The effect of l-thymidine, acyclic thymine and 8-bromoguanine on the stability of model G-quadruplex structures
Background Guanine-rich oligonucleotides are capable of forming tetrahelical structures known as G-quadruplexes with interesting biological properties. We have investigated the effects of site-specific substitution in the loops and in the tetrads model G-quadruplexes using thymine glycol nucleic acid (GNA) units, l-thymidine and 8-Br-2′-deoxyguanosine. Methods Modified oligonucleotides were chemically synthesized and spectroscopic techniques were used to determine the relative stability of the modified G-quadruplex. The double 8-BrdG-modified quadruplexes were further characterized by Nuclear Magnetic Resonance. Binding to thrombin of selected quadruplex was analyzed by gel electrophoresis retention assay. Results The most interesting results were found with a 8-bromoG substitution that had the larger stabilization of the quadruplex. NMR studies indicate a tight relationship between the loops and the tetrads to accommodate 8-bromoG modifications within the TBA. Conclusions The substitutions of loop positions with GNA T affect the TBA stability except for single modification in T7 position. Single l-thymidine substitutions produced destabilization of TBA. Larger changes on quadruplex stability are observed with the use of 8-bromoG finding a single substitution with the highest thermal stabilization found in thrombin binding aptamers modified at the guanine residues and having good affinity for thrombin. Double 8-BrdG modification in anti positions of different tetrads produce a conformational flip from syn to anti conformation of 8-Br-dG to favor loop-tetrad interaction and preserve the overall TBA stability
MANEJO DE LOS ELECTRODOS DE AGUJAS EN EL LABORATORIO DE EMG. EXPERIENCIA PRÁCTICA DE ENFERMERÍA
In practice, nursing plays an important role in the management of needle electrodes in the electromyography (EMG) lab, particularly in the sterilization and storage. Special attention should be taken with the manipulation of the electrodes, washing hands and using materials in good condition. All these aspects reverberate in the service quality and improve the patient’s care. The main aim of this report is to explain the processes of the use of electrode needles on patients, as well as to describe the precautions, regulations, and care that should be taken with electrodes needles to obtain the best results in the EMG lab. It is important for nurses as well as medical and technical personnel, before using one of the electrodes and its accessories, to wash their hands. To complete actions that avoid mistakes because of the incorrect use of the needle electrodes. Compliance with biosecurity measures to avoid the contraction of diseases, as well as knowing how to adequately operate electrodes needles and the execution of any electromiographic technique, and also bearing in mind the disinfection and sterilization of electromiographic needles in compliance with established procedures should be carried out. The realization of these procedures for the operation of electrode needles, ensures the quality of service and above all prevents that the patient suffers health complications due to a bad procedure.En la práctica, el personal de enfermería participa en el cuidado de los electrodos de agujas para los estudios electrofisiológicos, en especial de su esterilización y almacenamiento adecuado. Se debe de respetar la atención especial a la manipulación de los electrodos, la realización del lavado de manos, y al uso del material en buen estado. Estas prácticas repercuten positivamente en la calidad del servicio y benefician directamente al paciente. El objetivo de nuestro trabajo es dar a conocer el proceder del uso de los electrodos de agujas en los pacientes, así como exponer las precauciones, normas y cuidados que se deben tener con los electrodos de agujas para un mejor trabajo en el laboratorio de electromiografía. Es importante para el enfermero que, antes de utilizar alguno de los electrodos y sus accesorios, realice el lavado de las manos, así como también el personal médico y técnico que opera con estos instrumentos. Realizar acciones que eviten errores que conllevan a un uso inadecuado de los electrodos de aguja. Cumplir con las medidas de bioseguridad para evitar el contagio de enfermedades, conocer además el manejo adecuado de los electrodos de agujas para un mejor uso de ellos y la ejecución de cualquier técnica electromiográfica, y también tener en cuenta la desinfección y esterilización de agujas de electromiografía cumpliendo con el procedimiento establecido. El cumplimiento de los procederes para el manejo de los electrodos de aguja, garantiza la calidad del servicio y principalmente evita que el paciente sufra complicaciones de salud por la realización de un mal procedimiento
