    Usefulness of implementing the OHIP-14 questionnaire to assess the impact of xerostomia and hyposalivation on qualityof life in patients with primary Sjögren's syndrome

    Background: The aim of this study is to analyze if the results of the Oral Health Impact Profile-14 questionnaire (OHIP-14) in patients with primary Sjögren's syndrome (pSS) are correlated with salivary flow and level of xerostomia. Methods: This observational cross-sectional study was conducted in 61 patients (60 women, one man, mean age 57.64 [13.52]) diagnosed of pSS according to the American-European Criteria (2002). After recording demographic, medical and dental data (decayed-missing-filled teeth index [DMFT]), unstimulated (UWS) and stimulated (SWS) salivary flows were collected. Subsequently, UWS flow was categorized into two groups (0.7 ml/min). Patients also filled out a visual analog scale (VAS) for xerostomia and OHIP-14 for self-reported quality of life (QoL). Results: Data showed positive and significant correlation between OHIP-14 and xerostomia, based on VAS results (r = 0.52; p = 0.001). Furthermore, there was a negative correlation between UWS and OHIP-14 scores (r = -0.34; p = 0.006) and VAS for xerostomia (r = -0.22; p = 0.09). No significant correlation was found between SWS and OHIP-14 or VAS neither between DMFT and OHIP-14. When assessing the level of QoL by the UWS and SWS flow categories a significant association was found for UWS (p = 0.001) but not for SWS (p = 0.11). The OHIP-14 values were higher in the groups with lower salivary flow. The multiple linear regression to predict OHIP-14 only selected VAS for xerostomia as a statistically significant predictor. Conclusions: Increased level of xerostomia and reduced UWS flow decrease oral health-related QoL in patients with pSS

    A bacterial antirepressor with SH3 domain topology mimics operator DNA in sequestering the repressor DNA recognition helix

    Direct targeting of critical DNA-binding elements of a repressor by its cognate antirepressor is an effective means to sequester the repressor and remove a transcription initiation block. Structural descriptions for this, though often proposed for bacterial and phage repressor–antirepressor systems, are unavailable. Here, we describe the structural and functional basis of how the Myxococcus xanthus CarS antirepressor recognizes and neutralizes its cognate repressors to turn on a photo-inducible promoter. CarA and CarH repress the carB operon in the dark. CarS, produced in the light, physically interacts with the MerR-type winged-helix DNA-binding domain of these repressors leading to activation of carB. The NMR structure of CarS1, a functional CarS variant, reveals a five-stranded, antiparallel β-sheet fold resembling SH3 domains, protein–protein interaction modules prevalent in eukaryotes but rare in prokaryotes. NMR studies and analysis of site-directed mutants in vivo and in vitro unveil a solvent-exposed hydrophobic pocket lined by acidic residues in CarS, where the CarA DNA recognition helix docks with high affinity in an atypical ligand-recognition mode for SH3 domains. Our findings uncover an unprecedented use of the SH3 domain-like fold for protein–protein recognition whereby an antirepressor mimics operator DNA in sequestering the repressor DNA recognition helix to activate transcription

    Effectiveness of a cognitive behavioral intervention in patients with medically unexplained symptoms: cluster randomized trial

