Tinnitus in elderly patients and prognosis of mild-to-moderate congestive heart failure: a cross-sectional study with a long-term extension of the clinical follow-up
<p>Abstract</p> <p>Background</p> <p>The complex mechanism responsible for tinnitus, a symptom highly prevalent in elderly patients, could involve an impaired control of the microcirculation of the inner ear, particularly in patients with poor blood pressure control and impaired left ventricular (LV) function.</p> <p>Methods</p> <p>In order to define the relationship between the presence of tinnitus and the severity and clinical prognosis of mild-to-moderate chronic heart failure (CHF) in a large population of elderly patients (N = 958), a cross-sectional study was conducted with a long-term extension of the clinical follow-up. Blood pressure, echocardiographic parameters, brain natriuretic peptide (BNP), hospitalization, and mortality for CHF were measured. Multivariate logistic regression analysis was used to assess the association between the presence of tinnitus and some of the prognostic determinants of heart failure.</p> <p>Results</p> <p>The presence of tinnitus was ascertained in 233 patients (24.3%; mean age 74.9 ± 6 years) and was associated with reduced systolic and diastolic blood pressure (123.1 ± 16/67.8 ± 9 vs 125.9 ± 15/69.7 ± 9; <it>P </it>= .027/<it>P </it>= .006), reduced LV ejection fraction (LVEF%; 43.6 ± 15 vs 47.9 ± 14%, <it>P </it>= .001), and increased BNP plasma levels (413.1 ± 480 vs 286.2 ± 357, <it>P </it>= .013) in comparison to patients without symptoms. The distribution of CHF functional class was shifted toward a greater severity of the disease in patients with tinnitus. Combined one-year mortality and hospitalization for CHF (events/year) was 1.43 ± 0.2 in patients with tinnitus and 0.83 ± 0.1 in patients without tinnitus, with an adjusted hazard ratio (HR) of 0.61 (95% confidence interval (CI): 0.37 to 0.93, <it>P </it><.002).</p> <p>Conclusions</p> <p>Our preliminary data indirectly support the hypothesis that tinnitus is associated with a worse CHF control in elderly patients and can have some important clinical implications for the early identification of patients who deserve a more aggressive management of CHF.</p
Search for Third Generation Vector Leptoquarks in p anti-p Collisions at sqrt(s) = 1.96 TeV
We describe a search for a third generation vector leptoquark (VLQ3) that
decays to a b quark and tau lepton using the CDF II detector and 322 pb^(-1) of
integrated luminosity from the Fermilab Tevatron. Vector leptoquarks have been
proposed in many extensions of the standard model (SM). Observing a number of
events in agreement with SM expectations, assuming Yang-Mills (minimal)
couplings, we obtain the most stringent upper limit on the VLQ3 pair production
cross section of 344 fb (493 fb) and lower limit on the VLQ3 mass of 317
GeV/c^2 (251 GeV/c^2) at 95% C.L.Comment: 7 pages, 2 figures, submitted to PR
Observation of Exclusive Gamma Gamma Production in p pbar Collisions at sqrt{s}=1.96 TeV
We have observed exclusive \gamma\gamma production in proton-antiproton
collisions at \sqrt{s}=1.96 TeV, using data from 1.11 \pm 0.07 fb^{-1}
integrated luminosity taken by the Run II Collider Detector at Fermilab. We
selected events with two electromagnetic showers, each with transverse energy
E_T > 2.5 GeV and pseudorapidity |\eta| < 1.0, with no other particles detected
in -7.4 < \eta < +7.4. The two showers have similar E_T and azimuthal angle
separation \Delta\phi \sim \pi; 34 events have two charged particle tracks,
consistent with the QED process p \bar{p} to p + e^+e^- + \bar{p} by two-photon
exchange, while 43 events have no charged tracks. The number of these events
that are exclusive \pi^0\pi^0 is consistent with zero and is < 15 at 95% C.L.
The cross section for p\bar{p} to p+\gamma\gamma+\bar{p} with |\eta(\gamma)| <
1.0 and E_T(\gamma) > 2.5$ GeV is

We observe no exclusive Z->ll candidates and place the first upper limit on the exclusive Z cross section in hadron collisions, sigma(exclu) gammagamma->p+ll+pbar, and measure the cross section for M(ll) > 40 GeV/c2 and |eta(l)|This Letter presents a search for exclusive Z boson production in proton-antiproton collisions at √s=1.96 TeV, using the CDF II detector. No exclusive Z→l+l- candidates are observed and the first upper limit on the exclusive Z cross section in hadron collisions is found to be σexcl(Z)40 GeV/c2 and |ηl|<4 is found to be σ=0.24-0.10+0.13 pb, which is consistent with the standard model prediction.Peer reviewe

Percutaneous CT-guided biopsy of the spine: results of 430 biopsies
Biopsies of lesions in the spine are often challenging procedures with significant risk of complications. CT-guided needle biopsies could lower these risks but uncertainties still exist about the diagnostic accuracy. Aim of this retrospective study was to evaluate the diagnostic accuracy of CT-guided needle biopsies for bone lesions of the spine. We retrieved the results of 430 core needle biopsies carried out over the past fifteen years at the authors’ institute and examined the results obtained. Of the 430 biopsies performed, in 401 cases the right diagnosis was made with the first CT-guided needle biopsy (93.3% accuracy rate). Highest accuracy rates were obtained in primary and secondary malignant lesions. Most false negative results were found in cervical lesions and in benign, pseudotumoral, inflammatory, and systemic pathologies. There were only 9 complications (5 transient paresis, 4 haematomas that resolved spontaneously) that had no influence on the treatment strategy, nor on the patient’s outcome. In conclusion we can assert that this technique is reliable and safe and should be considered the gold standard in biopsies of the spine
Measurement of charged-particle multiplicities in gluon and quark jets in p(p)over-bar collisions at root s=1.8 TeV
We report the first largely model independent measurement of charged particle multiplicities in quark and gluon jets, N-q and N-g, produced at the Fermilab Tevatron in p (p) over bar collisions with a center-of-mass energy of 1.8 TeV and recorded by the Collider Detector at Fermilab. The measurements are made for jets with average energies of 41 and 53 GeV by counting charged particle tracks in cones with opening angles of θ(c)=0.28, 0.36, and 0.47 rad around the jet axis. The corresponding jet hardness Q=E-jetθ(c) varies in the range from 12 to 25 GeV. At Q=19.2 GeV, the ratio of multiplicities r=N-g/N-q is found to be 1.64&PLUSMN; 0.17, where statistical and systematic uncertainties are added in quadrature. The results are in agreement with resummed perturbative QCD calculations
