allow NC. In addition to conventional magnitude averaging, phase-corrected real data were averaged in an attempt to reduce rician noise-bias, with a range of phase-correction low-pass filter (LPF) sizes (8-128 pixels) tested. Each method was also tested using simulations. Pixelwise maps of apparent diffusion (D) and apparent kurtosis (K) were calculated for magnitude data with and without NC and phase-corrected real data. Average values were compared in tumor, normal transition zone (NTZ), and normal peripheral zone (NPZ).Results Simulations indicated LPF size can strongly affect K metrics, where 64-pixel LPFs produced accurate metrics. Relative to metrics estimated from magnitude data without NC, median NC K were lower (P < 0.0001) by 6/11/8% in tumor/NPZ/NTZ, 64-LPF real-data K were lower (P < 0.0001) by 4/10/7%, respectively.Conclusion Compared with magnitude data with NC, phase-corrected real data can produce similar K, although the choice of phase-correction LPF should be chosen carefully

    A Two Micron All-Sky Survey View of the Sagittarius Dwarf Galaxy: VI. s-Process and Titanium Abundance Variations Along the Sagittarius Stream

    We present high-resolution spectroscopic measurements of the abundances of titanium (Ti), yttrium (Y) and lanthanum (La) for M giant candidates of the Sagittarius (Sgr) dwarf spheroidal (dSph) + tidal tail system pre-selected on the basis of position and radial velocity. The majority of these stars show peculiar abundance patterns compared to those of nominal Milky Way (MW) stars. The overall [Ti/Fe], [Y/Fe], [La/Fe] and [La/Y] patterns with [Fe/H] of the Sgr stream plus Sgr core do resemble those seen in the Large Magellanic Cloud (LMC) and other dSphs, only shifted [Fe/H] by ~+0.4 from the LMC and by ~+1 dex from the other dSphs; these relative shifts reflect the faster and/or more efficient chemical evolution of Sgr compared to the other satellites, and show that Sgr has had an enrichment history more like the LMC than the other dSphs. By tracking the evolution of the abundance patterns along the Sgr stream we can follow the time variation of the chemical make-up of dSph stars donated to the MW halo by Sgr. This evolution demonstrates that while the bulk of the stars currently in the Sgr dSph are quite unlike those of the MW halo, an increasing number of stars farther along the Sgr stream have abundances like MW halo stars, a trend that shows clearly how the MW halo could have been contributed by present day satellite galaxies even if the present chemistry of those satellites is now different from typical halo field stars. Finally, we analyze the chemical abundances of a moving group of M giants among the Sgr leading arm stars at the North Galactic Cap, but having radial velocities unlike the infalling Sgr leading arm debris there. Through use of "chemical fingerprinting", we conclude that these northern hemisphere M giants also are Sgr stars, likely trailing arm debris overlapping the leading arm in the north.Comment: Accepted for publication in Ap

    Gene expression and matrix turnover in overused and damaged tendons

    Chronic, painful conditions affecting tendons, frequently known as tendinopathy, are very common types of sporting injury. The tendon extracellular matrix is substantially altered in tendinopathy, and these changes are thought to precede and underlie the clinical condition. The tendon cell response to repeated minor injuries or “overuse” is thought to be a major factor in the development of tendinopathy. Changes in matrix turnover may also be effected by the cellular response to physical load, altering the balance of matrix turnover and changing the structure and composition of the tendon. Matrix turnover is relatively high in tendons exposed to high mechanical demands, such as the supraspinatus and Achilles, and this is thought to represent either a repair or tissue maintenance function. Metalloproteinases are a large family of enzymes capable of degrading all of the tendon matrix components, and these are thought to play a major role in the degradation of matrix during development, adaptation and repair. It is proposed that some metalloproteinase enzymes are required for the health of the tendon, and others may be damaging, leading to degeneration of the tissue. Further research is required to investigate how these enzyme activities are regulated in tendon and altered in tendinopathy. A profile of all the metalloproteinases expressed and active in healthy and degenerate tendon is required and may lead to the development of new drug therapies for these common and debilitating sports injuries

    Clinical study of the factors affecting radioulnar deviation of the wrist joint

    <p>Abstract</p> <p>Background</p> <p>The radioulnar carpal joint is critical for hand and wrist function. Radioulnar deviation indicates distal radioulnar joint flexibility and reflects the structure and function of the carpal bones, ulna, radius and ligaments. The present study examined whether radioulnar deviation is affected by gender, manual labor, playing a musical instrument, playing sport, handedness, previous fracture or prior inflammation. The study used clinical findings based on anatomical landmarks</p> <p>Methods</p> <p>The ulnar, radial and total deviations for both left and right hands were measured in 300 subjects (157 men and 143 women) of mean age 21.7 years. Measurements were made with the forearm in a fixed pronated position using a novel specially designed goniometer. The gender of each subject was recorded, and information on playing of sport, playing a musical instrument, manual labor, handedness, and history of fracture or inflammation was sought. Data were analyzed using a multifactor ANOVA test.</p> <p>Results</p> <p>No statistically significant difference (p-value > 0.05) was found between those comparing groups except the total deviation of athletes' left hand versus the total deviation of non athletes' left hand (p-value 0.041 < 0.05) and the radial deviation of manual workers' left hand and non manual workers' left hand (p-value 0.002 < 0.05).</p> <p>Conclusions</p> <p>This study was based on clinical findings using anatomical landmarks. We found that manual workers and athletes showed greater left hand flexibility. This suggests that activities that place chronic stress on the radiocarpal joint can independently affect radioulnar deviation.</p
