Shared decision making in patients with low risk chest pain: prospective randomized pragmatic trial.
OBJECTIVE: To compare the effectiveness of shared decision making with usual care in choice of admission for observation and further cardiac testing or for referral for outpatient evaluation in patients with possible acute coronary syndrome.
DESIGN: Multicenter pragmatic parallel randomized controlled trial.
SETTING: Six emergency departments in the United States.
PARTICIPANTS: 898 adults (aged \u3e17 years) with a primary complaint of chest pain who were being considered for admission to an observation unit for cardiac testing (451 were allocated to the decision aid and 447 to usual care), and 361 emergency clinicians (emergency physicians, nurse practitioners, and physician assistants) caring for patients with chest pain.
INTERVENTIONS: Patients were randomly assigned (1:1) by an electronic, web based system to shared decision making facilitated by a decision aid or to usual care. The primary outcome, selected by patient and caregiver advisers, was patient knowledge of their risk for acute coronary syndrome and options for care; secondary outcomes were involvement in the decision to be admitted, proportion of patients admitted for cardiac testing, and the 30 day rate of major adverse cardiac events.
RESULTS: Compared with the usual care arm, patients in the decision aid arm had greater knowledge of their risk for acute coronary syndrome and options for care (questions correct: decision aid, 4.2 v usual care, 3.6; mean difference 0.66, 95% confidence interval 0.46 to 0.86), were more involved in the decision (observing patient involvement scores: decision aid, 18.3 v usual care, 7.9; 10.3, 9.1 to 11.5), and less frequently decided with their clinician to be admitted for cardiac testing (decision aid, 37% v usual care, 52%; absolute difference 15%; P
CONCLUSIONS: Use of a decision aid in patients at low risk for acute coronary syndrome increased patient knowledge about their risk, increased engagement, and safely decreased the rate of admission to an observation unit for cardiac testing.Trial registration ClinicalTrials.gov NCT01969240
The role of bacteria in the pathogenesis and progression of idiopathic pulmonary fibrosis
Rationale:Idiopathic pulmonaryfibrosis (IPF)isa progressivelung disease of unknown cause that leads to respiratory failure and death within 5 years of diagnosis. Overt respiratory infection and immunosuppression carry a high morbidity and mortality, and polymorphisms in genes related to epithelial integrity and host defense predispose to IPF. Objectives: To investigate the role of bacteria in the pathogenesis and progression of IPF. Methods: We prospectively enrolled patients diagnosed with IPF according to international criteria together with healthy smokers, nonsmokers, and subjectswithmoderate chronic obstructive pulmonary disease as control subjects. Subjects underwent bronchoalveolar lavage (BAL), from which genomic DNA was isolated. The V3âV5 region of the bacterial 16S rRNA gene was amplified, allowing quantification of bacterial load and identification of communities by 16S rRNA quantitative polymerase chain reaction and pyrosequencing. Measurements and Main Results: Sixty-five patients with IPF had double the burden of bacteria in BAL fluid compared with 44 control subjects. Baseline bacterial burden predicted the rate of decline in lung volume and risk of death and associated independently with the rs35705950 polymorphism of the MUC5B mucin gene, a proven host susceptibilityfactorfor IPF. Sequencing yielded912,883 high-quality reads from all subjects.WeidentifiedHaemophilus, Streptococcus,Neisseria, and Veillonella spp. to be more abundant in cases than control subjects. Regression analyses indicated that these specific operational taxonomic units as well as bacterial burden associated independently with IPF. Conclusions: IPF is characterized by an increased bacterial burden in BAL that predicts decline in lung function and death. Trials of antimicrobial therapy are needed to determine if microbial burden is pathogenic in the disease
XMMU J0541.8-6659, a new supernova remnant in the Large Magellanic Cloud
The high sensitivity of the XMM-Newton instrumentation offers the opportunity
to study faint and extended sources in the Milky Way and nearby galaxies such
as the Large Magellanic Cloud (LMC) in detail. The ROSAT PSPC survey of the LMC
has revealed more than 700 X-ray sources, among which there are 46 supernova
remnants (SNRs) and candidates. We have observed the field around one of the
most promising SNR candidates in the ROSAT PSPC catalogue, labelled [HP99] 456
with XMM-Newton, to determine its nature. We investigated the XMM-Newton data
along with new radio-continuum, near infrared and optical data. In particular,
spectral and morphological studies of the X-ray and radio data were performed.
The X-ray images obtained in different energy bands reveal two different
structures. Below 1.0 keV the X-ray emission shows the shell-like morphology of
an SNR with a diameter of ~73 pc, one of the largest known in the LMC. For its
thermal spectrum we estimate an electron temperature of (0.49 +/- 0.12)keV
assuming non-equilibrium ionisation. The X-ray images above 1.0 keV reveal a
less extended source within the SNR emission, located ~1' west of the centre of
the SNR and coincident with bright point sources detected in radio-continuum.
This hard component has an extent of 0.9' (i.e. ~13 pc at a distance of ~50
kpc) and a non-thermal spectrum. The hard source coincides in position with the
ROSAT source [HP99] 456 and shows an indication for substructure. We firmly
identify a new SNR in the LMC with a shell-like morphology and a thermal
spectrum. Assuming the SNR to be in the Sedov phase yields an age of ~23 kyr.
We explore possible associations of the hard non-thermal emitting component

