New genetic loci link adipose and insulin biology to body fat distribution.
Body fat distribution is a heritable trait and a well-established predictor of adverse metabolic outcomes, independent of overall adiposity. To increase our understanding of the genetic basis of body fat distribution and its molecular links to cardiometabolic traits, here we conduct genome-wide association meta-analyses of traits related to waist and hip circumferences in up to 224,459 individuals. We identify 49 loci (33 new) associated with waist-to-hip ratio adjusted for body mass index (BMI), and an additional 19 loci newly associated with related waist and hip circumference measures (P < 5 × 10(-8)). In total, 20 of the 49 waist-to-hip ratio adjusted for BMI loci show significant sexual dimorphism, 19 of which display a stronger effect in women. The identified loci were enriched for genes expressed in adipose tissue and for putative regulatory elements in adipocytes. Pathway analyses implicated adipogenesis, angiogenesis, transcriptional regulation and insulin resistance as processes affecting fat distribution, providing insight into potential pathophysiological mechanisms

Pitfalls in assessing stromal tumor infiltrating lymphocytes (sTILs) in breast cancer
Abstract: Stromal tumor-infiltrating lymphocytes (sTILs) are important prognostic and predictive biomarkers in triple-negative (TNBC) and HER2-positive breast cancer. Incorporating sTILs into clinical practice necessitates reproducible assessment. Previously developed standardized scoring guidelines have been widely embraced by the clinical and research communities. We evaluated sources of variability in sTIL assessment by pathologists in three previous sTIL ring studies. We identify common challenges and evaluate impact of discrepancies on outcome estimates in early TNBC using a newly-developed prognostic tool. Discordant sTIL assessment is driven by heterogeneity in lymphocyte distribution. Additional factors include: technical slide-related issues; scoring outside the tumor boundary; tumors with minimal assessable stroma; including lymphocytes associated with other structures; and including other inflammatory cells. Small variations in sTIL assessment modestly alter risk estimation in early TNBC but have the potential to affect treatment selection if cutpoints are employed. Scoring and averaging multiple areas, as well as use of reference images, improve consistency of sTIL evaluation. This recognition has come about thanks to standardized visual reporting guidelines, which helped to reduce inter-reader variability. Now, there are ripe opportunities to employ computational methods that extract spatio-morphologic predictive features, enabling computer-aided diagnostics. We detail the benefits of computational TILs assessment, the readiness of TILs scoring for computational assessment, and outline considerations for overcoming key barriers to clinical translation in this arena. Specifically, we discuss: 1. ensuring computational workflows closely capture visual guidelines and standards; 2. challenges and thoughts standards for assessment of algorithms including training, preanalytical, analytical, and clinical validation; 3. perspectives on how to realize the potential of machine learning models and to overcome the perceptual and practical limits of visual scoring

Application of a risk-management framework for integration of stromal tumor-infiltrating lymphocytes in clinical trials
Funder: Breast Cancer Research Foundation (BCRF); doi: https://doi.org/10.13039/100001006Abstract: Stromal tumor-infiltrating lymphocytes (sTILs) are a potential predictive biomarker for immunotherapy response in metastatic triple-negative breast cancer (TNBC). To incorporate sTILs into clinical trials and diagnostics, reliable assessment is essential. In this review, we propose a new concept, namely the implementation of a risk-management framework that enables the use of sTILs as a stratification factor in clinical trials. We present the design of a biomarker risk-mitigation workflow that can be applied to any biomarker incorporation in clinical trials. We demonstrate the implementation of this concept using sTILs as an integral biomarker in a single-center phase II immunotherapy trial for metastatic TNBC (TONIC trial, NCT02499367), using this workflow to mitigate risks of suboptimal inclusion of sTILs in this specific trial. In this review, we demonstrate that a web-based scoring platform can mitigate potential risk factors when including sTILs in clinical trials, and we argue that this framework can be applied for any future biomarker-driven clinical trial setting
Point-of-Care Testing in Pediatric Emergency Care
Les progrès récents de la médecine d'urgence pédiatrique ont mené à l'implémentation au sein des services d'urgence de tests de diagnostic rapide (Point-of-Care Testing, POCT). Ces tests montrent de nombreux avantages: réduction du délai diagnostique et du temps de séjour aux urgences, réduction du nombre de tests additionnels et traitements non nécessaires, réduction du délai jusqu'à la prescription du traitement approprié, réduction du volume sanguin nécessaire, et possibilité pour un personnel non entrainé de réaliser le test facilement. Cependant, plusieurs éléments sont à prendre en considération: le contexte clinique dans lequel le test est effectué afin de pouvoir en interpréter le résultat, son exactitude par rapport à un test de référence, l'assurance du maintien de la qualité, ainsi que la mise en place de protocoles cliniques de prise en charge, afin de pouvoir offrir aux patients pédiatriques une médecine d'urgence sécurisée et plus personnalisée
Impact du Lab-score sur la prescription d'antibiotiques chez l'enfant avec état fébrile sans foyer
Le Lab-score, combinant les résultats de protéine C-Réactive (CRP), procalcitonine et stix urinaire, a démontré une excellente capacité de détection de l'infection bactérienne sévère chez l'enfant avec état fébrile sans foyer. Notre étude visait à analyser son impact prospectif sur la réduction du taux de prescription d'antibiotiques chez l'enfant de 7 jours à 3 ans souffrant d'un état fébrile sans foyer. Malgré les excellentes caractéristiques diagnostiques du Lab-score, l'utilisation de ce dernier n'a pas montré d'efficacité sur la réduction de la prescription d'antibiotiques par rapport à un groupe contrôle d'enfants investigués selon les biomarqueurs usuels (globules blancs, neutrophiles immatures et CRP), essentiellement en raison de l'absence d'adhérence aux recommandations du Lab-score. Appliquées strictement, une réduction significative du taux d'antibiotiques aurait pu être observée tout en maintenant une excellente sécurité diagnostique. Renforcer l'éducation médicale et réviser les guidelines de prise en charge de l'état fébrile sans foyer chez l'enfant sont nécessaire
