Swift and Fermi observations of X-ray flares: the case of Late Internal Shock
Simultaneous Swift and Fermi observations of gamma-ray bursts (GRBs) offer a
unique broadband view of their afterglow emission, spanning more than ten
decades in energy. We present the sample of X-ray flares observed by both Swift
and Fermi during the first three years of Fermi operations. While bright in the
X-ray band, X-ray flares are often undetected at lower (optical), and higher
(MeV to GeV) energies. We show that this disfavors synchrotron self-Compton
processes as origin of the observed X-ray emission. We compare the broadband
properties of X-ray flares with the standard late internal shock model, and
find that, in this scenario, X-ray flares can be produced by a late-time
relativistic (Gamma>50) outflow at radii R~10^13-10^14 cm. This conclusion
holds only if the variability timescale is significantly shorter than the
observed flare duration, and implies that X-ray flares can directly probe the

Experience of loneliness associated with being an informal caregiver: A qualitative investigation.

encounter difficulties which may negatively affect their quality of life as well as their mental and physical health. Loneliness is one
important challenge that caregivers face, with this psychological state being associated with morbidity and premature mortality.
Although previous research has identified loneliness as an issue associated with being an informal caregiver, there is a paucity of
evidence that attempts to understand this phenomenon in depth. This study aimed to examine informal caregivers' reflections on,
and accounts of, experiences of loneliness linked to their caregiving situation. A cross-sectional, qualitative study was designed.
Sixteen semi-structured interviews were conducted with 8 spousal caregivers, 4 daughters caring for a parent, 3 mothers caring
for a child (or children) and 1 woman looking after her partner. The cared-for persons were suffering from a range of mental and
physical health conditions (e.g. dementia, frailty due to old age, multiple sclerosis, depression, autism). Data were analyzed using
an inductive thematic analysis. Experiences of loneliness were described by reference to a context of shrunken personal space and
diminished social interaction caused by the restrictions imposed by the caregiving role. Loneliness was also articulated against a
background of relational deprivations and losses as well as sentiments of powerlessness, helplessness and a sense of sole
responsibility. Social encounters were also seen to generate loneliness when they were characterized by some form of distancing.
Though not all sources or circumstances of loneliness in caregivers are amenable to change, more opportunities for respite care
services, as well as a heightened sensibility and social appreciation of caregivers' valued contributions could help caregivers

How countries plan to address agricultural adaptation and mitigation
Agriculture is well represented in Parties’ adaptation and mitigation strategies as communicated in their Intended Nationally Determined Contributions (INDCs) to the United Nations Framework Convention on Climate Change (UNFCCC). There is much attention to conventional agricultural practices that can be climate-smart (e.g. livestock and crop management), but less to the enabling services that can facilitate uptake (e.g. climate information services, insurance, and credit). Considerable finance is needed for agricultural adaptation and mitigation by Least Developed Countries (LDCs) – in the order of USD 3 billion annually for adaptation and 2 billion annually for mitigation. Parties need better information in order to refine their finance needs. Non-Annex 1 Parties raise issues of climate justice, social inequality and food security in their INDCs
The Study of TeV Variability and Duty Cycle of Mrk 421 from 3 Years of Observations with the Milagro Observatory
TeV flaring activity with time scales as short as tens of minutes and an
orphan TeV flare have been observed from the blazar Markarian 421 (Mrk 421).
The TeV emission from Mrk 421 is believed to be produced by leptonic
synchrotron self-Compton (SSC) emission. In this scenario, correlations between
the X-ray and the TeV fluxes are expected, TeV orphan flares are hardly
explained and the activity (measured as duty cycle) of the source at TeV
energies is expected to be equal or less than that observed in X-rays if only
SSC is considered. To estimate the TeV duty cycle of Mrk 421 and to establish
limits on its variability at different time scales, we continuously observed
Mrk 421 with the Milagro observatory. Mrk 421 was detected by Milagro with a
statistical significance of 7.1 standard deviations between 2005 September 21
and 2008 March 15. The observed spectrum is consistent with previous
observations by VERITAS. We estimate the duty cycle of Mrk 421 for energies
above 1 TeV for different hypothesis of the baseline flux and for different
flare selections and we compare our results with the X-ray duty cycle estimated

Discovery of Localized Regions of Excess 10-TeV Cosmic Rays
An analysis of 7 years of Milagro data performed on a 10-degree angular scale
has found two localized regions of excess of unknown origin with greater than
12 sigma significance. Both regions are inconsistent with gamma-ray emission
with high confidence. One of the regions has a different energy spectrum than
the isotropic cosmic-ray flux at a level of 4.6 sigma, and it is consistent
with hard spectrum protons with an exponential cutoff, with the most
significant excess at ~10 TeV. Potential causes of these excesses are explored,

