    Analysis and characterization of slab-coupled optical waveguide amplifiers and lasers

However, these devices traditionally suffer from low fiber coupling efficiency and have been limited to relatively low power applications. This thesis explores the potential of the slab-coupled optical waveguide (SCOW) semiconductor gain medium for use in high power optical amplifiers and external cavity lasers. The thesis begins by introducing the SCOW concept and describing the benefits of utilizing a low optical confinement design for high power operation. Detailed analysis and measurements of the output power, gain, and noise properties of slab-coupled optical waveguide amplifiers (SCOWAs) and slab-coupled optical waveguide external cavity lasers (SCOWECLs) are also presented. It will be shown that these devices not only exhibit Watt class output power with high coupling efficiency (> 90 %) but also demonstrate the capability for low noise operation.by William Loh.S.M

    Catheter-based intervention for symptomatic patient with severe mitral regurgitation and very poor left ventricular systolic function - Safe but no room for complacency

    Many patients with left ventricular systolic dysfunction have concomitant mitral regurgitation (MR). Their symptoms and prognosis worsen with increasing severity of MR. Percutaneous MitraClip((R)) can be used safely to reduce the severity of MR even in patients with advanced heart failure and is associated with improved symptoms, quality of life and exercise tolerance. However, a few patients with very poor left ventricular systolic function may experience significant haemodynamic disturbance in the peri-procedural period. We present three such patients, highlighting some of the potential problems encountered and discuss their possible pathophysiological mechanisms and safety measures.published_or_final_versio

    High platelet reactivity in patients with acute coronary syndromes undergoing percutaneous coronary intervention: Randomised controlled trial comparing prasugrel and clopidogrel

    Background: Prasugrel is more effective than clopidogrel in reducing platelet aggregation in acute coronary syndromes. Data available on prasugrel reloading in clopidogrel treated patients with high residual platelet reactivity (HRPR) i.e. poor responders, is limited. Objectives: To determine the effects of prasugrel loading on platelet function in patients on clopidogrel and high platelet reactivity undergoing percutaneous coronary intervention for acute coronary syndrome (ACS). Patients: Patients with ACS on clopidogrel who were scheduled for PCI found to have a platelet reactivity ≥40 AUC with the Multiplate Analyzer, i.e. “poor responders” were randomised to prasugrel (60 mg loading and 10 mg maintenance dose) or clopidogrel (600 mg reloading and 150 mg maintenance dose). The primary outcome measure was proportion of patients with platelet reactivity <40 AUC 4 hours after loading with study medication, and also at one hour (secondary outcome). 44 patients were enrolled and the study was terminated early as clopidogrel use decreased sharply due to introduction of newer P2Y12 inhibitors. Results: At 4 hours after study medication 100% of patients treated with prasugrel compared to 91% of those treated with clopidogrel had platelet reactivity <40 AUC (p = 0.49), while at 1 hour the proportions were 95% and 64% respectively (p = 0.02). Mean platelet reactivity at 4 and 1 hours after study medication in prasugrel and clopidogrel groups respectively were 12 versus 22 (p = 0.005) and 19 versus 34 (p = 0.01) respectively. Conclusions: Routine platelet function testing identifies patients with high residual platelet reactivity (“poor responders”) on clopidogrel. A strategy of prasugrel rather than clopidogrel reloading results in earlier and more sustained suppression of platelet reactivity. Future trials need to identify if this translates into clinical benefit

    Enhanced external counter pulsation in treatment of refractory angina pectoris: two year outcome and baseline factors associated with treatment failure

    <p>Abstract</p> <p>Background</p> <p>Enhanced external counter pulsation (EECP) is a non-invasive treatment option for patients with refractory angina pectoris ineligible to further traditional treatment. The aim of this study was to evaluate the effect of EECP on patients at a Scandinavian medical centre and to investigate if outcome can be predicted by analysing baseline factors.</p> <p>Methods</p> <p>86 consecutive patients (70 male, 16 female) were treated with EECP and followed for two years post treatment. Canadian cardiovascular society (CCS) class was analysed, and medication and adverse clinical events were researched prior to EECP, at the end of the treatment, and at six, 12 and 24 months thereafter. Patients responding to therapy by improving at least one CCS class were compared with those who failed to respond. Any differences in background factors were recorded and analysed.</p> <p>Results</p> <p>79% of the patients responded to therapy by improving at least one CCS class. In general, the CCS class improved by one class after EECP treatment (3.05 before versus 2.14 after treatment). A total of 61.5% of the initial responders showed sustained improvement at the 12 month follow-up while 29% presented sustained improvement after 24 months. Treatment was most effective among patients suffering from CCS class III-IV angina pectoris, while patients suffering from CCS class II angina pectoris improved transiently but failed to show sustained improvement after the 12 month follow-up. Diabetes mellitus and calcium channel antagonists were more common among the non-responders (<it>p </it>< 0.05).</p> <p>Conclusion</p> <p>This study confirms the safety and efficiency of EECP as a treatment option for patients suffering from refractory angina pectoris. The therapy is most beneficial in patients suffering from severe angina (CCS III-IV) while sustained response to therapy could not be verified among patients suffering from CCS class II angina pectoris.</p