    BACKGROUND: Medically unexplained symptoms are an important mental health problem in primary care and generate a high cost in health services.Cognitive behavioral therapy and psychodynamic therapy have proven effective in these patients. However, there are few studies on the effectiveness of psychosocial interventions by primary health care. The project aims to determine whether a cognitive-behavioral group intervention in patients with medically unexplained symptoms, is more effective than routine clinical practice to improve the quality of life measured by the SF-12 questionary at 12 month. METHODS/DESIGN: This study involves a community based cluster randomized trial in primary healthcare centres in Madrid (Spain). The number of patients required is 242 (121 in each arm), all between 18 and 65 of age with medically unexplained symptoms that had seeked medical attention in primary care at least 10 times during the previous year. The main outcome variable is the quality of life measured by the SF-12 questionnaire on Mental Healthcare. Secondary outcome variables include number of consultations, number of drug (prescriptions) and number of days of sick leave together with other prognosis and descriptive variables. Main effectiveness will be analyzed by comparing the percentage of patients that improve at least 4 points on the SF-12 questionnaire between intervention and control groups at 12 months. All statistical tests will be performed with intention to treat. Logistic regression with random effects will be used to adjust for prognostic factors. Confounding factors or factors that might alter the effect recorded will be taken into account in this analysis. DISCUSSION: This study aims to provide more insight to address medically unexplained symptoms, highly prevalent in primary care, from a quantitative methodology. It involves intervention group conducted by previously trained nursing staff to diminish the progression to the chronicity of the symptoms, improve quality of life, and reduce frequency of medical consultations.     Coordinators: Josep Martí Pérez (IMF, CSIC), Idoia Murga Castro (IH, CSIC).This challenge is formulated in terms of “humanities in transition,” that is, their approach and articulation in the face of the changes they must undergo to achieve the social weight that, due to their intrinsic relevance, should correspond to them. Faced with these situations that would demand a reinforcement in research and dissemination in diverse aspects of the humanities, from multiple perspectives, paradoxically an adverse panorama is drawn for the development and dissemination of humanistic knowledge, which concerns different factors. Some are related to the consideration of the area of knowledge itself, its organization within the scientific system, the questioning of its own limits, and the interaction with another knowledge. Considering current transition scenarios does not mean having to abandon old objectives, but it adds to the work conducted new objects of study closely related to current reality, such as: the informational revolution; the relations with the ecosystem and the environmental crisis; globalization; the intensification of human mobility and migration flows; the growing economic and social inequality; the frictions derived from the articulation of collective identities; the decolonization of discourses; demographic dynamics; integration of technological advances; and viability and support for alternative models of society.Peer reviewe

    Spatiotemporal Characteristics of the Largest HIV-1 CRF02_AG Outbreak in Spain: Evidence for Onward Transmissions

    Background and Aim: The circulating recombinant form 02_AG (CRF02_AG) is the predominant clade among the human immunodeficiency virus type-1 (HIV-1) non-Bs with a prevalence of 5.97% (95% Confidence Interval-CI: 5.41–6.57%) across Spain. Our aim was to estimate the levels of regional clustering for CRF02_AG and the spatiotemporal characteristics of the largest CRF02_AG subepidemic in Spain.Methods: We studied 396 CRF02_AG sequences obtained from HIV-1 diagnosed patients during 2000–2014 from 10 autonomous communities of Spain. Phylogenetic analysis was performed on the 391 CRF02_AG sequences along with all globally sampled CRF02_AG sequences (N = 3,302) as references. Phylodynamic and phylogeographic analysis was performed to the largest CRF02_AG monophyletic cluster by a Bayesian method in BEAST v1.8.0 and by reconstructing ancestral states using the criterion of parsimony in Mesquite v3.4, respectively.Results: The HIV-1 CRF02_AG prevalence differed across Spanish autonomous communities we sampled from (p < 0.001). Phylogenetic analysis revealed that 52.7% of the CRF02_AG sequences formed 56 monophyletic clusters, with a range of 2–79 sequences. The CRF02_AG regional dispersal differed across Spain (p = 0.003), as suggested by monophyletic clustering. For the largest monophyletic cluster (subepidemic) (N = 79), 49.4% of the clustered sequences originated from Madrid, while most sequences (51.9%) had been obtained from men having sex with men (MSM). Molecular clock analysis suggested that the origin (tMRCA) of the CRF02_AG subepidemic was in 2002 (median estimate; 95% Highest Posterior Density-HPD interval: 1999–2004). Additionally, we found significant clustering within the CRF02_AG subepidemic according to the ethnic origin.Conclusion: CRF02_AG has been introduced as a result of multiple introductions in Spain, following regional dispersal in several cases. We showed that CRF02_AG transmissions were mostly due to regional dispersal in Spain. The hot-spot for the largest CRF02_AG regional subepidemic in Spain was in Madrid associated with MSM transmission risk group. The existence of subepidemics suggest that several spillovers occurred from Madrid to other areas. CRF02_AG sequences from Hispanics were clustered in a separate subclade suggesting no linkage between the local and Hispanic subepidemics