Understanding innovators' experiences of barriers and facilitators in implementation and diffusion of healthcare service innovations: A qualitative study
This article is made available through the Brunel Open Access Publishing Fund - Copyright @ 2011 Barnett et al.Background: Healthcare service innovations are considered to play a pivotal role in improving organisational efficiency and responding effectively to healthcare needs. Nevertheless, healthcare organisations encounter major difficulties in sustaining and diffusing innovations, especially those which concern the organisation and delivery of healthcare services. The purpose of the present study was to explore how healthcare innovators of process-based initiatives perceived and made sense of factors that either facilitated or obstructed the innovation implementation and diffusion. Methods: A qualitative study was designed. Fifteen primary and secondary healthcare organisations in the UK, which had received health service awards for successfully generating and implementing service innovations, were studied. In-depth, semi structured interviews were conducted with the organisational representatives who conceived and led the development process. The data were recorded, transcribed and thematically analysed. Results: Four main themes were identified in the analysis of the data: the role of evidence, the function of inter-organisational partnerships, the influence of human-based resources, and the impact of contextual factors. "Hard" evidence operated as a proof of effectiveness, a means of dissemination and a pre-requisite for the initiation of innovation. Inter-organisational partnerships and people-based resources, such as champions, were considered an integral part of the process of developing, establishing and diffusing the innovations. Finally, contextual influences, both intra-organisational and extra-organisational were seen as critical in either impeding or facilitating innovators' efforts. Conclusions: A range of factors of different combinations and co-occurrence were pointed out by the innovators as they were reflecting on their experiences of implementing, stabilising and diffusing novel service initiatives. Even though the innovations studied were of various contents and originated from diverse organisational contexts, innovators' accounts converged to the significant role of the evidential base of success, the inter-personal and inter-organisational networks, and the inner and outer context. The innovators, operating themselves as important champions and being often willing to lead constructive efforts of implementation to different contexts, can contribute to the promulgation and spread of the novelties significantly.This research was supported financially by the Multidisciplinary Assessment of

Fermi-LAT Study of Gamma-ray Emission in the Direction of Supernova Remnant W49B
We present an analysis of the gamma-ray data obtained with the Large Area
Telescope (LAT) onboard the Fermi Gamma-ray Space Telescope in the direction of
SNR W49B (G43.3-0.2). A bright unresolved gamma-ray source detected at a
significance of 38 sigma is found to coincide with SNR W49B. The energy
spectrum in the 0.2-200 GeV range gradually steepens toward high energies. The
luminosity is estimated to be 1.5x10^{36} (D/8 kpc)^2 erg s^-1 in this energy
range. There is no indication that the gamma-ray emission comes from a pulsar.
Assuming that the SNR shell is the site of gamma-ray production, the observed
spectrum can be explained either by the decay of neutral pi mesons produced
through the proton-proton collisions or by electron bremsstrahlung. The
calculated energy density of relativistic particles responsible for the LAT
flux is estimated to be remarkably large, U_{e,p}>10^4 eV cm^-3, for either
gamma-ray production mechanism.Comment: 9 pages, 10 figure
Detection of 16 Gamma-Ray Pulsars Through Blind Frequency Searches Using the Fermi LAT
Pulsars are rapidly-rotating, highly-magnetized neutron stars emitting
radiation across the electromagnetic spectrum. Although there are more than
1800 known radio pulsars, until recently, only seven were observed to pulse in
gamma rays and these were all discovered at other wavelengths. The Fermi Large
Area Telescope makes it possible to pinpoint neutron stars through their
gamma-ray pulsations. We report the detection of 16 gamma-ray pulsars in blind
frequency searches using the LAT. Most of these pulsars are coincident with
previously unidentified gamma-ray sources, and many are associated with
supernova remnants. Direct detection of gamma-ray pulsars enables studies of
emission mechanisms, population statistics and the energetics of pulsar wind

GeV Gamma-ray Flux Upper Limits from Clusters of Galaxies
The detection of diffuse radio emission associated with clusters of galaxies
indicates populations of relativistic leptons infusing the intracluster medium.
Those electrons and positrons are either injected into and accelerated directly
in the intracluster medium, or produced as secondary pairs by cosmic-ray ions
scattering on ambient protons. Radiation mechanisms involving the energetic
leptons together with decay of neutral pions produced by hadronic interactions
have the potential to produce abundant GeV photons. Here, we report on the
search for GeV emission from clusters of galaxies using data collected by the
Large Area Telescope (LAT) on the Fermi Gamma-ray Space Telescope (Fermi) from
August 2008 to February 2010. Thirty-three galaxy clusters have been selected
according to their proximity and high mass, X-ray flux and temperature, and
indications of non-thermal activity for this study. We report upper limits on
the photon flux in the range 0.2-100 GeV towards a sample of observed clusters
(typical values 1-5 x 10^-9 ph cm^-2 s^-1) considering both point-like and
spatially resolved models for the high-energy emission, and discuss how these
results constrain the characteristics of energetic leptons and hadrons, and
magnetic fields in the intracluster medium. The volume-averaged
relativistic-hadron-to-thermal energy density ratio is found to be < 5-10% in