    Trends and predictions of metabolic risk factors for acute myocardial infarction: findings from a multiethnic nationwide cohort

    BACKGROUND: Understanding the trajectories of metabolic risk factors for acute myocardial infarction (AMI) is necessary for healthcare policymaking. We estimated future projections of the incidence of metabolic diseases in a multi-ethnic population with AMI. METHODS: The incidence and mortality contributed by metabolic risk factors in the population with AMI (diabetes mellitus [T2DM], hypertension, hyperlipidemia, overweight/obesity, active/previous smokers) were projected up to year 2050, using linear and Poisson regression models based on the Singapore Myocardial Infarction Registry from 2007 to 2018. Forecast analysis was stratified based on age, sex and ethnicity. FINDINGS: From 2025 to 2050, the incidence of AMI is predicted to rise by 194.4% from 482 to 1418 per 100,000 population. The largest percentage increase in metabolic risk factors within the population with AMI is projected to be overweight/obesity (880.0% increase), followed by hypertension (248.7% increase), T2DM (215.7% increase), hyperlipidemia (205.0% increase), and active/previous smoking (164.8% increase). The number of AMI-related deaths is expected to increase by 294.7% in individuals with overweight/obesity, while mortality is predicted to decrease by 11.7% in hyperlipidemia, 29.9% in hypertension, 32.7% in T2DM and 49.6% in active/previous smokers, from 2025 to 2050. Compared with Chinese individuals, Indian and Malay individuals bear a disproportionate burden of overweight/obesity incidence and AMI-related mortality. INTERPRETATION: The incidence of AMI is projected to continue rising in the coming decades. Overweight/obesity will emerge as fastest-growing metabolic risk factor and the leading risk factor for AMI-related mortality.     Measurement of the mass and lifetime of the Ωb\Omega_b^- baryon

    A proton-proton collision data sample, corresponding to an integrated luminosity of 3 fb1^{-1} collected by LHCb at s=7\sqrt{s}=7 and 8 TeV, is used to reconstruct 63±963\pm9 ΩbΩc0π\Omega_b^-\to\Omega_c^0\pi^-, Ωc0pKKπ+\Omega_c^0\to pK^-K^-\pi^+ decays. Using the ΞbΞc0π\Xi_b^-\to\Xi_c^0\pi^-, Ξc0pKKπ+\Xi_c^0\to pK^-K^-\pi^+ decay mode for calibration, the lifetime ratio and absolute lifetime of the Ωb\Omega_b^- baryon are measured to be \begin{align*} \frac{\tau_{\Omega_b^-}}{\tau_{\Xi_b^-}} &= 1.11\pm0.16\pm0.03, \\ \tau_{\Omega_b^-} &= 1.78\pm0.26\pm0.05\pm0.06~{\rm ps}, \end{align*} where the uncertainties are statistical, systematic and from the calibration mode (for τΩb\tau_{\Omega_b^-} only). A measurement is also made of the mass difference, mΩbmΞbm_{\Omega_b^-}-m_{\Xi_b^-}, and the corresponding Ωb\Omega_b^- mass, which yields \begin{align*} m_{\Omega_b^-}-m_{\Xi_b^-} &= 247.4\pm3.2\pm0.5~{\rm MeV}/c^2, \\ m_{\Omega_b^-} &= 6045.1\pm3.2\pm 0.5\pm0.6~{\rm MeV}/c^2. \end{align*} These results are consistent with previous measurements.Comment: 11 pages, 5 figures, All figures and tables, along with any supplementary material and additional information, are available at https://lhcbproject.web.cern.ch/lhcbproject/Publications/LHCbProjectPublic/LHCb-PAPER-2016-008.htm

