    Parental Knowledge, Attitudes and Practices Towards Paediatric Ear Infections in Riyadh, Saudi Arabia: A quantitative study

    ABSTRACT: Objectives: This study aimed to assess the knowledge, attitude and care-seeking practices of parents towards childhood ear infections in Riyadh, Saudi Arabia. Methods: This quantitative cross-sectional study was conducted in Riyadh, Saudi Arabia, between April and June 2018. An online survey was sent to a random selection of parents who were ≥18 years old with children of ≤10 years old. Associations between parental knowledge, attitude, care-seeking practices, socioeconomic status, level of education and age were assessed using the chi square test and logistic regression analysis. Results: A total of 782 subjects participated in this study (response rate: 78.2%). Most respondents were knowledgeable (76.2%), had a positive attitude (78.9%) and had positive care-seeking practices (89.5%). There was a statistically significant difference on both bivariate and multivariate analyses (P <0.05) between parental care seeking practice and knowledge. Parents who sought modern approaches to treatment were found to be almost nine times more likely to have a good attitude towards ear infections than those who did not seek treatment (odds ratio = 8.907, 95% confidence interval = 2.655–34.928; P <0.001). A statistically significant relationship was found between good care-seeking practice and age, as well as socioeconomic status (P <0.05 each). Conclusion: This study showed that knowledge, attitude and practice regarding paediatric ear infections were positive in the majority of parents in Riyadh, Saudi Arabia. Future studies should assess other factors that enhance the knowledge, attitude and practices regarding ear infections.Keywords: Otitis Media; Infection; Knowledge; Attitude; Pediatrics; Parents; Saudi Arabia

    هدف المقال إلى تطوير وحدة دراسية من مادة العلوم الصف الثالث األساسي وفق منحى STEAM وذلك إلستقصاء أثر الوحدة المطورة في مادة العلوم وفق منحى STEAM في تنمية الدافعية العقلية والتحصيل لدى طلبة الصف الثالث االساسي في لواء الجامعة، ولتحقيق أهداف الدراسة تم اعتماد المنهج شبه التجريبي، تكونت عّينة الدراسة من )60( طالبا وطالبة تم ً توزيعها عشوائيا الى مجموعتين تجريبية وضابطة اشتملت كل ً منهما على )30( طالباً وطالبة من طلبة الصف الثالث األساسي في مدرسة الكمالية االساسية التابعة لمديرية التربية والتعليم لواء الجامعة . تكونت أداة الدراسة من مقياس للدافعية العقلية يشتمل على )35( فقرة شملت مجاالت الدافعية العقلية جميعها، واختبار تحصيلي لمادة العلوم. أظهرت النتائج المتعلقة باإلجابة عن السؤالين األول والثاني وجود فروق ذات داللة إحصائية عند مستوى الداللة )0.05 =α )في األداء البعدي لمجموعتي الدراسة، لصالح طلبة المجموعة التجريبية حيث كان هناك أثر للمنحى في تنمية الدافعية العقلية والتحصيل. وأظهرت النتائج المتعلقة باإلجابة عن السؤال الثالث وجود عالقة ارتباطية دالة احصائيا عند مستوى الداللة )0.05 =α )بين الدافعية العقلية ً تعزى إلى الطريقة المستخدمة )STEAM )لطلبة والتحصيل ُ الصف الثالث األساسي.من أهم التوصيات تبني منحى STEAM عند تطوير المناهج الدراسية المختلفة والمواقف الصفية المتعددة؛ لفعاليتها في تنمية الدافعية العقلية والتحصيل وتعليم العلوم، تنظيم وترتيب المحتوى الدراسي الذي يحقق األهداف التعليمية بحيث يكون هناك سهولة في عرض المادة التعليمية بما يتناسب مع الخصائص النمائية والعمرية لطلبة الصف الثالث األساسي