    Impacto Psicosocial de la Pandemia COVID-19 en el Desgaste Psíquico de Trabajadores Mexicanos

    Objective: Identifying the psychosocial impact of the COVID-19 pandemic on the psychic exhaustion of Mexican workers (March–October 2020). Material and methods: Observational, cross sectional and analytical study.  1 050 Mexican workers from ten states filled an online battery.  A questionnaire was designed to identify the psychosocial variables (labour, social, family, and individual) and health conditions under COVID-19.  The psychic exhaustion dimension of the CTCPS-MAC questionnaire was applied.  Data was analyzed using the Ward method of logistical regression with the SPSS v.26. Results: 17% of cases infected by COVID-19, from 1% to 3% presented comorbidity with hypertension, diabetes, obesity, or heart disease. The risk of cognitive-emotional response was 2% for infected, for the others 14% and 7% in behavioral response. 4% and 27% presented a risk of psychic exhaustion, respectively. Conclusions: Safety dimensions: occupational health; social: housing; family and organization; expression of negative emotions and individual (physical and mental health) were predictors of psychic exhaustion.Objetivo. Identificar el impacto psicosocial de la pandemia COVID-19 en el desgaste psíquico de trabajadores mexicanos (marzo-octubre 2020). Material y métodos. Estudio observacional, transversal y analítico. Participaron, mediante una batería en línea, 1050 trabajadores mexicanos de diez estados. Se diseñó un cuestionario para identificar las variables psicosociales (laboral, social, familiar e individual) y condiciones de salud ante la COVID-19. Se aplicó la dimensión de desgaste psíquico del cuestionario CTCPS-MAC. Los datos se analizaron mediante el método Ward de regresión logística. Resultados. 17% de casos contagiados por COVID-19, del 1 al 3% presentó comorbilidad con hipertensión, diabetes, obesidad o cardiopatías. El riesgo de respuesta cognitivo-emocional fue del 2% para contagiados, para los demás el 14% y 7% en respuesta conductual. Presentó riesgo por desgaste psíquico el 4% y 27% respectivamente. Conclusiones. Dimensiones de seguridad: salud laboral; social: vivienda; familia y organización; expresión de emociones negativas y las individuales (salud física y mental) fueron predictoras del desgaste psíquico

    Differential gene expression patterns and physiological responses improve adaptation to high salinity concentration in pepper accessions

    [EN] High salinity decreases the productivity of crops worldwide. Pepper is particularly sensitive to high salt concentrations. Herein, we subjected three tolerant pepper accessions (C12, B14 and A25) to high sodium chloride concentration (70 mM NaCl). The aerial and root biomass, leaf and root osmotic potential (¿¿), Na+, Cl , K+ and proline concentrations and the relative expression of the putative genes CaSOS1, CaHKT1, three CaNHXs and CaP5CS were measured. Different salinity tolerance strategies depending on the pepper accession were identified. In C12, tolerance was attributed to the accumulation of Na+ in vacuoles and endosomes by the activation of vacuolar CaNHXs genes and the reduction in ¿¿; additionally, the activation of CaHKT1 and CaSOS1 in leaves and roots moved and accumulated Na+ ions in the xylem and xylem parenchyma cells (XPC) as well as expulsed it out of the root cells. A25 accession, on the contrary, was specialized in compartmentalizing Na+ ions in root and leaf vacuoles and root XPC by the up-regulation of CaNHXs and CaHKT1, respectively, avoiding a toxic accumulation in leaves. Finally, B14 accession moved and accumulated Na+ in xylem and XPC, reducing its concentration in roots by the activation of CaSOS1 and CaHKT1.