    Study of Bc+B_c^+ decays to the K+Kπ+K^+K^-\pi^+ final state and evidence for the decay Bc+χc0π+B_c^+\to\chi_{c0}\pi^+

    A study of Bc+K+Kπ+B_c^+\to K^+K^-\pi^+ decays is performed for the first time using data corresponding to an integrated luminosity of 3.0 fb1\mathrm{fb}^{-1} collected by the LHCb experiment in pppp collisions at centre-of-mass energies of 77 and 88 TeV. Evidence for the decay Bc+χc0(K+K)π+B_c^+\to\chi_{c0}(\to K^+K^-)\pi^+ is reported with a significance of 4.0 standard deviations, resulting in the measurement of σ(Bc+)σ(B+)×B(Bc+χc0π+)\frac{\sigma(B_c^+)}{\sigma(B^+)}\times\mathcal{B}(B_c^+\to\chi_{c0}\pi^+) to be (9.83.0+3.4(stat)±0.8(syst))×106(9.8^{+3.4}_{-3.0}(\mathrm{stat})\pm 0.8(\mathrm{syst}))\times 10^{-6}. Here B\mathcal{B} denotes a branching fraction while σ(Bc+)\sigma(B_c^+) and σ(B+)\sigma(B^+) are the production cross-sections for Bc+B_c^+ and B+B^+ mesons.     BB flavour tagging using charm decays at the LHCb experiment

    An algorithm is described for tagging the flavour content at production of neutral BB mesons in the LHCb experiment. The algorithm exploits the correlation of the flavour of a BB meson with the charge of a reconstructed secondary charm hadron from the decay of the other bb hadron produced in the proton-proton collision. Charm hadron candidates are identified in a number of fully or partially reconstructed Cabibbo-favoured decay modes. The algorithm is calibrated on the self-tagged decay modes B+J/ψK+B^+ \to J/\psi \, K^+ and B0J/ψK0B^0 \to J/\psi \, K^{*0} using 3.0fb13.0\mathrm{\,fb}^{-1} of data collected by the LHCb experiment at pppp centre-of-mass energies of 7TeV7\mathrm{\,TeV} and 8TeV8\mathrm{\,TeV}.     Model-independent evidence for J/ψpJ/\psi p contributions to Λb0J/ψpK\Lambda_b^0\to J/\psi p K^- decays

    The data sample of Λb0J/ψpK\Lambda_b^0\to J/\psi p K^- decays acquired with the LHCb detector from 7 and 8~TeV pppp collisions, corresponding to an integrated luminosity of 3 fb1^{-1}, is inspected for the presence of J/ψpJ/\psi p or J/ψKJ/\psi K^- contributions with minimal assumptions about KpK^- p contributions. It is demonstrated at more than 9 standard deviations that Λb0J/ψpK\Lambda_b^0\to J/\psi p K^- decays cannot be described with KpK^- p contributions alone, and that J/ψpJ/\psi p contributions play a dominant role in this incompatibility. These model-independent results support the previously obtained model-dependent evidence for Pc+J/ψpP_c^+\to J/\psi p charmonium-pentaquark states in the same data sample.Comment: 21 pages, 12 figures (including the supplemental section added at the end

    Study of WW boson production in association with beauty and charm

    The associated production of a WW boson with a jet originating from either a light parton or heavy-flavor quark is studied in the forward region using proton-proton collisions. The analysis uses data corresponding to integrated luminosities of 1.0 and 2.0fb12.0\,{\rm fb}^{-1} collected with the LHCb detector at center-of-mass energies of 7 and 8 TeV, respectively. The WW bosons are reconstructed using the WμνW\to\mu\nu decay and muons with a transverse momentum, pTp_{\rm T}, larger than 20 GeV in the pseudorapidity range 2.0202.0 20 GeV and 2.2<η<4.22.2 < \eta < 4.2. The sum of the muon and jet momenta must satisfy pT>20p_{\rm T} > 20 GeV. The fraction of W+W+jet events that originate from beauty and charm quarks is measured, along with the charge asymmetries of the W ⁣+ ⁣bW\!+\!b and W ⁣+ ⁣cW\!+\!c production cross-sections. The ratio of the W+W+jet to Z+Z+jet production cross-sections is also measured using the ZμμZ\to\mu\mu decay. All results are in agreement with Standard Model predictions