    Survival outcomes of in-hospital cardiac arrest in pediatric patients in the USA

    We report on in-hospital cardiac arrest outcomes in the USA. The data were obtained from the National (Nationwide) Inpatient Sample datasets for the years 2000–2017, which includes data from participating hospitals in 47 US states and the District of Columbia. We included pediatric patients (< 18 years of age) with cardiac arrest, and we excluded patients with no cardiopulmonary resuscitation during the hospitalization. Primary outcome of the study was in-hospital mortality after cardiac arrest. A multivariable logistic regression was performed to identify factors associated with survival. A total of 20,654 patients were identified, and 8226 (39.82%) patients survived to discharge. The median length of stay and cost of hospitalization were significantly higher in the survivors vs. non-survivors (LOS 18 days vs. 1 day, and cost 187,434vs.187,434 vs. 45,811, respectively, p < 0.001). In a multivariable model, patients admitted to teaching hospitals, elective admissions, and those admitted on weekdays had higher survival (aOR=1.19, CI: 1.06–1.33; aOR=2.65, CI: 2.37–2.97; and aOR=1.17, CI: 1.07–1.27, respectively). There was no difference in mortality between patients with extracorporeal cardiopulmonary resuscitation (E-CPR) and those with conventional cardiopulmonary resuscitation. E-CPR patients were likely to have congenital heart surgery (51.0% vs. 20.8%). Conclusion: We highlighted the survival predictors in these events, which can guide future studies aimed at improving outcomes in pediatric cardiac arrest

    Effects of Lanthanum on Redox Systems in Plasma Membranes of Casuarina equisetifolia Seedlings Under Acid Rain Stress

    The effects of La3 + on the growth and the ATPases activities of plasma membrane (PM) vesicles isolated from Casuarina equisetifolia seedlings under artificial acid rain (pH 4. 5) stress were studied. The results show that the height , length of roots , fresh weight and PM H+2ATPase activites of Casuarina equisetifolia seedlings increase by the treatments of soaking seeds in LaCl3 solutions with lower concentrations , and those can reach their peak values by treating with 200 mg ·L - 1 La3 + . However , in comparison with the CK, those are inhibited by the higher La3 + concentrations ; PM Ca2 +2AT2 Pase activity is inhibited with the treatments of La3 + . The results also reveal that the H+2ATPase activity and the growth of cell enlarge have a remarkable positive correlation , and La3 + activating H+2ATPase can facilitate plant growth. La3 + al2 so can alleviate cytosolic acidification of plant under acid rain stress and indirectly maintain the stability of intracellular environment . In order to resistant to acid rain and accelerate the growth of Casuarina equisetifolia , the suitable range of La3 + concentrations to soak seeds for 8 h is 50~200 mg·L - 1

    The Effect of Ephedra Foeminea Extract as an Antimicrobial and Antifungal Agent

    Drugs derived from natural sources play a significant role in the prevention and treatment of human diseases. In many developing countries, traditional medicine is an essential part of primary healthcare systems (Abdallah, 2011). Due to the emergence of drug-resistant bacteria, it is essential to investigate new drugs with lesser resistance especially ones that can be derived from natural resources like plants. Ephedra is likely one of the oldest medicinal plants that are still currently in use. Antimicrobial and antifungal activities of some ephedra species have been noticed in recent years (ZHANG Ben-Mei et al,2018). The aim of the study is to observe and understand the effects of E. foeminea extracts as antimicrobial and antifungal agents. It is an experimental study; four different types of bacteria including, Staphylococcus aureus, Pseudomonas aeruginosa, MRSA, and Escherichia coli as well as two different types of fungi including, Klebsiella pneumoniae and Candida albicans were used as test microorganisms. Maceration extraction technique (William P. Jones,2012) for the dried stem of E. foeminea will be used by methanol / water 90/10 for 2 days (Ali Parsaeimehr et el,2010). Three concentrations of the extract will be used on 30 plates for each type of microorganism in the laboratories of the University of Palestine. Antimicrobial activity will be tested by using plate methods in which a variable diameter of a growth inhibition zone in most types of bacteria will appear. The MIC values may also be evaluated using the broth serial dilution method according to standard methods (CLSI, 2012)