Study of a nonlinear Kirchhoff equation with non-homogeneous material
In this paper we study a non-homogeneous elliptic Kirchhoff equation with nonlinear reaction term. We analyze the existence and uniqueness of positive solution. The main novelty is the inclusion of non-homogeneous term making the problem without a variational structure. Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)
This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes. Unambiguous observation of blocked states reveals altered, blocker-induced, cardiac ryanodine receptor gating
The flow of ions through membrane channels is precisely regulated by gates. The architecture and function of these elements have been studied extensively, shedding light on the mechanisms underlying gating. Recent investigations have focused on ion occupancy of the channel’s selectivity filter and its ability to alter gating, with most studies involving prokaryotic K+ channels. Some studies used large quaternary ammonium blocker molecules to examine the effects of altered ionic flux on gating. However, the absence of blocking events that are visibly distinct from closing events in K+ channels makes unambiguous interpretation of data from single channel recordings difficult. In this study, the large K+ conductance of the RyR2 channel permits direct observation of blocking events as distinct subconductance states and for the first time demonstrates the differential effects of blocker molecules on channel gating. This experimental platform provides valuable insights into mechanisms of blocker-induced modulation of ion channel gating

los
estudiosos
interesados
y
a
la
sociedad
en
general
a
presentar
trabajos
para
analizar,
debatir
y
proponer
estrategias
de
acción
y
dirección,
que
fortalezcan
una
convivencia y bienestar con sentido humanista para una cultura de paz.
El
presente
texto
es
producto
de
esta convocatoria
que
recoge
los
trabajos
de
los
interesados
en
la
temática,
de
diferentes
países
(España,
Argentina,
Cuba,
Brasil,
Costa
Rica
y
México)
retomando
con
ello
sus
experiencias
relativas
al
estudio,
análisis,
comprensión
e
instrumentación
de
la
cultura
de
paz
en
los
distintos
ámbitos
institucionales
en
los
que
participan:
educativo,
salud,
penitenciario,
social,
laboral,
familia,
alimentario,
psicológico,
por
mencionar
algunos.
El
presente
libro,
propicia
un
espacio
de
reflexión,
diálogo
y
posicionamiento
de
las
ciencias
de
la
conducta
para
la
apropiación,
análisis,
debate
y
propuestas
que
fortalezcan
una
cultura
de
paz
a
través
de
la
convivencia
y
el
bienestar
social
con
sentido
humanista.
El
sistema
económico
neoliberal
y
el
proceso
de
globalización
han
contribuido
al
logro
de
avances
significativos
en
la
ciencia
y
la
tecnología,
pero
también
han
propiciado
la
polarización
de
las
sociedades
lo
que
ha
impactado
de
manera
negativa
a
la
sociedad
en
su
conjunto,
pero
en
mayor
medida
a
los grupos
vulnerables. Dicha
polarización
ha
traído
consigo
un
desarrollo
desigual
del
mundo
que
se
expresa
de
diferentes
maneras
tanto
en
países
desarrollados
como
en
los
llamados
del
tercer
mundo,
en
donde
no
están
satisfechas
las
necesidades
humanas
elementales
de
todos
los
sectores
de
la
población,
siempre
falta
algo.
Si
a
esto
le
sumamos
los
conflictos
internacionales por
diferentes
motivos
que
enfrentan
algunas
naciones,
una
insuficiente
cobertura
educativa
y
de
salud,
desempleo
y
pobreza
extrema,
entre
otras
cosas;
estamos
frente
a
retos
de
gran
envergadura
para
los
gobiernos,
para
los
estudiosos
y
para
la
sociedad
civil
en
general. Uno
de
los
intentos
para
frenar
y prevenir
la
agudización
de
estas
problemáticas
es
la
cultura
de
paz,
cuyo
estudio
y propuestas
han
ido
avanzando
en
diferentes
sentidos
y
de
manera
favorable,
el
tema
está
presente
en
diferentes
Organismos
Internacionales
como
la
ONU,
la
UNESCO,
la
OCDE,
El
Banco
Mundial,
entre
otros.
Pero
falta
mucho
por