Weak Lensing with SDSS Commissioning Data: The Galaxy-Mass Correlation Function To 1/h Mpc
(abridged) We present measurements of galaxy-galaxy lensing from early
commissioning imaging data from the Sloan Digital Sky Survey (SDSS). We measure
a mean tangential shear around a stacked sample of foreground galaxies in three
bandpasses out to angular radii of 600'', detecting the shear signal at very
high statistical significance. The shear profile is well described by a
power-law. A variety of rigorous tests demonstrate the reality of the
gravitational lensing signal and confirm the uncertainty estimates. We
interpret our results by modeling the mass distributions of the foreground
galaxies as approximately isothermal spheres characterized by a velocity
dispersion and a truncation radius. The velocity dispersion is constrained to
be 150-190 km/s at 95% confidence (145-195 km/s including systematic
uncertainties), consistent with previous determinations but with smaller error
bars. Our detection of shear at large angular radii sets a 95% confidence lower
limit , corresponding to a physical radius of
kpc, implying that galaxy halos extend to very large radii. However, it is
likely that this is being biased high by diffuse matter in the halos of groups
and clusters. We also present a preliminary determination of the galaxy-mass
correlation function finding a correlation length similar to the galaxy
autocorrelation function and consistency with a low matter density universe
with modest bias. The full SDSS will cover an area 44 times larger and provide
spectroscopic redshifts for the foreground galaxies, making it possible to
greatly improve the precision of these constraints, measure additional
parameters such as halo shape, and measure the properties of dark matter halos

Social democracy, embeddedness and decommodification: On the conceptual innovations and intellectual affiliations of Karl Polanyi
Of the several debates that revolve around the work of the economic historian and political economist Karl Polanyi, one that continues to exercise minds concerns his analysis of, and political attitudes toward, post-war capitalism and the welfare state. Simplified a little, it is a debate with two sides. To borrow IvĂĄn SzelĂŠnyi's terms, one side constructs a âhardâ Karl Polanyi, the other a âsoftâ one. The former advocated a socialist mixed economy dominated by redistributive mechanisms. He was a radical socialist for whom the market should never be the dominant mechanism of economic coordination. His âsoftâ alter ego insisted that the market system remain essentially intact but be complemented by redistributive mechanisms. The âdouble movementâ â the central thesis of his âGreat Transformationâ â acts, in this reading, as a self-correcting mechanism that moderates the excesses of market fundamentalism; its author was positioned within the social-democratic mainstream for which the only realistic desirable goal is a regulated form of capitalism. In terms of textual evidence there is much to be said for both interpretations. In this article I suggest a different approach, one that focuses upon the meaning of Polanyi's concepts in relation to their socio-political and intellectual environment
Colors of 2625 Quasars at 0<z<5 Measured in the Sloan Digital Sky Survey Photometric System
We present an empirical investigation of the colors of quasars in the Sloan
Digital Sky Survey (SDSS) photometric system. The sample studied includes 2625
quasars with SDSS photometry. The quasars are distributed in a 2.5 degree wide
stripe centered on the Celestial Equator covering square degrees.
Positions and SDSS magnitudes are given for the 898 quasars known prior to SDSS
spectroscopic commissioning. New SDSS quasars represent an increase of over
200% in the number of known quasars in this area of the sky. The ensemble
average of the observed colors of quasars in the SDSS passbands are well
represented by a power-law continuum with (). However, the contributions of the bump
and other strong emission lines have a significant effect upon the colors. The
color-redshift relation exhibits considerable structure, which may be of use in
determining photometric redshifts for quasars. The range of colors can be
accounted for by a range in the optical spectral index with a distribution
(95% confidence), but there is a red tail in the
distribution. This tail may be a sign of internal reddening. Finally, we show
that there is a continuum of properties between quasars and Seyfert galaxies
and we test the validity of the traditional division between the two classes of

The First Hour of Extra-galactic Data of the Sloan Digital Sky Survey Spectroscopic Commissioning: The Coma Cluster
On 26 May 1999, one of the Sloan Digital Sky Survey (SDSS) fiber-fed
spectrographs saw astronomical first light. This was followed by the first
spectroscopic commissioning run during the dark period of June 1999. We present
here the first hour of extra-galactic spectroscopy taken during these early
commissioning stages: an observation of the Coma cluster of galaxies. Our data
samples the Southern part of this cluster, out to a radius of 1.5degrees and
thus fully covers the NGC 4839 group. We outline in this paper the main
characteristics of the SDSS spectroscopic systems and provide redshifts and
spectral classifications for 196 Coma galaxies, of which 45 redshifts are new.
For the 151 galaxies in common with the literature, we find excellent agreement
between our redshift determinations and the published values. As part of our
analysis, we have investigated four different spectral classification
algorithms: spectral line strengths, a principal component decomposition, a
wavelet analysis and the fitting of spectral synthesis models to the data. We
find that a significant fraction (25%) of our observed Coma galaxies show signs
of recent star-formation activity and that the velocity dispersion of these
active galaxies (emission-line and post-starburst galaxies) is 30% larger than
the absorption-line galaxies. We also find no active galaxies within the
central (projected) 200 h-1 Kpc of the cluster. The spatial distribution of our
Coma active galaxies is consistent with that found at higher redshift for the
CNOC1 cluster survey. Beyond the core region, the fraction of bright active
galaxies appears to rise slowly out to the virial radius and are randomly
distributed within the cluster with no apparent correlation with the potential