    Viral shedding and antibody response in 37 patients with MERS-coronavirus infection

    Background. The Middle East respiratory syndrome (MERS) coronavirus causes isolated cases and outbreaks of severe respiratory disease. Essential features of the natural history of disease are poorly understood. Methods. We studied 37 adult patients infected with MERS coronavirus for viral load in the lower and upper respiratory tracts (LRT and URT, respectively), blood, stool, and urine. Antibodies and serum neutralizing activities were determined over the course of disease. Results. One hundred ninety-nine LRT samples collected during the 3 weeks following diagnosis yielded virus RNA in 93% of tests. Average (maximum) viral loads were 5 × 106 (6 × 1010) copies/mL. Viral loads (positive detection frequencies) in 84 URT samples were 1.9 × 104 copies/mL (47.6%). Thirty-three percent of all 108 serum samples tested yielded viral RNA. Only 14.6% of stool and 2.4% of urine samples yielded viral RNA. All seroconversions occurred during the first 2 weeks after diagnosis, which corresponds to the second and third week after symptom onset. Immunoglobulin M detection provided no advantage in sensitivity over immunoglobulin G (IgG) detection. All surviving patients, but only slightly more than half of all fatal cases, produced IgG and neutralizing antibodies. The levels of IgG and neutralizing antibodies were weakly and inversely correlated with LRT viral loads. Presence of antibodies did not lead to the elimination of virus from LRT. Conclusions. The timing and intensity of respiratory viral shedding in patients with MERS closely matches that of those with severe acute respiratory syndrome. Blood viral RNA does not seem to be infectious. Extrapulmonary loci of virus replication seem possible. Neutralizing antibodies do not suffice to clear the infection

    Immunohistochemical evaluation of human epidermal growth factor receptor 2 and estrogen and progesterone receptors in breast carcinoma in Jordan

    INTRODUCTION: Although breast carcinoma (BC) is the most common malignancy affecting Jordanian females and the affected population in Jordan is younger than that in the West, no information is available on its biological characteristics. Our aims in this study are to evaluate the expression of estrogen receptor (ER) and progesterone receptor (PR) and Her-2/neu overexpression in BC in Jordan, and to compare the expression of these with other prognostic parameters for BC such as histological type, histological grade, tumor size, patients' age, and number of lymph node metastases. METHOD: This is a retrospective study conducted in the Department of Pathology at Jordan University of Science and Technology. A confirmed 91 cases of BC diagnosed in the period 1995 to 1998 were reviewed and graded. We used immunohistochemistry to evaluate the expression of ER, PR, and Her-2. Immunohistochemical findings were correlated with age, tumor size, grade and axillary lymph node status. RESULTS: Her-2 was overexpressed in 24% of the cases. The mean age of Her-2 positive cases was 42 years as opposed to 53 years among Her-2 negative cases (p = 0.0001). Her-2 expression was inversely related to ER and PR expression. Her-2 positive tumors tended to be larger than Her-2 negative tumors with 35% overexpression among T3 tumors as opposed to 22% among T2 tumors (p = 0.13). Her-2 positive cases tended to have higher rates of axillary metastases, but this did not reach statistical significance. ER and PR positive cases were seen in older patients with smaller tumor sizes. CONCLUSION: Her-2 overexpression was seen in 24% of BC affecting Jordanian females. Her-2 overexpression was associated with young age at presentation, larger tumor size, and was inversely related to ER and PR expression. One-fifth of the carcinomas were Her-2 positive and ER negative. This group appears to represent an aggressive form of BC presenting at a young age with large primary tumors and a high rate of four or more axillary lymph node metastases

    Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10&nbsp;years; 78.2% included were male with a median age of 37&nbsp;years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     The global burden of adolescent and young adult cancer in 2019 : a systematic analysis for the Global Burden of Disease Study 2019

    Background In estimating the global burden of cancer, adolescents and young adults with cancer are often overlooked, despite being a distinct subgroup with unique epidemiology, clinical care needs, and societal impact. Comprehensive estimates of the global cancer burden in adolescents and young adults (aged 15-39 years) are lacking. To address this gap, we analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, with a focus on the outcome of disability-adjusted life-years (DALYs), to inform global cancer control measures in adolescents and young adults. Methods Using the GBD 2019 methodology, international mortality data were collected from vital registration systems, verbal autopsies, and population-based cancer registry inputs modelled with mortality-to-incidence ratios (MIRs). Incidence was computed with mortality estimates and corresponding MIRs. Prevalence estimates were calculated using modelled survival and multiplied by disability weights to obtain years lived with disability (YLDs). Years of life lost (YLLs) were calculated as age-specific cancer deaths multiplied by the standard life expectancy at the age of death. The main outcome was DALYs (the sum of YLLs and YLDs). Estimates were presented globally and by Socio-demographic Index (SDI) quintiles (countries ranked and divided into five equal SDI groups), and all estimates were presented with corresponding 95% uncertainty intervals (UIs). For this analysis, we used the age range of 15-39 years to define adolescents and young adults. Findings There were 1.19 million (95% UI 1.11-1.28) incident cancer cases and 396 000 (370 000-425 000) deaths due to cancer among people aged 15-39 years worldwide in 2019. The highest age-standardised incidence rates occurred in high SDI (59.6 [54.5-65.7] per 100 000 person-years) and high-middle SDI countries (53.2 [48.8-57.9] per 100 000 person-years), while the highest age-standardised mortality rates were in low-middle SDI (14.2 [12.9-15.6] per 100 000 person-years) and middle SDI (13.6 [12.6-14.8] per 100 000 person-years) countries. In 2019, adolescent and young adult cancers contributed 23.5 million (21.9-25.2) DALYs to the global burden of disease, of which 2.7% (1.9-3.6) came from YLDs and 97.3% (96.4-98.1) from YLLs. Cancer was the fourth leading cause of death and tenth leading cause of DALYs in adolescents and young adults globally. Interpretation Adolescent and young adult cancers contributed substantially to the overall adolescent and young adult disease burden globally in 2019. These results provide new insights into the distribution and magnitude of the adolescent and young adult cancer burden around the world. With notable differences observed across SDI settings, these estimates can inform global and country-level cancer control efforts. Copyright (C) 2021 The Author(s).     Estimating global injuries morbidity and mortality : methods and data used in the Global Burden of Disease 2017 study

    Background: While there is a long history of measuring death and disability from injuries, modern research methods must account for the wide spectrum of disability that can occur in an injury, and must provide estimates with sufficient demographic, geographical and temporal detail to be useful for policy makers. The Global Burden of Disease (GBD) 2017 study used methods to provide highly detailed estimates of global injury burden that meet these criteria. Methods: In this study, we report and discuss the methods used in GBD 2017 for injury morbidity and mortality burden estimation. In summary, these methods included estimating cause-specific mortality for every cause of injury, and then estimating incidence for every cause of injury. Non-fatal disability for each cause is then calculated based on the probabilities of suffering from different types of bodily injury experienced. Results: GBD 2017 produced morbidity and mortality estimates for 38 causes of injury. Estimates were produced in terms of incidence, prevalence, years lived with disability, cause-specific mortality, years of life lost and disability-adjusted life-years for a 28-year period for 22 age groups, 195 countries and both sexes. Conclusions: GBD 2017 demonstrated a complex and sophisticated series of analytical steps using the largest known database of morbidity and mortality data on injuries. GBD 2017 results should be used to help inform injury prevention policy making and resource allocation. We also identify important avenues for improving injury burden estimation in the future
