    Optical coherence tomography in patients with chronic migraine: Literature review and update

    Migraine is a chronic disease characterized by unilateral, pulsating, and often moderate-to-severe recurrent episodes of headache with nausea and vomiting. It affects approximately 15% of the general population, yet the underlying pathophysiological mechanisms are not fully understood. Optical coherence tomography (OCT) is a safe and reproducible diagnostic technique that utilizes infrared wavelengths and has a sensitivity of 8-10 µm. It can be used to measure thinning of the retinal nerve fiber layer (RNFL) in some neurological disorders. Although ophthalmologists are often the first specialists to examine patients with migraine, few studies have addressed the involvement of the optic nerve and retino-choroidal structures in this group. We reviewed the literature on the etiological and pathological mechanisms of migraine and the relationship between recurrent constriction of cerebral and retrobulbar vessels and ischemic damage to the optic nerve, retina, and choroid. We also assessed the role of OCT for measuring peripapillary RNFL thickness and macular and choroidal changes in migraine patients. There is considerable evidence of cerebral and retrobulbar vascular involvement in the etiology of migraine. Transitory and recurrent constriction of the retinal and ciliary arteries may cause ischemic damage to the optic nerve, retina, and choroid in patients with migraine. OCT to assess the thickness of the peripapillary RNFL, macula, and choroid might increase our understanding of the pathophysiology of migraine and facilitate diagnosis of retino-choroidal compromise and follow-up of therapy in migraine patients. Future studies should determine the usefulness of OCT findings as a biomarker of migraine

    Aplicación de nuevas técnicas de investigación biomecánica en cirugía de los tendones flexores

    Con el objeto de investigar la tensión intratendinosa, la distribución de la carga y la orientación de las líneas de fuerza, a nivel de las suturas tendinosas, se proponen 2 métodos: fotoelasticidad y elementos finitos, basados en técnicas utilizadas en ingeniería mecánica. Obtenida previamente una probeta de resina epoxi, de comportamiento similar al tendón y propiedades fotoelásticas adecuadas para el estudio, se ha mecanizado según el método de sutura y sometido a tracción continua en el banco de carga de un polariscopio, constituyendo 3 grupos con 72 estudios cada uno. Todos los datos obtenidos fueron procesados con un programa para estudios,fotoelásticos utilizado por el Departamento de Mecánica y valorados estadísticamente con el Sigma. Posteriormente se ha realizado el mismo estudio mediante el método de elementos finitos utilizando el módulo Geostar del Cosmos/M v 1.61. La carga intratendinosa más alta y su distribución central se obtiene en los métodos de lazo (BUNNELL y KLEINERT). Por el contrario, los métodos en cuadro son menos isquemiantes, con menor carga y distribución periférica de la tensión, siendo el método de Kessler el que mejor orientación axial presenta. Proponemos el método de Kesslev, como técnica de elección biomecánicamente, en cirugía de los tendones flexores.Peer Reviewe

    Clinical factors associated with discontinuation of ts/bDMARDs in rheumatic patients from the BIOBADASER III registry

Persistence of therapy provides an index of a drug's overall effectiveness. The objective of the study was to identify factors associated with discontinuation of ts/bDMARDs in a real-world dataset. The study population comprised patients diagnosed with RA, PsA, and AS included in the BIOBADASER registry for whom follow-up data were available until November 2019. Patient features and treatment data were included in the analysis. The Kaplan-Meier method was used to study survival of the different drugs according to the reason for discontinuation. Factors associated with discontinuation were studied using Cox regression models and bivariate and multivariate analyses. P values of less than 0.05 were regarded as statistically significant. The study population comprised 4,752 patients who received a total of 8,377 drugs, of which 4,411 (52.65%) were discontinued. The Kaplan-Meier curves showed that survival for first-line treatment was greater in all 3 groups (p < 0.001). Patients with RA had a greater risk of discontinuation if they were younger (HR, 0.99; 95% CI 0.99-1.00), if they were receiving anti-TNFα agents (HR, 0.61; 95% CI 0.54-0.70), and if they had more comorbid conditions (HR, 1.09; 95% CI 1.00-1.17). Patients with PsA had a higher risk if they were women (HR, 1.36; 95% CI 1.15-1.62) and if they were receiving other ts/bDMARDs (HR, 1.29; 95% CI 1.05-1.59). In patients with AS, risk increased with age (HR, 1.01; 95% CI 1.00-1.02), as did the number of comorbid conditions (HR, 1.27; 95% CI 1.12-1.45). The factors that most affected discontinuation of ts/bDMARDs were line of treatment, age, type of drug, sex, comorbidity and the year of initiation of treatment. The association with these factors differed with each disease, except for first-line treatment, which was associated with a lower risk of discontinuation in all 3 diseases

    Arquitectura pública de Santa Criz de Eslava (Navarra, conventus Caesaraugustanus) en época altoimperial: el criptopórtico del foro y su almacén anexo

    A partir de un castrum de la Edad del Hierro surgió en Santa Criz de Eslava una civitas en territorio vascón, cuya base económica fue la agricultura. La ampliación de la muralla en época tardorrepublicana explica el papel territorial que alcanzó en el valle medio del Ebro y la buena situación respecto a la red de comunicaciones terrestres. Alcanzó 13 ha de extensión. Integrada en el conventus Caesaraugustanus, en época augustea inició importantes trabajos de urbanización a la manera romana con la construcción de un conjunto público de carácter monumental. Las excavaciones recientes (2007-2016) han puesto al descubierto el criptopórtico, que sostenía el porticado sur de la plaza forense, y un almacén contiguo. El estudio completo de su estructura arquitec-tónica y decoración permite ahora el análisis de su interpretación funcional, el establecimiento de la fase constructiva inicial y reformas y una propuesta de restitución de su espacio público.With origin as an Iron Age II settlement, Santa Criz de Eslava was a Roman civitas in the Vascones’ territory with an economy based in agriculture. The ampliation of the city walls during the Late Republic shows the territo-rial importance that the city reached in the center of the Ebro Valley and its well-connected position concerning the road-system. During that Roman period the city covered 13 Has. Integrated in the conventus Caesaraugustanus, in the Augustan times the city began to undertake important public works leading to its urban development. Recent archaeological excavations (2007-2016) has discovered the cryptoporticus, which sustained the porticoes of the south side of the forum, and its adjacent depot. A complete study of the architectonic structure and its decoration allow us now to the whole anal-ysis of both its function and its final aspect, the fixing of its early stage, its reforms and a proposal of reconstruction to the whole public area

    GWAS for Systemic Sclerosis Identifies Multiple Risk Loci and Highlights Fibrotic and Vasculopathy Pathways

    Systemic sclerosis (SSc) is an autoimmune disease that shows one of the highest mortality rates among rheumatic diseases. We perform a large genome-wide association study (GWAS), and meta-analysis with previous GWASs, in 26,679 individuals and identify 27 independent genome-wide associated signals, including 13 new risk loci. The novel associations nearly double the number of genome-wide hits reported for SSc thus far. We define 95% credible sets of less than 5 likely causal variants in 12 loci. Additionally, we identify specific SSc subtype-associated signals. Functional analysis of high-priority variants shows the potential function of SSc signals, with the identification of 43 robust target genes through HiChIP. Our results point towards molecular pathways potentially involved in vasculopathy and fibrosis, two main hallmarks in SSc, and highlight the spectrum of critical cell types for the disease. This work supports a better understanding of the genetic basis of SSc and provides directions for future functional experiments.Funding: This work was supported by Spanish Ministry of Economy and Competitiveness (grant ref.     Cut-offs and response criteria for the Hospital Universitario la Princesa Index (HUPI) and their comparison to widely-used indices of disease activity in rheumatoid arthritis

    Objective To estimate cut-off points and to establish response criteria for the Hospital Universitario La Princesa Index (HUPI) in patients with chronic polyarthritis. Methods Two cohorts, one of early arthritis (Princesa Early Arthritis Register Longitudinal PEARL] study) and other of long-term rheumatoid arthritis (Estudio de la Morbilidad y Expresión Clínica de la Artritis Reumatoide EMECAR]) including altogether 1200 patients were used to determine cut-off values for remission, and for low, moderate and high activity through receiver operating curve (ROC) analysis. The areas under ROC (AUC) were compared to those of validated indexes (SDAI, CDAI, DAS28). ROC analysis was also applied to establish minimal and relevant clinical improvement for HUPI. Results The best cut-off points for HUPI are 2, 5 and 9, classifying RA activity as remission if =2, low disease activity if >2 and =5), moderate if >5 and <9 and high if =9. HUPI''s AUC to discriminate between low-moderate activity was 0.909 and between moderate-high activity 0.887. DAS28''s AUCs were 0.887 and 0.846, respectively; both indices had higher accuracy than SDAI (AUCs: 0.832 and 0.756) and CDAI (AUCs: 0.789 and 0.728). HUPI discriminates remission better than DAS28-ESR in early arthritis, but similarly to SDAI. The HUPI cut-off for minimal clinical improvement was established at 2 and for relevant clinical improvement at 4. Response criteria were established based on these cut-off values. Conclusions The cut-offs proposed for HUPI perform adequately in patients with either early or long term arthritis

    Database of spatial distribution of non indigenous species in Spanish marine waters

    Research in marine Spanish waters are focused on several actions to achieve an effectively management on protected areas, with the active participation of the stakeholders and research as basic tools for decision-making. Among these actions, there is one about the knowledge and control on NIS. One of its objectives is the creation of NIS factsheets, which are going to be added to the National Marine Biodiversity Geographical System (GIS) providing complementary information about taxonomic classification, common names, taxonomic synonyms, species illustrations, identification morphological characters, habitat in the native and introduced regions, biological and ecological traits, GenBank DNA sequences, world distribution, first record and evolution in the introduced areas, likely pathways of introduction, effects in the habitats and interaction with native species, and potential management measures to apply. The database will also provide data for (1) the European online platforms, (2) the environmental assessment for the Descriptor 2 (D2-NIS) of the EU Marine Strategy Framework Directive (MSFD), as well as (3) supporting decisions made by stakeholders. It is the result of extensive collaboration among scientist, manager’s and citizen science in the Spanish North-Atlantic, South-Atlantic, Gibraltar Strait-Alboran, Levantine-Balearic and Canary Islands marine divisions, providing an updated overview of the spatial distribution of relevant extended and invasive NIS of recent and established NIS introduced by maritime transport and aquaculture pathways, as well as on cryptogenic or native species in expansion due to the climatic water warming trend
